There is no single characteristic clinical or radiological feature to diagnose xanthogranulomatous pyelonephritis, so it makes accurate preoperative diagnosis difficult. In our case, the renal parenchyme of affected kidney was well preserved on excretory urogram. The abnormal finding was only a filling defect within the right upper pelvocalyceal system. Herein we report a case of xanthogranulomatous pyelonephritis in a 18 years old girl which was confused with renal pelvic tumor.

Xanthogranulomatous pyelonephritis is an atypical form of severe chronic parenchymal infection and usually occurs in associated with urinary tract infection, obstruction and/or calculi. An accurate preoperative diagnosis is difficult because of its clinical and radiological similarities to various other renal lesions. We experienced two cases of xanthogranulomatous pyelonephritis, so we report with review of some literatures.

3.The value of CT scanning in the diagnosis of pyelonephritis of yellow granulatio

The study on 6 cases of pyelonephritis of yellow granulatio at Hue Central Hospital showed a majority of female patients (2/3 of cases) aged 50-60 years old), having classic trilogy of great kidney with calculus and loss of function. Patients were admitted because of lumbago in 2/3 of cases. Urimia, fever and purulent fistula at lumbar side, each 1 case; large kidney touched in 1/2 of cases. Unprepared abdomen X ray image, UIV and ultrasound had found calculus in 1/2 of cases, 1/4 with pyhydrostasis, 3/4 loss of function. Especially color Doppler ultrasound found kidney arterio venous communicatio fistula in 1/2 of cases. CT scanning showed 6/6 cases with fat structure in the kidney parenchyma, 5/6 with large shade kidney, irregular border and heterogenous stain absorption.

4.Two cases of acute focal bacterial nephritis.

Acute focal bacterial nephritis (AFBN), synonymous with acute lobar nephronia (ALN) or focal pyelonephritis, is an inflammatory mass without liquefaction caused by severe acute bacterial infection of kidney. Unless treated effectively, progression to a renal abscees may ensue. Clinically, AFBN presents as acute pyelonephritis, and radiologically, as focal swelling or mass. The distinction between AFBN and renal abceess or even tumor may be confusing and difficult. Ultrasonography and computerized tomography(CT) aid in establishing the correct diagnosis. However, the appearance of AFBN on sonography and CT is by no means pathognomonic. Benign or malignant renal tumors may have a similar appearance. With the clinical history, a high index of suspicion, and follow-up studies after institution of medical treatment may further aid in the differentiation. Herein we report 2 cases of AFBN, which were presented as renal masses.

Emphysematous pyelonephritis is a rare complication of severe acute pyelonephritis and a suppurative infection of the renal parenchyma characterized by the production of gas by bacteria through fermentation. Emphysematous pyelonephritis occurs in patient with diabetes mellitus or urinary obstruction. We review the diagnosis and management of emphysematous pyelonephritis and report a case of emphysematous pyelonephritis developed in renal tuberculosis.

Renal angiogram is a safe and excellent procedure for an early detection and accurate diagnosis of benign or malignant diseases. Selective renal angiography has been used mainly in the diagnosis of surgical disease, but now many informations are available regarding the vascular disease, pyelonephritis and renal tuberculosis. Emphysematous pyelonephritis is characterized by acute necrotizing renal parenchymal infection with gas formation. It is a serious, life-threatening infection that tends to occur in patients with diabetes mellitus and/or urinary tract obstruction. Early diagnosis and prompt intervention combined with surgical and medical treatment are required for survival. In Korea, there have been a number of cases of emphysematous pyelonephritis reported in patients with diabetes mellitus or urinary tract obstruction. We describe a case of emphysematous pyelonephritis in a patient without diabetes mellitus or urinary tract obstruction.

9.Bilateral emphysematous pyelonephritis: a case report

Authors reported a case of bilateral emphysematous pyelonephritis. Diagnosis is based on CT scan images of urinary system, or urography, and ultrasound of urinary system. This disease is a serious infection with high rate of death, usually occurs in diabetic patients, or in advanced stage. Treatment options are based on classifications according to CT scan, usually drainage of pus in pelvis renalis and bladder, and using antibiotics according to antibiogram. In unpreservable case, nephrectomy is applied. Healing is depended on severity and needs 1-5 years of follow-up.

10.Two Cases of the Metastatic Endophthalmitis.

The endophthalmitis is an ophthalmic emergency and it usually takes a tragic course, so its early diagnosis and appropriate treatment are urgently mandatory to preserve the vision and the eyeball. The authors found metastatic endophthalmitis developed during the course of acute pyelonephritis of two diabetic patients. In spite of aggressive topical and systemic managements, the disesed eyes were brought about to enucleation. The pathogens were proved to be Escherichia coli by culture of the specimen obtained from the vitreous. In accordance with cases and book review, it is emphasized that early recognition and aggressive treatments are essential in prevention of phthisis and blindness.
