1.Efficacy of caffeine combined with non-invasive respiratory support on the treatment of apnea in very low birth weight preterm infants

Objective To compare the efficacy and safety of caffeine combined with humidified high-flow nasal cannula(HHHFNC) and nasal continuous positive airway pressure(nCPAP) on the treatment of apnea in very low birth weight (VLBW) preterm infants.Methods Totally 80 VLBW preterm infants with neonatal apnea,who were enrolled in the NICU of Hebei Province Children Hospital from September 2013 to March 2014,were randomly assigned to either HHHFNC group(39 cases) or nCPAP group(41 cases) according to respiratory support mode by random number table method,both groups were given caffeine.The incidence of severe apnea,incidence of reintubation,oxygen exposure time,duration of non-invasive ventilation time,feeding conditions and incidence of adverse events were compared.Results There were no significant differences in male/female ratio,gestational age,age at randomization and birth weight between the two groups(P >0.05).There were no significant differences in the incidence of severe apnea[15.4% (6/39) vs 12.2 % (5/41)],incidence of reintubation [17.9 % (7/39) vs 19.5 % (8/41)],oxygen exposure time [(183.1 ± 31.2) h vs (175.9 ± 32.1) h],duration of non-invasive ventilation time [(163.3 ± 25.1) h v s (153.0 ± 26.2) h] between the two groups (P > 0.05).The occurrence of abdominal distention [7.7 % (3/39) vs 24.4% (10/41)],nasal trauma[2.6% (1/39) vs 19.5 % (8/41)],head shaping [0.0% (0/39)vs 29.3% (12/41)] during treatment were lower in HHHFNC group than those of nCPAP group (P < 0.05).Age began feeding was earlier in HHHFNC group than nCPAP group [(67.5 ± 19.1) h vs (96.3 ± 18.7) h,P < 0.05],and day to full oral feeding was reduced in HHHFNC group than nCPAP group [(346.8±28.6) h vs (371.0 ±29.4),P <0.05].Conclusion HHHFNC combined with caffeine is effective on the treatment of apnea in VLBW preterm infants,and easier to make newborn tolerated.HHHFNC is an effective non-invasive respiratory support mode with few side effects.

3.Therapeutic effect analysis of three internal fixation methods in the treatment of intertrochanteric femur fracture

[Objective] To discuss the therapeutic effect of different methods in the treatment of intertrochanteric femur fracture. [Methods]Totally 118 middle aged or old patients with intertrochanteric femur fracture were fixed with dynamic hip screw (DHS), Gamma nail or PFNA.[Results]Totally 112 cases were followed up, and the average time was 16 months, in those 45 patients were fixed with DHS: 29 were excellent, 11 cases were good, and 5 were poor, and the excellent to good rate was 88.9%; 36 patients were treated with Gamma nail, and the excellent, good, poor results were 28, 2, 1, respectively, and the excellent to good rate was 96.8%; 36 patients were fixed with PFNA, and the excellent, good, poor results were 29, 6, 1, respectively. The differences were significant (P=0.02, analyzed by Logistic regression with SPSS software). The excellent and good superiority of Gamma nail and PFNA were 5.1552 and 2.3726 times compared to DHS, and there was significant differences between the groups.[Conclusion]Surgical treatment of intertrochanteric femur fracture can improve the functional recovery and reduce the complication incidence, and PFNA fixing method can shorten the operative time, reduce the bleeding and the therapeutic effect is reliable, and suitable for old patients combined with osteoporosis.
4.Bibliometric analysis of Chinese ophthalmological papers indexed in SCI database from 2007 to 2011

[Objective] To learn the bibliometric characteristics of Chinese ophthalmological papers indexed in SCI database from 2007 to 2011.[Methods] All the ophthalmological papers published in the source journals indexed in the SCI database from 2007 to 2011 were retrieved.The papers of first authors were manually selected for bibliometric analysis.[Results] The ophthalmological papers published by Chinese scholars as the first author were 478,482,698,791,and 1049 from 2007 to 2011 (total 3498).The five institutions that published papers in the most were Sun Yat-sen University,Capital University of Medical Sciences,Fudan University,Hong Kong Chinese University and Shanghai Jiaotong University.The papers were distributed in 625 journals.The top five journals the papers were published in were Molecular Vision (332),International Journal of Ophthalmology (268),Investigative Ophthalmology & Visual Science (206),Chinese Medical Journal (109),and Graefe′s Archive for Clinical and Experimental Ophthalmology (104).The 3498 papers were cited 12 030 times,3.44 times per paper.The rate of non-cited articles for 5-year,3-year and 2-year periods were 12.55%,24.21% and 38.43% respectively.[Conclusion]s Chinese ophthalmological papers indexed in SCI database have gradually increased.Chinese ophthalmological papers mainly originate in the affiliated hospital of universities and colleges.There are four ophthalmologic professional periodicals included in the top five in the quantity of articles.

6.Individualized Pharmaceutical Care for One Patient with End-stage Renal Disease and Atrial Fibrillation after Aortic Valve Replacement

Objective:To explore the importance and the breakthrough points of pharmaceutical care for patients with renal disease,especially using drugs with narrow therapeutic window and notable individual difference.Methods:Digoxin concentration and international normalized ratio (INR) were detected by clinical pharmacists,and then individualized pharmaceutical care was given to the patient according to the monitoring results.Results:Physician increased the monitoring frequency and adjusted the drug usage according to the patient's renal function.The patient's condition was improved and understood the necessity of related index monitoring.Conclusion:Clinical pharmacists can assist physicians in optimizing the treatment,and ensure safe,reasonable and effective medication by providing pharmaceutical care in various aspects.
7.Determination of Antioxygen 1178 in 5-Layer Co-extrusion Bags Used for Infusion by HPLC

Objective:To establish a method for the determination of antioxygen 1178 in 5-layer co-extrusion bags used for infu-sion. Methods:A Uitimat XB-C8 (250 mm × 4. 6 mm,5μm) column was used,the mobile phase was methanol-water with gradient elu-tion, the flow rate was 1. 0 ml·min-1 ,the detection wavelength was 223 nm,the column temperature was 35℃ and the injection vol-ume was 20 μl. Results:The concentration of antioxygen 1178 had the linear relationship within the range of 1. 64-205. 10 μg·ml-1 (r=0. 999 9),and the average recovery was 92. 05% (RSD=1. 94%, n=9). Conclusion:The method is accurate,stable and spe-cific,and can be used to determine antioxygen 1178 in 5-layer co-extrusion bags used for infusion.

9.Research of urinary incontinence and depression among aged female patients

Objective To explore the prevalence and relationship of urinary incontinence and depression among elderly female patients.Methods A total of 118 female patients older than 60 years old were interviewed face to face.The prevalence of urinary incontinence and depression were estimated using the International Consultation on Incontinence Questionnaire Urinary Incontinence Short Form-Chinese (ICIQ-SF-Chinese) and Geriatric Depression Scale-15 (GDS-15).Linear regression and Spearman correlation were used to identify the relationship between urinary incontinence and depression.Results Urinary incontinence was found in 27.1% (32/118),and depression was in 39.0% (46/118) of the survey population.Urinary incontinence with depression was found in 13.6% (16/118) of the overall population.Urinary incontinence was positively correlated with depression,r=0.211,P<0.05.The frequency of entertainment and urinary incontinence were significantly related to depression symptoms,P<0.05.Conclusions Urinary incontinence appears to possess an additive effect on incidence of depression,which seriously affects both physical and mental health of patients.Clinicians should give patients health education and intervention measures at early stage.

Calcium/calmodulin-dependent protein kinaseⅡ (CaMKⅡ) has multiple functions, which made it play a central role in cardiovascular disease. Especially it activates numerous downstream targets in various signaling pathways that promotes vascular disease, heart failure, myocardial hypertrophy and arrhythmias. CaMKⅡcan impact calcium balance and increase calcium leak in myocardial cell via phosphorylating L type calcium channel, Ryanodine receptor (RyR 2) and phos?pholamban (PLN), and regulate ATP sensitive potassium current (IKATP) and late sodium current by affecting sodium channels and potassium channels. In addition, It can directly regulate transcription via activating the silk crack the original activated protein kinases (MAPKs) and acetylation enzyme (HDAC). These mechanisms have important roles in myocardial hypertro?phy, heart failure and arrhythmia. So we focus to demonstrating the structure and action mechanism of CaMKⅡto improve a new therapy of cardiovascular disease.
