3.Influence of rehabilitation for patients with dysphagia following stroke: observation of patients' nutriture and analysis of medical costs

Objective To explore the effects of comprehensive rehabilitation on nutriture and medical costs in the treatment of patients with dysphagia following stroke. Methods Eighty-seven patients with dysphagia were divided into an intervention group (IG) and a control group (CG). Comprehensive rehabilitation therapy including deglutition training, electric neuromuscular stimulation and acupuncture were used with the IG patients, but not with those in the CG. Nutriture parameters included a biochemistry index and a body measurement index. Medical costs were observed before and after one month of treatment for both groups. Results After one month of treatment, average deglutition scores and nutriture indices were significantly higher in the IG than in the CG. Medical costs for the IG had decreased significantly compared with the CG. Conclusions Comprehensive rehabilitation therapy improved deglutition function and slowed nutriture deterioration, while decreasing medical costs in the treatment of patients with dysphagia following stroke.
4.Application value of cerebrospinal fluid MRLCs combined with lactate dehydrogenase detection in diagnosis of central nervous system leukemia

Objective To study the apllication value of cerebraspinal fluid minimal residual leukemia cells(MRLCs) combined with lactate dehydrogenase(LDH) detection in early diagnosis of central nervous system leukemia(CNSL) to provide a laboratory basis for clinical diagnosis and evaluation of disease progression and therapeutic effect.Methods Thirty inpatiernts without blood disease and central nervous system(CNS) organic diseases,and normal CSF routine and detected biochemical indexes were selected as the control group and 96 cases of acute leukemia (AL) diagnosed by bone marrow puncture FAB morphology and flow cytometry typing served as the disease group.Flow cytometry was used to detect the MRLCs in CSF,the activity of LDH in CSF was detected by the rate method,and the detection results were performed the comparative analysis.Results Among 96 cases of AL,30 patients with CNSL had 13 cases of positive MRLCs in CSF,the positive rate was 43.33%,among 66 cases of non-MRLCs,MRLCs was not detected detected.Among 96 cases of AL,CSF LDH level in 30 cases of CNSL was (30.54±10.29)U/L,which was significantly higher (16.16±7.12)U/L in the patients without CNSL,the difference between the two groups was statistically significant (P<0.05).Conclusion The detection of MRLCs and LDH in CSF of AL can be used as a laboratory diagnostic index for early diagnosis of CNSL,and MRLCs combined with LDH detection can improve the positive diagnosis rate of CNSL.
5.Expression of KPNA2 in hepatocellular carcinoma

Objective To detect the mRNA expression of KPNA2 in hepatocellular carcinoma(HCC) tissue and to investigate its relationship with the clinicopathological features of HCC so as to provide the basis for predicting HCC infiltration ,metastasis and early treatment .Methods The HCC tissue and paracarcinoma normal tissue from 30 operative cases of HCC in the hepatobiliary surgery department of this hospital from January 2011 to November 2013 were collected and detected KPNA2 expression at the transcriptional level by RT-PCR .The correlation between the mRNA expression of KPNA2 with the clinicopathological factors of HCC was analyzed by consulting the medical record data .Results Among 30 cases of HCC ,the ratio of KPNA2 mRNA in the HCC tissue and the paracarcinoma normal tissue in 25 cases(83 .33% ) was up-regulated (grey value >2 times) .Therefore ,the KP-NA2 gene exhibited the high expression in the HCC tissue and low expression in the paracarcinoma normal tissue .Conclusion KP-NA2 is highly expressed in the HCC tissue and lowly expressed in the paracarcinoma normal tissue ;the high expression of KPNA2 is positively correlated with the stage and pathological grading of HCC ;KPNA2 may be used as an important indicator for early di-agnosing HCC and judging the malignant degree and potential metastais ability of HCC .
6.Management for late complications after transurethral resection of the prostate

Objective To analyze the causes and management of the late complications after transurethral resection of the prostate (TURP) for benign prostatic hyperplasia. Methods The clinical data of 102 patients who were hospitalized for the late complications after TURP were analyzed retrospectively. Results The causes of readmission were residual prostatic hyperplasia for 32 patients (31.37%),bladder neck restriction for 22 patients (21.57%),urethral stricture for 18 patients (17.65%),hematuria for 15 patients (14.71%),epididymitis for 6 patients (5.88%),urinary tract infection for 4 patients (3.92%),carcinoma of prostate for 3 patients (2.94%),and neuropathic bladder dysfunction for 2 patients (1.96%). All patients were satisfied with their therapeutic efficacy after different treatments. Conclusion The main complications after TURP include residual prostatic hyperplasia,bladder neck restriction,urethral stricture and hematuria. Best therapeutic approach should be chosen according to different causes of the late complications after TURP.
7.UV-Visible Spectrophotometric Assay for Saikosaponins in Bupleurum Chinense DC

Objective To set up a simple and accurate method for the determination of saikosaponins in Bupleurum chinense DC. by UV-Visible Spectrophotometry. Methods The content of saikosaponins was determined by UV-Visible Spectrophotometry. The absorbency was measured at 536nm. Results The calibration curves was linear within 194~1940mg/L(r=0.9996). The average recovery was 97.35%,and the RSD was 1.02%(n=9). Conclusion The method was proved to be simple, precise and reproduciable. It can be used for the quantitative determination of B upleurum chinense DC.
8.Effect of over Expression of P53 and C-erbB-2 on Prognosis of Breast Cancer

Objective The purpose of this study is to assess the prognostic significance of over expression of P53, C-erbB-2 in breast cancer. Methods In 48 patients with breast cancer the expression of P53, C-erbB-2, PCNA and ER was determined by the S-P staining immunohistochemical technique. Results The positive rate of P53, C-erbB-2, PCNA and ER were 54%, 43.7% 52%, 41.6% seperateiy. Expression of P53 and C-erbB-2 increased with the class of the tissuess. There is significant difference between grade Ⅰ and Ⅱ , Ⅲ (P<0.05) as well as between lymph node metastasis (+) and lymph node metastasis (-) patients (P<0.05). Conclusion The over expression of P53, C-erbB-2 may be a useful prognostic indicator in breast cancer.
9.Iodine nutrition survey of children aged 8 to 10 in coastal and non-coastal areas of Rizhao city Shandong province

Objective To investigate the iodine nutritional status of children aged 8 to 10 in both costal and non-costal areas of Rizhao city Shandong province,and provide a basis for scientific supplementation of iodine.Methods In 2009 and 2010,townships of Lanshan and Donggang were chosen as survey points which situated close to the coast,the counties of Wulian and Juxian that were more than 50 kilometers away from the coast were selected as controls.The iodine concentration of drinking water in every village and water supply point was determined.At the county level,5 to 9 towns were selected according to their sub-area positions of east,west,south,north and center,4 villages were selected in each chosen township,8 - 15 households were selected in each chosen village,edible salt from the households was collected; 5 primary schools were selected in each chosen township,60 students aged 8 - 10 were selected to take thyroid examination in each chosen school.Twenty copies of urine samples were collected from the 60 students to detect the iodine concentration.Iodine in drinking water was tested by cerous sulfate catalytic spectrophotometry,in iodized salt by direct titration,in urine by arsenic-cerium contact method,and thyroids were examined by palpation.Results Totally 3483 copies of drinking water samples,1164 copies of edible salt samples,and 476 copies of urine samples were tested,and a total of 1200 children aged 8 to 10 were investigated; and the ratio of water iodine frequency distribution ≤ 10 μg/L (in costal and non-costal area) accounted for 90.03% ( 1011/1123 ) and 91.10% (2150/2360),respectively,the medians of water iodine were 5.6 and 4.2 μg/L,respectively; the iodine medians of edible salt were 29.03 and 29.99 mg/kg,respectively; the consumption rates of qualified iodized salt were 96.77% (569/588) and 97.05% (559/576),respectively; total goiter rates were 1.17%(7/600) and 1.33%(8/600),respectively; the medians of urinary iodine were 144.05 and 159.15 μg/L,respectively; the percentages of urinary iodine that bellow 100 μg/L were 26.58% (63/237) and 22.59% (54/239),respectively; between 100 - 300 μg/L were 66.75% (158/237) and 64.02% (153/239),respectively;above 300μg/L were 6.75% (16/237)and13.39% (32/239),respectively.ConclusionsThe costal of Rizhao is an iodine deficient area.The current popularity of household's consumption of qualified iodized salt is suitable.The levels of iodine nutrition of children aged 8 to 10 is appropriate,which met the national standard of elimination of iodine deficiency disorders (IDD).There is no iodine excess.There are no significant differences in levels of iodine nutrition between costal and non-costal areas on the premise that the same iodized salt is supplied in both the areas,and there is no need to make differentiated supply of iodized salt.
10.Effects of Alcohol Extract of Toddalia asiatica on the Inflammation-associated Cytokines of Model Rats with Adjuvant Arthritis

OBJECTIVE:To study the effect of alcohol extract of Toddalia asiatica on the inflammation-associated cytokines of model rats with adjuvant arthritis(AA). METHODS:70 SD rats were randomly divided into a normal control group,a model con-trol group,a positive control chemical medicine group(Leflunomide tablets,0.012 g/kg),a positive control TCM group(Tripter-ygium glycosides tablets,0.012 g/kg)and the groups of low,medium and high-dose [1,4,6 g(crude drug)/kg] alcohol extract of T. asiatica,with 10 rats in each group. The rats in all groups except for the normal control group were given complete Freund’s complete adjuvant id for the establishment of AA models. At the same time,the rats in the drug administration groups were given corresponding drugs ig,while those in the normal control group and the model control group were given isometric normal saline ig,twice a day,for 28 consecutive days. The degree of toe swelling,arthritis index and the levels of interleukin(IL)-1β,IL-6, IL-10,tumor necrosis factor α(TNF-α)and prostaglandin E2(PGE2)in serum and synovial membranes of all groups of rats were determined. RESULTS:Compared to the normal control group,the model control group demonstrated higher degree of primary and secondary toe swelling,arthritis index and levels of IL-1β,IL-6,TNF-αand PGE2 in serum and joint synovial membrane,and low-er level of IL-10 therein(P<0.01). Compared to the model control group,all the above-mentioned indexes of the rats in drug ad-ministration groups significantly improved(P<0.05 or P<0.01). CONCLUSIONS:The alcohol extract of T. asiatica. has a preven-tive and therapeutic effect on the model rats with AA by regulating the expression of anti-inflammatory and proinflammatory cyto-kines in serum and synovial membrane.
