1.Clinical efficacy of the technique of reticular basket through cystic duct stone removal during laparoscopic common bile duct exploration

ObjectiveTo explore the efficacy of the technique of reticular basket through cystic duct stone removal during laparoscopic common bile duct exploration.MethodsFrom January 2008 to February 2011,185 patients received laparoscopic bile duct exploration in our hospital.These patients underwent intraoperative cholangiography (IOC) through the cystic duct with a reticular basket.With the help of a choledochoscope,a reticular basket was used to take out stones for patients who were shadow-positive.Re-cholangiography was then performed to ascertain that no stone was retained in the biliary tract.ResultsOf the 185 patients,60 were found to be positive on IOC.Stones were successfully removed by using a reticular basket through the cystic duct in 45 patients,and through the cystic duct and common bile duct after a micro-incision was made under choledochoscopic assistance in 10 patients.After laparoscopic common bile duct exploration re-cholangiography found no residual stones in these patients. In 5 patients,stones were removed after conversion to open surgery. Conclusions During laparoscopic common bile duct exploration,using reticular baskets via the cystic duct for intraoperative cholangiography (IOC) and removing stones is of great value.
2.The Role of FUS/TLS in the Regulation of Gene Expression and the Correlation with Neurodegenerative Diseases

Neurodegenerative diseases are chronic lesions caused by neuronal degeneration and secondary demyelination.Recent studies have shown that a DNA/RNA binding protein named fused in sarcoma/translocated in liposarcoma protein (FUS/TLS) is associated with neurodegenerative diseases,such as amyotrophic lateral sclerosis (ALS),frontotemporal dementia (FTD) and polyglutamine diseases.Mutations or dysfunction in FUS/TLS are responsible for the abnormal processes in the related gene transcription,RNA processing,transportation and translation and are highly correlated with the development of neurodegenerative disease.Discovery and the research of FUS/TLS not only contribute to the understanding of neurodegenerative disease at RNA level,but also provide a new way for the prevention and treatment of neurodegenerative diseases.
3.Effect of recombinant human brain natriuretic peptide on sepsis induced myocardial dysfunction

Objective To explore the interventional effect of recombinant human brain natri-uretic peptide(rhBNP)on the patients with sepsis induced myocardial dysfunction by comparing N-terminal pro-B-type natriuretic peptide(Nt-proBNP),troponin and left ventricular ejection fraction (LVEF).Methods Forty septic patients were admitted into department of critical care medicine from February 2013 to May 201 5.They were randomly divided into control group and rhBNP group,with 20 patients in each group.Both groups received early goal directed therapy(EGDT),and rhBNP group re-ceived additional therapy of rhBNP 1.5 μg/kg bolus,and then 0.075 μg·kg-1 ·min-1 for 48 h.Con-trol group received saline in the same way.Vasoactive agents were used to maintain blood pressure. Concentration of plasma Nt-proBNP and troponin were determined at baseline,on days 1,3,and 7 day by point of care (POCT ).LVEF was also detected by transthoracic echocardiography. Results Concentrations of plasma Nt-proBNP and troponin in rhBNP group were lower than control group on days 1 and 3 (P <0.05).On day 3,levels of Nt-proBNP and troponin in both groups were higher than the days 1 and 7 (P <0.01).LVEF in the rhBNP group was higer than control group on days 3 and 7 (P <0.05 or P <0.01);On day 3,LVEF in both groups was significantly lower than the pre-therapy and day 7 (P < 0.05 or P < 0.01 ).Conclusion rhBNP can improve sepsis induced myocardial dysfunction by decreasing the Nt-proBNP and troponin levels with increased left ventricular ejection fraction.
4.Analysis on results difference of two methods for detecting 146 cases of anti-nuclear antibody

Objective To investigate the results difference of the indirect immunofluorescence (IIF) and immunoblotting (LIA) for detecting ant-inuclear antibodies (ANA) and clinical value .Methods One hundred and forty-six ANA detection specimens of IIF negative and LIA positive were collected and performed the detection of HBV antibodies and anti-HCV antibodies .Results (1) In 146 specimens of IIF (-)LIA (+ ) ,the positive specimen numbers of single anti-Ro52 antibody ,anti-SSA antibody and anti-AMA-M2 antibody were 69 cases ,42 cases and 15 cases respectively ,which of anti-RNP antibody ,anti-PCNA antibody and PM-Scl antibody were 3 cases ,5 cases and 2 cases respectively ,the combined 2-item positive was in 8 cases .(2)Among positive specimens of single anti-Ro52 antibody ,HBsAg(+ ) ,anti-HBe(+ ) ,anti-HBc(+ ) model and HBsAg(+ ) ,HBeAg(+ ) ,HBcAb(+ ) model of hepatitis B ,and hepatitis C were 51 cases ,4 cases and 7 cases respectively ,7 cases were non-hepatitis patients .(3) Among positive specimens of single ant-SSA antibody ,6 cases were hepatitis B patients and 36 cases were the patients with non-hepatitis B .Conclu-sion Anti-Ro52 antibody and anti-SSA antibody are easier to be missed by the IIF detection .Anti-Ro52 antibody positive has a cer-tain relation with small three positive of hepatitis B .
5.Skinfold change with aging in adult females of Maonan nationality in Guangxi province of China

BACKGROUND: Skinfold thickness instrument was used to analyze the distribution and development of subcutaneous fat. This method is simple, economic, and widely used. OBJECTIVE: To investigate the body fat development and age changes in female adults from Maonan nationality in Guangxi province. DESIGN, TIME AND SETTING: The cluster sampling investigation was performed at the Department of Anatomy, Basic College, Youjiang Medical College for Nationalities from September to October 2006. PARTICIPANTS: Using random sampling method, 533 healthy adult females of Maonan nationality and aged 25-75 years were selected from Huanjiang county of Guangxi province. They all live in Huanjiang count for over 10 years and their parents were also Maonan nationality. Taking five years were served as a group, totally 10 groups. METHODS: The indexes were measured and calculated by professionals according to Handbook of Human Measure. The equipment was modified skinfold thickness instrument. The database was set up in computer. MAIN OUTCOME MEASURES: Skinfold thickness of triceps brachii, anterior superior iliac spine, inferior angle of scapula and gastrocnemius. Four indexes were compared with other groups in China. RESULTS: The torso skinfold was thicker than limb ones. Skinfold thickness decreased with age after it came to the maximum. Compared with other minornationalities, the skinfold of Maonan adult females in Huanjiang of Guangxi province was thinner than Daur and Uzbek, but thicker than Bouyei nationality. CONCLUSION: The skinfold thickness of Chinese Maonan nationality in Guangxi province has a wavy change. It is thicker than Bouyei nationality and thinner than those in Dong nationality and Daur nationality and Uzbek nationality.

7.The impact of serum uric acid level to CSFP

ObjectiveTo investigate on the impact of the serum uric acid (UA) level coronary slow flow phenomenon(CSFP).MethodsSelected 30 cases of coronary slow flow patients as CSFP group,30 cases of patients with coronary heart disease,30 cases of patients with angiographically normal as the control group.The serum UA level was measured in three groups.The CSFP group and the control group were measured left anterior descending,circumflex and right coronary artery TIMI frame count(TFC) value.To analyze the mean TFC value of CSFP patients and the relationship between the serum UA level.ResultsCompared with the control group,the left anterior descending,circumflex and right coronary TFC results were significantly higher(all P < 0.05) in the CSFP group.The mean serum UA level of CSFP group ,control group and CHD group were(340.79 ± 79.40)μmol/L, (220.45 ± 82.34)μmol/L,(42625 ± 87.02) μmol/L, the difference was statistically significant (all P < 0.05).The mean serum UA level and TFC value of CSFP group was positively correlated(r = 0.943, P = 0.007).ConclusionElevated level of serum UA is closely related to the occurrence and development of the CSFP.
8.Analysis of Application of Antihypertensive Drugs in Wuhan Xiehe Hospital

Objective:To review the application of antihypertensive drugs in Wuhan Xiehe Hospital in the period of 1996 to 1998.Methods:Statistically analyze the application of antihypertensive drugs during the period in terms of their types,sales income,DDDs*d-1 as well as the changes of daily expense on drugs.Results:During the period analyzed,the newly applied drugs are mainly ACEI drugs.The DDDs*d-1 of metoprotol,benazepril,nifedipine and nimodipine had been among the highest.The increase of DDDs*d-1 was rapid.The newly prescribed drugs were mainly oral and retard preparations.Conclusion:The Prescription of antihypertensive drugs in Wuhan Xiehe Hospital was rational and in accordance to the principles of convenience and cost-effectiveness.
9.Clinical,image and pathological features of Wernicke's encephalopathy

Objective To investigate the clinical, imaging and pathological features of Wernicke's encephalopathy (WE).Methods The clinical, imaging and pathologic datas of 10 patients with WE were analyzed retrospectively.Results 10 patients were not ethylism. 9 cases presented various mental and conscious disturbance, 6 cases initially presented vertigo, nausea and vomiting. 5 cases showed ophthalmoparalysis. 3 cases displayed hypotension.2 cases showed ataxia and 1 case showed severe peripheral neuropathy. 3 of the 5 patients with MRI examinations showed symmetric T_1 and T_2 high signal in encephalocoele and periphery of aqueduct of midbrain, the other 2 cases were no positive finding . 4 cases with supplement VitB_1 were cured, 5 cases died.1 case withdrawed. 5 autopsy cases showed congestion, edema and multiple petechial hemorrhages in encephalocoele and periphery of aqueduct of midbrain.Conclusions The clinical manifestation of WE is atypical, and MRI imaging is helpful for early diagnosis of WE. It is the key treatment to supply the thiamine as early as possible.
