1.Effect of recombinant human brain natriuretic peptide on sepsis induced myocardial dysfunction

Objective To explore the interventional effect of recombinant human brain natri-uretic peptide(rhBNP)on the patients with sepsis induced myocardial dysfunction by comparing N-terminal pro-B-type natriuretic peptide(Nt-proBNP),troponin and left ventricular ejection fraction (LVEF).Methods Forty septic patients were admitted into department of critical care medicine from February 2013 to May 201 5.They were randomly divided into control group and rhBNP group,with 20 patients in each group.Both groups received early goal directed therapy(EGDT),and rhBNP group re-ceived additional therapy of rhBNP 1.5 μg/kg bolus,and then 0.075 μg·kg-1 ·min-1 for 48 h.Con-trol group received saline in the same way.Vasoactive agents were used to maintain blood pressure. Concentration of plasma Nt-proBNP and troponin were determined at baseline,on days 1,3,and 7 day by point of care (POCT ).LVEF was also detected by transthoracic echocardiography. Results Concentrations of plasma Nt-proBNP and troponin in rhBNP group were lower than control group on days 1 and 3 (P <0.05).On day 3,levels of Nt-proBNP and troponin in both groups were higher than the days 1 and 7 (P <0.01).LVEF in the rhBNP group was higer than control group on days 3 and 7 (P <0.05 or P <0.01);On day 3,LVEF in both groups was significantly lower than the pre-therapy and day 7 (P < 0.05 or P < 0.01 ).Conclusion rhBNP can improve sepsis induced myocardial dysfunction by decreasing the Nt-proBNP and troponin levels with increased left ventricular ejection fraction.
2.Value of volume rendering technique of MSCT in the diagnosis of pediatric pectus carinatum

Objective To evaluate the value of volume rendering technique of MSCT in the diagnosis of pediatric pectus carinatum. Methods MSCT data of 36 children with pectus carinatum were collected.Volume rendering and multiplanar reformation recon-struction techniques were adopted at a postprocessing station.Results There were 35 cases of chondrogladiolar variant and 1 case of chondromanubrial variant,27 cases were symmetric and other 9 cases were asymmetric.VR technique clearly demonstrated the abnormal changes of sternum,costae and costal cartilages.Conclusion VR technique of MSCT can fully display the three-dimension morpho-logical changes of pediatric pectus carinatum,which can supply accurate information for minimally invasive treatment procedure.
3.Probing the timing of treatment of neonatal dacryocystitis

Objective To find earliest or the best time for children with neonatal dacryocystitis acceptable treatment of lacrimal to shorten the course and improve treatment Methods Different time points after birth accepted probing treatment efficacy in children with neonatal dacryocystitis were retrospectively analyzed. Results Probing technique was effective for newborn infant with dacryocystitis. Symptoms recovered in 47 eyes,and 26 eyes were significantly improved with only a simple washing,there are 33 eyes need to wash and rinse probe or the expansion of the sclera again,and it was more common in the disease course of 4 ~6 months. Once probing technique treatment in the group( within 3 days) were effective in 23/34(67. 7% ) ,more than it in the group(more than 4 days) with effect of 24/72(33. 3%), One probing technique treatment in the group(within 10 days) were effect of 12/53(22.6%). Conclusion Press and simple washing techniques ineffective in children with neonatal dacryocystitis, was depended on operator proficiency, and time of lacrimal passage sounding could really advance the treatment to patients within 10 days after birth.
5.The facial nerve functional recovery law and tumor size impact after acoustic neuroma surgery

Objective To explore the facial nerve functional recovery law after resection of acoustic neuroma,and the influence of tumor size on postoperative facial nerve function. Methods According to the House-Brackman (HB) facial nerve function classification method, 89 patients with acoustic neuroma were performed microsurgical resection with the ret?rosigmoid approach and facial nerve preservation. The HB classification method was used to evaluate the facial nerve func?tion at operation, 15 d, 45 d, 3 m, 6 m, 12 m and more than 12 m after surgery. The recovery pattern of neurological function after operation was analyzed. al. According to the tumor size, patients were divided into three groups: diameter < 30 mm group (n=23), 30-40 mm group (n=31) and≥40 mm group (n=35). The facial nerve function was compared between different groups with early postoperative (within 15 days) and long-term (more than 12 months). Results The facial nerve function was the worst in 15 days after operation (excellence rate was 52.81%), but the function was returned to normal in postopera?tive 3 months (excellent rate reached 80.90%). After postoperative 12 months, almost all patients returned to normal func?tion (excellent rate was 91.01%), and the facial nerve recovery was more smoothly (excellent rate was 92.13%). Tumor size had remarkable effect on facial nerve function in the early postoperative period (χ2=23.34, P<0.05), and long-term period (χ2=14.46, P<0.05). And tumor size was positively correlated with classification of facial nerve function in the early stage (r=0.476, P<0.05) and long-term stage (r=0.379, P<0.05). The excellent rates of postoperative facial nerve function were decreased with the increased diameters of tumor size. Conclusion The facial nerve function may appear deterioration in early postoperative period (within 15 days) in patients with acoustic neuroma, which can return to the normal level in 12 months. The diameter of tumor is one of important factors influencing the early and long-term prognosis of postoperative fa?cial nerve function.
6.Influence of intimacy and adaptability with family members on postpartum recovery

Objective To explore the influence of the intimacy and adaptability to the other fa- mily members on the postpartum recovery of parturients. Methods One hundred and twenty-two pregnant women were randomly selected among all the admissions from March to August of 2004 in the hospital. FACESⅡ-CV and (EPDS) were applied in this study. Results Postpartum depression and postpartum recovery were significantly correlated with the degree of intimacy and adaptability between the mother and other family members. Conclusions A higher degree of intimacy and adaptability in the family is helpful in postpartum recovery, breast-feeding and alleviating symptoms of postpartum depression.

8.Atrioventricular nodal ablation for patients with chronic atrial fibrillation

Objective: To evaluate effects of atrioventricular nodal ablation, pacemaker and pharmaco logic agents on ventricular rate control. Methods:Three patients with chronic sustained atrial fibrillation ( AF) were treated by atrioventricular nodal ablation. Fast-slow junctional cardiac rhythm in this course meant the ablation was successful. Unless the ablation was able to get Ⅱ? Ⅱ type or Ⅲ? AVB, R-R intervals ≥3 s, ventricular rate≤40 bpm, it would be canceled. Otherwise implantation of a permanent pacemaker was performed. Results:One patients having complete atrioventricular block and a ventricular rate of 38 -40 bpm had implantation of a permanent pacemaker. Two patients having no atrioventricular block, prolongation of R-R intervals and ventricular rate of 90 - 120 bpm were given antiarrhythmic drugs. Conclusion: Patients with chronic sustained AF can be treaded with atrioventricular nodal ablation, pacemaker and pharmacologic agents effectively.
9.Association of combined CYP2E1 gene polymorphism with the risk for esophageal squamous cell carcinoma in Huai'an population, China.

BACKGROUNDCytochrome P450 2E1 (CYP2E1) has an important role in the metabolic activation of precarcinogens such as N-nitrosoamines and other low relative molecular mass, organic compounds. This study examined whether CYP2E1 RsaI and DraI polymorphism are associated with susceptibility to esophageal squamous cell carcinoma and the correlation between the genotypes and expression levels of CYP2E1 mRNA.
METHODSSeventy-seven patients with newly diagnosed, untreated esophageal squamous cell carcinoma and 79 healthy controls matched in age, gender and residence were recruited for the control study. An RsaI polymorphism in the 5'-flanking region and a DraI polymorphism in the sixth intron of the CYP2E1 gene, which could possibly affect its transcription, were determined in this study by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) and mRNA level of CYP2E1 was measured by quantitative real-time reverse transcription PCR.
RESULTSNo significant association of RsaI or DraI polymorphism of CYP2E1 with susceptibility of esophageal squamous cell carcinoma were demonstrated (OR = 1.67, 95% CI: 0.89 - 3.15, P = 0.11; OR = 1.11, 95% CI: 0.59 - 2.09, P = 0.74, respectively). With SHEsis software, no linkage disequilibrium was detected between RsaI and DraI polymorphism (D' = 0.528, r(2) = 0.27). When combined RsaI polymorphism with DraI polymorphism, the association between that carrying c2 allele and DD genotype and the risk for esophageal squamous cell carcinoma were found (OR = 5.77, 95% CI: 1.65 - 20.22). Compared with the normal controls, the mRNA levels with RsaI polymorphism, DraI polymorphism, or any combined genotypes in cases showed no statistical difference.
CONCLUSIONSThis study suggests that carrying c2 allele and DD genotype conferreded an elevated risk for esophageal squamous cell carcinoma. There was no significant statistical relationship between the genotypes c1/c2, D/C, or the combined allele and mRNA expression.

10.TESTIN tumor suppressor gene in endometrial cancer tissues

ObjectiveTo investigate the expression of TESTIN gene in endometrial carcinoma and explore the functions of this gene in tumor development and progression.MethodsqRT-PCR and immunochemical staining assay were used to determine the mRNA and protein level of TESTIN in the tumor tissues,and the relationship between TESTIN expression and clinical pathology characteristics was analyzed.Results Compared to normal tissue,76.5% (52/68) tumor tissues showed TESTIN reduced ( P < 0.01 ),furthermore,this reduction in the subgroup of endometrioid adenocarcinoma was significant,but it was rarely observed in the subgroup of serous papillary adenocarcinoma.ConclusionsTESTIN was obviously down regulated in endometrail carcinoma,especially in endometrioid adenocarcinoma,which indicated TESTIN played an important role in tumorigenesis of uterine.
