1.Clinical Significance of Judgement About RA Carpal Subsidence by Carpal Height Ratio

Objective To evaluate the clinical significance of judgement about rheumatoid arthritis(RA)carpal subsidence by carpal height ratio(CHR).Methods The materials which have been maken definite diagnosis,including pair of carpal films of 40 cases,were devided into 6 groups to measure carpal height ratio by X-ray stage and deviation of metacarpal and phalanx toward ulna side.Results Early stage ? 36 =0.512,Middle stage ? 26 =0.514,Later stage? 18 =0.455.The group without deviation toward ulna side ? 69 =0.502,the group which deviation was smaller than 15?:? 10 =0.484,the group which deviation was larger than 15 ?:? 1 =0.450.Conclusion Carpal height ratio(CHR)has very importmant clinical significance to study the development and evolution of rheumatoid arthritis(RA)carpal subsidence.
2.Orthodontic treatment for AngleⅡ~2 malocclusion accompanying one side of TMJ click:contrast before and after treatment

Objective To investigate the imaging and condylar movements during mandibular opening/closing movement in AngleⅡ2 malocclusion accompanying one side of temporomandibular joint(TMJ)click before and after orthodontic treatment.Methods There were 20 AngleⅡ2 malocclusion patients with one side of TMJ click in treatment group,and 23 with the individual normal occlusion in control group.This study was performed taking the films of Sch?ller position before and after treatment,using CADIAX to record the condylar tracings during opening/closing movement and analyze the qualitative and quantitive changes.Results In AngleⅡ2 malocclusion patients,the condylar position of 58.33% patients recovered from retroposition to mesoposition.The condyle movement presented conspicuous verticality,the smoothing and symmetry were not well,and obvious improvements were observed after the treatment.All the index changed remarkably except the TCI and TCI of 5 mm.Conclusion It is suggested that orthodontic treatment can make the disc-condyle relationship return to normalization,and the condylar movement become much more coherent.
3.Clinical characteristics of atherosclerosis in extracranial carotid arteries in patients with ischemic cerebrovascular disease

The higher-grade stenosis of carotid artery may be related to soft plaque and mixed plaque, the plaque rupture is concerned with in the incidence of ischemic stroke. The diagnostic information about atherosclerotic plaque in one carotid artery can be used to infer information about atherosclerosis severity degree in contralateral artery.Color-Doppler ultrasound examination for ECCA had important reference value for estimating atherosclerosis severity degree in ECCA in patients with ICVD of carotid artery system and the pathogenesis of ischemic stroke.
4.Cell-killing and sensitization effect of 6-gingerol on human hepatoma carcinoma cell in chemotherapy in different environment

Objective To compare the cell-killing and sensitization effect of 6-gingerol on human hepatoma carcinoma (HepG-2) cell in normal mode versus hypoxia-hypoglycemia mode in chemotherapy.Methods The HepG-2 cells was cultured to logarithmic phase and treated with adriamyein doxorubicin hydrochloride (ADM) (5,10,15,20,40,60 mg/L) and 6-gingerol(25,50,100,200 μmol/L)in different concentrations.Then the cell counting kit (CCK-8) assay kit was used to determine the proliferation inhibition of HepG-2 cells.Cell apoptosis was detected by combining flow cytometry and AnnexinV-FITC PI double staining after treated with different drugs.The expressions of bcl-2,bax and birc-5 mRNA in HepG-2 cells was detected by real time fluorescence quantitative polymerase chain reaction(RT-PCR)assay.Results 6-gingerol and ADM had a certain degree of growth inhibition on HepG-2 cells.In two modes,the inhibition ratios of the 6-gingerol and ADM were both increased along with the increase of the concentration,which showed a dose-dependent manner.Flow cytometry analysis demonstrated that the apoptosis rate in the control group,6-gingerol group,ADM group and the 6-gingerol+ADM group in the normal mode was (7.98±0.76)%,(9.63 ± 1.00) %,(12.70 ± 2.13) % and (19.92 ± 1.41) % respectively.The apoptosis rate in the control group,6-gingerol group,ADM group and the 6-gingerol+ ADM group in the hypoxia-hypoglycemia mode was (13.92 ± 2.02)%,(19.36 ±-1.22)%,(27.87 ± 0.99)% and (38.63 ± 2.25)% respectively.It demonstrated that the apoptosis rate was increased in the experimental groups as compared to the control group under the two culture conditions(the normal mode and the hypoxiahypoglycemia mode)(t=7.250,5.259,12.185,8.140,15.000,47.576,respectively,all P<0.05,0.01 or 0.001).The combination group had the highest number of apoptosis cells,and the number of apoptosis cells was higher in hypoxia-hypoglycemia group than in normal culture group.Real-time PCR analysis showed that,compared with the control group,the expressions of bcl-2 and birc-5 mRNA were decreased and the expression of bax mRNA had no significant changes in experimental group under the normal culture conditions.The expressions of bcl-2 and birc-5 mRNA were significantly decreased and the expression of bax mRNA was increased in experimental group as compared with the control group under the hypoxia-hypoglycemia conditions.Under the hypoxiahypoglycemia environment,the expression of bcl-2 mRNA was significantly increased,the expressions of bax and birc-5 mRNA was significantly reduced,and the ratio of bcl-2 and bax was significantly increased as compared with the normal culture conditions.Conclusions 6 gingerol may decrease the inhibitory effect of survivin protein on tumor cells apoptosis by reduced the expression of birc-5,which generates the cell-killing and sensitizing effect on HepG-2 cell in chemotherapy.This performance is more obvious in the hypoxia-hypoglycemia environment.
5.Effect of alteplase intra- arterial thrombolysis combined with continuous monitoring of transcranial Doppler in patients with acute cerebral infarction

Objective To observe the effect of alteplase intra-arterial thrombolysis combined with continuous monitoring of transcranial Doppler (TCD) in patients with acute cerebral infarction. Methods Fifty-four patients with acute cerebral infarction were divided into TCD group and control group by random digits table method. The patients of 2 groups were given alteplase intra-arterial thrombolysis, and the patients of TCD groups were combined with continuous monitoring of TCD. The degree of clinical neurologic impairment was evaluated by National Institutes of Health stroke scale (NIHSS) and Barthel index (BI). The condition of vascular recanalization was evaluated by the change of blood flow signal of TCD. CT was performed within 1 d after thrombolysis to detect intracranial hemorrhage and other adverse reaction. The degree of clinical neurologic impairment, blood vessel recanalization rate after thrombolysis and recanalization time and incidence of intracranial hemorrhage and other adverse reaction were compared between 2 groups. Results There were no statistical difference in NIHSS score before thrombolysis and 1 h after thrombolysis between 2 groups (P>0.05). The NIHSS scores 7 and 30 d after thrombolysis in TCD group were significantly lower than those in control group:(8.1 ± 4.9) scores vs. (12.1 ± 4.2) scores and (6.9 ± 3.1) scores vs. (10.9 ± 3.9) scores, there were statistical differences (P<0.05). The BI scores 7 and 30 d after thrombolysis in TCD group were significantly higher than those in control group: (78.5 ± 13.8) scores vs. (60.8 ± 12.4) scores and (82.6 ± 13.5) scores vs. (63.2 ± 12.9) scores, and there were statistical differences (P<0.05). The blood vessel recanalization rate 24 h after thrombolysis in TCD group was significantly higher than that in control group: 70.4% (19/27) vs. 44.4% (12/27), the recanalization time after thrombolysis was significantly shorter than that in control group: (36.4 ± 9.5) min vs. (58.5 ± 12.4) min, and there were statistical differences (P<0.05). No adverse reaction like intracranial hemorrhage and so on was found by CT detection within 1 d after thrombolysis in the 2 groups. Conclusions Alteplase intra-arterial thrombolysis combined with continuous monitoring of TCD in acute cerebral infarction has good curative effect, and no obvious adverse reaction.
6.A Retrospective Analysis of Distribution and Antibiotic Resistance Changes of Nonfermenters in Lower Respiratory Tract Infection

OBJECTIVE To investigate the distribution and antibiotic resistance of nonfermenting gram-negative bacilli isolated from lower respiratory tract infection patients from 2003 to 2007 for rational antimicrobial therapy in practice.METHODS The data of distribution and antibiotic resistance of nonfermenting gram-negative bacilli isolated from sputum samples of lower respiratory tract infection in our hospital from 2003 to 2007 were retrospectively analysed.RESULTS The most predominant nonfermenting gram-negative bacilli isolates were Pseudomonas aeruginosa,Acinetobacter baumannii,Acinetobacter lwoffii and Stenotrophomonas maltophilia.From 2003 to 2007,the resistance rates of these isolates to 15 antibiotics increased,especially P.aeruginosa to ceftazidime.The resistance changed from 30.2% in 2003 to 55.4% in 2005,54.8% in 2006,and reduced to 41.5% in 2007.The resistance to gentamicin,imipenem,netilmicin,ticarcillin and tobramycin varied in the similar way.The resistance of A.baumannii to imipenem increased from 21.1% to 59.7% within the five years.The resistance rate of A.lwoffi was still relatively low.S.maltophilis isolates were highly multi-resistant to antimicrobial agents tested with a rate ranging from 61.9%-100.0% except ceftazidime,ticarcillin and trimethoprim-sulfamethoxazole.CONCLUSIONS P.aeruginosa,A.baumanii and S.maltophilia are the main nonfermentive pathogens of lower respiratory tract infections and not susceptive to many antibiotics.The tendency of susceptibility and distribution changes of those bacteria should be considered in practice for proper antimicrobial therapy.
7.Brachial-ankle pulse wave velocity correlates to urinaryprotein in patients with type 2 diabetes mellitus

Objective Urinary protein is closely related to the development and progression of major vascular lesionsin diabetic patients.This study was to investigate the correlation between brachial-ankle pulse wave velocity(baPWV) and urinary protein in patients with type 2 diabetes mellitus ( T2DM) . Methods According to the level of 24-hour urinary albumin ( UAL) , 142 T2DM patients were divided into three groups:control (UA<30 mg, n=84), microalbuminuria(UAL<300 mg, n=26), and clinical albuminuria group ( UAL≥300 mg, n=33) .All the patients underwent examination offasting blood glucose, blood lipid, renal function, HbA1c, islet func-tion, C-reactive protein, blood pressure, and BMI.Their baPWV was determined with an Omron atherosclerosis detector and the correla-tion of baPWV with other factors evaluated by Pearson analysis and multivariate linear regression analysis. Results The baPWVwas significantly higher in themicroalbuminuria([1649.19±229.36]cm/s) and clinical albuminuria ([1759.21±291.05]cm/s) than in the control group ([1522.33±248.37]cm/s) (P<0.05).Pearson analysis showed a positive correlation between 24-hour UAL and baPWV ( r=0.347, P<0.01) and multivariate linear regression analysis manifested the same correlation ( P<0.05) . Conclusion The level of urinary protein is closely correlated with baPWV in T2DM patients.
8.Clinical study on the treatment of 29 cases of HIV/AIDS with the traditional Chinese medicine combined with HAART

Objective To explore a reasonable therapeutic plan for treating HIV/AIDS with traditional Chinese medicine plus western medicine. Methods Twenty-nine patients with HIV/AIDS were selected and recruited into a HIV group and a AIDS group according to the clinical diagnosis. Each group was further divided into comparison stage and treatment stage. In HIV group, observations and statistics of observation indexes were performed during the comparison stage; while TCM treatment was applied in the treatment stage. In AIDS group, HAART was applied in the comparison stage, and HAART combined with TCM therapy were used in the treatment stage. Both comparison stage and the treatment stage lasted 12 months. Results In HIV group, self control results showed that immune and virus indexes did not have significant changes, but also with no side or toxic effects, In the AIDS group, contrasting between the comparison stage and the treatment stage showed that there were significant improvement of symptoms (χ2=4.9231, 2.5000, P<0.05) , anti-toxic effects (χ2=9.333、 16.4091、10.2273, P<0.01) and immune indexed(t=3.1990,P<0.01) after treated additionally with traditional Chinese medicine. Conclusion Traditional Chinese medicine could improve the immunity function and clinical symptoms, reduce side effect of HAART medicine and stabilize CD4+ T cells of patient with HIV/AIDS.
9. Analysis of physique types and their causes of modern Chinese population by head-to-face measurements

Objective: To review the physical characteristics in modern Chinese populations, and to exploring the relationships between physical traits and the geo-environmental factors. Methods: This paper took the principal component and cluster analysis of the craniofacial physical characters of 134 populations in China (65 Han nationalities, 59 minority nationalities and 10 unidentified populations) with 15 measurements. We also focused the linear relationship between the craniofacial measurements and 8 geo-environmental factors. Results: The physical characteristics of Modern Chinese was divided into the northern and southern groups: North Han was close to north minority nationalities; Han nationalities were close to each other in southern region, as well as the minority nationalities. In addition, the physiognomic ear length and physiognomic ear breadth decreased when the annual mean temperature increases, which was not consistent with Allen's rule. Conclusion: The differentiation of northern and southern Chinese population is a result of the physical adaptation which may be caused by the selective pressures under different regional environments. And the north-south demarcation zone (Qinling mountain - Huaihe river) can be considered as the most suitable boundary between the southern and northern Chinese.
10.Analysis of 1 731 Cases of ADR in a Hospital

OBJECTIVE:To investigate the regularity and characteristics of adverse drug reaction (ADR) in our hospital,to reduce the incidence of ADR,and to provide reference for clinical rational drug use. METHODS:1 731 ADR cases reported by our hospital during Jan. 2002 to Jul. 2015 to national ADR monitoring center through the network system were selected and analyzed statistically in respects of gender,age,related drugs,route of administration,causal relationship evaluation,reporting personnel status,ADR results and drug dosage form organs or systems involved in ADR and manifestation. RESULTS:There were a total of 1 731 ADR patients,among which 640 cases were male,and 1 091 cases were female;patients aged 41-60,≥61 were 676,568 cases;there were 86 cases of severe ADR and 1 645 cases of general ADR,249 cases of new ADR,include 19 cases of severe ADR;causal relationship evaluation of ADR was“impossible”(572 cases) and“very likely”(859 cases) as the vast majority of staff reporting;the most of reporters were doctors (1 290 cases,74.52%),followed by pharmacists (323 cases, 18.66%) and nurses (118 cases, 6.82%);ADR of most patients were improved and recovered. There were 16 routes of administration in ADR cases,among which intravenous infusion and oral administration were the main route of administration, accounting for 92.95%;ADR reports involved 32 kinds of dosage form,which mainly were injection,tablets and capsules, accounting for 86.76%. CONCLUSIONS:Great importance should be attached to ADR monitoring and reporting. We also should reduce the use of intravenous drugs,pay attention to the safety of drug use in elderly patients,promote clinical rational drug use, and ensure the safety of patients.
