8.A Case of Prurigo Nodularis Treated with Narrow-band UVB and Naltrexone.

Prurigo nodularis is a distressing, chronic disease, which is considered to be very resistant to therapy. Treatment choice is quite limited. Among them, opiate antagonist and phototherapy have been used with satisfactory results. We report a case of treatment-resistant prurigo nodularis. Narrow band-UVB phototherapy was performed twice weekly for 3 months. Naltrexone 50mg was also taken daily before sleep. Pruritus was midly reduced after 1 week, and had almost completely disappeared after 3 months of treatment.

Prurigo pigmentosa(PP) is a rare dermatosis with an unknown cause characterized by intensely pruritic papules that resolve with reticulate pigmentation. It is a disease entitity commonly reported in Japan but seemingly rare in other parts of the world. We report 5 cases that clinically and histopathologically fulfilled the characterisitics of PP and were treated with dapsone resulting in significant clinical improvement.

A 65-year-old man presented with simple neurogenic claudication for 3 yea rs. He chronically used steroid (betamethasone) medication for about 6 yea rs due to prurigo simplex. Imaging studies(myelography, CT, and MRI ) showed circumferential constriction of the central thecal sac at L5 by excessive epidural fat. After decompressive laminectomy and debulking of fat tissue, the symptom was subsided. The authors reviewed the literature relevant to spinal epidural lipomatosis.
