2.Term, live, primary ovarian pregnancy: A case report

Primary ovarian pregnancy accounts for less than 1% of all cases of ectopic pregnancies. Its diagnosis is most commonly made intra-operatively and the approach in its management has been geared towards conservative measures such as oophorectomy and resection. Use of intrauterine device still remains the most established risk factor for the development of ovarian pregnancy. The diagnosis is established following the criteria first described by Spiegelberg in 1878. Several cases of ovarian gestation have been described in literature, although very few cases with live term fetus have been reported. Majority of the cases were diagnosed intra-operatively, and the management was tailored depending on the complexity of each of the different cases. This paper reports a case of primary ovarian pregnancy with a live term fetus, which was only diagnosed intra-operatively. Total hysterectomy with right salpingo-oophorectomy was performed due to difficulties encountered brought about by dense adhesions.

3.Ovarian adenocarcinoma in twin pregnancy

This is the first time a case of ovarian adenocarcinoma was diagnosed in a twin pregnant woman at the Department of Obstetrics and Gynecology of HuÕ Central Hospital. The incidence of ovarian cancer during pregnancy is approximately 1 to 25000 pregnancies. About 70 cases of ovarian adenocarcinoma during pregnancy have been reported. to our knowledge, this is the first report of poorly differentiated papillary serious adenocarcinoma in twin pregnancy. Overall, the ovarian adenocarcinoma has poor prognosis. It is necessary to enhance the primary health care system to detect the disease at the earlier stage, obtain better treatment result and improve the prognosis. The maternal prognosis depends solely on the extension of the tumor. The fetal prognosis is poor during the first six months of pregnancy, especially if the tumor is advanced of progressive. The wishes of the parents and their family should be taken into account when making therapeutic decision for these cases.

Ovarian fibromas originating primarily from the ovarian stroma are rare, accounting for approximately 4% of all ovarian neoplasm. The ovarian fibromas are mostly benign tumors, but infrequently they have histologically malignant appearances such as hypercellularity, high mitotic rate, marked nuclear pleomorphism. Malignant fibromatous tumor can be categorized on the basis of prognosis into two separate types of tumors, one of which is cellular fibroma (one to three miotic counts per 10 high power fields) and the other of which is fibrosarcoma (over four miotic counts per 10 high power fields) that has extremely poor prognosis. We report a case of large ovarian fibrosarcoma accompanied with pregnancy and have a brief review of literatures.

The ovarian hyperstimulation syndrome (OHSS) is one of the notorious complications of assisted reproduction procedure. Severe and moderate OHSS associated with a spontaneous pregnancy is rare. We experienced a case of two normal, spontaneously conceived pregnancies with OHSS and her past obstetric history included prior spontaneous OHSS in the first trimester and term vaginal delivery of normal baby.

8.Effect of electroacupuncture on endometrial receptivity and IVF-ET pregnancy outcomes in patients with diminished ovarian reserve.

OBJECTIVE:
To explore the effect of electroacupuncture on endometrial receptivity and the pregnancy outcomes of in vitro fertilization and embryo transfer (IVF-ET) in the patients with diminished ovarian reserve (DOR).
METHODS:
Sixty-eight patients of DOR undertaken IVF-ET were randomized into an observation group (34 cases, 2 cases dropped off) and a control group (34 cases, 1 case dropped off). In the control group, endometrial preparation was performed according to the routine protocol. In the observation group, on the base of the treatment as the control group, acupuncture was applied to Geshu (BL 17), Shenshu (BL 23), Mingmen (GV 4), Shiqizhui (EX-B 8), Ciliao (BL 32), Zhongliao (BL 33), Tianshu (ST 25), Qihai (CV 6) and Guanyuan (CV 4), etc. Electric stimulation was given at Ciliao (BL 32)-Zhongliao(BL 33), Tianshu (ST 25)-Zigong (EX-CA 1), with disperse-dense wave, 2 Hz/15 Hz in frequency and tolerable current in intensity. Electroacupuncture was given once every two days, 3 times weekly, lasing 3 menstrual cycles till 1 day before embryo transfer. The endometrial thickness and morphology were observed on the day of human chorionic gonadotropin (HCG) of egg retrieval cycle, the day of endometrial transformation in frozen-thawed embryo transfer (FET) cycle and the day of embryo transfer in both groups successively; as well as HCG positive rate, clinical pregnancy rate, embryo implantation rate and live birth rate.
RESULTS:
In the observation group, the proportion of type A endometrium on the embryo transfer day was higher than those on HCG day of the egg retrieval cycle and the endometrial transformation day of FET cycle (P<0.05), and also higher than that of the control group (P<0.01). In the observation group, HCG positive rate, clinical pregnancy rate, embryo implantation rate and live birth rate were 75.0% (24/32), 71.9% (23/32), 47.4% (27/57) and 56.3% (18/32) respectively, and all higher than 36.4% (12/33), 30.3% (10/33), 18.0% (11/61) and 15.2% (5/33) in the control group separately (P<0.01).
CONCLUSION
Electroacupuncture improves the endometrial receptivity and IVF-ET pregnancy outcomes in the patients of diminished ovarian reserve.

9.Successful delivery in a women with natural pregnancy and severe ovarian hyperstimulation syndrome: a case report and literature review.

Spontaneous severe ovarian hyperstimulation syndrome-complicated natural pregnancy (SOHSSp) is a rare clinical entity and can be easily misdiagnosed as ovarian tumors. We report a case of SOHSSp in a 26-year-old women with successful delivery, and reviews the clinical data and the diagnosis and treatment of this case. The patient underwent a diagnostic laparoscopy and was followed-up for 1 year with a favorable prognosis.

10.Ovarian Mature Cystic Teratoma Containing Homunculus: A Case Report.

We report a partial fetus-like structure (homunculus) in benign ovarian mature cystic teratoma removed from a 23-yr-old female. The cyst displayed various tissues derived from the three germ layers. The homunculus had a distinguished fetal cranial structure with one upper extremity. A partially developed osteocartilagenous skeleton was identified in the cranial structure of the homunculus. Intracranial structures such as cerebral cortex, retinal pigment, and leptomeninges were extruded from the partially disrupted calvarium.
