1.A Case of Abdominal Pregnancy Developed after the Previous Unilateral Adnexectomy.

Abdominal pregnancy is one of the rare but extremely harzardous complications of childbearing. The diagnosis of the abdominal pregnancy is rarely made before surgery and the management of it also has many difficult problems. I experienced a case of abdominal pregnancy developed after the previous unilateral adnexectomy and it was a suspicious primary abdominal pregnancy as though did not fulfilled the Studdiford's the following criteria for primary abdominal pregnancy. 1) Normal tubes and ovaries with no evidence of recent or remote injury, 2) absence of any evidence of uteroplacental fistula, and 3) presence of a pregnancy related exclusively to the peritoneal surface and young enough to eliminate the possibility of secondary implantation following primary nidation in the tube. And now I present a case with brief review of literatures.

Abdominal pregnancy is a rare entity, which has been classified as primary or secondary by Studifford criteria. The clinical characteristics of abdominal pregnancy is extremely variable, so early diagnosis is very difficult. Abdominal pregnancy is a rare form of ectopic pregnancy, which has been classified as early and advanced abdominal pregnancies. In case of early abdominal pregnancy, we may miss the diagnosis because the symptoms and clinical characteristics are non-specific. However, recently, the early diagnosis is made because of the development of quantitative beta-hCG measurement, transvaginal ultrasonography and the development of laparoscopy. Omental pregnancy is a very rare form of abdominal pregnancy, which is hard to detect early. When ruptured, it accompanies with massive hemorrhage. So, early diagnosis and treatment is essential to prevent high morbidity, mortality and serious complications. Definite diagnosis is made by cytopathologic examination of tissue specimen after surgery. We have experienced a case of ruptured early omental pregnancy and reviewed it briefly.

The diagnosis of omental pregnancy is hardly ever made prior to laparotomy. Small bowel obstruction in pregnancy is a rare critical condition with high morbidity and mortality. The diagnosis of small bowel obstruction in pregnancy is often delayed because the symptoms such as nausea, vomiting, and abdominal pain mimic typical pregnancy-associated complaints. So, a high level of suspicion especially in patients with a history of previous abdominal surgery is essential for early diagnosis. Once the diagnosis is made, surgery should be performed immediately. We present the first case of small bowel obstruction during pregnancy in Korea with a brief review of the literature.

Abdominal pregnancy is very rare and potentially life-threatening variation of ectopic pregnancy. Early diagnosis is very difficult. In the last several decades prenatal care has improved and noninvasive procedures such as ultrasound scanning and fetal cardiotocography have led to us a better assessment of fetal and maternal disorders. In the last 30 years an increase of diagnosed ectopic pregnancies has been noted and attributed to the greater frequency of pelvic inflammatory disease. Although the incidence of ectopic pregnancy is rising, the incidence of abdominal pregnancy, which is primarily a sequel of a missed ruptured ectopic pregnancy, remains low. We presented a case of secondary abdominal pregnancy in 14 wks with its review of literature.

Abdominal pregnancy is a rare form of extrauterine pregnancy which is account for 1.4% of all ectopic pregnancy cases. Among these omental pregnancy is extremely rare and may cause life-threatening massive hemorrhage as a complication. But the diagnosis of early abdominal pregnancy is difficult because of non specific findings about clinical history, physical examination and ultrasonography. Risk factors related to ectopic pregnancy include inflammation, infection and surgery, but endometriosis is an uncommon factor. We present a case of the ruptured primary omental pregnancy associated with endometriosis with a review of the literature.

Abdomianl pregnancy is rare, occurring 9.2 cases per 1,000 ectopic pregnancies and encountered in 1 in 10,000-25,000 births. Omental pregnancy is extremely rare form of abdominal pregnancy that may cause life-threatening massive hemorrhage in case of rupture. The clinical characteristics is variable and physical examination by itself may be insufficient for diagnosis. Early and accurate diagnosis is necessary to decrease fetal and maternal mortality and morbidity, serious complications. We report a case of ruptured early omental pregnancy with a review of literature.

An ectopic pregnancy in the abdominal organs is very rare. Primary intestinal pregnancy is considered the rarest form of extrauterine pregnancy, and only a few well-documented cases have been reported. Herein, a case of an abdominal pregnancy in a 25-year-old woman, with massive lower gastrointestinal bleeding, is reported. The source of bleeding could not be identified, despite gastroscopy, ultrasonography and angiography investigations. A diagnostic laparotomy disclosed an abdominal pregnancy, causing an erosion of the jejunal wall at the site of the pregnancy, with massive lower gastrointestinal bleeding. An abdominal pregnancy is seldom included in the differential diagnosis of lower gastrointestinal bleeding. The possibility of intestinal erosion in an abdominal pregnancy should be borne in mind in cases of lower gastrointestinal bleeding.

Diaphragmatic hernias presenting during pregnancy or in the puerperium are very rare. The diagnosis is often delayed because the presenting symptoms of nausea, vomiting and mild abdominal pain are commonly seen in the normal antenatal or postpartum women. The most common etiology of diaphragmatic hernia in pregnancy is trauma and spontaneous diaphragmatic hernia is rare. We report a case of maternal diaphragmatic hernia in pregnancy with a brief review of literatures.
