2.New developments for endoscopic management of Barrett's esophagus with high grade dysplasia.

Barrett's esophagus is now clearly recognized as a preneoplasic condition. Progression of metaplasia through dysplasia to adenocarcinoma is a widely accepted theory for esophageal carcinogenesis. That high grade dysplasia is frequently found in association with esophageal adenocarcinoma. Long-term endoscopic surveillance of high grade dysplasia in Barrett's esophagus facilitates detection and treatment of esophageal cancers in the early stage.

6.Effects of He-Ne laser on gastric mucosa in rat with chronic atrophic gastritis.

This study sought to explore the effects of He-Ne laser irradiation on the gastric mucosa of experimental chronic atrophic gastritis (CAG) in rat. Fifty-two male adult Wistar rats were randomly divided into five groups including normal control group, model group and three different doses He-Ne laser groups. The CAG model in rats was made by administering (per gastrogavage) a compositus liquor, including 2% sodium salicylate and 30% alcohol, for 8 weeks to stimulate the rat's gastric mucosa, combined with irregular fasting and compulsive sporting as pathogenic factors. He-Ne laser therapy was used at different doses of He-Ne laser, once a day for twenty days, then the morphological changes and the expression of cyclinD1 were observed. Compared with untreated group, the gastric mucosa of 3.36J x cm(-2) He-Ne laser group was significantly thicker (P < 0.01), the inflammatory cells of gastric mucosa were decreased (P < 0.05), the morphology, structure and volume of the cells were restored or nearly normal and the expressions of cyclinD1 were higher (P < 0.05). In a word, small dose He-Ne laser (3.36 J x cm(-2)) has a good adjuvant therapeutic effect on rat's CAG.

8.Controversies in borderline ovarian tumors.

Borderline ovarian tumors (BOTs) represent about 15% to 20% of all ovarian malignancies and differ from invasive ovarian cancers (IOCs) by many characters. Historically, standard management of BOT is peritoneal washing cytology, hysterectomy, bilateral salpingo-oophorectomy, omentectomy, complete peritoneal resection of macroscopic lesions; in case of mucinous BOTs, appendectomy should be performed. Because BOTs are often diagnosed at earlier stage, in younger age women and have better prognosis, higher survival rate than IOCs, fertility-sparing surgery is one of the option to preserve childbearing capacity. The study of such conservative surgery is being released, and still controversial. After surgery, pregnancy and ovarian induction followed by in vitro fertilization are also significant issues. In surgery, laparoscopic technique can be used by a gynecologic oncology surgeon. So far postoperative chemotherapy, radiotherapy and hormone therapy are not recommended. We will discuss controversial issues of BOTs on this review and present the outline of the management of BOTs.

9.Effect of Helicobacter pylori Eradication on Subsequent Dysplasia Development after Endoscopic Resection of Gastric Dysplasia.

BACKGROUND/AIMS: Eradication of Helicobacter pylori reduces the incidence of gastric cancer, and may inhibit gastric dysplasia progression into gastric cancer. The aim of this study was to investigate the effect of eradication of Helicobacter on the incidence of subsequent gastric dysplasia development after endoscopic resection. METHODS: Medical records of patients who underwent endoscopic resection for gastric dysplasia were retrospectively reviewed. Presence of H. pylori was assessed by the Campylobacter-like organism test and histology. The rate of subsequent dysplasia development after endoscopic resection between the eradication group and non-eradication group was compared. RESULTS: Total of 129 patients positive for H. pylori infection were included for analysis. Of these, 85 patients received successful eradication therapy and 44 patients did not receive eradication therapy or failed to achieve successful eradication. Sex, mean age and pathologic grade of dysplasia did not differ between the two groups. In univariate analysis, the grade of intestinal metaplasia (p=0.013) significantly differed between metachronous dysplasia group and non-metachrounous dysplasia group. In multivariate analysis, eradication of H. pylori (p=0.014) was related to reduced incidence of subsequent gastric dysplasia development after endoscopic resection. CONCLUSIONS: Eradication of H. pylori likely has a beneficial effect in preventing the development of subsequent gastric dysplasia, a premalignant lesion of gastric cancer, after endoscopic resection.

10.Clinical study on effects of kangwei granule on precancerous lesion in patients with chronic atrophic gastritis.

UNLABELLED(GPL) in patients suffered from chronic atrophic gastritis (CAG) differentiated as Pi-deficiency with damp-heat retention and blood stasis in TCM Syndrome differentiation.
METHODSSixty-eight patients fitting to the admission criteria were randomly divided into two groups, 36 patients were treated with KWG in the treated group and 32 were treated with Weifuchun in the control group, all were treated for 2 treatment courses (12 weeks as one course).
RESULTSThe curative effects on gastroscopy and pathologic changes in the treated group were significantly superior to those in the control group (P < 0.05). The comparison of clinical efficacy, symptom improvement, anti-Helicobactor pylori effect between the two groups was insignificantly different (P > 0.05).
CONCLUSIONKWG is an effective drug for GPL.
