1.Relationship between serum and placental interleukin-18 levels and preeclampsia.

OBJECTIVETo investigate the relationship of interleukin (IL)-18 concentrations in serum and placenta and preeclampsia.
METHODSTwenty-seven patients with preeclampsia and 28 women with normal pregnancy were recruited. Blood and placenta samples were taken and serum and placental IL-18 concentrations were determined by enzyme-linked immunosorbent assay (ELISA). The levels of IL-18 in serum and placenta were compared.
RESULT(1) The median of serum IL-18 in patients with preeclampsia was 704 ng/L, which was significantly higher than that of control group (470 ng/L, P<0.05). (2) The median of placental IL-18 in patients with preeclampsia was 105 ng/L, which was also significantly higher than that of control group (31.5 ng/L, P<0.005).
CONCLUSIONIncreased serum and placental levels of IL-18 were presented in preeclampsia as compared to normal pregnancy, which might offer insight into the pathogenesis of the disease.

2.The Relationship between Aldosterone to Renin Ratio and RI Value of the Uterine Artery in the Preeclamptic Patient vs. Normal Pregnancy.

PURPOSE: Plasma levels of renin, angiotensin II and aldosterone are increased during normal pregnancy. However, these values in preeclampsia are decreased to nearly that of a nonpregnant subject, and vascular sensitivity to angiotensin II is increased. In preeclampsia, aldosterone is decreased less than rennin. Therefore current studies were undertaken to determine the relationship between aldosterone to renin ratio (ARR) and uterine artery perfusion via RI value. MATERIALS AND METHODS: In this study, the relationship between plasma aldosterone and renin concentration was determined in 27 preeclamptic women and 50 normal pregnant women, whose gestational weeks were matched. The aldosterone to renin ratio was calculated and compared between the two groups. Doppler velocimetry of the uterine artery, which was used to calculate resistance index (RI), was performed on all subjects. The relationship between ARR and RI value was reviewed. RESULTS: In the preeclampsia group, RI value of the uterine artery was significantly higher than that of normal pregnant women. Both plasma renin and aldosterone concentrations were lower in the preeclampsia group. However, the ratio of these two parameters was significantly higher (38.3 vs. 16.1, p < 0.001); the greater ARR, the higher the RI of the uterine artery (r(2)=0.053, p=0.048). CONCLUSION: This study demonstrates that a high aldosterone to renin ratio may have a negative effect on perfusion of the uterine artery and play an important role in the pathophysiology of preeclampsia.

3.Expression of Fas and FasL in serum and placenta of preeclamptic pregnancy and its significance.

OBJECTIVETo observe the serum and placental levels of FAS and FASL in preeclampsia (PE) and to study its relationship with the disease.
METHODSForty women with preeclampsia and 39 healthy pregnant women were recruited and samples of serum and placentas were collected. The expression of Fas and FasL in placentas was detected with Western blot and the concentration of soluble Fas and FasL in serum was detected with ELISA method.
RESULTSerum levels of soluble Fas in PE group were significantly higher than those of healthy pregnant women (2.11+/-0.95 mg/L compared with 1.57+/-0.60 mg/L, P<0.05), and serum levels of soluble FasL in PE group were also significantly higher than those in controls (4.43+/-1.90 g/L compared with 3.48+/-1.53 g/L, P<0.01). There were no significant differences in Fas and FasL levels in placentas between PE group and healthy pregnant women (P>0.05 for both).
CONCLUSIONThe elevated serum Fas and FasL levels are closely associated with preeclampsia, which may play an important role in the pathogenesis of the disease.

4.Imbalance of serum T helper 1- and 2-type cytokines in preeclampsia and gestational hypertension.

OBJECTIVETo investigate the changes in serum levels of Th1- (IL-2 and TNF-alpha) and Th2-type cytokines (IL-10) and the ratios of Th1/Th2 (IL-2/IL-10 and TNF-alpha/IL-10) in preeclampsia and in gestational hypertension.
METHODSLevels of IL-2, IL-10 and TNF-alpha were determined with radioimmunoassay in serum samples from 22 women with preeclampsia, 15 women with gestational hypertension and 32 normal term pregnant women. The Th1/Th2 ratios were calculated accordingly.
RESULTThere were no significant differences in serum levels of IL-2, IL-10 and TNF-alpha (P>0.05 for all) among normal pregnancy, gestational hypertension and preeclampsia. The ratio of serum IL-2/IL-10 was significantly higher in preeclampsia than that in controls (P < 0.05), and the ratio of TNF-alpha/IL-10 significantly higher in patients with preeclampsia than that in either controls or gestational hypertension (P<0.025 for both).
CONCLUSIONAlterations of serum cytokine balance with predominance of Th1 immunity were observed in preeclampsia. These associations may offer insight into the pathogenesis of preeclampsia.

5.Predictive value of placenta-derived RASSF1A sequence expression in maternal plasma for pre-eclampsia.

OBJECTIVETo investigate the expression of placenta-derived RASSF1A gene in maternal plasma during first and second trimesters, and to explore its value for the prediction of pre-eclampsia.
METHODSFor 325 pregnant women of the first trimester, free DNA of plasma samples was extracted at 7-12, 13-18, and 19-24 gestational weeks, respectively. Methylation-sensitive restriction enzyme digestion followed by fluorescence quantitative PCR (MSRE+ PCR) was employed for analyzing the concentrations of hypermethylated RASSF1A gene. Blood pressure, proteinuria and clinical feature were monitored at the same time. Those who had subsequently developed pre-eclampsia were selected as the pre-eclamptic group, 30 normal pregnant women were selected as the control group. Hypermethylated RASSF1A gene in maternal plasma was retrospectively analyzed. The relationship between clinical classification, type of pre-eclampsia and concentrations of the gene were further analyzed.
RESULTSTwenty-six out of the 325 pregnant women developed pre-eclampsia as their only complication. At 13-18 gestational weeks, the mean concentrations of fetus-specific RASSF1A sequences were 141.62 copies/mL in maternal plasma of pre-eclamptic pregnancies, which was significantly greater than that of the controls (98.90 copies/mL). Fetus-derived RASSF1A levels were 2.03 fold higher in pre-eclamptic subjects than controls at 19-24 gestational weeks. There was a significant difference in the level of hypermethylated RASSF1A gene between the mild and severe pre-clamptic subjects at 13-24 gestational weeks (P< 0.05). The concentrations of the sequences were significantly higher in early-onset severe pre-eclampsia than late-onset severe pre-eclampsia at 19-24 gestational weeks (P< 0.05).
CONCLUSIONAltered expression of hypermethylated RASSF1A gene may be detected in maternal plasma during second trimester, which has important significance for early prediction of pre-eclampsia.

6.Alteration of heat shock protein 20 expression in preeclamptic patients and its effect in vascular and coagulation function.

Preeclampsia (PE) is a pregnancy-specific, multi-system disorder and the leading cause of maternal and perinatal morbidity and mortality in obstetrics worldwide. Excessive vasoconstriction and dysregulated coagulation function are closely associated with PE. Heat shock protein 20 (HSP20) is ubiquitously expressed under normal physiological conditions and has important roles in vascular dilatation and suppression of platelet aggregation. However, the role of HSP20 in the pathogenesis of PE remains unclear. In this study, we collected chorionic plate resistance arteries (CPAs) and serum from 118 healthy pregnant women and 80 women with PE and detected the levels of HSP20 and its phosphorylated form. Both HSP20 and phosphorylated HSP20 were downregulated in CPAs from women with PE. Comparison of the vasodilative ability of CPAs from the two groups showed impaired relaxation responses to acetyl choline in preeclamptic vessels. In addition to the reduced HSP20 in serum from women with PE, the platelet distribution width and mean platelet volume were also decreased, and the activated partial thromboplastin time and thromboplastin time were elevated.With regard to the vital roles of HSP20 in mediating vasorelaxation and coagulation function, the decreased HSP20 might contribute to the pathogenesis of PE.

7.Quantitative detection of the hypermethylated RASSF1A gene in maternal plasma of pre-eclampsia.

OBJECTIVETo investigate the level of hypermethylated ras association domain family 1A (RASSF1A) gene in maternal plasma of pre-eclampsia and its clinical value.
METHODSSixty pre-eclampsia women including 30 mild and 30 severe cases were selected, 60 women with normal pregnancy were studied as control. Free DNA from plasma samples was extracted, fluorescence quantitative polymerase chain reaction (FQ-PCR) was used to detect the concentrations of RASSF1A gene before and after methylation-sensitive restriction digestion. Meanwhile, beta-actin gene was detected as a control to confirm complete enzyme digestion.
RESULTSThe median concentration of hypermethylated RASSF1A gene was 3.31-fold higher in samples from pre-eclamptic pregnancies than that in controls. There was significant difference between the mild and severe pre-eclamptic subjects (P<0.05), with the median concentrations of 1659 copies/mL and 2036.50 copies/mL, respectively.
CONCLUSIONHypermethylated RASSF1A gene in pre-eclampsia plasma was significantly increased and the concentrations were related to the severity of pre-eclampsia.

8.Expression of aquaporin-4 protein in the brain of preeclampsia model rats.

OBJECTIVETo study the expression of aquaporin-4 (AQP4) protein in preeclampsia(PE) model rats.
METHODSAdult SD rats were divided into PE model group (n=7) induced by endotoxin, normal pregnancy group (n=6) and non-pregnancy group (n=12) treated with an equal volume of saline. AQP4 protein expression in the brain of the rats was detected with immunohistochemical staining and Western blotting.
RESULTSThe blood pressure in PE model rats (135∓9 mmHg) was significantly higher than that in normal pregnancy rats (116∓8 mmHg) and non-pregnancy rats (112∓6 mmHg) (P<0.02). The rats in PE model group showed obvious proteinuria compared with the other two groups (3.8∓0.5 vs 2.6∓0.6 and 2.3∓0.4, P<0.05). Immunohistochemistry showed that AQP4 protein was localized primarily around the brain parenchymal blood vessels. Western analysis revealed a significantly elevated AQP4 protein expression in PE model group (39.6∓4.9) compared with that in normal pregnancy group (26.5∓4.3) and non-pregnancy group (9.7∓2.1) (P<0.01).
CONCLUSIONThe up-regulation of AQP4 protein around the intraparenchymal blood vessels of the brain may play a role in the development of eclampsia.

9.The expression of PLF in placenta in pregnancy-induced hypertension and the relationship between the PLF and serum VCAM-1.

To study the expression of placental isoferritin (PLF) in placental tissue of pregnancy-induced hypertension (PIH) and the relationship between the level of expression of PLF and the amount of vascular cell adhesion molecule-1 (VCAM-1) in serum, immunohistochemical technique was used to detect the expression of PLF in placenta tissue in 45 PIH patients (PIH group) and 15 normal pregnant women (normal group). High resolution pathological image analysis system (HPIAS-100) was employed to determine the quantity of PLF. The VCAM-1 in serum was examined by enzyme linked immunoabsorbent assay (ELISA). The results showed that the levels of PLF expressions in moderate and severe PIH patients were significantly lower than that of normal group (P < 0.01). The serum VCAM-1 was significantly decreased in PIH group (1310 +/- 177 rho/ng/ml) than that of normal group (609 +/- 72 rho/ng/ml, P < 0.01). The significant negative correlation existed between the expression of PLF in placental tissue and the serum VACM-1 (r = -0.58, P < 0.01). It was concluded that the level of PLF expression in PIH decreases and is negatively correlated with the amount of serum VCAM-1, indicating that these may be involved in the pathogenesis of PIH.

10.Effect of lipoxin A₄ on IL-1β production of monocytes and its possible mechanism in severe preeclampsia.

This study examined in vitro effect of lipoxin A(4) (LXA(4)) on interleukin-1β (IL-1β) production of monocytes and its possible mechanism in severe preeclampsia (PE). Peripheral venous blood was drawn from 15 patients with severe preeclampsia (PE group) and 20 normal pregnant women (control group) to prepare monocytes which were then treated with LXA(4) at different concentrations of 0, 10, 100 nmol/L respectively. IL-1β level in the supernatant of monocytes was detected by enzyme linked immunoassay. The [Ca(2+)](i) of monocytes was measured by laser scanning confocal microscopy. The results showed that the IL-1β level and the [Ca(2+)](i) of monocytes in the PE group were significantly higher than those in the control group. LXA(4) significantly decreased the generation of IL-1β in a dose-dependent manner in the PE group. After treatment with 100-nmol/L LXA(4), in the PE group, the [Ca(2+)](i) concentration of monocytes was significantly reduced. It was concluded that LXA(4) may inhibit the IL-1β production of monocytes from severe preeclampsia women by inhibiting extracellular calcium influx.
