4.Erythema Nodosum Probably Induced by Kerion Celsi.

A 6-year-old girl visited us with a 4-week history of inflammatory, pustular, tender patches and plaques on the scalp, and a 3-day history of multiple, erythematous, indurated, tender, subcutaneous nodules on both pretibial areas. A swab was taken from the scalp lesion and colonies of Tiichophyton mentagrophytes grew on culture. The histopathological findings of the leg nodule were consistent with erythema nodosum. The patient was treated with oral itraconazole and deflazacort, combined with topical potassium permanganate solution. Erythema nodosum regressed two weeks later and the kerion of the scalp regressed six weeks after starting the treatment, leaving residual scarring alopecia. The patient was diagnosed as erythema nodosum probably induced by kerion celsi, that has not been reported in the Korean literature.

6.Flow injection chemiluminescent detection of acemetacin in KMnO4 - Na2 SO3 system.

AIMTo study the sensitizing effect of acemetacin (ACE) on the weak chemiluminescent (CL) reaction of KMnO4 with sulfite and establish a fast and convenient method for CL detection of ACE.
METHODSUsing the sensitizing effect of ACE on KMnO4-Na2SO3 system and flow injection technique to determine the concentration of ACE.
RESULTSUnder optimal conditions, the CL intensity of 1.0 x 10(-2) mol x L(-1) H3PO4 - 5.0 x 10(-5) mol x L(-1) KMnO4 - 4.0 x 10(-4) mol x L(-1) Na2SO3 was proportional to the concentration of ACE ranging from 1.0 x 10(-7) to 1.0 x 10(-5) mol x L(-1). The detection limit of ACE was 6.9 x 10(-8) mol x L(-1) at 3sigma. Satisfactory results were obtained for determination of ACE at 2.5 x 10(-6) mol x L(-1).
CONCLUSIONThe present method showed good precision, high sensitivity and selectivity and could be used for fast and convenient detection of ACE. It would be of significance to the clinical and pharmacological study of acemetacin.

7.A Case of pustulosis Palmaris et Plantaris.

A case of 42 years old male with pustulosis palmaris et plantaris was reported. The skin lesions were characterized by several vesicles, pustule, scaly crusts with a erythematous infiltrated base on the both palms and lateral side of the fingers. The physical examination showed dental caries on the left second molar teeth. The pus culture on the Sabouraud's media and the blood agar showed no growing organism. The gram stain of the lakes of pustules reveald many WBC but no organism. He was treated with oxytetracycline, DDS and wet dressing with potassium permanganate solution for 2 weeks and favorable effect was noted.

9.Tinea Manus Caused by Trichophyton mentagrophytes var. erinacei Transferred from a Hedgehog.

The hedgehog has recently become a fashionable pet in South Korea, especially among the younger persons. However, hedgehogs have been rarely reported to carry fungus that can cause human dermatomycosis. We report such a case. A 12-year-old boy was bitten by his hedgehog one week prior to presentation; he developed two clearly defined erythematous plaques with some pustules on the fingers. Periodic acid-Schiff stain of the biopsy specimen showed long, septated fungal hyphae in the keratin layer. KOH examination and fungus culture showed Trichophyton(T.) mentagrophytes. The subtype was identified as T. mentagrophytes var. erinacei by sequence analysis of the internal transcribed spacer regions of theribosomal DNA. The patient was treated with oral itraconazole (3.3 mg/kg, twice a day for 4 weeks) and topical ketoconazole cream with potassium permanganate wet dressings twice a day, resulting in complete resolution of the skin lesions.

10.A Case of Unusual Clinical Course in Patient with Hemorrhagic Fever with Renal Syndrome Associated with Secondary Amyloidosis.

Hemorrhagic fever with renal syndrome (HFRS) is a symptom complex of fever, chill, myalgia, hemorrhagic diathesis and acute renal failure. We experienced a case of non-oliguric HFRS associated severe thrombocytopenia. Peripheral blood smear showed large atypical lymphocytosis and hemophagocytic histiocytes on bone marrow aspiration. Hantaan virus antibody titer was 1:5,120. Congo-red stain after potassium permanganate treatment of bone marrow biopsy revealed yellow-green birefringence and immunohistochemistry for AA amyloid antibody showed weak positive staining in the stroma of the bone marrow. Herein, we report a case of unusual HFRS associated with secondary amyloidosis and review the literature.
