4.Clinical features of children with colorectal polyps and the efficacy of endoscopic treatment: an analysis of 1 351 cases.

OBJECTIVES:
To study the clinical features of children with colorectal polyps and the efficacy of endoscopic treatment.
METHODS:
A retrospective analysis was performed on the medical data of 1 351 children with colorectal polyps who were admitted and received colonoscopy and treatment in the past 8 years, including clinical features and the pattern and outcomes of endoscopic treatment.
RESULTS:
Among the 1 351 children, 893 (66.10%) were boys and 981 (72.61%) had an age of 2-<7 years, and hematochezia (1 307, 96.74%) was the most common clinical manifestation. Of all the children, 89.27% (1 206/1 351) had solitary polyps, and 95.77% (1 290/1 347) had juvenile polyps. The polyps were removed by electric cauterization with hot biopsy forceps (6 cases) or high-frequency electrotomy and electrocoagulation after snare ligation (1 345 cases). A total of 1 758 polyps were resected, among which 1 593 (90.61%) were pedunculated and 1 349 (76.73%) had a diameter of <2 cm. Postoperative complications included bleeding in 51 children (3.77%), vomiting in 87 children (6.44%), abdominal pain in 14 children (1.04%), and fever in 39 children (2.89%), while no perforation was observed. The children aged <3 years had the highest incidence rates of postoperative bleeding and fever (P<0.0125), and the children with a polyp diameter of ≥2 cm had significantly higher incidence rates of postoperative bleeding, vomiting, and fever (P<0.05).
CONCLUSIONS
Solitary polyps, pedunculated polyps, and juvenile polyps are common types of pediatric colorectal polyps. Electric cauterization with hot biopsy forceps or high-frequency electrotomy and electrocoagulation after snare ligation can effectively remove colorectal polyps in children, with good efficacy and few complications. Younger age and larger polyp diameter are associated with a higher risk of postoperative bleeding.

7.Synchronous Lesions in Colorectal Cancer.

PURPOSE: The detection and removal of synchronous cancer and polyps in colorectal surgery is important to prevent the future development of metachronous cancer. However, it is occasionally impossible to evaluate the entire colon with colonoscopy preoperatively due to luminal obstruction by tumors. The aim of this study is to evaluate the incidence of synchronous colorectal tumors and to emphasize the importance of their peri-operative detection through vigorous application of colonoscopy. METHODS: Three hundreds two patients underwent a potentially curative resection for colorectal cancer in Ilsan Paik Hospital from January 2000 to March 2005. Colonoscopy was performed preoperatively or intraoperatively. All the synchronous polyps detected by colonoscopy and contained in the surgical specimen were included in the analysis. The data on synchronous cancer and polyps were collected through medical records and colonoscopic databases. The incidence, the risk factors, and the effects of synchronous polyps on surgery were analyzed. RESULTS: A total of 268 polyps were detected in 112 patients (37%) when all types of polyp were included whereas true adenomatous polyps were present in 78 patients (26%) among 302 patients. Synchronous cancers were found in 14 patients (4.6%). The detection of polyps on preoperative colonoscopy changed the surgical strategy in 30 out of the 223 patients (13.5%) who had completed preoperative colonoscopy. Neither the age and the gender of the patient, the location and the stage of the tumor, nor the family history were related with increased risk of synchronous polyps. Seventeen among 46 intraoperative colonoscopy cases had synchronous lesions which were not known on preoperative evaluation. Another bowel resection was needed to remove the lesions in nine of them. CONCLUSIONS: Synchronous colorectal polyps or cancer is frequent, and preoperative detection is important. Intraoperative colonoscopy can provide valid information if a preoperative colonoscopic evaluation is not available.

8.Bilateral antro-choanal polyps in an elderly female.

OBJECTIVE: To report the first case of primary bilateral antro-choanal polyps in the elderly age group.
METHODS:
Design: Case report
Setting: Tertiary Government Hospital
Patient: One
Result: A 60-year-old, non-allergic female with progressive bilateral nasal obstruction was subsequently diagnosed with bilateral antro-choanal polyps. Endoscopic sinus surgery was performed and the patient remained asymptomatic on one year follow-up.
Conclusion: Antro-choanal polyps can occur bilaterally in the elderly age group. To the best of our knowledge, this is the first reported case of primary bilateral antro-choanal polyps in an elderly female.

9.Fibrovascular polyp of the esophagus in infant.

A five month female was referred complaining of intermittent vomiting with protrusion of a sausage-like mass through the oral cavity. Esophageal endoscopy and esophagogram revealed a mass in the upper esophagus, which was diagnosed as a fibrovascular polyp. Under general anesthesia, the mass was grasped through the oral cavity with a forcep and ligated and excised at the base, where a stump arose from the posterior wall of the cervical esophagus. The pathology was confirmed as a fibrovascular polyp, which is a rare benign esophageal lesion occurring mostly in adult males, and has not been reported in infancy.
