The hem-o-lock is useful tools for the laparoscopic surgeon. We herein describe a simplified technique for suturing and knotting technique using hem-o-loc. After enucleation of the myoma, uterine parenchymal defect was closed laparoscopically using a 1-0 vicryl suture on a needle, also prepared with a hem-o-lock. Before suturing, the vicryl is tied up with hem-o-loc on the tail of continuous suture that is start point of suture site. Once the suture is passed, a hem-o-loc is used to cinch it down on the suture site and secure the knot tightly. All sutures can be completed with just one suture, continuously. We experienced no major surgical complication.

Delayed reattachment of the muscle to the sclera may be desirable to correct the angle of deviation developed after the surgery of strabismus. In order to know the possible time for the delayed adjustment, the authors tried Viscoat(R). Interceed(TC7) and polyglactin 910 mesh(Vicryl mesh(R)) as physical barriers for preventing the formation of postoperative adhesions. These materials were placed around between the detached end of the muscle and the sclera in 29 rabbits and adjustment was performed thereafter. Viscoat(R) was effective in delaying the adjustment up to 6 days after surgery. Interceed(TC7) and polyglactin 910 mesh were effective in delaying the adjustment up to 1 week after surgery.

Although the surgical excision of the fat pad has been the mainstay of the management of lower palpebral bulge in cosmetic blepharoplasty, procedures that depend on fat removal may render the patient deficient of fat, with the appearance of a hollow orbit. The concept of fat conservation during lower blepharoplasty has been developed by several authors in recent years. Attenuated orbital septum was released along the arcus marginalis and transposed to cover the bony framework. The orbital septum meticulously sutured to the pre-periosteal soft tissue with 5-0 polyglactin. This technique of septal draping over the inferior orbital rim was performed in 30 consecutive individuals (60 lower eyelids) for an year period. The overall results obtained were satisfactory and morbidity was minimal.

Eighteen experimentally induced blunt and sharp renal injury in rabbits were treated either with an alloplastic renal capsule made of Vicryl mesh or with through and through chromic catgut sutures or with no suture as controls. A simple apparatus was used to cause uniform renal injury and the device was modification of an AO Kuntscher nail extractor. The Vicryl mesh capsule was made by ourselves with #2 and #4 Vicryl. The postoperative healing course were followed up until 6 weeks and isotope renogram showed good renal function in 15 cases. However, when compared to the controls, the kidney which had been repaired with Vicryl mesh contained considerably less scar and showed neither perirenal fibrosis or atrophy of the parenchyma in the vicinity of the capsule. Our preliminary results seem to confirm that simple and rapid surgical treatment of intermediate degree renal injuries is possible using the alloplastic Vicryl mesh capsule. The method may be also suitable for reconstruction of the parenchyma following nephrotomies, such as for removal of staghorn calculi.

PURPOSE: To report a case with a rare form of laceration through the upper punctum, vertical canaliculus, ampulla, and upper eyelid. A modified method of repair involving the use of a bicanalicular silicone stent in the canalicular systems was used successfully. METHODS: A 7-year-old child with laceration through the upper punctum, vertical canaliculus, ampulla, and upper eyelid visited our hospital and was treated with the repair methods. A Crawford Probe(R) (FCI Ophthalmics, Marshfield Hills, MA, U.S.A.) was intubated. No direct suture was done to the canaliculus. Two 8-0 Vicryl sutures were passed through the orbicularis around the ampulla in circular fashion and tied. The authors evaluated the effectiveness and stability of the silicone stent. RESULTS: On the 13th day after surgery, all the lid sutures were removed. One and two months after surgery, the patient had no tearing. Three and a half months after operation, the patient visited our clinic because of prolapse of the silicone stent to the ocular side. Irrigation through the lacrimal passage was normal and the stent was removed. Two months after removal, the patient was free from symptom with a patent canaliculus.

PURPOSE: We prospectively examined the effects on refractive power after strabismus surgery using suturing materials of different thickness. METHODS: Postoperative sutures were done using 6-0 vicryl on the right eye and 7-0 vicryl on the left eye in 16 patients (32 eyes) who received bilateral horizontal rectus muscle recessions. Cycloplegic refraction was performed preoperatively and at 1 week, 1 month, 3 months, and 6 months after surgery. RESULTS: We found a significant decrease in the refractive power at 180 degrees meridian resulting in with-therule astigmatism during the first month after surgery in the 6-0 vicryl group and during the first week in the 7-0 vicryl group. The changes in refractive power gradually returned to preoperative states in each group. There was no statistical significance between the two groups. However, the decrease in refractive power at the 180( meridian resulting in with the rule astigmatism was less in the 7-0 vicryl group. CONCLUSION: With-the-rule astigmatism after horizontal muscle recession surgery is a temporary phenomenon and this tendency is decreased with the use of thinner suture materials.

Deepening of the nasolabial crease is an esthetically unpleasing aging phenomenon occurring in the midface. Various treatment modalities have been introduced to improve the appearance of prominent nasolabial folds, all of which have pros and cons. Currently, a minimally invasive technique using synthetic dermal fillers is most commonly used. A simple and easy subcision procedure using a wire scalpel has also been used and reported to be effective for prominent nasolabial fold correction, with minimal complications. As an alternative to the wire scalpel, we used a 20-gauge metal type spinal needle cannula (Hakko Co.) and 4-0 Vicryl suture (Ethicon Inc.) for subcision of nasolabial folds. This technique is less expensive than the use of a wire scalpel and easily available when needed. Therefore, on the basis of favorable results, our modified subcision technique may be considered effective for prominent nasolabial fold correction.

The ideal suture for use in ophthalmic surgery is one that is fine yet strong, non-irritant, and non allergic, predictably and reliably absorbed after a suitable period of time, and with good knot holding properties. Polyglactin (Vicryl) 8-0 suture, newly developed absorbable suture was used in 10 cases of cataract extractions and trabeculectomy, The results were excellent with minimal complications. comparing with ordinary 10-0 monofilament nylon or 8-0 virgin silk.

PURPOSE: The purpose of this study was to evaluate the tensile strength of surgical synthetic absorbable sutures over a period of 14 days under simulated oral conditions. METHODS: Three suture materials (polyglycolic acid [PGA], polyglactin [PG] 910, and poly (glycolide-co-small je, Ukrainian-caprolactone) [PGC]) were used in 4-0 and 5-0 gauges. 210 suture samples (35 of each material and gauge) were used. All of the samples were tested preimmersion and 1 hour and 1, 3, 7, 10, and 14 days postimmersion. The tensile strength of each suture material and gauge was assessed. The point of breakage and the resorption pattern of the sutures were also assessed. RESULTS: During the first 24 hours of immersion, all 4-0 and 5-0 samples of PGA, PG 910, and PGC maintained their initial tensile strength. At baseline (preimmersion), there was a statistically significant (P<0.001) difference in the tensile strengths between the 4-0 and 5-0 gauge of PGA, PG 910, and PGC. PGA 4-0 showed the highest tensile strength until day 10. At 7 days, all the 4-0 sutures of the three materials had maintained their tensile strength with PGA 4-0 having significantly greater (P=0.003) tensile strength compared to PG. CONCLUSIONS: 4-0 sutures are stronger and have greater tensile strength than 5-0 sutures. The PGA 4-0 suture showed the highest tensile strength at the end of day 10.

Webbed penis without chordee or hypospadias is uncommon malformation of the genital integument which is repaired easily once the condition is recognized. Examination reveals a web of skin extending between the median scrotal raphe and the ventral aspect of penis. The web was incised transversely and sutured vertically in two patients. In other three patients the redundant skin was excised and sutured with interrupted 4-0 vicryl. We report five cases of webbed penis which are good functional and cosmetic results.
