We report a case of cutaneous horn originated frorn condyloma acuminata in a 25 year-old man. The patient showed yellowish brown hyperkeratotic masses on the prepuce, coronal sulcus and multiple asymptomatic pinkish cauliflower-like projections in both inguinal folds and perianal area. Skin biopsies were taken from the base of the penile lesions andinguinal, perianal area exhibited marked hyperkeratosis and parakeratosis in addition to the typical findings of condylorna acuminaturn. In situ hybridization technique for Human papilloma virus(HPV) showed positive reaction to HPV 11. All lesions were removed by surgical excision, electrocauterization and podophyllin application, but condyloma acuminatum recurred several times thereafter.

Giant condyloma acuminatum (also known as Buschke-Lowenstein tumor) is a rare disease, for which the treatment of choice is still controversial. In the case described in this report, we used a combination of electrocautery and a surgical scalpel to resect a huge genital wart, followed by long-term local treatment with podophyllin. This procedure was relatively fast, easy to perform, involved minimal bleeding, and did not result in any complications. Moreover, combination therapy enabled us to avoid deep resection of the lesion and consequently, sexual function was preserved completely.

Bowenoid papulosis is manifested by clinically the benign-appearing papules of anogenital region and histopathologically similar features of squamous cellurar inoma in situ in human. A cause is thought to be Human Papillomavirus infection, type 16. The treatment is mainly through conservative management but others are topical application of podophyllin, shave excision, local excision, cryotherapy, electrodessication, topical applcation of 5-FU ointment and subcutaneous injection of recombinant interferon gamma. We present a case of Howenoid papulosis treated with DNCB imriunotherapy. The patient, had multiple papules on the penis with mild pruitus. Histopatnologic findings showed crowding and irregular arrangement of the nucleic many of which are large, hyperchromatic, and pleomorphic. Dyskeratotic and multinucleated keratinocytes wire also present, as were atypical mitoses, scattered in epidermal layer. We treated him with DNCB immunotherapy. Initially he was sensitized with 2,000 ug/0.1ml and two weeks later he received 50 ug/0.1ml per every week. After 3 weeks, he had showed no improvement and we elevated the dose to 100 ug/0.1ml. He was improved after 6th trial and his skir. lesions were almostly disappeared at present.

Podophyllin is known as a neurotoxin, We recently observed a 34-year-old woman with podophyllin induced peripheral neuropathy. She was treated with topical podophyllin for perianal multiple condyloma acuminata. One day after application, she complained of G-I symptoms ( abdominal pain, nausea, vomiting, diarrhea), followed by tingling sensation on all extremities. We confirmed peripheral neuropathy with electrophysiologic study and sural nerve biopsy. She has improved on follow-up.

Since penile prosthesis were introduced clinically, higf operation success rates and good patient satisfaction ratings have been reported. Nevertheless, the complication rates have been reported to range from 4% to 7% penile prosthesis (AMS-CXM) and widespread condylomata on the genitaile. Physical examination revealed that the right prosthesis cylinder had perforated the distal medial side of the corpus cavernosum and almost perforated the distal urethra, and the left cylinder had perforated the distal midline septum and was overhanging the right cylinder. Additionally, condylomata acuminata infected the whole penile skin and lower pubic area. The patient underwent multiple treatments for condyloma, including podophyllin, electrocoagulation, and coagulation using a laser, for 6 months and a "windsock" operation using Gore-Tex in both corpora cavernosa with a penoscrotal skin incision for his prosthetic probiem. There have been no complications at 9 months'follow-up.

A 20 year old woman with noninsulin-dependent diabetes mellitus (NIDDM) developed an extensive warty and focally cauliflower like mass located at anogenital area. Its histopathologic study and in situ DNA hybridization revealed this lesion an ordinary condyloma acuminatum without large bulbous downward proliferation of which HPV type was 6 and/or 11. The mass lesion was successfully treated by excision and electrodesiccation under spinal anesthesia. Intermittent recurrence of several papular condylomata acuminata was observed during a follow up period of 6 months, which were easily cured by podophylline application on each occasion.

A case is reported ofsystemic toxicity by topically applied podophyllin. The patient was a 18-year-old girl who was treated at private gynecological clinic with 25% podophyllin resin for multiple vulval condyloma acuminata. Her apparent podophyllin toxicity begun 4 hours after topical application, presenting gastrointestinal symptoms such as nausea, vomiting, diarrhea and abdominal distension, followed by tingling sensation on hands and feet with motor weakness predominantly of distal limbs. She had never experienced disturbance of consciousness or other CNS symptoms. The symptoms of severe peripheral neuropathy had been improved by 72 days followup when she had mild impairement of vibration and position sense and mild dorsiflexion weakness of the feet.
