Plummer-Vinson syndrome is characterized by dysphagia associated with anemia, glossitis and esophageal web. The cause of this syndrome is still not clear, but the most important one is thought to be the iron deficiency. The patient with small sized ageal web is treated by administration of iron but large sized web with dyspha gia requires breakage of web. In the following case report, we describe a patient with Plummer-Vinson syndrome who showed clinical improvement after treatmeat with endoscopic balloon dilatation of upper esophageal web with a review of literatures.

Plummer-Vinson syndrome(Paterson-Kelly syndrome)is characterized by dysphagia due to upper esophageal or hypopharyngeal web,iron deficiency anemia,and atropic oral and glossal mucosa.This syndrome is usually known as precancerous lesion that develops into postcricoid carcinoma.Universally,the clinical manifestations of this syndrome were markedly improved after oral iron replacement therapy or endoscopic balloon or electrocatery treatment. 63 year-old woman was received a short segment,free jejunal transfer to be released from esophageal stricture.After the operation,the stenotic lesion proved to be Plummer-vinson syndrome with carcinoma in situ by pathologic study.

Plummer-Vinson syndrome is characterized by dysphagia, iron deficiency anemia, and upper esophageal web. The associated symptoms can be resolved by administering iron supplements as well as by endoscopic intervention. Relapse in patients with Plummer-Vinson syndrome is very rare. We describe a case of a 42-year-old woman with Plummer-Vinson syndrome whose symptoms were successfully treated with endoscopic dilatation and iron supplementation at first admission; however, 1 year later, she revisited our hospital because of dysphagia. On second admission, investigations revealed esophageal web relapse in Plummer-Vinson syndrome. She was again successfully treated with endoscopic dilatation and iron supplementation. After first admission, her anemia was not normalized due to poor compliance and loss of follow-up. We experienced a case of esophageal web relapse due to uncorrected iron deficiency anemia in a patient with Plummer-Vinson syndrome. This experience indicates that continuous iron supplementation and long-term follow-up is important in patients with Plummer-Vinson syndrome.

The combination of symptomatic hypopharyngeal webs and iron-deficiency anemia in middle-aged women constitutes Plummer-Vinson syndrome. It is uncommon but important syndrome because of inceased risk of squamous cell carcinoma of the pharynx and esophagus. The cause of this syndrome is still not clear. A small sized web is treated by correction of anemia, but a large sized one with dysphagia is treated by breakage of web. Recently, we experienced a 41-year-old woman with Plummer-Vinson syndrome. She complained of dysphagia for 10 years. Esophagogram and endoscopic examination showed a hypopharyngeal web and peripheral blood profile was compatible with iron-deficiency anemia. Her symptom improved after endoscopic balloon dilatation of the upper esophageal web.

10.A Case of Plummer-Vinson Syndrome.

Plummer-Vinson syndrome is a clinical entity characterized by dysphagia, iron deficiency anemia, cheilosis, glossitis, and cervical esophageal web, especially in middle aged women. Recently, the authors experienced a case of Plummer-Vinson syndrome. A 53-year-old female was admitted due to intermittent solid food dysphagia for 18 months. She had a 2 years history of iron deficiency anemia. On admission glossitis, fissures at the angle of the mouth, spoon nails, and iron deficiency anemia were noted. Esophagogram and esophagoscopic examination revealed thin walled concentric web at upper esophagus. Esophageal web was succefully teared by endoscopic balloon dilatation with subseguant improvement of dysphagia. Skin manifestations as well as anemia were markedly improved after oral iron replacement therapy.
