1.The Clinical Application Status and Development Trends of Hydrogen Peroxide Low Temperature Plasma Sterilizers.

The hydrogen peroxide low temperature plasma sterilization technology solved the problems of thermo-sensitive materials' disinfection and sterilization based on its development and unique characteristics. This paper introduced the researches of clinical application quality control, and showed the hydrogen peroxide low temperature plasma sterilizers were being widely used in hospitals and highly recognized. According to the clinical data and the literatures of the domestic equipment in preliminary application, it could be concluded that the technology maturity of domestic hydrogen peroxide low temperature plasma sterilizers was in a high level. The advantages of using domestic hydrogen peroxide low temperature plasma sterilizers to do disinfection and sterilization included lower cost, safer, faster and non-toxic, etc. Also the management system should be improved and the clinical staff should master the technical essentials, obey the procedures strictly, verify periodically and offer full monitoring to upgrade the quality of sterilization.

2.On the active mechanism of hydroxyapatite modified by cold plasma.

The surface modification using cold plasma technique was introduced to hydroxyapatite(HA). The methods adopted in the study included the formation of bone-like apatite in simulated body fluid and the use of SEM, XPS and XRD. The results showed that the formation of bone-like apatite on HA modified by cold plasma was easier than that without modification. The active mechanism involves the impact of the particles with high energy and high activity against HA, which roughens and etches the surface of HA, heads to the distortion of HA crystal, and thus increases the dissolvability of HA and the local concentration of the Ca and P ions. This approach is helpful to the formation of bone-like appetite. The data demonstrate that the surface modification using cold plasma technique can increase the activity of HA.

3.A Choice and Precautions of Replacement Fluids for Therapeutic Plasma Exchange.

BACKGROUND: Therapeutic plasma exchange (TPE) is an effective and practical treatment for separation and removal of harmful antibodies or pathogenic substances from the blood. The volume of plasma removed must be replaced by a replacement fluid such as 4~5% albumin solution or Fresh frozen plasma (FFP). We conducted a study of coagulopathy using albumin solution and checked the chemical composition of fresh frozen plasma. METHODS: We measured pre- and post-TPE PT/aPTT for evaluation of the effect of albumin replacement on coagulation from 192 TPE sessions of 19 patients. We also investigated routine chemistry test items including glucose and electrolytes from 10 randomly selected FFP. RESULTS: The post PT and aPTT within four hours after TPE were prolonged due to a transient decrease in coagulation factors, but were normalized within 2 days after TPE. All coagulation time was corrected to the level of the pre-TPE status within four hours before the next TPE except the patients who received TPE 6 times or more. FFP showed higher level in glucose, sodium and inorganic phosphate. CONCLUSION: Albumin exchange produces temporary coagulation factor deficiency. However, this transient factor deficiency rarely causes clinical problems and the factors are rapidly corrected by redistribution and resynthesis. We should be careful about hypocalcemia, hyperglycemia, and hypernatremia when using FFP replacement.

4.Reorganization of Automated Outpatient Laboratory to improve Total.

BACKGROUND: Asan Medical Center ran a fully automated outpatient laboratory to serve outpatient departments with a rapid turnaround time (TAT; one hour from reception to reporting) for frequently requested test items and thus to make `one stop service' possible. As the number of samples increased, the TAT gradually became longer and eventually showed over one hour for almost all items. METHODS: In October 1998, reorganization of the outpatient laboratory took place. Newly introduced were a priority system for samples, a plasma separate tube instead of the serum separate tube, an on-line simultaneous sample reception system at the time of sampling, and a real-time monitoring system for TAT. RESULTS: With the sample priority system, samples processed for one stop service were 61.0% (476 priority samples out of 780 total samples) for routine hematology, 59.2% (527 out of 890) for routine chemistry, 40.1% (122 out of 304) for urinalysis, 43.2% (89 out of 206) for coagulation tests, and 47.9% (75 out of 157) for diabetic tests. We monitored samples processed as `one stop service' with the real time monitoring system for TAT; among the samples processed as such, 80.1% of rou-tine hematology, 91.9% of routine chemistry, 99.5% of urinalysis, 92.6% of coagulation tests, and 97.6% of diabetic tests showed TAT less than one hour. Average TAT, from the specimen acquisition to the reporting, decreased dramatically after introduction of the system. Average TAT of routine hematology decreased to 53.9 minutes, routine chemistry to 54.6 minutes, urinalysis to 35.2 minutes, coagulation tests to 46.6 minutes, and diabetic tests to 31.9 minutes. And the patient satisfaction index for the outpatient laboratory rose 15% from 81% to 96%. CONCLUSIONS: The outpatient laboratory of AMC shortened the TAT substantially without sacrificing quality and fully met the needs of patients and clinicians after reorganization.

6.Comparison of rheologic properties between Ca-alginate hydrogel microspheres suspension and whole blood.

Starting from the form of red blood cells and the hematocrit (Hct, about 45 vol% of whole blood), we tried to prepare a kind of microspheres suspension to imitate non-Newtonian fluid property of whole blood, exploring its potentiality to be applied in blood viscosity quality control substance. In our study, we produced Ca-alginate hydrogel microspheres using emulsion polymerization, then we suspended the microspheres in 0.9 wt% NaCl solution to obtain a kind of liquid sample with the microspheres taking 45% volume. Then we used two types of viscometers to measure and analyse the changes of sample viscosity at different shear rate. We observed the forms of Ca-alginate hydrogel microspheres with microscope, and found them to be relatively complete, and their diameters to be normally distributed. Diameters of about 90% of the microspheres were distributed in a range from 6 to 22 micron. The samples were examined with viscometer FASCO-3010 and LG-R-80c respectively, both of which have shown a shear-thinning effect. After 5-week stability test, the CV of viscosity results corresponding to the two instruments were 7.3% to 13.8% and 8.9% to 14.2%, respectively. Although some differences existed among the results under the same shear rate, the general variation trends of the corresponding results were consistent, so the sample had the potentiality to be widely used in calibrating a different type of blood viscometer.

The evaluation of the adsorption of methylene blue (MB) in plasma by sulfonated polyethersulfone (SPES) adsorbent column was carried out in this study. The results indicated the adsorption of MB by SPES adsorbent column was more efficient than that by polyethersulfone (PES). In addition, the changes of the concentration of BSA solution passing through adsorbent column along with the time and the biochemical indices of plasma before and after adsorption treatment were also investigated. The results showed that the adsorption amount of BSA by PES adsorbent column was larger than that by SPES, and the biochemical parameters such as total protein, albumin, glucose, triglyceride and total cholesterol in plasma varied slightly before and after passing through the column, which were still within the clinical indices.

8.Expression of IgG4 in Rosai-Dorfman disease and its significance.

OBJECTIVETo study the prevalence of IgG4-positive plasma cells in Rosai-Dorfman disease and to assess the association between Rosai-Dorfman disease and IgG4-related sclerosing disease (IgG4-SD).
METHODSThe clinicopathologic features of 12 tissue samples of Rosai-Dorfman disease (11 extranodal and one nodal) from nine patients were reviewed. The degree of fibrosis and occlusive phlebitis was studied by HE staining. The expression of IgG4 and IgG in plasma cells were studied by immunohistochemistry (EnVision) and quantitatively analyzed by medical image analysis system.
RESULTSNine tissue samples showed different degree of fibrosis (four tissue samples were mild, one tissue sample was moderate and four tissue samples were severe) and two tissue samples showed occlusive phlebitis in the lesional tissue. Immunohistochemical study showed marked infiltration by IgG4-positive plasma cells (> 50 per high-power field) in four tissue samples, moderate infiltration (30 to 50 per high-power field) in two tissue samples, mild (10 to 29 per high-power field) in three cases and negative infiltration (< 10 per high-power field) in three tissue samples (P < 0.01). Three tissue samples fulfilled the diagnostic criteria of IgG4-SD (> 50 IgG4-positive plasma cells per high-power field and IgG4-to-IgG ratio > 40%), including one tissue sample each of Rosai-Dorfman disease in the left facial skin, above the left eye socket, and in the right parotid.
CONCLUSIONSSome cases of Rosai-Dorfman disease fulfill the diagnostic criteria and show the histologic features of IgG4-SD. They may represent members of the IgG4-SD spectrum. The detection of IgG4-positive plasma cells in the lesional tissues of Rosai-Dorfman disease may have clinical pathological significance.

9.Arginase Level in Suspended Red Blood Cells Storaged for Different Time.

OBJECTIVETo explore the effect of storage time on arginase level, and the possible source of arginase in suspended red blood cells (RBC).
METHODSThe arginase and myeloperoxidase (MPO) levels in suspended RBC and control plasma were detected by ELISA. The free hemoglobin level in suspended RBC and control plasma were detected by colorimetric method. The relationship between arginase level, MPO level and free hemoglobin level in suspended RBC was analyzed by the related methods.
RESULTSThe arginase and free hemoglobin levels in suspended RBC were higher than those in control plasma. Otherwise, MPO level was not significantly different between suspended RBC and control plasma. All of them did not increase along with prolonging of storage time. There was not a significant correlation between arginase level and free hemoglobin level in suspended RBC of different storage time (r = 0.03), but arginase level positively correlated with MPO level in the suspended RBC of different storage time (r = 0.76).
CONCLUSIONThe arginase level in suspended RBC storaged for different time increases significantly, but not along with prolonging of storage time. The main possible source of arginase in the suspended RBC is the residual white blood cell, especially neutrophils.

10.Pharmacokinetic profiles of methylprednisolone concentration in perilymph and plasma of guinea pig following local administration.

OBJECTIVETo determine pharmacokinetics profiles of methylprednisolone (MTH) in perilymph and plasma of guinea pig after intratympanic injection of MTH in different concentrations.
METHODSBy auripuncture, three different concentrations of MTH were used in guinea pigs: 32.0 mg/ml, 40.0mg/ml and 62.5 mg/ml. Samples of plasma and perilymph of the scala tympani were collected at 1-12 h after administration and the concentrations of MTH were assayed using high-performance liquid chromatography. Auditory brainstem response (ABR) was detected before and 12h after MTH administration.
RESULTSThe concentrations of MTH in perilymph were dose-related. The differences of MTH concentrations in perilymph before and at series sampling time after administration were significant in statistics (P < 0.05), except for the intervals of 5 h, 6 h and 12 h. In addition, the concentration of MTH in perilymph was the highest in 62.5 mg/ml group. No MTH was detected in plasma after local administration, except for 1.5 h and 2 h after administration with 62.5 mg/ml. There were not differences in the thresholds of wave III of ABR before and 12h after local MTH administration among three groups (P > 0.05).
CONCLUSIONSThe experimental results indicates that the concentration of 62.5 mg/ml of MTH is optimal for treatment inner ear diseases by intratympanic administration among three groups, it get a high concentration in perilymph and not effect on the function of inner ear.
