Many malignant tumors, most commonly small cell lung cancer, merge with syndrome of inappropriate secretion of antidi-uretic hormone (SIADH). SIADH is caused by the neuroendocrine function of tumor cells or is a side effect of chemotherapeutics. As a chemotherapeutic side effect, SIADH presents as hyponatremia. Therapy for SIADH includes both treatment for primary diseases and hyponatremia. Given that SIADH-induced hyponatremia aggravates the prognosis and shortens the survival time of patients with malig-nant tumors, SIADH is an independent risk factor for prognosis. Therefore, the early diagnosis, monitoring, and treatment of SIADH will improve the prognosis of patients with malignant tumors.

Objective To discuss the characteristics of pulmonary function in children patients with positive bronchodilating response using impulse oscillation system(IOS) and provide the evidence for clinical diagnosis and performing IOS.Methods Seventy children patients with bronchodilating response using IOS were recruited randomly for this study.Forty children patients with positive bronchodilating response were regarded as positive group and the other thirty children patients with negative bronchodilating response were regarded as negative group.The IOS parameters,for example,total respiratory impedance (Z5),respiratory resistance at 5 Hz(R5),reactance at 5 Hz(X5),peripheral respiratory resistance(Rp) in the two groups were analyzed.Results Z5,R5,X5 of the positive group [(123.45 ± 24.75)％,(120.09 ± 25.40)％,(138.62 ± 33.48)％] were higher than those of the negative group [(103.89 ± 16.30)％,(102.67 ± 18.16)％,(111.85 ± 14.93)％].There were significantly different between the two groups (P ＜ 0.05).Moreover,Rp ≥ 19.89 cmH2O/ (L·s)(1 cmH2O =0.098 kPa) in the positive group was 50.00％(20/40),which was higher than that in the negative group[23.33％(7/30)].There was significant difference between the two groups(P＜ 0.05).Conclusions The impedance of children patients with positive bronchodilating response using IOS is significantly increased,especially X5.When IOS parameters ≥ 100％,especially X5 ≥ 100％,which suggest that perform the bronchodilation test in order to decrease the rate of missed diagnosis of asthma.

Objective To investigate the expression of 5-lipoxygenase (5-LOX) in endometriosis and analyze the relationship of 5-LOX and angionesis in endometriosis. Methods Immunochemical method was used to determine the expression of 5-LOX, VEGF, and the value of MVD in ectopic endometrium (ec-topic group) and eutopic endometrium (eutopic group) of 32 patients with EMs and in eutopic endometrium ( control group) of 25 patients without EMs. Result The expressions of 5-LOX and VECF and the value of MVD in ectopic groups were 0. 43 ± 0.09,0. 39 ±0.07 and 0.40 ± 0.09, respectively, which were higher than that of eutopic group and control group. The expressions of 5-LOX and VEGF and the value of MVD in eutopic group were 0. 35 ± 0. 08, 0.30 ± 0. 06 and 0. 33 ± 0. 08, respectively, which were higher than that of control group ( P <0.05). In ectopic group, the expressions of 5-LOX and VEGF, and the value of MVD in stage III/IV, were much higher than that of stage I/II ( P <0. 05); the expressions of 5-LOX and VEGF and the value of MVD were related to the severe of endometriosis through Spearman correlation analysis. The expressions of 5-LOX were shown to be significantly related to the expressions of VEGF and the value of MVD in ectopic and eutopic groups ( P <0. 05). Conclusion There is a high expression of 5-LOX in endometriosis, suggesting that 5-LOX may be participate in the pathogenesis of endometriosis. 5-LOX may promote angiogenesis of EMs through up - regulating the expression of VEGF.

Objective To compare the clinical effect of povidone-iedine cream plus vagina scrubbing with traditional preoperative vaginal preparation method in order to seek a more safe,effective,convenient and efficient method to prepare the vagina.Methods 100 cases of married patients undergoing elective gynecological surgery were divided into the observation group and the control group with 50 patients in each.The observation group adopted povidone-iodine cream plus vagina scrubbing and the control group used traditional method.Two groups were prepared in the vagina before and after taking cervical swabs for bacterial culture to compare the positive rate of bacterial culture.Results Intra-group comparison of vaginal bacterial culture before and after vaginal preparation was significantly different,but no statistical significance was seen between the two groups.Conclusions Vagina preparation with povidoneiodine cream plus vagina scrubbing can not only reduce the workload of nurses but also improve patient comfort degree.

Objective To develop measles and Japanese encephalitis(JE)combined attenuated live vaccine. Methods The independent development of China′s Shanghai 191 measles vaccine strains and Japanese encephalitis SA14-14-2 vaccine strains were selected to prepare two of monovalent vaccines solution,which were used to conduct the interference test,the ratio test,the screening test of protection agent,the screening test of dilution,and the alternative of the curve of freeze-dried.Results The preparation technology of measles and Japanese encephalitis combined live attenuated vaccine was determined.The batches of the products met the quality requirement of the three kinds of monovalent vaccine in the Pharmacopoeia of the people's Republic of China(edition 2005,volume 3).The cross-interference and increased response were not found in the animal test,and the immune response of mouse was not influenced,which indicated the safety and immunogenicity of with good combined vaccine.Conclusion Measles and Japanese encephalitis combined attenuated live vaccine was developed successfully.

Objective To investigate the right ventricular function of patients with chronic pulmonary hypertension using Doppler tissue imaging(DTI)and to evaluate the relationship between DTI parameters and pulmonary artery systolic pressure(PASP).Methods Thirty-five patients with chronic pulmonary hypertension and thirty-five healthy subjects underwent Doppler tissue echocardiographic evaluation of right ventricular function.The tricuspid lateral annular systolic velocity(Sm),early(Em)and late(Am)peak diastolic velocities,isovolumic contraction time(IVCT)and isovolumic relaxation time(IVRT)were acquired from apical 4-chamber view.Results When compared with control group,Sm,Ea and Em/Am values were found to be lower and Am,IVRT values higher in patients with chronic pulmonary hypertension.When all patients were analyzed,there was a significant positive correlation between PASP and IVRT(r=0.78,P

Objective To evaluate the clinical efficiency and safety of thiazolidinediones (TZDs) on treat-ment of patients with nonalcoholic fatty liver disease (NAFLD). Methods The published controlled clinical trials about TZDs on treatment of patients with NAFLD from 1998 to 2008 were identified by searching in the full-text data base,such as Medline, PubMed, Ovid, EMCC, CBM Disc, CNKI, Wanfang, Weipu data and so on, as well as by searching of cross references from original articles and reviews. The enrollment standard was formulated strictly ac-cording to the designs of trials, and Meta analysis was performed by software package of Review Manage 4.2. Further-more, the statistic test, sensitivity and publication bias about the combined statistic data were revealed by referring to forest-graph and funnel-plot. Results All of the 473 patients in 18 controlled clinical trials were enrolled in this study. The weighted mean diffrerence of serum ALT, AST,TC,TG and BMI, HOMA-IR before and after treatment with TZDs 32. 24 (18.98-45.49) (Z=4.77,P<0.05),20.32 (2.30-38.35) (Z=2.21,P<0.05),0.26 (-0.01-0.54)(Z=1.88,P=0.06),0.63 (0.33-0.93) (Z=4.14,P<0.01),and -0.06 (-0.66-0.55) (Z=0.19, P=0.85),2.63 (1.33-3.93) (Z=3.96, P<0.01). In all papers, the improvement of hepatic tissue after treatment with TZDs in patients with NAFLD was observed in only 6 articles, which demonstrated the hepatic steatosis, inflammation and fibrosis [WMD (95% CI) 1.59 (1.08-2.10), 1.45 (0.90-2.00), 0.72 (0.12-1.32), Z= 6.08,5.16,2.33,P<0.05],especially the degree of NASH activity index,whose WMD (95% CI) was 4.21 (3.13-5.28) and Z=7.66,P<0.01, were improved significantly in the 104 NASH patients taken this kinds of drugs. There were 7 patients exiting from the all chnical trial, and the main adverse effects in patients accepting treat-ment with TZDs were debility, dizziness, edema of lower extremity, weight gain, indisposition of gastrointestinal tract, elevated liver enzyme and so on, which were similar to the control group. Conclusions The therapeutic effects of TZDs on NAFLD is confirmed. Not only are the serum liver enzyme,TC,TG and HOMA-IR in patients with NAFLD improved, but this new euglycemic agent has no acutely adverse effect in clinical application,which confirms the clin-ical efficiency and safety of TZDs on treatment of patients with NAFLD.

A simple preparative strategy was proposed for the preparation of water-soluble, bright blue-emitting carbon nanoparticles ( CNPs ) at different temperatures by hydrothermal treatment using bean curd residue as carbon source for the first time. When the temperature reaches 240 ℃, quantum yield ( QY) can be up to 15. 24%. Most importantly, such fluorescent CNPs can be served as an effective sensing platform for label-free sensitive and selective detection of Fe3+ions with a detection of limit as low as 50 nmol/L, which is nearly 2 orders of magnitude lower than the maximum level of Fe3+ions in the drinking water permitted by the European Community. These outstanding properties give promising potential for reliable detection of Fe3+ in intracellular fluid and water samples, even bioimaging and optical imaging without being hazard in the future.

BACKGROUND: Progressive body mass losses and physical strength decrease are common complications of malignant tumors, and they are important factors that affect the quality of life of the patients.OBJECTIVE: To probe into the curative effect of a Chinese herbal compound Yiqi Xiaozheng granules combined with antitumor drugs in the treatment of malignant tumors and its effect on the quality of life.DESIGN: A randomized controlled observation.SETTING:Clinical Tumor College of Peking University.PARTICIPANT: Totally 61 patients with malignant pulmonary carcinoma,breast or intestinal carcinoma hospitalized in the Wards of Integrated Traditional Chinese and Western Medicine of Beijing Cancer Hospital from June 1999 to March 2002 were recruited.METHODS: Altogether 61 cases of malignant tumor were randomized into the treatment group (treated with Chinese herbal compound Yiqi Xiaozheng granules supplemented with chemotherapy)(n=34) and the control group (treated with only chemotherapy was used)(n=27). The difference in curative effect, survival period with tumor and the quality of life of the two groups were observed. The comparison of the curative effects of the two groups was performed according to Clinical Curative Effect Criteria for Solid Tumors stipulated by World Health Organization: completely response, partly response, stable disease, and progressive disease. X-ray examination was performed on the patients before the therapy and two courses of treatment after the therapy to evaluate the curative effect. Physical strength grading was made according to the evaluation criteria of Eastern Oneology Cooperative Group of the United States. The two groups were graded before and after the treatment. The group with decreased grade after the treatment was considered to be improving, but the group with increased grade after the treatment was considered to be worsening.symptom before and after therapyparison of the curative effect of the two groups: The cure rate of the treatment group was higher than that of the control group (partial response rate,stability rate and progression rate of the two groups was 36% ,7% ;52%,survival with tumors: The mean period of survival with tumors of the treatment group was longer than that of the control group until March 2002ing: Physical strength grading status was better after the therapy in the treatment group than that in the control group [ In the treatment group, 9cases (26%)were ameliorated, 25 (74%)appeared stable , none was aggravated; in the control group, 1 case (4%) was ameliorated, 25(93%) appeared of qi-deficiency symptoms in traditional Chinese medicine before and after the treatment between the two groups. Symptoms such as sweating and qi deficiency manifested as disclination to talk, listlessness, debility, etc. were significantly reduced in the treatment group (The numbers of cases before and after the treatment were 20, 9; 17,9; 20,13 respectively). In the control group, the cases either were stable or showed an increase in symptoms (There were 13, 13; 14, 17; 18, 20 respectively before and after treatment).CONCLUSION: Compared with the control group, in the treatment of malignant tumors, Yiqi Xiaozheng granules supplemented with chemotherapy not only enhanced curative effect, but also improved the quality of life and prolonged the period of survival with tumors.

ObjectiveTo determine the diversity of outer membrane protein expression of Leptospira interrogans during infection of human THP-1 monocytes,and protein antigen candidates for developing leptospiral genetically engineering vaccines.MethodsThe outer membrane proteins(OMPs) of Leptospira interrogans serogroup Icterohaemorrhagiae serovar Lai strain Lai before or after infection of human THP-1 monocyte line were extracted using Triton X-114 method.The OMPs extracts were separated by two-dimensional gel electrophoresis(2-DGE) and then detected using silver staining method.Four up-regulated and four down-regulated OMPs were selected for hydrolysis by trypsin and then identified by using liquid chromatography-tandem mass(LC-MS/MS) method.The transmembrane regions and signal peptides in the eight target OMPs were analyzed using bioinformatic software.Several real-time fluorescent quantitative RT-PCRs were performed to determine the mRNA level changes of the eight target genes of L.interrogans strain Lai before or after infection of THP-1 cells.The prokaryotic expression systems of target genes were generated and the immunoprutection of target recombinant proteins was determined using leptospire-infected guinea pigs model.ResultsAfter a 60 min infection of THP-1 cells,four OMPs( Loa22,GroEL,F0F1 ATP synthase alpha- and beta-subunits) of L interrogans strain Lai were noticeably up-regulated ( P＜0.05 ),while the other four OMPs( FlaB2,LigB,OmpA family protein and OmpA) were significantly down-regulated (P＜0.05).The results of real-time fluorescent quantitative RT-PCRs were coincident with those confirmed by the 2-DGE phus silver staining.The bioinformatic analysis indicated that among the eight OMPs,OmpA and OmpA family protein belonged transmembrane proteins,while the rest had no membrane structure.Loa22,LigB and OmpA family protein contained a signal peptide in their sequences.200 μg of the recombinant proteins rLoa22 and rGroEL presented 75.0％ immunoprotective rates in the infected guinea pigs.Conclusion An obvious change of the OMP expression map of L.interrogans strain Lai occurs during infection of host cells.The up-regulated leptospiral OM Ps during infection,especially for the GroEL and Loa22 proteins,can be used as the immunogen candidates for developing leptospiral genetically engineering vaccines.
