2.The Effect of Stasis-eliminating and Meridian-unblocking Moxibustion on Vascular Endothelial Cell Proliferation and Migrationin VD Rats

Objective To investigate the in vitrointervening effect of ratserum effector substances on vascular endothelial cell proliferation and migrationafter stasis-eliminating and meridian-unblocking moxibustion and explore the possible proangiogenic mechanism of stasis-eliminating and meridian-unblocking moxibustion treatment for vascular dementia.Method Male adult Wistar rats were used.Arat model ofvascular dementia was made by four-vessel occlusion(4-VO).The rats were randomized to moxibustion,Western medicationand model groups.Asham operation group was established as a control.There was a total of four groups.The moxibustion group received aconite cake moxibustion on point Baihui(GV20)and suspended moxibustion on points Dazhui(GV14)and Shenting(GV24) and the Western medication, an oralgavage of aniracetam tablets.Treatment was given 15 days as one course for a total of two courses.Blood was taken from the Orbital cavity in the moxibustion, sham operation and model groups after each course of treatment.After stasis-eliminating and meridian-unblocking moxibustion, animal serum was prepared tointervene in human umbilical vein endothelial cell (HUVEC-12) culture.Another parallel liquid group was set up. RPMI-1640 culture medium containing 10% fetal bovine serum (FBS) was prepared.Endothelial cell proliferationwas examined bymethyl thiazolyl tetrazolium (MTT) colorimetric assay.Endothelial cell migration was examined by cell scratch labeling.Result After two courses of treatment, the examination of endothelial cell proliferation by methyl thiazolyl tetrazolium (MTT) colorimetric assay showed that rat serum intervened significantly in human umbilical vein endothelial cell (HUVEC-12) proliferation in the moxibustion serum group compared with the model serum, sham operation serum and parallel liquid groups (P<0.01).At 24 hrs after two courses of treatment, the examination of endothelial cell migrationby cell scratch labelingshowed that rat serum intervention significantly increased the HUVEC-12 migration rate in the moxibustion serum group compared with the model serum, sham operation serum and parallel liquid groups (P<0.01).Conclusion Stasis-eliminating and meridian-unblocking moxibustion has a marked improving effect on vascular endothelial cell proliferation and migration in vascular dementiarats, which provides strong evidence for its in vitro proangiogenic effect.
3.Assessment and direction of Objective Structured Clinical Examination on clinical practice in Department of Obstetrics and Gynecology

Objective To analyze the results of the Objective Structured Clinical Examination(OSCE) of medical interns in Department of Gynecology and Obstetrics,and to assess the role of OSCE in clinical teaching and practice. MethodsAtolal of 104 medical interns of five-year education program in 2002 and seven-year education program in 2003 in School of Medicine had taken part in OSCE of gynecology and obstetrics.The OSCE consisted of six stations: gynecologic examination,obstetric examination,oral test,interrogation of standardized-patients(SP),drawing partogram,and non-stress test(NST) explanation.After gathering the data of each station,the software of SPSS 11.0 was used to make statistical analysis. Results The median scores were as follows: 92.0 for gynecologic examination,91.0 for obstetric examination,83.5 for oral test,80.0 for interrogation of SP,80.0 for drawing partogram,and 70.0 for NST explanation.There was no significant impact of familiarity to OSCE on the results of the test.The results showed that medical interns did good job in basic performance,but lack of clinical practice skills in some complicated practice of gynecology and obstetrics.Conclusion The grading system and stations setup of OSCE in Department of Gynecology and Obstetrics in Shanghai Sixth People's Hospital show objectivity and equity.The OSCE can be widely applied in the after-department examination and even graduation examination.
4.Killing efficacy of a medical disinfection ultrasonic coupling agent on five kinds of multidrug-resistant organisms

Objective To observe the efficacy of a medical disinfectant ultrasonic coupling agent on the killing of five clinically isolated multidrug-resistant organisms(MDROs).Methods From March 2016 to May 2017,a disin-fection ultrasonic coupling agent containing active ingredient,including triclosan and propylene glycol,was used to conduct carrier quantitative germicidal test on five clinically isolated MDROs,the killing efficacy to MDROs was ob-served.Results After 1.5,3.0,and 4.5 minute disinfection time,the killing logarithms values of disinfection ultra-sonic coupling agent to five MDROs(multidrug-resistant Acinetobacter bau m annii[MDR-AB],methicillin-resistant Staphylococcus aureus[MRSA],multidrug-resistant Pseudomonas aeruginosa[MDR-PA],carbapenem-resistant Klebsiella pneumoniae[CRKP],and extended-spectrum β-lactamase Escherichia coli[ESBLs-EC])were all>3.0. Conclusion Medical disinfection ultrasonic coupling agent can effectively kill five common MDROs,and can take the place of disinfectant during ultrasonic examination.
5.Associations of SUMO4 polymorphisms with tacrolimus concentrations in Chinese renal transplant recipients.

The study aims to investigate the associations of SUMO4 polymorphisms with tacrolimus concentrations in Chinese renal transplant recipients. Blood samples and clinical data were collected from 132 renal transplant recipients with tacrolimus treatment. CYP3A5*3 genotypes were detected by PCR-RFLP, and SUMO4 (rs237024, rs237025) genotypes were detected by Sequenom® MassARRAY system. SUMO4 rs237024 and rs237025 genotypes were in complete linkage disequilibrium (D' = 1). The dose-adjusted concentration of tacrolimus in SUMO4 rs237024A-rs237025A (GA-GA +AA-AA) carriers was considerably higher than that in GG-GG carriers (P < 0.05). After stratification by CYP3A5*3 genotypes, SUMO4 rs237024A-rs237025A carriers (GA-GA+AA-AA) had a higher dose-adjusted tacrolimus concentration than that in GG carriers in CYP3A5 expresser (P < 0.05). The results illustrated that SUMO4 rs237024 and rs237025 polymorphisms were associated with tacrolimus concentrations, and the test of these genotypes may be useful for individualized medicine of tacrolimus.

6.Killing activity in DC and CIK co-culture against hepatocarcinoma cells.

This study was aimed to investigate the proliferation activities and phenotype changes of DC, CIK and DC-CIK, and their cytotoxicity against hepatocarcinoma cells in co-culture of DC with CIK. Peripheral blood mononuclear cells (PBMNC) were isolated from healthy adult donors. After incubation of PBMNC for 2 hours, DCs were induced from adherent cells by some cytokines and CIKs were generated from non-adherent cells. Mature DCs were harvested after incubation for 9 days, and then were co-cultured with CIK at ratio of 1:5 for 3 days. The cytotoxicity activity against SMMC-7721 hepatocellular carcinoma cell line was detected by MTT assay. The results showed that CIK cells were able to lyse SMMC-7721 hepatocellular carcinoma cells at low ratios of effector to target. This effect was significantly enhanced by co-culture with DCs. It is concluded that CIK cells have high lytic activity against 7721 hepatocellular carcinoma cell line, which can be enhanced by co-culture with DC. DC-CIK cells are highly effective immune cells.

7.Simultaneous determination of the four effective components in Huaijiao pill by HPLC.

OBJECTIVETo develop an HPLC method for determination of the four effective components (genistin; rutin; quercetin; genistein) in Huaijiao pill.
METHODThe chromatographic separation was performed on a Shim-packODS (4.6 mm x 150 mm, 5.0 microm) eluted with a mobile phase of MeOH-H2O-HAc (40:60:0.25). The detection wavelength was set at 256 nm and column temperature was set at 30 degrees C .
RESULTNice linear relation between the peak area and injected amount exists when the amount is within 0.059-0.352 microg for genistin, within 0.435-2.610 microg for rutin, within 0.020-0.121 microg for quercetin and within 0.053-0.319 microg for genistein. The correlation coefficient of each component is 0.999 6, 0.998 2, 0.998 9 and 0.999 9 respectively. The average recoveries of the four components are from 98.7% to 100.2%. The RSD of each group are 1.21%, 1.36%, 0.47% and 1.54% (n = 5).
CONCLUSIONThe method was accurate, repeatable and suitable to determine four effective components in Huaijiao pill. It can be use for quality control of Huaijiao pill.

8.G-protein Coupled Estrogen Receptor 1 Expression in Primary Breast Cancers and Its Correlation with Clinicopathological Variables.

PURPOSE: G-protein coupled estrogen receptor 1 (GPER) probably play important roles in the progression of breast cancer including endocrine therapeutic resistance. We evaluated GPER in primary breast cancers. METHODS: Immunohistochemistry was used to detect GPER in paraffin-embedded tissues of primary breast cancers from 423 patients and GPER expression was correlated with clinicopathological factors. RESULTS: GPER was expressed in 63.8% of specimens, coexpressed with estrogen receptor alpha (ERalpha) in 36.6% of tumors and was positive in 62.5% of the ERalpha-negative tumors. The expression of GPER had no relationship with the status of ERalpha, progesterone receptor and HER2. Although the expression of GPER was significantly inversely related with nodal status (p=0.045), no correlation between GPER expression and other clinicopathological variables (age, menstruation status, tumor size, stage, histologic grade, Nottingham Prognostic Index or pathological type) was found. CONCLUSION: GPER and ERalpha exhibited independent expression pattern of distribution in primary breast cancers. A long-term follow-up and a more definite molecular phenotype for ER are necessary in confirming studies.

9.Expression of bcl-10 protein in MALT lymphoma.

OBJECTIVETo investigate the significance of bcl-10 protein expression in extranodal marginal zone B-cell lymphoma of mucosa-associated lymphoid tissue (MALT) lymphoma.
METHODSSixty-two cases of MALT lymphoma were reviewed and immunohistochemical studies for bcl-10 and Ki-67 were performed.
RESULTSSixty out of the 62 cases studied (96.8%) were positive for bcl-10. Thirty-three (53.2%) showed bcl-10 expression in both the nuclei and cytoplasm, while 27 cases (43.6%) showed only cytoplasmic staining. The 10 cases with Hashimoto's thyroiditis demonstrated bcl-10 expression in the cytoplasm. The mean age of patients with bcl-10 nuclear expression (51.4 years old) was 5.2 years younger than those (56.6 years) without bcl-10 nuclear expression. The former category also showed a male predilection (male to female ratio = 19:14, in contrast to 10:19 in the latter category). The frequency of bcl-10 nuclear expression was lower in cases from thyroid but higher in cases from lung, stomach and intestine (P < 0.05). There was no statistically significant correlation between bcl-10 nuclear expression and clinical tumor stage (P > 0.05) or tumor cell morphology (P > 0.05). Amongst the 40 cases of gastrointestinal MALT lymphoma, bcl-10 nuclear expression correlated with extent of tumor involvement. The protein was expressed in 36.4% (4 out of 11 cases) of MALT lymphoma confined to mucosa or submucosa, 65.2% (15 out of 23 cases) of those invading down to muscularis propria or subserosa, and 100% (all 6 cases) of those extending beyond serosa (P < 0.05). There was no statistically significant difference in Ki-67 proliferative index between bcl-10-positive and bcl-10-negative groups (P < 0.05). Follow-up data were available in 52 patients (83.9%) and the five-year survival rate was no statistically significant difference in survival between bcl-10-positive (29 patients, 96.3%) and bcl-10-negative groups (23 patients, 66.4%, P > 0.05).
CONCLUSIONSTwo expression patterns of bcl-10 protein were observed in MALT lymphoma: mixed nuclear-cytoplasmic and cytoplasmic only. The bcl-10 nuclear expression appears more important and correlates with anatomic site of tumor and extent of tumor involvement. Immunohistochemical detection of bcl-10 may carry some diagnostic and prognostic implications in assessment of MALT lymphoma.

10.Expressions of survivin and nm23 and the relationship among the expressions and the axillary lymph node metastasis in breast cancer.

OBJECTIVE:
To investigate the expressions of Survivin protein and nm23 protein and the relationship among the expressions and axillary lymph node metastasis in breast cancer.
METHODS:
The expression of Survivin and nm23 in 80 cases of breast cancer tissues were detected by immunohistochemistry SP method, and their correlation with axillary lymph node metastasis and 5-year disease free survival (DFS) were analysed.
RESULTS:
Survivin protein positive expression rate was 68.75% (55/80) in breast cancer tissues, which had positive correlation with the axillary lymph nodes metastasis but negative correlation with 5 years FS (P < 0.05); nm23 protein expression had negative correlation with the axillary lymph nodes metastasis but positive to 5 years FS (P < 0.05). Survivin and nm23 proteins expression had no obvious correlation with the breast cancer pathology type, patient age and clinical stage (P > 0.05).
CONCLUSION
The anti-apoptosis effect of Survivin protein and the anti-metastasis effect of nm23 protein may be important in the occurrence and advancement of breast cancer, suggesting that it may be a new indicator of prognostic and judgement in breast cancer.
