1.Effects of penehyclidine hydrochloride on the diaphragm of rats with omethoate poisoning

Objective To study the morphological changes of diaphragm of rats with omethoate poisoning and the protection of penehyclidine hydrochloride.Methods The experimental model of Wistar rats was made by eliac injection ofomethoate,96 rats was divided randomly into four groups in average:the hrinematched control group (Group NO);the omethoate intoxication matched control group (Group PO);atropine and pralidoxime chloride cure group (Group AC); penehyclidine hydrochlofide and pralidoximc chloride cure group (Group PC).The whole blood cholinesterase (ChE) and creatinekinase (CK) activtitise were measured 2 h after poisoning.To observe the morphological changes of diaphragmat different time from 1 to 7 days.Results All the poisoned rats showed that the diaphragmatic histologic damage of the penehyclidine hydrochloride and pralidoxime chloride cure group was slighter than that of atropine cure group.Conclusion A possible reasons of the respiratory muscle paralysis conduced by AOPP was the putrescence of diaphragm muscle fiber, and penehyclidine hydrochloride could definitely protect the diaphragm of rats with omethoate poisoning,and we could deduce that it prevented from intermediate myasthenia syndrome.

Objective To evaluate the cost-effectiveness of early enteral nutrition (EEN) in elder patients undergoing gastrectomy for gastric carcinoma. Methods An outcome-based retrospective review of 52 patients who had undergone gastrectomy for gastric carcinoma between July 1999 and June 2002 was performed .There were 27 patients in the EEN group, and 25 in the TPN group. Objectives To discuss the CT features of primary tuberculosis in adults Methods CT images of 39 adult patients with primary tuberculosis were retrospectively analyzed and 30 cases were also examined after injection of contrast materials Results 39 patients had both mediastinal tuberculous lymphadenitis and parenchymal infiltration There were 25 cases (64 1%) with primary lesions involving the right lung and 14 cases (35 9%) in left side 28 cases(71 8%) appeared as patchy shadows,nodules,and lobular consolidation; 11 cases (28 2%) were with segmental or lobar consolidation In 30 cases with contrast enhanced CT,the mediastinal tuberculous lymphadenitis appeared as homogenous enhancement in 10 cases, irregular enhancement in 18 cases,central low attenuation and peripheral rim enhancement in 20 cases 14 cases showed that the nodes were confluenced with inhomogenous enhancement There were 26 cases with acute bronchogenic spread tuberculosis Conclusion The parenchymal infiltration and mediastinal tuberculous lymphadenitis are the basic features in adult primary tuberculosis Acute bronchogenic spread into other fields and the features of node enhancement after injection of contrast materials are very important findings in the diagnosis of primary tuberculosis
4.The CT findings of endobronchial spread in lung adenocarcinoma

Objective To analyze the CT findings of endobronchial spread in lung adenocarcinoma. Methods The CT findings of 15 lung adenocarcinomas or bronchioloalveolar carcinomas with endobronchial spread were reviewed,the distribution and the progression of the spread were evaluated.Results All of the primary tumors were consolidation form.The spread lesions distributed in one side of the lung or both sides along the bronchus.The pleural surface was spared.The CT findings of the spread included centrilobular nodules(n=5),tree-in-bud(n=7),acinar nodules(n=2),ground-glass opacities(n=10)and air- space consolidations(n=13)in the first CT examination.5 cases of the spread lesions only presented centrilobular nodules(single form)and 10 cases presented several appearances(complex form).All of the cases were diagnosed as tuberculosis or pneumonia,and antituberculotic or antibiotic therapy was taken with no effect.The follow-up CT scans showed progression in all cases,and the spread lesions with single form became multiple consolidations.The spread lesions with complex form deteriorated faster than the single one. Conclusion Although the CT findings of the endobronchial spread of the lung adenocarcinoma is specific, the clinical history and laboratory examination also are important for the differential diagnosis with tuberculosis and other infectious diseases.

Objective To determine whether the susceptibility to ischemia-reperfusion injury in aged heart is higher than that in adult heart and,if so,to clarify the mechanisms underlying this change.Methods Wister rats(5-or 20-month-old)were randomly divided into 4 groups(6 animals in each group).The rats were subjected to 30 minutes of myocardial ischemia via ligating the left anterior descending coronary artery,followed by 3 hours of reperfusion(Young-MI/R group and Old-MI/R group);A silk suture around the left anterior descending coronary artery was not ligated in young and old rats(Young-sham group and Old-sham group).Myocardial apoptosis was detected by terminal deoxynueleotidyl transferase biotin-d UTP nick end labeling(TUNEL)staining and caspase-3 activity was detected by using a caspase-3 colorimetrie assay.Nitrotyrosine content,a footprint of in vivo ONOO~-formation,and total NO content were determined by ELISA and chemiluminescence method respectively.Results A significantly exacerbated cardiac reperfusion injury was found in Old-MI/R group as evidenced by increased TUNEL positive myocytes[(19.0?2.1)% vs.(14.6?1.7)%],and increased myocardial caspase-3 activity[(436?35)?mool/mg vs.(340?32)?mol/mg] compared with Young-MI/R group(P<0.05).Aged hearts had a markedly increased basal NOx level compared with young adult hearts.Marked higher myocardial nitrotyrosine content was found in OId-MI/R group[(7.25?0.18)nmol/g]than that in Young-MI/R group[(4.68?0.15)nmol/g] (P<0.05).Conclusions In aged hearts,high levels of NO might form highly toxie derivant, ONOO~-,and its subsequent nitrified protein.This may attribute to the increased susceptibility of the aged heart to isehemic-reperfusion injury.

Objective To understand family' s feeling in ICU,analyze the influencing factors of family satisfaction,and provide references for effectively improving medical care quality.Methods 129 family members were investigated by family satisfaction with care in the intensive care unit (FS-ICU),2 family members were interviewed,and their satisfaction was summarized and influencing factors were analyzed.Results The average score of family satisfaction with ICU was (85.86 ± 20.18).Their satisfaction was related to the attention to family needs,polite,respect and compassion to the family,the atmosphere of ICU,honesty of the disease,support during decision-making (t =2.980,4.118,3.704,2.539,2.871,receptively; P < 0.05).Conclusions Medical staff' s respect and compassion,and family' s demand meeting,involvement in decisionmaking and getting support play an important role in improving family satisfaction.The improvement of service ideology and hardware facility is helpful to enhance medical and nursing quality.

OBJECTIVETo report the clinical, genetic, and molecular characterization of two Chinese families with Leber's hereditary optic neuropathy (LHON).
METHODSOphthalmological examinations showed that only probands in two families exhibited visual loss at the age of 10 and 17 years respectively. The entire mitochondrial genome of two probands was PCR amplified in 24 overlapping fragments using sets of oligonucleotide primers.
RESULTSMutational analysis of mitochondrial DNA (mtDNA) in these pedigrees revealed the absence of three common LHON associated G11778A, G3460A and T144484 mutations but the presence of homoplastic LHON associated ND4 G11696A mutation, which was present in one out of 167 Chinese healthy controls.
CONCLUSIONSequence analysis of the complete mitochondrial genomes in two pedigrees showed the distinct sets of mtDNA polymorphisms, belonging to Eastern Asian haplogroup D4. The incomplete penetrance of visual loss and the presence of one in 167 controls suggested that this mutation itself is insufficient to produce a clinical phenotype and other modifier factors play a role in the phenotypic manifestation. The lack of functional mtDNA variants in these pedigrees ruled out the role of mitochondrial background in the phenotypic expression of visual loss. Therefore, nuclear modifier gene(s) or environmental factor(s) may play a role in the phenotypic expression of the LHON-associated G11696A mutation in two Chinese pedigrees.
