1.In vitro analysis of the effect of cyclic loading on the fracture resistance of teeth restored with different post and core systems.

OBJECTIVEThis study aims to compare the fracture resistance of teeth restored with different post and core systems by cyclic loading test and to predict long-term effects.
METHODSA total of 15 recently extracted mandibular premolars without crowns were endodontically treated. The teeth were randomly divided into three groups with five teeth in each group. Group A, teeth restored with cast metal post and core. Group B, teeth with fiber post and resin core. Group C, teeth with resin filling. All of the teeth were restored with full-cast metal crowns. The specimens were exposed to 300,000 cyclic loadings in a cyclic loading machine and 1,000 thermal cycles between 5 and 55 °C. All of the samples were loaded in AG-IS mechanical testing machine with a compressive load until fracture occurred. The fracture load and the fracture mode of each tooth were recorded.
RESULTSThe fracture resistance in group B and C were higher than that of the cast metal post and core restoration (P < 0.05). Repairable fractures were observed in group B and C, whereas unrestorable and catastrophic fractures were found in group A.
CONCLUSIONFiber post and resin filling is a good choice to restore endodontically treated teeth.

2.Gene therapy and cutaneous wound healing

Cutaneous wound healing is a complicated multiatep process with numerous mediators that act in a network of activation and inhibition processes. Nonhealing chronic wounds decrease the quality of Life. Recombinant growth factors, currently used in clinic, fail to provide a sustained repair function at the wound due to their inadequate biological availability and transient half-time. Recent attention has focused on gene therapy, which might become a significant treatment modality for those wound healing pathologies refractory to other wound management approaches. This review discusses the potentials and limitations of current genetherapy for the treatment of wounds, current ongoing clinical trials, and possible future directions in this exciting field.
3.Efficacy comparison of different operations in treatment of papillary thyroid carcinoma

Objective To compare the operative efficacy of low collar incision and L-shaped incision plus cervical plexus preser-ving lymphadenectomy in treating papillary thyroid carcinoma.Methods 9 3 inpatients with papillary thyroid carcinoma were ran-domly divided into the low collar incision group (n=37)and the L-shaped incision group(n=56)according to the random number table.The low collar incision group underwent the low collar type cervical plexus preserving lymphadenectomy,while the L-shaped incision group received the L-incision cervical plexus preserving lymphadenectomy.The postoperative 24-month follow up was per-formed.Then the clinical efficacy and the life quality of the patient were evaluated and compared between the two groups.Results (1)1 case of lymph node recurrence in the district Ⅱ after 20 months was found in the low collar incision group,1 case of lymph node recurrence in the district Ⅰ after 12 months and 1 case of lymph node recurrence in the districtⅣ after 22 months was found in the L-shaped incision group;no case of distant metastasis in the low collar incision group occurred during the follow up period, but 1 case of pulmonary metastasis in postoperative 5 months and 1 case of bone metastasis in postoperative 24 months occurred in the L-shpaed incision group.The differences in the occurrence rates of postoperative recurrence and metastasis had no statistical sig-nificance (2.70%vs.7.14%).(2)The scores of neck shoulder pain and numbness,incision appearance satisfaction degree in the low collar incision group were significantly higher than those of the L-shaped incision group (P<0.05).Conclusion Compared wtih L-shaped incision cervical lymphadenectomy,low collar incision cervical plexus preserving cervical lymphadenectomy has simi-lar high clinical efficacy,can significantly reduce neck shoulder pain and numbness,meet aesthetic requirements and especially is suitable for the patients with higher demands of the neck function preserving and the neck appearance.
4.The mechanism of the action of dipeptidyl peptidase-4 inhibitors and its use in type 2 diabetes

The critical role of α- and β-cell dysfunction in the development of type 2 diabetes has been established.Dipeptidyl peptidase-4 (DPP-4) inhibitors are new class of oral anti-hyperglycemia agents that acts to increase active levels of the incretin hormone glucagon-like peptide-1 ( GLP-1 ) and glucose-dependent insulinotropic polypeptide ( GIP),which improved α- and β-cell function.This action is shown as improved α- and β-cell sensitivity to glucose,increased glucose-dependent insulin secretion and decreased glucagon secretion.DPP-4 inhibitors treatment has also been associated with beneficial extrapancreatic effects,including improved peripheral insulin sensitivity and lipid metabolism.Additionally,DPP-4 inhibitors have shown a minimal risk of hypoglycemia and bodyweight neutrality without effects on gastric emptying.Numerous clinical trials have verified the favorable efficacy,safety,and tolerability of DPP-4 inhibitors treatment in monotherapy or combination with other anti-diabetic agents for patients with type 2 diabetes.
5.Technique and its significance of detecting colorectal cancer cells by fluorescent amplification catalyzed with T7 RNA polymerase

Objective To develop a new method for detecting colorectal cancer cells in peripheral blood using a hybrid technology referred to as fluorescent amplification catalyzed with T7 polymerase technique (FACTT). Method A model of colorectal cancer circulating tumor cells in peripheral blood was reproduced by mixing serial dilutions of HT29 cells with 1?107 peripheral blood monocytes from healthy subjects. After all nucleated cells were isolated from the model, biotinylated monoclonal antibody CEA against the Gold epitopes Ⅳ, which was expressed on the cell surface of human colorectal cancer cells but not on the normal cells, was added followed by the adding of avidin-biotinylated-DNA. Then T7 RNA polymerase and nucleoside triphosphates (NTPs) were added to perform RNA amplification at room temperature for 3h. Finally, RNA products were quantified by adding the RNA intercalating dye RiboGreen. RT-PCR was used to detect the expression of CEA mRNA as a control to assess the sensitivity of fluorescent amplification catalyzed with T7 polymerase technique. Results A method of detecting circulating colorectal cancer cells in peripheral blood by fluorescent amplification catalyzed with T7 polymerase technique was established. It was shown that the technique had a detection level of 5 HT29 cells among 1?107 monocytes, and it was more sensitive than RT-PCR technique, by which the level of detection was 1?102 cells among 1? 107 monocytes. Conclusion Fluorsescent amplification catalyzed with T7 polymerase technique is a new and sensitive technology for detecting circulating colorectal cancer cells in peripheral blood.

7.The effect of several cytokines combinations on in vitro expansion of mononuclear cells from cord blood and their expression of adhesive molecules and CXCR4

8.Radiotherapy of primary cardiac neoplasm

Objective To evaluate the result of radiotherapy for primary cardiac neoplasm.Methods A patients with primary malignant mucinous Schwannoma in th left ventriclular wall had been treated by conservative surgery. He,then,received fast neutron radiotherapy one year after wards. Radiotherapy was given through opposing anterior and posterior fields of 7.8 cm?7.8 cm. The total radiation dose was 1?004.95 ncGy with 11 fractions of 100 ncGy in 48 days.Results The function of the patient's heart recovered after treatment. The patients has been followed for 5 years without evidence of recurrence or distance metastased. Conclusion Fast neutron radiotherapy is effective in the treatment of primary cardiac neoplasms by surgery combined with postoperative radiotherapy. It could improve the prognosis of patients with primary cardiac neoplasms.

10.Clinical characteristics and classification of ketosis-prone diabetes

Objective To investigate the clinical characteristics and classification of ketosis-prone diabetes. Methods Firstly,according to with or without autoantibodies,patients with an initial onset of unprovoked ketosis or ketoacidosis were classified to two different groups. Secondly,based on weather or not depending on insulin,patients without autoantibodies were further divided into two groups,insulin dependent and non-insulin dependent. Clinical characteristics,biochemical parameters were compared between different groups. Results The autoantibodies negative group showed a strong male predominance and family history of diabetes.BMI was higher than type 1 diabetes. Plasma triglyceride level was highest. Fasting and postprandial C-Peptide level was between the other two group. Compared with non-insulin dependent group,insulin dependent group showed a strong male predominance,lower ratio of overweight and obesity,more severe ketosis status. There were no significant differences with regard to blood glucose level and fasting C-Peptide level of initial. But fasting C-Peptide after 3 months was higher in non-insulin dependent group . Conclusion Ketosis-prone diabetes can divided into two groups,based on autoantibody negative or positive. The latter consist of type 1A diabetes mellitus. The former can further classified as ketosis-prone type 2 diabetes and idiopathic type 1 diabetes(type 1B diabetes)according to insulin dependent or non-insulin dependent.
