Based on the concept of environmental factors of ICF, together with the practice of environmental assessment and reform in the recent years, this paper discussed the principle, specification, procedures and content of environmental assessment.

Objective To improve the knowledge about controlling plague in cadres, masses, and the medical staff in plague affected area in Dinghian county of Shaanxi province and to assess the efforts of health education activities. Methods In 2008, the education activities carded out by the government-related departments were investigated. The awareness of plague control and assessment was obtained through a written survey, questionnaire and interviews on clinic. Results Some education activities were carried out in plague area of Dingbian county, such as training, issuing educational materials, broadcasting plague scientific educational films and arranging knowledge grand prix. The rates of knowing plague clinic, epidemiology, prevention and the "three prohibitions and three alerts to report" were as follows: the cadres were 50.50%(101/200),63.69%(414/650),78.67%(118/150), the masses were 64.71% (66/102),87.91% (269/306),76.47% (78/102) and the medical staff were 64.18% (543/846) ,68.51%(322/470),67.02%(63/94). The passing rates of the cadres, the masses and the medical staff were 70.00% (35/50),92.16% (47/51),74.47% (35/47). Conclusions Health education can strengthen health consciousness of cadres and masses and improve the ability of the medical stalf on controlling sudden epidemic situation. Reinforcing plague control knowledge and training of medical staff are still important for health education in the future.

OBJECTIVETo study the effects of rhizoma Astilbes chinensis in abirritation, blood activation, cough relieving and sputum elimination.

METHODThe antalgic function of rhizoma A. chinensis was tested by hot-plate method and writhing reaction. The acute blood-stasis model rats were made by Injection of adrenaline hydrochloride along with stimulation by ice water. The effects of cough relieving and sputum elimination were observed by the ammonia water-induced tussive mice and excretion of phenol red in the airway of mice. The maximum tolerance dose of rhizoma A. chinensis was also determined during the acute toxicity test.

RESULTThe data were analyzed for statistical significance by t-test, which shows that the decoction of rhizoma A. chinensis is significantly effective in reducing the frequency of licking behavior of mice on hot-plate and writhing response induced by acetic acid, improving the hemarheology of acute blood-stasis model rats, prolonging the latent period, reducing the frequency of cough induced byammonia, and in increasing the quantity of phenol red output from the trachea in mice. The result acute toxicity test shows that maximum tolerance dose of gastrogavage in mice was 400 g x kg(-1). Which was 666. 7 times of that clinically used for human.

CONCLUSIONRhizoma A. chinensis has the effects of abirritation, blood activation, cough relieving and sputum elimination, and is safe in clinical application.

objective To identify the bacterial pathogen in food and the source of infection that might be responsible for a food poisoning outbreak.Methods All the probable cases that had attended the banquet in a villige of Sichuan province who had developed fever(≥37.5℃),diarrhea (≥3 times/dav)and vomiting since August 1,2009,were identified.A case-control study was conducted to identify the foods that might be responsible for this food poisoning event.50 cases were randomly selected from the probable cases and 50 controls were randomly selected from those without the symptoms.Results The attack rate of this food poisoning event was 43.4%(132/304).86.0%(43/50)of the cases and 34.0%(17/50)of the controls had eaten pork-mixed-vermicelli(OR=12.0,95%CI:4.4-32).Data from further tests showed that dose-effect relation existing between eating pork mixing vermicelli behavior and the incidence rates(Chi-square trend:x2=21.06,P=0.00)of the disease.94.0%(47/50)of the cases and 68.0%(34/50)of the controls had eaten mushroom braising chicken(OR=7.4.95%CI:2.0-27).Data from the crossover analysis showed that there appeared an effect modification between behaviors of eating pork-mixed-vermicelli and eating mushroom braising chicken.Three cases were detected carrying Salmonella blegdam from their anus swabs and the same bacteria type was also detected in the pork-mixed-vermicelli. Conclusion This outbreak of salmonellosis was caused by Salmonella blegdam contaminated pork-mixed-vermicelli or mushroom braising chicken during food processing.

OBJECTIVETo evaluate the role of buccal-fat-pad flap in the lined coverage of the maxillary bone graft for the maxillary reconstruction after the tumor excision.

METHODSBased on the cadaver anatomical observation of the buccal-fat-pad, 38 patients with the maxillary tumors were undertaking the treatment in this paper. Under general anesthesia, the buccal-fat-pad flap was elevated and transferred to the anterior or nasal cavities as a line of the sinus after the tumor was immediately excised through an intro-oral or external approach. The bone from the ribs was then grafted in the place for the maxillary support.

RESULTSAfter the anatomical evaluation, 38 patients were carried out the operation successfully. All of the grafted bones were well covered without bone exposure. The aesthetic appearance was achieved.

CONCLUSIONBone graft lined by the pedicled buccal fat pad flap may be a safe and good method for reconstruction of the maxilla with a lower rate of re-absorption and sequestration.

Objective To evaluate the condition of rebuilding stove for preventing coal-buming fluorosis in Ankang City of Shaanxi Province in 2006.Methods Guided by local health administration department in Shaanxi.five counties-Zhengping,Langao,Hanyin,Ziyang,Shiquan-were chosen as rebuilding stove regions in Ankang City.Fifteen housewives were chosen randomly in each village in each county,they were quizzed for the knowledge of health.Fifty students were chosen randomly in each school in each county,they were quizzed for the knowledge of health too.Data of rebuilding stove were refferred.Results Ninty-seven point two per cent(243/250)of the ovens were appropriately sealed,96.8%(242/250)were efficiently exhausted,the rate of oven correctly used was 97.2%(243/250),the awareness rates of healthful knowledge were 85.6%(214/250)in adults and 97.8%(485/496)in children.Data of rebuilding stove were documented systemically,completely and precisely.ConelusionFive model counties in Ankang City have fulfilled the demand.

OBJECT: To obtain values of normal human lung and diffuse pathological changes, in order to provide a simple and convenient diagnostic method for measuring human visceral organs in the autopsy of pathology and forensic pathology. METHODS: The exact mass, volume and density of normal and pathological lung were synchronously measured with the intelligentized volume-densimeter made by authors. RESULTS: The date-base about the volume and density of human lung with different races were established for anatomy, anthropology and biodynamics. In two cases of over-load in circulation and water toxicosis, the severe lung edema was proved with the scale of lung density, which was atypical edema in the lung tissue. CONCLUSION Measurement of visceral organs in the autopsy can assist to an integrative pathological diagnosis.

OBJECTIVETo use randomized controlled clinical research method to assess therapeutic effect of picking therapy on cervical spondylosis.

METHODSOne hundred and fifty-eight cases were randomly divided into a picking therapy group (n=56), a routine acupuncture group (n=55) and a local anesthesia group (n=47). They were treated respectively with picking therapy, routine acupuncture and local anesthesia at Jing bailao (EX HN 15), Dazhui (GV 14), Jianjing (GB 21), etc. Brief McGill Pain Questionaire was used for score, which was combined with clinical symptoms and signs to analyze the therapeutic effect.

RESULTSThe cured rate was 57.1% in the picking therapy group, better than 23.6% in the acupuncture group and 14.9% in the local anesthesia group (P < 0.01), and adverse reaction was basically not found in the picking therapy group.

CONCLUSIONPicking therapy is a highly effective and safe therapy for cervical spondylosis.

Objective During June 2-8, 2009, 11 cases of the novel influenza A (H1N1)occurred in Sichuan Province, China. We investigated this outbreak to identify the source of infection,mode of transmission and risk factors for infection. Methods The primary case, a U.S. citizen,developed disease on June 2. From June 3 to 5, she joined Tour Group A for a trip to Jiuzhaigou. We telephoned passengers of the three flights on which the primary case had traveled in China, and members of Tour Group A. We asked whether they had any influenza-like symptoms during May 27 to June 12. Health authorities placed passengers whose seats were within three rows of the primary case on flights and members of Tour Group A on medical observation, and isolated individuals if they developed symptoms. We used real-time RT-PCR to test the throat swabs from symptomatic persons for the novel influenza virus and defined a confirmed case as one with influenza-like symptoms and laboratory confirmation. A retrospective cohort investigation to identify the risk factors for infection was conducted. We interviewed all members of Tour Group A about their detailed contact history with the primary case. Results During June 5 to 6, 9 (30%) of the primary case' s 30 fellow tour group members developed disease, compared with none of her 87 fellow passengers to Jiuzhaigou and 1 of her 87 fellow passengers on the returning trip (when several of the members of Tour Group A were symptomatic). 56% of the tourists who had talked with the primary case in close range ( ＜2 m) for ≥2 minutes developed disease, whereas none of the 14 other tour group members developed disease (RR= ∞; exact 95%CI: 2.0- ∞ ). Having conversed with the primary case for ≥ 10 minutes (vs. 2-9 minutes) increased the risk by almost five fold (RR=4.8, exact 95%CI: 1.3-180). Conversely, other kinds of contact, such as dining at the same table, receiving chewing gum from the primary case and sharing bus rides or planes with the primary case played no roles during this outbreak. Conclusion This novel influenza A (H1N1) outbreak was caused by an imported case, and transmitted mainly via droplet transmission when the primary case was talking with her fellow tourists during a tour. These findings highlight the importance of preventing droplet transmission during a pandemic.
