1.Minimally Invasive Percutaneous Nephrolithotomy for the Treatment of Impacted Upper Ureteral Calculi: A Report of 49 Cases

Objective To investigate the efficacy of minimally invasive percutaneous nephrolithotomy(MPCNL) for impacted upper ureteric calculi.Methods Retro-urethral catheterization was conducted in the diseased ureter under cyctoscope.After a channel from the skin surface into the middle calyx was established under the guidance of X-ray or B ultrasonography,pneumatic lithotripsy was conducted to break up stones under ureteroscopy.Results The success rate of pneumatic lithotripsy was 100%.The rate of postoperative macroscopic hematuria was 55.1%(27/49).The symptom lasted one to three days in 26 cases,and 1 case had apparent hematuria with blood clot flowing out from nephrostomy tube,which lasted 8 days.10.2%(5/49) of cases was complicated with postoperative pyrexia.The stone-free rate in one week and one month postoperatively was 93.9%(46/49) 98.0%(48/49),respectively.Follow-up observations in 49 cases for 1-12 months(mean,5 months) showed no major complications.Conclusions MPCNL for impacted upper ureteral calculi has the advantages of simple performance,less complications and satisfactory efficacy.

Objective To investigate the existence of epithelial-mesenchymal transition(EMT)and its relation to the expression of HBx protein in hepatocellular carcinoma(HCC).Methods The expression of HBx protein,E-cadherin,β-catenin,N-cadherin and fibronectin were detected by immumnohistochemistry technique in 76 cases of HCC.Results Among 76 HCC samples,the loss of expressions of E-cadherin and β-catenin were 34%(26/76)and 20%(15/76),respectively.The positive expressions of HBx protein,N-cadherin and frbronectin were observed in 68%(52/76),55%(42/76),46%(35/76)of HCC samples,respectively.The loss of expression of E-cadherin significantly correlated with the positive expression of N-cadherin and HBx protein (P<0.01).The positive expression of N-cadherin and fibronectin significantly correlated with the loss of expression of β-catenin and the positive expression of HBx protein(P<0.05).Conclusions EMT exists in hepatocellular carcinoma,and the expression of HBx is significantly associated with EMT.

6.The effect of comprehensive intervention program on early postoperative enteral nutrition tolerance and recovery of patients with esophageal cancer

Objective:To explore the effect of comprehensive intervention program based on the concept of fast track surgery in early enteral nutrition tolerance and rehabilitation in patients with esopha geal cancer.Methods:93 selected patients were divided into the intervention group and the conventional control group according to the admission time.Patients in intervention group were treated with comprehensive intervention,and control group were given routine nursing care.Enteral nutrition intolerance incidence,the first time of bowel sounds,the first time exhaust and defecation and complications were compared between the two groups.Results:36 patients from 47 patients (76.60%) in the intervention group could tolerate the early feeding,while 24 patients from 46 patients (52.17%) in the control group could tolerate.There was significant difference between two groups (P < 0.05).The first time of exhaust,bowel sounds and defecation,and the hospitalization time of patients in the intervention group were better than that in the conventional control group (P < 0.01).Incidence rate of incision infection,pulmonary infection and intestinal obstruction in patients of the comprehensive intervention group was lower than that in the control group (P < 0.05).Conclusion:Compared with the traditional treatment,the comprehensive intervention can effectively reduce the enteral nutrition intolerance incidence,promote the recovery,reduce hospitalization time and the incidence of postoperative complications,and improve the life quality of patients after surgery.
7.Research on climatic factors of ecology suitability regionalization of atractylodis.

Through study on the correlation between atractylodis lactones ingredient content and climatic factors, we research regionalization from climatic of five main producing provinces of the country, in order to provide a scientific basis for atractylodis' conscious cultivation. By sampling from 40 origins which from five main producing provinces of the country, we use SPSS to analysis variation of atractylodis lactones ingredient content in different conditions of climatic factors and the effect of each factors. Then according to the relationship between atractylodis lactones ingredient content and climatic factors, we use ArcGIS to conduct ecological suitability regionalization based on climatic factors. The most suitable climatic condition for cultivation of atractylodis: the wettest month precipitation 220-230 mm, the warmest average temperature 25 degrees C, the average temperature of driest season 10 degrees C.

10.Influence of hepatic ischemia-reperfusion injury induced by Pringle maneuver on the prognosis of hepatocellular carcinoma patients after hepatectomy

Objective To evaluate the influence of hepatic ischemia-repeffusion injury (HIRI) induced by Pringle maneuver on the prognosis of hepatoceUular carcinoma (HCC) patients after hepatectomy. Methods The chnical data of 315 HCC patients who had been admitted to Southwest Hospital from January 2004 to December 2008 were retrospectively analyzed. The 194 patients who received Pringle maneuver during hepatectomy were in the HIRI group. The control group was composed of 121 patients without portal triad clamping. The pre- and peri-operative characteristics and the prognosis of the patients were analyzed by t test, chi-square test, Kaplan-Meier survival curve, Log-rank test and Cox regression model analysis. Results Patients in the HIRI group were significantly younger than those in control group (median age, 49 vs 59) (X2 =4. 12, P < 0.05). There were 108 patients (55.7%) with large HCC (diameter > 5 cm) in the HIRI group, while the number of patients with large HCC in the control group was 83 (68.6%), with statistical difference between the 2 groups (X2=4. 12, P <0.05). The serum levels of aspartate aminotransferase on postoperative day 3 and day 7 were 255 U/L and 112 U/L, which were significantly higher than 128 U/L and 35 U/L in the control group (X2 =4.57, 5.89, P <0.05). The level of total bilirubin on postoperative day 3 was 56 U/L in the HIRI group, which was significantly higher than 39 U/L in the control group (X2=4.79, P <0.05). The disease-free survival rate and cumulative survival rate in the HIRI group were significantly lower than those in the control group (X2 = 5.93, 8. 32, P < 0. 05). Perioperative blood loss, diameter of tumor, portal triad clamping and portal vein invasion were independent factors influencing the diseasefree survival rate. Conclusions HIRI induced by Pringle maneuver significantly decreases the disease-free survival rate and cumulative survival rate of HCC patients after hepatectomy.
