1.Repeated elevation of carcinoembryonic antigen induced by alendronate sodium in an elder osteoporosis woman

Objective One case of repeated elevation of carcinoembryonic antigen (CEA) induced by alendronate sodium in an elder osteoporosis woman was introduced to draw physicians'attention.Methods One elderly women with osteoporosis was treated with oral alendronate sodium 70 mg,once a week.The serum CEA was detected at an regular interval.The relationship between CEA elevation and alendronate sodium therapy was evaluated with Naranjo's assessment scale.Results and conclusion The serum level of CEA was repeatedly observed to gradually increase several weeks after treatment,while gradually decline after discontinuing the medication.The elevation of CEA took placed in a sequential manner in relation to the administration of alendronate sodium.Naranjo's assessment gave a total score of 8 points,indicating that CEA may be associated with the use of alendronate sodium.
2.Influence of intramuscular injection of vitamin D2 on serum 25-hydroxyvitamin D in aged women

Objective To evaluate the influence of intramuscular injection of vitamin D2 on serum 25-hydroxyvitamin D in aged women.Methods A total of 40 aged women were randomly divided into treatment group and control group,each group 20 patients.Patients in treatment group were given vitamin D2 injection 6.0 × 105 U every month,intramuscular injection,for 3 months.Control group was given sunshine treatment The levels of serum 25-hydroxyvitamin D and calcium were evaluated and compared before treatment and after 8 months treatment.Results After 3 months treatment,serum 25-hydroxyvitamin D in treatment group and control group were (19.76 ± 3.28),(16.37 ± 3.01)ng · mL-1,with significant difference (P < 0.01),and the levels of serum 25-hydroxyvitamin D in other times had no significant difference (all P > 0.05).There was no significant difference in serum calcium between treatment and control group during 9 months (all P > 0.05).Conclusion Vitamin D2 injection could be used to increase serum 25-hydroxyvitamin D level,and it had no effect on serum calcium.
3.Appraisal of clinical practice guidelines for ischemic stroke management in Chinese medicine with appraisal of guidelines for research and evaluation instrument: A systematic review.

OBJECTIVETo systematically review the clinical practice guidelines (CPGs) for ischemic stroke in Chinese medicine (CM) with the Appraisal of Guidelines for Research and Evaluation (AGREE II) instrument.
METHODSCM CPGs for ischemic stroke were searched in 5 online databases and hand-searches in CPGrelated handbooks published from January 1990 to December 2012. The CPGs were categorized into evidence based (EB) guideline, consensus based with no explicit consideration of evidence based (CB-EB) guideline and consensus based (CB) guideline according to the development method. Three reviewers independently appraised the CPGs based on AGREE II instrument, and compared the CPGs' recommendations on CM pattern classification and treatment.
RESULTSFive CM CPGs for ischemic stroke were identified and included. Among them, one CPG was EB guideline, two were CB guidelines and two were CB-EB guidelines. The quality score of the EB guideline was higher than those of the CB-EB and CB guidelines. Five CM patterns in the CPGs were recommended in the EB CPG. The comprehensive protocol of integrative Chinese and Western medicine recommended in the EB CPG was mostly recommended for ischemic stroke in the CPGs. The recommendations varied based on the CM patterns.
CONCLUSIONThe quality of EB CPG was higher than those of CB and CB-EB CPGs in CM for ischemic stroke and integrative approaches were included in CPGs as major interventions.

Patients with severe trauma require an extremely timely treatment and transfusion plays an irreplaceable role in the emergency treatment of such patients. An increasing number of evidence-based medicinal evidences and clinical practices suggest that patients with severe traumatic bleeding benefit from early transfusion of low-titer group O whole blood or hemostatic resuscitation with red blood cells, plasma and platelet of a balanced ratio. However, the current domestic mode of blood supply cannot fully meet the requirements of timely and effective blood transfusion for emergency treatment of patients with severe trauma in clinical practice. In order to solve the key problems in blood supply and blood transfusion strategies for emergency treatment of severe trauma, Branch of Clinical Transfusion Medicine of Chinese Medical Association, Group for Trauma Emergency Care and Multiple Injuries of Trauma Branch of Chinese Medical Association, Young Scholar Group of Disaster Medicine Branch of Chinese Medical Association organized domestic experts of blood transfusion medicine and trauma treatment to jointly formulate Chinese expert consensus on blood support mode and blood transfusion strategies for emergency treatment of severe trauma patients ( version 2024). Based on the evidence-based medical evidence and Delphi method of expert consultation and voting, 10 recommendations were put forward from two aspects of blood support mode and transfusion strategies, aiming to provide a reference for transfusion resuscitation in the emergency treatment of severe trauma and further improve the success rate of treatment of patients with severe trauma.
