1. Three treatments for moderate hypertensive intracerebral hemorrhage: A comparative therapeusis

Objective: To compare the therapeutic efficacy of small skull window microsurgery, minimally invasive surgery for crashing and aspirating of hematoma (MISCAH) , and conservative management for moderate hypertensive intracerebral hemorrhage treatment. Methods: A total of 106 consecutive patients with moderate hypertensive intracerebral hemorrhage (hematoma volume 20-40 mL; Glasgow coma scale[GCS] scores 9 - 12) in unilateral basal ganglion regions were selected. They were randomly assigned to small skull window microsurgery group (n = 31), MISCAH group (n = 36), and conservative treatment groups (n = 39). The recurrent rehemorrhage rate, mortality rate, and disability rate 3 months after the procedure were observed. Three patients in the MISCAH group and six patients in the conservative treatment group eventually underwent microsurgery through small skull window for evacuation of hematoma. Results: Circled digit oneThe volume of hematoma evacuated in the small skull window microsurgery group was >75%. Among them, 9 were almost evacuated completely. The volume of hematoma evacuated in the MISCAH group was <50%. Circled digit twoThe recurrent hemorrhage rates in the small skull window microsurgery group, MISCAH group, and conservative treatment group were 6.5% (2/31), 30.3% (10/33), and 45.5% (15/33), respectively. Their mortality rates were 3.2% (1/31), 24.2% (8/33), and 48.5% (16/33), respectively. The disability rates were 35.7% (10/28), 68.0% (17/25), and 73.3% (11/15), respectively. There were significant differences of the recurrent hemorrhage rate, mortality rate, and disability rate between the small skull window microsurgery group and both the MISCAH group and conservative treatment groups(P<0.01, P<0.05). There was no significant significance in the recurrent hemorrhage rate and disability rate between the MISCAH group and the conservative treatment group(P>0.05), except the mortality rate (P < 0.05). Conclusion: Microsurgery through small skull window for evacuation of hematoma is the most safe and effective method for treatment of hypertensive intracerebral hemorrhage. The MISCAH can decrease the mortality rate of the patients, but it can not reduce the recurrent hemorrhage rate and disability rate. The conservative treatment was only suitable for patients with small amount of bleeding, without disturbance of consciousness, and with mild neurological deficits.
2.Research progress in strategy of lateral femoral offset reconstruction in total hip arthroplasty.

As an important indicator of total hip arthroplasty (THA) the rate and degree of offset reconstruction play an important role in improving the prognosis and life quality of patients. The reconstruction of femoral offset is closely related to reserved length of calcar femorale, the head and neck length of prosthesis, angle degree of neck shaft and whether lower limb is isometric. Reconstruction strategy includes making a meticulous and standard measurement before the surgery, predicting the reserved length of calcar femorale, selecting a prosthesis with approximate anatomical neck-shaft angle and reconstructing offset by adjusting the head and neck length of the prosthetic during the operation. The aim of this article was to introduce the research progress and influence of offset on hip function, prosthetic wear and postoperative complications such as pain, limp and unequal leg length, and to discuss the reconstruction strategy.

5.Advances in nanocrystal technology and its application to improve the pharmacological efficacy for poorly-water soluble drugs

In order to solve the problems of erratic drug absorption and low bioavailability after oral administration for poorly-water soluble drugs due to low solubility, a series of novel pharmaceutical dosage forms as solid dispersion, liposome, microemulsion, vesicle, cyclodextrin inclusion complexes and drug nanocrystal have been developed in recent years. Among which drug nanocrystal attracts more attentions for its simpler preparation method, higher drug loading and easier manufacturing technology in the design of dosage forms suitable for different administration routes. In this paper, the nanocrystals of the poorly-water soluble drugs prepared based on bottom-up and top-down technologies were introduced. The characteristics and applications of the nanocrystal-based dosage forms as suspension, tablet and capsule were also introduced and carefully evaluated with the focus on their pharmacokinetics, pharmacodynamics and tissue targeted drug distribution after delivery by oral administration, intravenous injection and pulmonary inhalation. The advantages of drug nanocrystals in their therapeutics effects over the bulk drugs were discussed together with the inherent mechanism. Finally, the problems existing in basic research and scaled-up manufacture of drug nanocrystal as well as the possible ways of solution were listed out so as to make the nanocrystal-based preparations exert their maximum therapeutic effect after clinical application.

According to the theory of in vitro radioassay, using TSHAb as binder and 125I-staphylococcal protein A (125I-SPA) as tracer agent, a new method for detecting the abnormal immunoglobulin (aIgG) in the sera of patients with Graves disease (GD) was reported. In the initial study of the method, the most appropriate interacting conditions of TSHAb with aIgG were explored and compared with ELISA using ganglioside (GLS) as a binder. The relationship between aIgG and thyroid-stimulating imunoglobulin(TSI) was probed into. The results showed that TSHAb could interact specifically with aIgG in vitro; but it could not interact with IgG from sera of normal subjects, systemic lupus erythematosis (SLE) patients and diabetic patients. With aIgG as an evaluating index, the mean value in 29 normal subjects was 1. 06?0. 17. When the sera of 72 patients with GD in different clinical stages were studied, the aIgG index was positive in 83% of untreated GD patients (n = 24), in 12% of GD patients in clinical remission (n = 25) and in 82% of relapsed GD patients (n = 23). Objective To study the effect of Fas gene transfection on rectal carcinoma cells in vitro. Methods By using RT-PCR technique, a full length of Fas gene 1007 bp was cloned from actived peripheral mononuclear cells of healthy donors. The fragment was ligated with the pGEM-T Easy and sequenced. The constructed vector was transfected into 8348 cells with lipofectin, the change in expression of Fas gene was determined by RT-PCR. The apoptosis and proliferation of rectal carcinoma cells pre- and posttransfection induced by cisplatin were analysed by ladder and MTT methods. Results Transfection of Fas gene significantly upregulates the expression of Fas in human rectal carcinoma 8348 cells. 8.Clinicopathologic characteristics of intestinal T-cell lymphoma

Objective To study the clinicopathologic characteristics and differential diagnosis of T-cell immunophenotype in intestinal non-Hodgkin's lymphoma(NHL).Methods The clinicopathologic characteristics of 13 cases with intestinal T-cell lymphoma were analyzed by light microscopy and immunohistochemistry(Envision detection method).Results The lesions of 8 cases with T-cell lymphoma were found on the small intestine and 5 on the colon.Grossly,8 cases showed ulcer pattern,3 polypoid pattern and 2 presented as a regional thickening of intestinal wall.The tumor cells were medium to large size with pleomorphic nuclei and inflammatory background.The neoplastic lesions expressed the immunophenotype of peripheral T cells.The neoplastic cells of 13 cases(100%)expressed leukocyte common antigen(LCA);10(76.9%)cases expressed CD3;9(69.2%)CD45RO;5(38.5%)EB virus(EBV);3(23.1%)CD56 and 2(15.4%)vimentin(VIM).All the cases were negative for CD20,CD79a,CK,CDX2,NSE,CgA and CD117.ConclusionIntestinal T-cell lymphoma is a rare,aggressive neoplasm with poor prognosis and should be distinguished from other malignant tumors of intestine.
9.Methods for Components in Chlorine Dioxide Disinfectant

Objective In order to evaluate the quality and performance of chlorine dioxide disinfectant,it is necessary to develop a set of methods to determine the components in chlorine dioxide disinfectant.Methods Iodometry,malonic acid iodometry,improved malonic acid iodometry and five-step iodometry were employed to determine the components including ClO_2, Cl_2,ClO_2~-,and ClO_3~-in chlorine dioxide disinfectant,the advantages and disadvantages of the test methods were compared.Results Using iodometry,the liquid stabilized chlorine dioxide products activated by hydrochloric acid,the content of ClO_2 was 20.23 mg/ml that was accord with the content which was marked in the product label.Using malonic acid iodometriy,the liquid stabilized chlorine dioxide contents of ClO_2 was 19.99 mg/ml,Cl_2 was 0.35 mg/ml,in which a little chlorine was detected.Using improved malonate iodometry and five-step iodometry,the activation rates of the liquid stabilized chlorine dioxide products were 88.0% and 75.6% respectively,ClO_2 and ClO_2~-in the liquid stabilized chlorine dioxide products could be distinguished.Conclusion Active chlorine dioxide and residual ClO_2~-can be detected by using iodometriy and improved malonic acid iodometry,but iodometriy and malonic acid iodometry can not.
