1.Effects of Dexmedetomidine Hydrochloride Injection on Elderly Patients Underwent Thoracic Surgery dur-ing Anesthesia Recovery Period

OBJECTIVE:To investigate the effects of Dexmedetomidine hydrochloride injection on elderly patients underwent thoracic surgery during anesthesia recovery period. METHODS:Totally 60 elderly patients underwent thoracic surgery in our hospi-tal during Jun. 2014-Feb. 2016 were selected as research objects and then divided into control group and observation group ac-cording to even and odd-numbered admission order,with 30 cases in each group. Control group was given Propofol injection 0.5-1 mg/kg intravenously 5 min after mask oxygen inhalation. Observation group was given Dexmedetomidine hydrochloride injec-tion 0.5-1.0 μg/kg intravenously. Both group were successively given Etomidate emulsion injection 0.1-0.2 mg/kg,Sufentanil citrate injection 0.3 μg/kg and Cisatracurium besilate for injection 0.2 mg/kg intravenously for anesthesia induction;tracheal intubation was performed and ventilator was used to assist ventilation after anesthesia induction. And then control group was given continuous intravenous dripping of propofol 3-6 mg/kg·h;observation group was given continuous intravenous dripping of Dexmedetomidine hydrochloride injection 0.2-0.5 μg/(kg·h). At the same time,both group were given continuous intravenous dripping of sufentanil 0.1-0.2 μg/(kg·min)and cis-atracurium 1.5-2 μg/(kg·min),and continuously inhaled 1%-2% sevoflurane for anesthesia mainte-nance. Hemodynamic index(HR,MAP,SpO2),postoperative recovery,the occurrence of delirium,restlessness,cough and involun-tary movement were observed in 2 groups before surgery (T0),after anesthesia induction (T1),at the moment of sputum suction and extubation(T2),5 min after extubation(T3),30 min after extubation(T4). RESULTS:At T2 and T3,HR and MAP of control group were increased significantly compared to at T0,T1,and higher than those of observation group,with statistical significance (P<0.05). At T0,T1,T4,there was no statistical significance in HR and MAP between 2 groups (P>0.05). At T0,T1,T2,T3, T4,there was no statistical significance in SpO2 level between 2 groups(P>0.05). The breathing recovery time,eye openning time and extubation time of observation group were significantly shorter than those of control group,with statistical significance (P<0.05). The incidence of postoperative restlessness,delirium,cough and involuntary movement in observation group were signifi-cantly lower than control group,with statistical significance (P<0.05). CONCLUSIONS:Dexmedetomidine hydrochloride injec-tion can keep hemodynamics stable in elderly patients underwent thoracic surgery during anesthesia recovery period.,shorten recov-ery time and reduce the occurrence of stress response as postoperative delirium,restlessness,cough,involuntary movement.
2.Studies on Individual Biomass and Alkaloid Content of Wild Coptis on Mt. Emei

The structure and trends of individual organic biomass and alkaloid content of two wilCoptis species, C. omeiensis and C. gulinensis, on Mt. Emei, were studied by testing and analy-psing samples collected from several points. The results were compared with two cultivated C.species, C. chinensis and C. deltoidea. The accumulative characters and laws of biomass andeffective composition of the two wild species were revealed.
3.T-SHAPED SUBTOTAL LARYNGECTOMY ACCOMPANIED BY RECONSTRUCTION WITH MYOCUTANEOUS FLAP FOR RESTORATION OF LARYNGEAL FUNCTIONS:REPORT OF 13 CASES

Thirteen cases of supraglottic cancer extending to various contiguous parts of the glottic region treated with horizontal laterovertical or horizontal anterovertical subtotal laryngectomy and repaired by the use of cervical myocutaneous flap are reported.All 13 patients were living and well up to the date of reporting, among whom. 11 cases had the laryngeal functions (phonation. deglutition and respiration) completely restored following the extraction of the tracheostomic tube while 2 cases had partially restored (phonation and deglutition) with the tracheostomy tube retained.The advantages of the cervical myocutaneous flap for reconstruction of post-operative laryngeal defects were discussed. Its use seems to have widened the indication for subtotal laryngectomy.
4.Clinical observation on the therapeutic efficacy of cyberknife for primary or metastastic retroperitoneal tumors

ObjectiveTo evaluate the early response rate and radiation toxicity of cyberknife in the treatment of primary or metastasticretroperitoneal tumors.MethodsTwenty-eightpatientswith retroperitoneal tumors were treated with cyberknife.The total doses were 2000-6000 cGy ( median 4500 cGy) and biological effective doses were 3750-10080 cGy (median 7680 cGy) in 2-10 fractions (median 5).Of all patients,3 received three dimensional conformal radiotherapy (3DCRT) or intensity modulated radiotherapy (IMRT) boost,1 was treated as second-course radiotherapy,and others were treated with cyberknife only.The survival rates were calculated by Kaplan-Meier method and compared with Logrank test.ResultsThe complete response,partial response,stable disease and progression disease rates were 43%(12/28),6% ( 10/28),18% ( 5/28 ),4% ( 1/28 ),respectively.The overall response rate was 96%.The number of patients who were followed up more than 1,2,3 years were 17,9,7,respectively.The 1-,2-and 3-year local control rates were 92%,86% and 86%,respectively.The 1-,2-and 3-year overall survival rates were 60%,49% and 49%,respectively.The difference between local progression-free survival and overall survival was not significant ( median 9.5 and 12.0 months,x2 =0.17,P =0.680).Moreover,if the patients did not have metastasis elsewhere and local treatment was effective,there was no significant difference between local progression-free survival and progression free survival (median 17 and 11 months,x2 =0.13,P=0.720).Acute radiation-induced side effects (≥ 2 grade) such as fatigue,anorexia,nausea,vomiting and epigastric discomfort occurred in 9,9,7,7 and 2 patients,respectively.Intestinal stenosis of 1 grade occurred in 1 patient.Conclusions Radiotherapy for retroperitoneal tumors with cyberknife has provided a high response rate with minimal side effects.It is a safe and effective local treatment method for retroperitoneal tumors.
5.Correlative factors study on alexithymia of patients with asthma

Objective To investigate factors related to alexithymia of patients with asthma.Methods 52 patients with asthma were assessed with TAS-26,POMS-SF and EPQ-RSC.Results Compared with normal model control , patients with asthma showed alexithymia(t=2.15~5.29,P<0.05~0.01). There was negative correlation between the total score of TAS,Ⅰ,Ⅱ, Ⅲ , Ⅳ and E(r=-0.372~-0.451,P<0.01), between Ⅲ and N (r=-0.37,P<0.01).There was opposite correlation between the total score of TAS,Ⅰ, Ⅱand N (r=0.312~0.415,P<0.01),and between Ⅱ, Ⅳ and P(r=0.531~0.621,P<0.01). There was close relation between alexithymia and mood state. There was opposite correlation between the total score of TAS,Ⅰ,Ⅱ, Ⅳ and the total score of POMS, TA,DD,AH,FI , CB(r=0.449~0.779,P<0.01). There was negative correlation between the total score of TAS,Ⅰ,Ⅱand VA (r=-0.418~-0.544,P<0.05~0.01),and between Ⅳ and VA(r=-0.320,P<0.05).Conclusion Patients with asthma have alexithymia.There was close relation between alexithymia,personality traits and mood state.
6.Tissue engineered neocartilage using polymer substrates and chondrocytes rrom residual ear

Objective To investigate the possibility of using poly-hydroxybutyate-hydroxyhexate p(3HB-co-3HH)), a modified extracellular matrix, the third generation of PHA family, as a scaffold for seeding of chondrocytes from the residual ear of microtia patients to generate tissue engineered cartilage in athymus mice. Methods The residual ear cartilage from eight 7-years-old microtia patients was enzymatically dissolved by collagenase, and chondrocytes were harvested and seeded into foams of PHB-PHH. After incubation for 1 week in vitro, chondrocyte-polymer constructs were implanted subcutaneously into 8 athymus mice. A control groups was established by subcutaneous implantation of PHB-PHH foams alone. One athymus mice were killed at 4 weeks postoperatively, and 6 were sacrificed at 8 weeks. The specimens were dissected and examined macroscopically and histologically. Results Specimens harvested from chondrocyte-polymer constructs subjected to gross morphologic and histology analysis demonstrated new cartilage formation, and those from control groups showed no cartilage formation. The one of 4 weeks still had some remains of the scaffold with nodules of neocartilage. After 8 weeks, all the 6 mice had neocartilage formed almost the same as natural. The PHB-PHH scaffold were totally absorbed. Conclusion These findings demonstrate that the foam of PHB-PHH is not only a good "matrix" for cartilage tissue engineering, but also optimal scaffold for the seeding of chondrocytes from the residual ear to generate new cartilage that would be useful in plastic and reconstructive surgery. Chondrocytes from the residual ear of microtia patients are good candidates for generation of tissue engineered cartilage.

[Objective]To investigate the clinical curative effect and superiority of subtotal corpectomy through the pedicle,titanium rete support and pedicle screws system fixation in the treatment of burst fracture of thoracic vertebrae with paralysis.[Method]Twenty-three patients who suffered from the burst fracture of thoracic vertebrae with paralysis were treated with subtotal corpectomy through pedicle,titanium rete supporting bone autograft and pedicle screws system fixation.[Result]The patients were followed up for 8 months to 2 years.All had their bone grafts fused,and no incision infection,pneumothorax,hemothorax,pulmonary infection,internal fixation loosening or breakage occurred.The mean loss of Cobb's angle was 3.5?.According to Frankel grading,6 of grade A had no obvious recovery.In 5 of grade B,3 improved to C,1 to D,and the last one to E.In 7 of grade C,3 improved to D,4 to E.Five cases of grade D improved to E.[Conclusion]There had relatively less operation wound and good clinical curative effect with subtotal corpectomy through pedicle,titanium rete supporting bone autograft and pedicle screws system fixation.
