Objective:To analyze the serum levels of 25OHD 3 in children.Methods:489 children who visited the Children's Hospital of Chongqing University Medical Sciences during 1994 to 2001 were included in the analysis.Serum level of 25OHD 3 was measured for each child by reversed-phase HPLC method,which was developed in our laboratory in early 1990s.Results:Of the 489 children, boys 287(58.69%) and girls 202(41.31%).30.67(150/489) children had serum level of 25OHD 3 above the normal range in this laboratory.23.31%(114/489) children had a serum 25OHD 3 below the normal range.The majority came from the Department of Primary Child Care of this hospital.77.86%(102/131) children clinically diagnosed as hypervitamin D were proved by our 25OHD 3 measurement,while only 37.29%(44/118) children clinically diagnosed as vitamin D deficiency had their serum 25OHD 3 below the normal range.In bone fracture patients,10.34%(3/29) children were found to be at lower level of serum 25OHD 3.Conclusion:Pediatricians should realize the possibility of herpervitamin D caused by incorrect treatment of rickets.The diagnosis of rickets,i.e.vitamin D deficiency,could not be just based on its clinical manifestations.25OHD 3 measured by HPLC is important for making correct diagnosis.

BACKGROUND: Psychological disorders, such as depression and anxiety are relevant to pathogenesis and management of type 2 diabetes mellitus.OBJECTIVE: It was designed to investigate the influence of improving depression and anxiety in diabetes on metabolism of blood glucose.DESIGN: It was a randomized controlled comparative study.SETTING: It was conducted at the Cousulting Center for Clinical Psychology and the Endocrinology Department of Southwest Hospital Affiliated Third Military Medical University of Chinese PLA .PARTICIPANTS: Totally 43 inpatients with type 2 diabetes mellitus in the Endocrinology Department of Southwest Hospital Affiliated toThird Military Medical University of Chinese PLA were selected from October, 1999 to November 2000. All of them participated the investigation voluntarily, 24males and 19 females, with an age of 51±9 years old. They were randomly divided into 2 groups, 23 in experimental group and 20 in control group.METHODS: For experimental group, besides routine medications for diabetes, patients also Accepted anti-depression and anti-anxiety treatment, including paroxetine (20 mg, taking after breakfast everyday) and Alprazolam (0.4 mg, taking at noon, at evening and before sleep everyday). For control group, patients only Accepted routine medication for diabetes. Patients in the two groups were assessed before medication and after 4 weeks medication by Zung Self-Rating Depression Scale (SDS) and Zung Self-Rating Anxiety Scale (SAS). They were identified as depression and anxiety according to the Scales. Reduction rate of SDS/SAS score was used to assess the therapeutic effects: When the reduction rate was 50%-100%, the medication was significantly effective, when it was 25%-49% the treatment was effective, and when it was less than 25%, the treatment was noneffective.Reduction rate =(pre-treatment score-post-treatment score)/post-treatment score×100%. The fasting blood glucose level, 2-hour postprandial blood glucose level and the Glycosylated Hemoglobin (GHB, HbAl) were measured before treatment and after 4 weeks medication. Meanwhile, preprandial blood glucose level and 1-hour postprandial blood glucose level (6 times per day) were detected before treatment and at the 10th, 20th and 30th day during treatment. Reduction rate of blood glucose-(pre-treatment blood glucose level-post-treatment glucose level)/pre-treatment blood glucose level.MAIN OUTCOME MEASURES: These were main outcome measureglucose ①SDS/SAS scores, fasting blood glucose, 2-hour postprandial blood glucose and GHB before treatment and after 4 weeks treatment were measured in two group, ② and reduction rate of blood glucose.RESULTS: Six cases were omitted, 5 was in experimental group for financial pressure, intolerance to side effects of paroxetine and fears of the addiction of psychotropic drugs; the other one was in control group due to drop-out. There were 37 patients finished the investigation, 18 in experimental group and 19 in control group.①Comparison of SDS score/SAS core: After 4 weeks medication, it was observed that the post-treatment scores were significantly lower than pre-treatment scores in experimental group compared to control group[(41.05±7.05,41.88±10.57); (59.80±10.24,55.03±8.31 ), (t=2.41, 2.21, P ＜ 0.05)].②Comparison of fasting blood glucose, 2-hour postprandial blood glucose and reduction rate of blood glucose: After 4 weeks medication, the fasting blood glucose and 2-hour postprandial blood glucose in experimental group were significantly lower than those in control group(t=2.11,2.98, P ＜ 0.05-0.01)and the reduction rate of fasting blood glucose and 2-hour postprandial blood glucose in experimental group were significantly greater than those in control group (t=2.94,2.93,P ＜ 0.01). ③Comparison of GHB:After 4 weeks medication,the posttreatment GHB level in experimental group were significantly lower than pre-treatment GHB level[(8.2±2.0)%,(9.1±1.9)%, (t=2.14, P ＜ 0.05)].CONCLUSION: Anti-depression and anti-anxiety drugs not only can alleviate depression and anxiety symptoms significantly, but also can improve blood glucose metabolism. This may be attributed to an effect of anti-depression and anti-anxiety drugs in increasing the compliance of the patients to the medication for diabetes and decreasing the insulin resistance induced by negative moods.

Background and purpose:The incidence and mortality of lung cancer are markedly increasing all over the world, new drug and new methods in the treatment of lung cancer has been continuously explored and reported. Our study was to evaluate the efficacy and toxicity of continuous infusion of paclitaxel combined with cisplatin in the treatment of advanced non-small-cell lung cancer. Methods:61 patients were randomized into two groups, in the study group (31cases), the patients were treated with continuously infused paclitaxel for 48 hour at day1-2 with total dose 150 mg/m 2 , and cisplatin at day3,4,5 by intravenous injection with total dose 75 mg/m 2 . The patients in the control group(30 cases) received paclitaxel by intravenous injection for at least 3 hours at day 1, 8 with 150 mg/m 2 , and the same dose and administration of cisplatin as study group. More than three cycles were deliveried with three weeks apart to each cycle.Results:The objective response rates(RR) in the study group and control group were 54.84%(17/31)vs 43.33%(13/30),respectively; time to progress (TTP) were 7.1 months in the study group vs 5.8 months in the control group; median survival time (MST) were 14.0 months vs 10.2 months; 1-year survival rates were 67.74%(21/31)vs 33.33%(10/30)(P0.05). The toxicities that occurred in the two group included bone marrow suppression,nausea/vomiting, alopecia, peripheral neurotoxicity,impaired liver function,impaired renal function and phlebitis. There was no significant difference between the two groups in terms of the incidence of toxicites.Conclusions:In the treantment of advanced NSCLC , compared to conventional administration in delivery of the drugs, continuous infusion of paclitaxel can increase response rate and significantly improve 1-year survival fraction without the increase of toxicities.

To prepare sustained phenylpropanolamine*HCl (PPA*HCl)-resin complexes and to study the factors influencing the in vitro dissolution of it. METHOD: The sustainedPPA*HCl-resin complexes were prepared by coating the complexes with different kinds of ethylcellulose (EC) as coating embranes. The factors influencing the release rate of PPA*HCl from the complexes, such as resin particle size, the viscosity and thickness of coating membrane, the dispersion medium of EC (water or alcohol) as well as thepH and ion strength of dissolution medium were studied in detail. RESULTS: Thefactors above all influenced the dissolution of the complexes to ifferent extent. When prepared by EC 200 Pa*s (150 g*kg-1), the dissolution of the coated complexes in 0.1 mol*L-1 HCl accorded with the requirements of sustained PPA*HCl capsulesof USP ⅩⅩⅢ. ONCLUSION: The sustained PPA*HCl can be prepared by coating the drug-resin complexes with EC.

Objective: To search for a rapid and efficient method for the isolation of 3 kinds of isoflavone glucosides from the ethanol extract of Semen Sojae Praeparatum. Methods: The crude isoflavones were extracted by 75% aqueous ethanol after removing the oil by Soxhlet extraction. Then 1300 macroporous resin, water and different concentrations of aqueous ethanol (10%, 20%, 30%, 40%, 50%, 70%, and 95%) were used to separate and elute the crude isoflavones. The fraction eluted by 40% aqueous ethanol was subjected to preparative HPLC analysis. The chromatographic conditions: A YWG C18 (10.0 mm × 200 mm, i. d. 10 μm) column was used; the mobile phase consisted of acetonitrile-water-acetic acid at volume ratio of 25: 75: 1 (V/V/V) and a flow rate of 3.0 ml/min; the injection volume was 750 μl; and the detection wave length was set at 260 nm. Results: Daidzin, glycitin and genistin were rapidly separated with the purities over 99% as determined by external standard HPLC. Conclusion: This technique is simple and suitable for the isolation of daidzin, glycitin and genistin from Semen Sojae Praeparatum.
