Objective To discuss the curative effect of small incision thyroidectomy on thyroid nodule . Methods 94 patients with thyroid nodule were selected and divided into the small incision group and control group according to different modes of operation ,47 cases in each group .The patients in small incision group were given small incision thyroidectomy ,while the patients in control group were given the traditional big incision thyroidectomy .The curative effect and security in the two groups were observed and compared .Results The operation time,intraoperative amount of bleeding ,length of incision and length of hospital stay in the small incision group were (48.76 ±10.37) min, (39.57 ±7.68)mL,(3.57 ±0.68)cm(4.89 ±1.13)d,which were significantly less than those in the control group [(54.07 ±11.64)min,(64.16 ±7.98)mL,(6.88 ±0.65)cm,(5.47 ±1.32)d](t=2.13,3.12,3.96,2.14,P<0.05 or P<0.01).The incidence rate of adverse reactions in small incision group was 10.6%,which was significant-ly lower than 29.8%in control group (χ2 =5.34,P<0.05).Conclusion Compared with traditional mode of opera-tion,small incision thyroidectomy has following advantages on thyroid nodule ,smaller operation wound ,less intraopera-tive amount of bleeding ,quicker postoperative recovery ,better cosmetic effect ,less adverse reactions ,etc.

Objective: To investigate the therapeutic effect of oat β-glucans on serum lipid levels of rats with experimental hyperlipemia. Methods: Rats were fed on high fat forage for 3 weeks to establish hyperlipemia models, then the model rats were orally given oat β-glucans at doses of 133, 266 and 533 mg/(kg · d) separately. At the same time, there were 3 control groups including chitosan ligosaccharide[200 mg/(kg · d)], normal, and high blood lipid (model rats fed with distilled water). Serum levels of total cholesterol (TC), high density lipoprotein cholesterol (HDL-C) and low density lipoprotein cholesterol (LDL-C) were assayed in rats of all groups and the results were compared. Results: β-glucans obviously decreased the levels of TC and LDL-C and increased HDL-C in rat hyperlipemia models. The reduction of serum lipid level in 4-week β-glucans treatment rats was more obvious than that in 3-week β-glucans treatment rats. Besides, β-glucans showed an obvious dosage dependent effect in reducing serum lipid level without adverse effect. Oat β-glucans had a better lipid-reducing effect than ligosaccharide did. Conclusion: Oat β-glucans can obviously decrease serum lipid in rats with hyperlipemia.

Objective To investigate the effects of Lycopene ( LP) on immune function of ovarian cancer rats′and its mechanism .Metho ds Sixty ovarian cancer rat models made by injecting ovarian cancer cell line NUTU-19 were divided into five groups randomly:model group,Cisplatin group(2 mg/kg)and LP(40,20,10 mg/kg)group,n =12;the LP and Cisplatin were given by intraperitoneal injection ,once every two days for 5 times.The rats general states and tumor growth conditions were observed ,the tumor tissue morphological changes was observed by H&E staining;the tumor weight ,inhibition rate,spleen lymphocyte transformation rate were de -tected;and the content of IL -2 and TNF-αin serum were detected .The present of CD4 +,CD8 +in blood were detected and the ratio of CD 4 +/CD8 +was calculated .Results The food situation of the rats in LP groups was improved,tumor was reduced,activity and hardness were poorer ,necrosis presented ,tumor cell shrinkage and oth-er pathological morphological changes appeared ,especially the rats in LP 40 mg/kg group .Compared with model group,the tumor weight in LP(40,20 mg/kg)groups was decreased,the inhibition rate and the spleen lymphocyte transformation rate were increased;the contents of IL-2 and TNF-αwere decreased;the CD8+present was de-creased,while the CD4 +present in LP 40 mg/kg group was increased,the ratio of CD4 +/CD8 +was increased. All of the difference above were significant (P<0.05,P<0.01).Conclusion LP can effectively improve the immune function of ovarian cancer rat;which perhaps relates to its effects of reducing the inflammatory cytokines levels and increasing the ratio of CD 4 +/CD8 +.

Objective to investigate the application of the whole course case tracking method in continuous improvement of nursing quality of out-patient operation unit. Methods Administration frames were established for this project,such as monitoring group and executive group. the medi-cal treatment procedure and patients′experience were followed by referees. the results were quantified for statistical analysis. the problems were rec-ognized by priorities and rectification was correspondingly performed. Afterward,another group of patients were traced to evaluate the results of the rectification. We focus on those items of lower scores and undertook rectifications accordingly. Particularly,we optimized signs in outpatient unit, made pamphlets to clarify the procedures,and were cautious to provide more easy-to-understand informs. Results By the second round evaluate, these scores were effectively improved. Especially,the item of“clear informed consent”was statistically significant(P = 0.003). Meanwhile,the team of outpatient operation unit became more cohesion,with enhanced service consciousness,and was initiative to improve the work. Conclusion the whole course case tracking method is feasible for short-range medical service evaluation such as outpatient surgery. the evaluation results clarify the direction for quality improvement,thus set the starting point of quality management aiming at continuously improvement of the patients′ experi-ence and quality of nursing service.

Objective To know the nursing countermeasures for the catheter associated complications during the course of hemofilitration by observation critical patients who had accepted hemofilitration.Methods Retrospective analyzed the nursing points and treatment effects among 68 critical patients with hemofilitration from June 2005 to October 2008.Results There were no venous catheters droped off,no blocked and obvious infection.6 cases with exudation,57 cases with vitro cogulation,35 cases with hypotension,21cases with hypothermia,2 cases with hypoglycemia.Conclusions Apropriate anticoagulant,observe the body temperature and keep the comfortable temperature can increase efficiency and reduce the venous catheter associated complications during the hemofilitration.

This paper aims to investigate the regulatory mechanism of blood-activating and stasis-dissipating drugs on fecal metabolic characteristics of rhubarb-peach kernel in mice with adenomyosis (AM) using fecal metabolome method. Adenomyosis was modeled by pituitary transplantation, and after the end of modeling administration, fecal samples were collected from mice. Non-targeted metabolomics studies were performed using liquid chromatography-mass spectrometry (LC-MS) to compare the metabolic characteristics of the feces of mice in each group and to find intestinal differential metabolites and potential differential metabolic pathways. The results showed that compared with the normal group, 5-hydroxy-L-tryptophan, histidine, L-acetylcarnitine, 16-hydroxy hexadecanoic acid, thromboxane B₂, etc. were significantly up-regulated, L-urobilin and prostaglandin D₃ were down-regulated in the feces of the model group, and were reversed after treatment with the rhubarb-peach kernel. The results of metabolic pathway enrichment analysis showed that tryptophan metabolism and histidine metabolism were the main intervention pathways of the rhubarb-peach kernel on AM intestinal metabolism. This study found that the underlying mechanism of the rhubarb-peach kernel in the treatment of AM is related to the intervention of intestinal metabolism of tryptophan, histidine, bile acid, choline and arachidonic acid, and the regulation of pro-inflammatory microenvironment and fatty acid metabolic homeostasis. Objective To observe the transformation features of children with complete atrioventricular block(CAVB), and to improve the diagnosis and treatment of this disease.Methods Seventeen children with primary diagnosis of CAVB were reviewed by retrospective and follow-up study and the clinical characteristics, treatment schemes and prognosis were evaluated.Results Four cases of congenital CAVB lived normally without obvious symptoms in the tracing period from 6 months to 9 years;1 case had the onset of Adams-Stokes syndrome induced by diarrhea;3 cases of the CAVB caused by virus myocarditis turned into sinus rhythm after comprehensive therapy;2 cases persistently presented with the CAVB Complicated with enlarged heart;1 case gave up treatment after deterioration, and one case died.Three cases of temporary CAVB after the open heart operation turned to sinus rhythm in transitory time, while other two cases presented with permanent CAVB and treated with epicardium pacemaker,among whom one had the pacemaker replaced for one time in the 8-year follow-up,and the follow-up of other cases were intermitted.Conclusions The congenital CAVB in the study group with normal QRS interphase and no obvious symptom might not require treatment but follow-up is needed. While infants with heart malformation and wide QRS wave could not endure the low ventricular rhythm are in high risk. Virus myocarditis induced CAVB children tend to present the Adams-Stokes syndrome, and require effective treatments. Partial cases of the CAVB caused by open heart operation may turn to normal sinus rhythm in 2-4 weeks after surgery. cases has persistent CAVB for over 4 weeks, or Adams-Stokes syndrome onset after the surgery demand epicardium pacemaker treatment.

There are many progresses in the therapy of MM with ASCT. ASCT improves the ratio of CR,EFS and OS,compared with conventional chemotherapy.Myeloma patients with renal failure or old age also can be candidate for ASCT.Tandem autologous transplantation improves the ratio of CR. Its impact to long-term EFS and OS still need to be judged.To improve conditional project,to choose suitable time,and- toselect consolidation schedule can boost therapeutic effect.
