1.Expression and significance of interleukin-18.12 and tumor necrosis factor-α in intrahepatic cholestasis of pregnancy

Objective To investigate the effect of Interleukin(IL)-18,IL-12 and tumor necrosis factor-α(TNF-α)in hepatic injury in intrahepatic cholestasis of pregnancy(ICP).Methods Sixty-two cases of ICP patients(ICP group),30 cases of normal pregnant women(control group)and 30 cases of hepatitis B(HBV) women (hepatitis group) were recruited. Serum IL-18, IL-12 and TNF-α were examined by ELISA. Serum alanine aminotransferase (ALT) and aspartate aminotransferase (AST) were examined by automatic biochemical analysis instrument. Results ( 1 ) In hepatitis group, serum concentrations of IL-18,IL-12 and TNF-α were (256±51 ) ng/L, ( 122±96) ng/L and (207±3) ng/L; serum levels of ALT and AST were(363±174) U/L and (359 ±237) U/L, respectively. In ICP group, serum concentrations of IL18, IL-12 and TNF-α were (72±32) ng/L, (42 ±28) ng/L and (48±14) ng/L; serum levels of ALT and AST were (201 ±128) U/L and ( 132±87) U/L, respectively. While in control group, serum concentrations of IL-18, IL-12 and TNF-α were (43 ± 13) ng/L, ( 10±3) ng/L and (33±9) ng/L; serum levels of ALT and AST were (13 ~ 4) U/L and (15 ± 3) U/L, respectively. Serum IL-18, IL-12, TNF-α, ALT and AST levels in hepatitis group were significantly higher than those in ICP group and control group ( P <0. 05 ).Serum IL-18, IL-12, TNF-α, ALT and AST levels in ICP group were significantly higher than those in control group(P < 0. 05 ). (2) In severe ICP subgroup, serum concentrations of IL-18, IL-12 and TNF-α were (81 ±32) ng/L, (50 ±25) ng/L and(50 ± 14) ng/L; serum levels of ALT and AST were (269 ± 111 ) U/L and (181±73) U/L In mild ICP subgroup, serum concentrations of IL-18, IL-12 and TNF-α were (48 ±18 ) ng/L, (17 ± 4 ) ng/L and (40 ± 10 ) ng/L; serum levels of ALT and AST were (87±46) U/L and (50 ±21 ) U/L, respectively. Serum IL-18, IL-12, TNF-α, ALT and AST levels in severe ICP subgroup were significantly higher than those in mild ICP subgroup and control group (P < 0. 05). And serum ALT and AST levels in mild ICP subgroup were significantly higher than those in control group(P <0. 05). (3) There were 16 cases with preterm birth (50%, 16/32 ) and 10 cases with meconium-stained amniotic fluid( 31%, 10/32 ) in severe ICP subgroup, significantly higher than those in mild ICP subgroup ( P< 0. 05 ), which contained 2 preterm births ( 7%, 2/30) and 1 meconium-stained amniotic fluid (3%, 1/30). While in control group, the numbers were 1(3%, 1/30)and 1(3%, 1/30),respectively. As for the cases of neonates whose 1 minute Apgar score were not more than 7, there were 2 cases, 1 case and 1 case in severe ICP subgroup, mild ICP subgroup and control group, respectively,which showed no significant difference(P> 0. 05). Conclusion Serum IL-18, IL-12 and TNF-α may be involved in the process of hepatic injury of ICP.
2.The clinical effects of metformin combined with cyproterone therapy on Dolyeystic ovary syndrome

Objective To observe the curative effects of metformin combined with cyproterone therapy on polycystic ovary syndrome(PCOS). Methods Ninety diagnosed PCOS patients were treated. The serum luteinizing hormone(LH), luteinizing hormone and follicule stimulating hormone(LH/FSH) ratio and T, waist hip ratio(WHR) of the patients before therapy were compared with using merformin combined with- cyproterone three cycles. Results A significant reduction(P<0.05 or P<0.01) in LH,LH/FSH,T and WHR of patients after the therapy was observed. Menses became regular after the therapy, and ache and hirsutism mitigated. Conclusion Metformin com- bined with cyproterone is a perfect means to treat polycystic ovary syndrome(PCOS).
3.Effect of Mcl-1 siRNA on TRAIL-induced apoptosis in gastric cancer cells

Objective To explore the effect of Mcl-1 small interference RNA ( siRNA) on tumor necrosis factor-re-lated apoptosis-inducing ligand ( TRAIL)-induced apoptosis in gastric cancer HGC-27 cells. Methods The apop-totic rates of cells treated with TRAIL and pan-caspase inhibitor ( z-VAD-fmk) alone or combination were measured by propidium iodide (PI) method using flow cytometry. The activation of caspase-3, cleavage of PARP-1, as well as the protein level of anti-apoptotic Bcl-2 proteins Bcl-2, Bcl-XL and Mcl-1 before and after TRAIL treatment were monitored by Western blot analysis. Transfection of Mcl-1 siRNA was performed using Lipofectamine 2000 reagent. The efficiency of gene silencing was quantified by Western blot and the effect of Mcl-1 siRNA on TRAIL-induced apoptosis was measured using PI method. Results HGC-27 cells were resistant to TRAIL-induced apoptosis, and z-VAD-fmk pretreatment could block apoptosis nearly completely. Activation of caspase-3 and cleavage of PARP-1 occurred in the late stage of apoptosis. The expression levels of Bcl-2, Bcl-XL and Mcl-1 were not altered after exposure to TRAIL. Transfection with Mcl-1 siRNA could obviously downregulate the expression evel of Mcl-1 in HGC-27 cells and enhanced the sensitivity of cells to TRAIL-induced apoptosis. Conclusion Overexpression of Mcl-1 may account for the resistance of HGC-27 cells to TRAIL.ownregulationof Mcl-1 by siRNA can effectively enhance the sensitivity of HGC-27 cells to TRAIL-induced apoptosis.

5.Nano targeted drug delivery system and the treatment of oral cancer

Oral cancer is one of the most common malignant tumor of the head and neck.Passive targeting is a targeted therapy for initial treatment of oral cancer,which is not always effective.In order to achieve better targeted therapy on oral cancer,researchers have turned to the study of active targeting with nano drug delivery system in recent years.With this treatment,nanoparticles are attached to the surface of the target molecules,and combine with specific expression or overexpression receptor on the surface of tumor cell to achieve target.This delivery system can transport anticancer drugs directed at the lesion site and achieve the purpose of reducing adverse drug reaction and increasing treatment efficacy.Targeted chemotherapy is increasing being used in the treatment of oral cancer.
6.Standardization of Package Inserts of Traditional Chinese Medicine Injections

OBJECTIVE:To standardize and improve the package inserts of Traditional Chinese medicine injections,and to guide clinical rational drug use and reduce incidences of adverse drug reactions(ADRs).METHODS:The package inserts of 34 kinds of traditional Chinese medicine injections commonly used in the clinic were subjected to an analysis.RESULTS & CONCLUSION:Part of the package inserts were not standard in writing contents in that they are unscientific and incompact,which thus remain to be improved.

8.Effects of thoracic epidural anesthesia combined with propofol on myocardial injury and apoptosis induced by acute myocardial infarction in rabbits

The rabbits were randomly divided into 4 equal groups of 12 animals in each group: Ⅰ control group in which normal saline 0.2-0.3 ml?kg-1 was injected through epidural catheter; Ⅱ epidural group in which 1% lidocaine 2-3 mg?kg-1 was injected epidurally; Ⅲ propofol group in which propofol was infused iv at 20 mg ? kg-1 ?h-1 for 30 min; Ⅳ combined epidural-propofol group in which epidural 1% lidocaine and propofol iv were both administered. Blood samples were taken before and 4 h, 8 h after ligation for determination of serum concentrations of myocardial troponin I (cTn I ) and MDA and serum SOD activity.The myocardial tissue was taken from the infarct area for determination of the apoptotic myocyte count (TUNEL) and p53 and Bcl-2 protein expressions (LSAB) . Results In control group serum cTn I and MDA increased and SOD decreased significantly after ligation of LAD. Thoracic epidural anesthesia, propofol iv infusion and especially the combination of them suppressed the increase in serum cTn I and MDA and decrease in serum SOD induced by ligation of LAD.In epidural-propofol group and propofol group the apoptotic myocytes count and p53 protein expression significantly decreased, the Bcl-2 protein expression increased as compared with those in control group. Conclusion Thoracic epidural anesthesia combined with propofol can significantly protect myocardium from injury induced by myocardial infarction. This action is mediated by the depression of myocyte apoptosis and the generation of oxygen-derived free radicals.
9.Clinlcal application of non-operative management in the treatment of external pancreatic fistula:a report of 18 cases

Objective To explore the effect of non-operative management in the treatment of external pancreatic fistula(EPF). Methods 18 cases of EPF underwent non-operative treatment were analyzed retrospectively . The causes of EPF, the criteria of case selection were summarized respectively. the non- operative treatments methods and the change of drainage volume after the treatment were analyzed.Results After the non-operative treatment,the drainage volume reduced gradually,and all the 18 cases were curred in 1 to 8 (averaged 5.5) months.No recurrence happened after the drainage withdrawn.There were no other severe complications except 4 pancreatic pseudocysts formation. Conclusions Non-operative treatment plays an important role in the treatment of EPF.It could reduce patients' sufferings.The choice of the method should be according to patients' condition.
10.Changes in Cell-mediated Immune Function among Older Inpatients with Pseudomonas aeruginosa Pneumonia

OBJECTIVE To study the cell-mediated immune function among older inpatients with Pseudomonas aeruginosa pneumonia.METHODS T-cell subsets and expression of membrane molecule on activated T-cell in peripheral blood were measured in 35 cases with P.aeruginosa pneumonia by using immunofluorescence and flow cytometry,and compared with those of control healthy older people.RESULTS The expressions of CD4+ /CD3+ and CD25+ /CD3+ in P.aeruginosa pneumonia patients were lower than in control group(P0.05).CONCLUSIONS There are disfunctions of lymphocgte and immunity among P.aeruginosa pneumonia patients.
