1.Comparison of 23 G and 20 G vitreous cut system in silicone oil extracting operation

AIM: To evaluate the safety and effectiveness of 23G and 20G vitreous cutting system in silicone oil extracting operation.
●METHODS:A total of 98 cases (98 eyes) patients, after 3- 6mo silicone oil filling with retinal attachment and suitable for retrieving silicone oil. Patients were randomly divided into two groups. 20G vitreous cutting system group including 48 cases ( 48 eyes ) and 23G vitreous cutting system group including 50 cases ( 50 eyes ) . A series of comparative analyses were conducted on the times for set up and closing the passage and the time for taking the oil time on the two groups:intraocular pressure of preoperative and of postoperative for 1, 3d, 1wk, 3 and 6mo. The comparisons were also made between the best corrected visual acuity of preoperative and postoperative. The section inflammation of postoperative 1, 3d, 1mo were compared. Complications were compared between intraoperative and postoperative.
●RESULTS:Establishing surgery channel for 20G and 23G group were ( 243. 54 ± 51. 17 ) s and ( 91. 16 ± 21. 37 ) s respectively;closing wound time were (235. 04 ± 42. 89) s and (86. 04±21. 76)s. Extracting oil time were (6. 7±1. 65) min and ( 7. 35 ± 2. 02 ) min. There was a significant difference ( P<0. 01 ) between establishing the operation channel time and close the incision time. There was no significant difference between two groups in the extracting oil time (P>0. 05). The first day was statistically different ( P< 0. 05 ) but the rest of the time point of intraocular pressure had no statistical difference ( P >0. 05). The best corrected visual acuity for 20G and 23G group preoperative and postoperative after 6mo were:4. 21 ± 0. 61, 3. 91 ± 0. 64; 4. 03 ± 0. 46 and 4. 22 ± 0. 39 respectively. There was no significant difference between the two groups (P>0. 05). There was significant difference ( P<0. 05) of cells in the anterior chamber between the 20G group and 23G group of postoperative 1 and 3d. There was no statistical difference after 1mo (P>0. 05). The findings also reported that 20G and 23G group had no intraoperative complications and postoperative. For 23G group, 5 cases of temporarily had lower intraocular pressure, 1 case was retinal again, 1 case was choroid detachment. For 20G group, 1 case of temporarily had lower intraocular pressure, 3 cases were retinal detachment and 2 cases were choroid detachment.
●CONCLUSION:23G vitreous cutting system has the advantages of low operation steps, intraoperative injury. The postoperative complications of low probability, used in silicone oil is safe and effective.

4.Adjuvant chemotherapy of stage Ⅱ colon cancer

For stage Ⅱ colon cancer patients after surgery,the benefit of adjuvant chemotherapy remains controversial.Several studies indicate that the patients with high-risk stage Ⅱ colon cancer can benefit from adjuvant chemotherapy after surgery.According to the clinical and pathological features,the stage Ⅱcolon cancer patients including T4 lesion,perforation,peritumoral lymphovascular invasion can benefit from the adjuvant chemotherapy of oxaliplatin.In the horizontal of molecular and genetic levels,the stage Ⅱ colon cancer patients can express high microsatellite instability (MSI-H) and cannot benefit from the chemotherapy of Fluorouracil.The guiding functions of 18q loss of heterozygosity (18q LOH),Cx43 and gene expression profiling in adjuvant chemotherapy are still unclear,and need further study.
5.Role of Nrf2/Keap1/ARE signaling pathway in respiratory diseases

Nrf2/Keap1/ARE is an important signaling pathway in maintaining the balance between peroxide and antioxidants. Once oxidative stress happened, Nrf2/Keap1/ARE would be ac-tivated to regulate the expression of downstream antioxidant pro-teins that could reduce the injury from oxidative stress and the level of oxidative stress. Recent researches show that Nrf2/Keap1/ARE signaling pathway is closely related to refractory re-spiratory diseases such as pulmonary fibrosis (PF), lung canc-er, chronic obstructive pulmonary disease (COPD), suggesting that Nrf2/Keap1/ARE signaling pathway may be a potential ther-apeutic target. This review focuses on the role of Nrf2/Keap1/ARE signaling pathway in refractory respiratory diseases in order to further understand the related mechanism and provide evidence for the diagnosis and treatment of refractory respiratory diseases.
6.Enrichment of lung cancer stem cells and expression of related markers

BACKGROUND:Lung cancer stem cel s are tightly related to the treatment and prognosis of lung cancer. We can provide more references for clinical diagnosis and treatment of lung cancer through the study on the tumorigenicity and surface markers of lung cancer stem cel s. OBJECTIVE:To explore the enrichment methods for lung cancer stem cel s and cel ular tumorigenicity. METHODS:Lung cancer stem cel s were induced in serum-free culture medium containing epidermal growth factor, insulin-like growth factor 1, and basic fibroblast growth factor. Then, the expressions of related surface markers were detected using immunofluorescence method. After that, mice were implanted subcutaneous with lung cancer stem cel spheres to understand the tumorigenicity of lung cancer stem cel s. RESULTS AND CONCLUSION:Lung cancer stem cel s under serum-free induction and culture were changed to sphere-forming cel s, and the immunofluorescence detection showed that over 80%of sphere-forming cel s were positive for CCSP, SP-C and OCT4. After transplantation of sphere-forming cel s, the mice showed a high tumorigenicity. These findings indicate that sphere-forming cel s are formed after serum-free suspension culture of lung cancer stem cel s, which have a higher tumorigenicity.
7.Current status of the treatment of colorectal cancer in China--development and limitations.

There is a significant gap between China and the optimal international level in terms of the treatment outcomes of colorectal cancer. In recent years, progresses have been made while limitations still exist in the management of colorectal cancer. Advances are seen including use of multi-disciplinary therapy, neoadjuvant chemoradiation, laparoscopic surgery and management of liver metastasis. Individualized therapy is also applied in colorectal cancer. However, randomized controlled trials based on Chinese population are still lacking. Radiation therapy is inadequate and standardization of surgical technique is to be improved. Although standardization of management of colorectal cancer has gained attention from the government, there is a long way to go. The establishment of subspecialty training and practice has not been satisfactory. National efforts are needed to improve the treatment outcomes of colorectal cancer in China.

8.Individualized therapy is the future of diagnosis and treatment for colorectal cancer.

Colorectal cancer has become one of the most common malignant tumors that affects Chinese's health. Recurrence and metastasis are main reasons for death of colorectal cancer patients. Making up individualized and comprehensive therapeutic strategy according to patients' specific matters is the developmental direction of diagnosis and treatment for colorectal cancer. Only if standardize clinical pathway, perfect postoperative follow-up system, and accelerate research development on biomarkers, individualized therapy of colorectal cancer can be accomplished.
