1.Evaluation of psychological health status and interventions in patients with myocardial in farction

Objective:To investigate the psychological health status and the intervention in patients with myocardial infarction. Methods:87 Patients with myocardial infarction were randomly divided into control group (n = 43) and psychological intervention group (n = 44). Both groups were assessed with Symptom checkist-90,Hamilton depression and behavior pattern test. The control group was treated with routine therapy. The psychological intervention group was treated with routine therapy,psychological intervention,and low dose fluoxetine hydrochloride. After three months of follow-up,New York Heart Association (NYHA) class,ejection fraction and the occurrence of adverse cardiac events were assessed. Results:The patients with myocardial infarction had high prevalence of psychological disorder (82.75%),depression was the major symptoms (type-A behavior 62.07%). Compared with the control group,psychological disorder,NYHA class,and ejection fraction were significantly improved in the intervention group. Moreover,the occurrence of cardiac events was much lower than the control group. Conclusion:The patients with myocardial infarction had high prevalence of psychological disorder and the major symptoms were depression. The combination of routine therapy and psychological intervention may be effective at alleviation the symptoms and improvement the prognosis.
2.Clinical study on evaluating the function of cardiac reserve in hypertensive patients with noninvasive techniques

Objective:To explore whether phonocardiogram exercise test(PCGET) or doppler echocardiography can sensitively and specifically detect the changes of cardiac reserve in patients with hypertension and whether there is a relationship between the two methods.Methods:The trail was performed with eighty volunteers, including twenty healthy persons as control group,and sixty hypertensive patients, which were divided into three subgroups:hypertension A, HA(patients without left ventricular hypertrophy,LVH and enlarged left ventricle);hypertension B, HB(patients with only LVH) and hypertension C,HC(patients with both LVH and enlarged left ventricle).The cardiac contractility reserve index(CCRI) was measured by PCGET, while the value of LV end-systolic and end-diastolic long and short axis dimensions, wall thickness and synchronous blood pressure were measured by the technique of simultaneous screen display of electrocardiography and synchronously monitoring blood pressure.According to the parameters measured by the latter technique, the values of maximum elasticity(Emax), the average maximum myocardial elasticity stiffness(maxEav) and ejection fraction(EF)were calculated by computer programmed process.Results:(1)Compared with control group,there was a statistically significant difference between the hypertension groups except HA group,and also an obvious difference between HB and HC about the indexes of Emax, maxEav and EF(P
3.Optimization of Processing Technology for Atractylodes lancea Stir-baking with Bran by Box-behnken Re-sponse Surface Methodology

OBJECTIVE:To optimize the processing technology for Atractylodes lancea stir-baking with bran. METHODS:Us-ing the content of atractylodin,yield of water-soluble extract and ethanol-soluble extract,appearance score as comprehensive in-dex,the effects of excipient amount,stir-baking time and temperature on the quality of A. lancea stir-baking with bran were investi-gated by Box-behnken design-response surface method,so as to optimize the processing technology parameters of A. lancea stir-bak-ing with bran,and verification test were conducted. RESULTS:The optimized parameters were that the excipients amount was 10% medicinal material,stir-baking at 140 ℃ for 3 min;average comprehensive score of verification test was 0.39 (RSD=2.88%,n=3),which was close to predicted value 0.38. CONCLUSIONS:Box-behnken response surface method is reasonable and feasible for the optimization of the processing technology for A. lancea stir-baking with bran.
4.Correlative Factors of Hospital Infection among Children with Infectious Diseases in a General Hospital

OBJECTIVE To investigate the distribution of infectious diseases and diagnostic and therapeutic conditions in pediatric department of a general hospital,and try to find out the measures to decrease hospital infection.METHODS From Jan 2004 to Dec 2005,507 cases of infectious diseases were retrospectively analyzed in pediatric department of our hospital.RESULTS Among them epidemic parotitis,infectious diarrhea and bacillary dysentery were 56.2%,20.1%,and 17.4%,respectively.Others were measles,scarlet fever,rebella,viral hepatitis,epidemic cerebrospinal meningitis,typhoid,and syphilis.Among the cases,the most were outpatients.CONCLUSIONS Because the species of infectious diseases are various and the amount are large,the pediatric department is important in a general hospital for management of hospital infection.Strengthening the standardized management of pediatric infectious diseases and taking strong measures may play the important role in control the cross infection.
5.Recent advances on the modified endostatin and ocular neovascularization

Endostatin(ES), the C-terminal fragment of collagen XVIII, is a potent angiogenesis inhibitor. At present, there are a large number of research papers on ES. It has already been on clinical stage Ⅱ and been widely used in inhibition of neovascularization(NV). However, how to improve the bioactivity of ES is still a matter of ongoing discussion. The objective of this review is to elucidate the relationship between the modified ES and ocular neovascualrization, and to discuss the superiority based on the structure modification. The structure can be changed either by covalent modification or by genetical mutation. It is proposed that the secondary structral ES enhance the anti-angiogenic activity. Studies on modified ES also shed light on our understanding of the molecular action mechanisms of ES. Modified ES may be exploited as a new angiogenesis inhibitor for therapeutic applica-tions, in substitution of the native ES. Activity
6.Relationship between vimentin and glucocorticoid-induced cataract

Long-term use of systemic or topical glucocorticoid can cause posterior subcapsular opacities ( PSO ) , named glucocorticoid-induced cataract ( GIC ) . There are many hypotheses on the pathogenesis of GIC. However, no one has well explained the formation of PSO, which leads to no effective approaches in the prevention and/or treatment. A new opinion is that hormones might affect lens epithelial cells through GR - mediated vimentin changes, which eventually result in the formation of GIC. Therefore, the association between vimentin and lens epithelial cell proliferation and differentiation, maybe a new direction for further studies in the pathogenesis of GIC.

9.Clinical observation on medulla oblongata palsy after brainstem infarction treated with electroacupuncture at eight-neck-occiput points.

OBJECTIVETo compare the differences in the efficacy on speech and swallowing dysfunction of medulla oblongata palsy (MOP) after brainstem infarction between electroacupuncture at eight-neck-occiput points and routine acupoints.
METHODSSeventy-two patients were randomized into a neck-occiput points group and a meridian points group, 36 cases in each one. In the neck-occiput points group, the eight-neck-occiput points (Neck 1-4 points, Occiput 1-4 points) were selected. In the meridian points group, Lianquan (CV 23), Futu (LI 18), Tongli (HT 5), Hegu (LI 4) and the others were selected. Electroacupuncture was used in the two groups, dense-dispersion wave, retaining for 30 min. The treatment was given once a day, 5 treatments a week. Totally, 4 weeks were required. The symptom scores of speech and swallowing dysfunction were observed before and after treatment in the two groups. The efficacy was compared between the two groups.
RESULTSThe scores of speech and swallowing dysfunction were improved significantly after treatment in the two groups (P < 0.05, P < 0.01). The results in the neck-occiput points group were better than those in the meridian points group (both P < 0.01). The curative rate of speech dysfunction was 30.6% (11/36) and that of swallowing dysfunction was 22.2% (8/ 36) in the neck-occiput points group, which were better than 11.1% (4/36) and 5.6% (2/36) in the meridian points group, respectively. The differences were significant in comparison of the two groups (P < 0.01, P < 0.05).
CONCLUSIONElectroacupuncture at eight-neck-occiput points achieves a better efficacy on speech and swallowing dysfunction of MOP after brainstem infarction as compared with the routine acupoints. This therapy is characterized as more accurate point localization and safer operation.

10.The repairation of long bone defect after getting rid of free bone in chronic osteomyelitis patients with vascularized fibula graft

Objective To probe the repair effect of bone defect after getting rid of free bone in chronic osteomyelitis patients. Method Eighteen chronic osteomyelitis patients were reconstructed with vascularized fibula graft at one stage after getting rid of free bone and 2 cases were reconstructed with regional transplantation of pedic fibula after inflammable bone segment were exsected. Result Twenty chronic osteomyelitis patients who were treated with vascularized fibula graft or regional transplantation of pedic fibula after getting rid of free bone were followed up 3 - 6 months (mean 3. 5 months). The reconstructed bone unioned well,the inflammation of all bones near to joints were controlled; the repaired limbs achieved satisfactory function and outline. Conclusion The operation to reconstruct bone defect after getting rid of free bone in chronic osteomyelitis patients with vascularized fibula graft or regional transplantation of pedic fibula primerally is a good method' The advantage lies in avoiding the long alteration period of free bone, bettering the blood circulation of receipt region, controlling inflammation, shortening the resident period,times and operation times,and so. The method is a good attempt to repair inflammatory bone defect using vascularized bone graft.
