Objective To investigate the health status and nutrition-related diseases of residents in Pudong New Area,Shanghai and provide information for shaping the relevant national policies and guiding the people''s health dietary.Methods By means of a stratified multi-stage cluster random household sampling survey were performed questionnaire,physical examination and laboratory assay for 737 residents above the age of 15.Results The mean±standard deviation of the men''s height was (168.79±6.81) cm,and the women''s mean height (157.47±6.53) cm.And the rates of malnourishment,overweight,obesity and central obesity were 4.37%,48.91%,14.48% and 49.25%,and the prevalence rate of hypertension was 45.18%.Among residents above 18 years old,the abnormal rate of total cholesterol and triglycerides were 28.69% and 23.50%.Conclusion Nutrition-related chronic diseases will be an important public health problem in Pudong New Area.

4.Posterior interosseous nerve entrapment after Monteggia fracture-dislocation in children.

OBJECTIVEAlthough most of nerve injuries associated with Monteggia fracture-dislocation in children are neurapraxias and will recover spontaneously after conservative treatment, surgical exploration of the involved nerve is always required in the cases with the entrapment of posterior interosseous nerve (PIN). However, the necessity and time frame for surgical intervention for specific patterns of nerve dysfunction remains controversial. The aim of the report is to observe and understand the pathology of PIN injury associated with Monteggia fracture-dislocation in children, and to propose the possible indication for the exploration of nerve.
METHODSEight cases, six boys and two girls, with Monteggia fracture-dislocation complicated by PIN injury, managed operatively at the authors?Hospital from 2007 to 2008 were retrospectively reviewed. All the patients underwent the attempted closed reduction before they received exploration of PIN, with open reduction and internal fixation or successful closed reduction.
RESULTSThe PIN was found to be trapped acutely posterior to the radiocapitellar joint in 4 out of 5 Type III Bado's Monteggia fractures. In the remaining cases, since there were longer time intervals from injury to operation, chronic compressive changes and epineural fibrosis of radial nerve were visualized. After a microsurgical neurolysis performed, the complete recovery in the nerve function was obtained in all the cases during the follow-up.
CONCLUSIONThe findings from this study suggest that every case of type III Monteggia fracture-dislocation with decreased or absent function of muscles innervated by PIN and an irreducible radial head in children should be viewed as an indication for immediate surgical exploration of the involved nerve to exclude a potential PIN entrapment.

5.Treatment of allergic rhinitis with nasal mometasone furoate.

OBJECTIVETo evaluate the effects of nasal mometasone furoate on moderate to severe allergic rhinitis.
METHODSPatients with moderate to severe allergic rhinitis were enrolled and received mometasone furoate nasal spray 200 microg once daily for four weeks. Four hundreds and sixty-three patients completed the study. We weekly interviewed the patients to evaluate the symptoms, and the affection of disease on night sleep and daily life. Mini Rhinoconjunctivitis Quality of Life Questionnaire (Mini-RQLQ) and Nocturnal Rhinoconjunctivitis Quality of Life Questionnaire (NRQLQ) were used to evaluate the quality of life.
RESULTSThe individual and total symptomatic scores of week 1, week 2 and week 4 decreased compared with baseline. The scores of Mini-RQLQ and NRQLQ of week 1, week 2 and week 4 decreased compared with baseline too. The scores of the next visit were significantly lower than those of the previous visit (P < 0.01).
CONCLUSIONSA four-week administration of mometasone furoate nasal spray can effectively treat allergic rhinitis.

6.Effects of ADRB2(rs1042713)Gene Polymorphism on Therapeutic Efficacy of Anticholinergic Drugs in the Treatment of Refractory Asthma Pediatric Patients

OBJECTIVE: To study the effects of ADRB2 (rs1042713) gene polymorphism on therapeutic efficacy of anticholinergic drug in the treatment for refractory asthma pediatric patients. METHODS: 171 children with refractory asthma were selected from outpatient department of Kunming Children’s Hospital during Nov. 2016 to Jul. 2019. The distribution of ADRB2 (rs1042713) genotype, the clinical efficacy [asthma control test (C-ACT) score, FEV1, FVC, PEF, maximal mid-expiratory flow (MMEF)] of anticholinergic drug were analyzed statistically; the response of different genotypes to the use of anticholinergic drug were also analyzed statistically. RESULTS: 148 of 171 refractory asthmatics pediatric patients were administered anticholinergic drug, among them 50 of the 71 AA genotype and 36 of the 77 GA genotype responded to anticholinergic drug treatment. Statistical analysis showed that 71 children with AA refractory asthma had improved C-ACT score, FEV1, FVC, PEF and MMEF, there was statistical significance, compared with GA genotype (P<0.05); the response rate of the AA genotype to anticholinergic drugs was 2.71 times that of the GA genotype [OR=2.71, 95%CI (1.38, 5.34), P=0.005]. CONCLUSIONS: The detection of ADRB2 (rs1042713) gene polymorphism has some guiding significance in the treatment of refractory asthma with anticholinergic drugs, and the response of AA genotype is better.
7.Prognostic Value of Morphology and Hans Classification in Diffuse Large B Cell Lymphoma.

OBJECTIVETo investigate the prognostic value of morphology and Hans classification in diffuse large B cell lymphoma(DLBCL).
METHODSClinical data of 249 patients diagnosed with DLBCL in our hospital and Hangzhou Xixi hospital during Jan 2006 to Dec 2016 were analyzed retrospectively. These patients were classified into 3 groups: immunoblastic variant(IB) group, centroblastic variant(CB) group and others group according to the cell morphology. And DLBCL was also divided into GCB(germinal center B-cell-like)or non-GCB(non-germinal center B-cell-like) group by analyzing the expression of CD10, BCL6 and MUM1 (GCB: CD10 ,BCL6,MUM1/CD10,BCL6,MUM1;non-GCB:CD10,BCL6,MUM1/CD10,BCL6,MUM1).
RESULTSThe univariate analysis displayed that the age,LDH level,IPI,IB,non-GCB,B-symptoms and rituximab all could influence the OS and EFS, the CR rate of CB subtype patients was significantly higher than that of the patients with IB subtype (68.3% vs 38.9%)(P=0.02). IB subtype was the in dependent prognostic factor for both EFS and OS in the whole study. In multivariate analysis, IPI and IB were the independent prognostic factors for OS and EFS. IB subtype was also an independent prognostic factor in EFS and OS with or without rituximab. The expression of BCL2 and BCL6 was related with prognosis in R-CHOP, but not in CHOP treated patients. Other markers (CD5, CD10, IRF4/MUM1, HLA-DR and Ki-67 proliferation index) were not of the significant prognostic value for DLBCL. When accepted rituximab, the GCB and non-GCB were not different significantly for prognosis. However, the non-GCB group showed a poor prognosis without using rituximab (EFS P=0.020;OS P=0.020). Multivariate Cox models showed that OS and EFS were not significantly different between GCB and non-GCB group, however, the IB subtype had a very significantly poor prognosis in OS and EFS (P=0.001, P=0.002). When the analysis was restricted to DLBCL with CB morphology only, no prognostic value was observed in Hans classification.
CONCLUSIONThe subtype of immunoblast is a major risk factor in patients treated with CHOP or R-CHOP. There is a significant association between the Hans classification and the morphologic subclassification. Results of this study have supplemented the data for the prognostic factor of DLBCL and demonstrated that the cytomorphologic diagnosis can be reproducible.
