3.Relationship of CPAP pressure titration methods and CPAP compliance

OBJECTIVE To study the relationship between the methods of CPAP pressure titration (under whole night PSG or by auto-set) and CPAP compliance. METHODS Forty-five patients with serious OSAHS who had been diagnosed in our sleep lab between 2001 and 2004 and were willing to try CPAP treatment were enrolled. They were divided into A and B group according to their wills: pressure titration under whole night PSG in lab (group A) or by auto-set in their families (group B). After pressure titration, patients and their families were educated for the knowledge of CPAP treatment in OSAHS patients. Questionnaires about QOL before CPAP treatment and half year later were carried out on the phone. The decreased score was used to evaluate the effect of CPAP treatment. The hours of per day using machine were used to evaluate the compliance of CPAP. RESULTS Group A and group B got the similar decreased score (14 VS 14.3) , compliance (4.91h VS 4.97h) and adherence (85.7 % VS 88 %) . CONCLUSION OSAHS patients under go pressure titration either in lab with PSG or in their home with auto-set have the same effect and same compliance of CPAP treatment. That means the simple auto-titration can substitute the traditional titration under whole night in lab.
4.The report of extreme lateral lumbar disc herniations (15 cases)

Objective To explore the clinical and CT characteristics of extreme lateral lumbar disc herniations and improve the diagnosis. Methods A retrospective analysis of the clinical and CT manifestations in 15 patients with extreme lateral lumbar disc herniations proved by operation was done. Results Extreme lateral lumbar disc herniation at L4/5 level is more common. Intraforaminal lumbar disc herniations combined with extraforaminal herniation were seen in most cases. The clinical findings often presented lumbar bi-root compression syndromes. Conclusion The clinical presentation combined with CT image is the key to the correct diagnosis.
5.Determination of berberine hydrochloride and palmatine hydrochloride in Ruyijinhuang Powder by HPCE

Objective: A capillary electrophoresis for the determination of berberine hydrochloride and palmatine hydrochloride in Ruyijinhuang Powder(Rhizoma Curcumae Longae, Radix et Rhizome Rhei, Cortex Phellodendri, Rhizoma Atractylodis, etc.) was established. Methods: The berberine hydrochloride and palmatine hydrochloride were determined using benzoic acid as an internal standard its detection wavelength was at 230nm. The optimized condition was achieved with a fused\|silica capillary 100?m?56.5cm, 10mmol?L -1 phosphate buffer(pH=4.80) containing 30% acetonitrile, a constant voltage of 20kv and temperature at 30℃. Results: Calibration curve was constructed in the range of 5?g?mL -1 ~100?g?mL -1 for berberine hydrochloride and palmatine hydrochloride, respectively. The regression equations were Y=0.02964X -0.00771 ( r =0.9998) and Y =0.01092 X -0.02939 ( r =0.9997) for berberine hydrochloride and palmatine hydrochloride, respectively. The average recovery was 98.7% and 102.1%( n =5), respectively. Conclusion: This method is sample, quick and sensitive.
6.Research on the correlation of morphologic changes in oropharynx of OSAHS patients before & after UPPP operation with its therapeutic effectiveness

7.Construction and Genetic Analysis of Murine Hepatitis Virus Strain A59 Nsp16 Temperature Sensitive Mutant and the Revertant Virus

Coronaviruses (CoVs) are generally associated with respiratory and enteric infections and have long been recognized as important pathogens of livestock and companion animals. Mouse hepatitis virus (MHV) is a widely studied model system for Coronavirus replication and pathogenesis. In this study, we created a MHV-A59 temperature sensitive (ts) mutant Wu"-ts18(cd) using the recombinant vaccinia reverse genetics system. Virus replication assay in 17C1-1 cells showed the plaque phenotype and replication characterization of constructed Wu"-ts18(cd) were indistinguishable from the reported ts mutant Wu"-ts 18. Then we cultured the ts mutant Wu"-ts 18(cd) at non-permissive temperature 39.5℃, which "forced" the ts recombinant virus to use second-site mutation to revert from a ts to a non-ts phenotype. Sequence analysis showed most of the revertants had the same single amino acid mutation at Nsp16 position 43. The single amino acid mutation at Nsp16 position 76 or position 130 could also revert the ts mutant Wu"-ts 18 (cd) to non-ts phenotype, an additional independent mutation in Nsp13 position 115 played an important role on plaque size. The results provided us with genetic information on the functional determinants of Nsp16. This allowed us to build up a more reasonable model of CoVs replication-transcription complex.
8.Effects of moxibustion on the expressions of hippocampal VEGF, flt-1, bFGF, and bFGF-r in vascular dementia rats.

OBJECTIVETo observe the expression levels of hippocampal vascular endothelial growth factor (VEGF), fms-like tyrosine kinase-1 (flt-1), basic fibroblast growth factor (bFGF), and basic fibroblast growth factor receptor (bFGF-r) in vascular dementia (VD) rats, thus studying the angiogenesis mechanism of moxibustion in VD.
METHODSSixty male elderly Wistar rats were selected. The VD rat model was prepared by bilateral carotid artery occlusion and reperfusion of sodium nitroprusside injection. The model rats were divided into 3 groups by the random digit table, i. e., the moxibustion group, the Western medicine group, and the model group. A sham-operation control group was also set up. In the moxibustion group rats was acupunctured at Baihui (GV20), Shenting (GV14), and Dazhui (GV24). Aniracetam was given to rats in the Western medicine group by gastrogavage for 2 therapeutic courses, 15 days as one course. The learning and memory results were observed by the neuroethological score in combination of step-down avoidance test before treatment and by the end of the 2nd course respectively. The expression levels of hippocampal VEGF, flt-1, bFGF, and bFGF-r of all rats were detected using immunohistochemical assay.
RESULTSAfter 2 courses of treatment, statistical difference existed in the latent period, the error times, and the neuroethological score in the moxibustion group and the Western medicine group when compared with the model group (P < 0.01, P < 0.05). Statistical difference existed in the latent period and the neuroethological score between the moxibustion group and the Western medicine group (P < 0.05), which indicated that moxibustion and Western medicine showed significant effects in improving the latent period, decreasing the error times and the neuroethological score. Better results were obtained in the moxibustion group than in the Western medicine group (P < 0.01, P < 0.05). Statistical difference of the average grey level (AGL) of hippocampal VEGF, flt-1, and bFGF existed in the moxibustion group and the Western medicine group when compared with the model group. Statistical difference of the bFGF-r expression existed only between the moxibustion group and the model group. Statistical difference of the VEGF and flt-1 expressions existed between the moxibustion group and the Western medicine group (P < 0.05).
CONCLUSIONSMoxibustion showed confirmative effects in improving the behavioral score and memory performance in VD rats. Its mechanisms might lie in that moxibustion regulated and controlled the expression levels of hippocampal VEGF, flt-1, bFGF, and bFGF-r in VD rats. Particularly it up-regulated the expression levels of key factors VEGF and flt-1, promoted the angiogenesis in the vital parts, and ultimately stimulated the repairing mechanisms of cerebral nerve injury.

9.Beneficial effect of cod-derived polypeptides (CDPs) on wound healing in a rat model of skin incisions

Objective To investigate the wound-healing process in a rat model of skin full-thickness incisions and to detect related possible mechanism.Methods Twenty-four female rats were selected and the dorsal skin of rats was used as the experimental area.A cutaneous excision (6 mm diameter) was made on the back of each animal,close to the cervical area.The dorsal skin of every rat was allocated to three groups which were treated with physiological saline,human recombinant epidermal growth factor (rhEGF),and CDPs,respectively.After making a rat model of skin incisions,we observed the wound healing process,took photos of the wounds under a digital microscope,and use sulfuric graph paper to record the size of every wound.At the 3rd,6th,9th,12th day after modeling,6 rats were killed,and mRNA expression of K10,K14,and EGF was detected in the skin tissues using a RT-PCR technique.Results At the 6th and 12th day after modeling,there were significant differences between the experimental group and the blank control group (P<0.05).The gene expression of EGF,K-14 in the third day and that of EGF,K-10 and K-14 in the 6th and 12th day were upregulated compared with control group,and there were significant differences between them (P < 0.05).Conclusions CDPs have a beneficial effect on the acceleration of skin wound healing,possibly due to increasing keratinocyte proliferation and up-regulating the expression of K10,K14 and EGF genes.

A woman aged 31 had recurrent urinary tract infection with bloody urine. A series image of medullary sponge kidney presented by intravenous urography (IVU) was detected dynamically by retrograde pyelography (RP). Other than ultrasonography and IVU, RP is also a reliable method to detect medullary sponge kidney.
