1. The advantages of early application of infliximab to Crohn's disease treatment

OBJECTIVE: To investigate the clinical efficacy of early application of infliximab to Crohn's disease treatment. METHODS: Forty-eight patients with Crohn's disease were randomly divided into ATI group and AZA group with 24 patients in each group. The patients in ATI group were treated with infliximab in the first 14 weeks while the patients in AZA group only used azathioprine. Both groups were followed up for 50 weeks. The clinical efficacy, adverse reactions, treatment effective time and recurrences were compared between the two groups. RESULTS: The clinical effective rate in the ATI group was 91.67%, significantly higher than that in AZA group (66.67%, P<0.05); the average time of cure for the ATI group was shorter than the control group (P<0.05); the mucosal healing rates was 25.00%, significantly higher than the AZA group (4.17%, P<0.05); the recurrence rate in the ATI group was 16.66%, lower than the AZA group (37.50%, P<0.05). CONCLUSION: The method has good efficacy for Crohn's disease. It can significantly improve the clinical efficacy, reduce adverse reactions, complications and recurrence rate, and should be widely used in clinic. 2.Stress analysis of fiber posts combined with auxiliary posts in restored anterior tooth with flared canals

Objective To assess the stresses of fiber posts combined with auxiliary posts in anterior teeth with flared canals by three dimensional finite element method .Methods Nine 3D models of maxillary first incisors with flared canals restored by two post systems(porcelain pile ,fiber and auxiliary pile) divided into 5 groups were generated .By static loading method ,dental crowns cut 1/3 and middle 1/3 connection link five points into 135 angles by 100 N force loading .The major stress for the dentin and the e‐quivalent Von Mises stress for resin cement and post and core were calculated .Results Biggest tooth stress value of 111 .9 Mpa and the largest resin adhesive stress value 54 .6 Mpa had appeared in the use of a model of the quartz fiber post after repair ,ceramics pile repair model of pile nuclear stress value and the maximum was 48 .1 Mpa .Combining with one auxiliary post ,stress values re‐duced significantly in dentin ,in cement and in post and core .However ,as more auxiliary posts were added ,the stresses did not de‐crease sequentially .Conclusion It is suggested that to restore the tooth with flared canals by quartz fiber post combining with one auxiliary post .
4.Effects of S-adenosylmethionine on hyperhomocysteinemia and the expression of TNF-? in ethanol-induced liver injury in rats

Objective To investigate the effects of S-adenosylmethionine (SAM) on ethanol-induced liver injury in rats. Methods 48 female SD rats were randomly divided into 4 groups as control, model, small and large dose SAM groups. Except control group, all rats were fed high fat-containing diet plus ethanol and fish oil gavage for 8 weeks. SAM was administered by intraperitioneal injection after 4 weeks of exposure to ethanol. Plasma total homocysteine (tHcy), serum aminotransferase activity (ALT), liver malondialdehyde (MDA), glutathione (GSH) contents were assayed, and expression of TNF-? was detected by immunohistochemistry, and liver histology was also examined. Results Compared with control group, model group rats developed marked liver damage, accompanied by an increase of tHcy, ALT, and MDA levels, but a decrease of GSH levels (P0.05). Conclusion SAM prevents alcohol-induced liver injury in rats by reducing liver lipid peroxidation, together with down-regalation of expression of TNF-?. SAM does not affect the plasma total homocysteine levels.

BACKGROUND:Interproximal contact loss is a complication of posterior crown/bridge restorations with a higher incidence, and it is induced by a variety of factors, such as age, tooth position, occlusal contact, tooth loss time, whether there is a repair on the free end.OBJECTIVE:To study and analyze the loss of interproximal contact between posterior crown/bridge restorations and adjacent teeth.METHODS: This was a single-center, prospective, observational clinical trial which has been completed at the Stomatology Hospital Affiliated to Southwest Medical University, China. Totaly 82 patients who had underwent posterior crown/bridge restorations from June 2015 to June 2016 in the Stomatology Hospital Affiliated to Southwest Medical University were selected. The time of denture wearing was 1-40 months, and the interproximal contact loss was defined by a 30 μm adjacent contact check or a flossing that can pass through the detection area with no resistance. Single-factor analysis and multiple correlation analysis were performed to analyze the interproximal contact loss at 1, 3, 6 months after posterior crown/bridge restorations. The study protocol was approved by the Ethics Committee of the Stomatology Hospital Affiliated to Southwest Medical University of China with an approval number of 2016024. All protocols were in accordance with Declaration of Helsinki, formulated by the World Medical Association. All patients and their relatives were informed of study protocols and provided a written informed consent prior to the beginning of the trial.RESULTS AND CONCLUSION:Of the 82 patients enroled, there were 41 adjacent contact zones with the emergency of interproximal contact loss, including 20 maxillary and 21 mandibular. It indicated that the interproximal contact loss had no association with the upper and lower jaw. Single-factor analysis showed that the time of denture wearing, tooth loss time and repair of the free end were confirmed as independent influencing factors for the complications (P < 0.05). Multiple correlation analysis found that free end repair and short-term tooth loss were independent factors of the interproximal contact loss. To conclude, free end repair and tooth loss time should be considered in the posterior crown/bridge restorations in order to minimize the occurrence of interproximal contact loss.
7.Three-dimensional digital model measurements of the temporomandibular joint

BACKGROUND:Guidances for design of Chinese people’s temporomandibular joint prosthesis are urgently needed because foreign prosthesis is unsuitable for Chinese people due to variant anatomy parameters.
OBJECTIVE:To provide a reference for improving the design of artificial temporomandibular joint through computer three-dimensional digital measures of the anatomy parameters of the temporomandibular joint. METHODS:Continuous CT data with layer thick 0.625 mm of 30 normal temporomandibular joints were selected. Three-dimensional digital reconstruction and eight related anatomy parameter measurements of the articular fossaandcondylar of the joints were performed using Mimics10.01 software.
RESULTS AND CONCLUSION:The internal and external diameter of the mandibular fossa, head of mandible and neck of mandible were greater thantheanterior and posteriordiameter. The mandibular foramen was located at middle-posterior of the ramus ofthe mandible. The distance between head of the mandible and coronoid process was longer than the depth of the mandibular notch. There were no
significant differences between two sidesregardingal above-mentioned parameters. There are certain regular patterns in anatomy parameters of the temporomandibular joint. The study results wil provide a reference for improving the design of artificial temporomandibular joint.
8.The efficacy and safety of human glucagon-like peptide-1 analogue liraglutide in newly diagnosed type 2 diabetes with glycosylated hemoglobin A1c > 9%

Objective To evaluate the efficacy and safety of human glucagon-like peptide-1 analogue liraglutide in newly diagnosed type 2 diabetes mellitus (T2DM) with glycosylated hemoglobin A1c (HbA1c) > 9%.Methods This was an open-labelled,randomized,parallel-group,treat-to-target trial.Newly diagnosed T2DM patients with HbA1c > 9% were enrolled.These patients were treated with metformin with repaglinide and randomized to receive once-daily liraglutide (LIRA,n =25) or the insulin glargine (IGla,n =24) at bedtime.Efficacy and safety were assessed and compared after 18-month treatment.Results (1) Compared with the baseline,patients with LIRA had significantly reduced mean body weight,BMI and waist circumference (P < 0.01),whereas,the above indexes were increased (P < 0.01) in patients treated with IGla.(2) After 18 months of treatment,fasting plasma glucose (FPG),2-hour plasma glucose after a 75g oral glucose load(2hPG) and HbA1c were significantly improved in all patients (P < 0.01),with 2hPG,mean blood glucose (MBG),the largest amplitude of glycemic excursions (LAGE),mean amplitude of glycemic excursions (MAGE) were significantly lower in LIRA group than in IGla group (all P < 0.05).(3) HOMA-IR decreased in both groups (P < 0.05).However,△I30/△G30,AUCCP180 and Matsuda index were only significantly increased in patients treated with LIRA (respectively,4.88 ± 1.55 vs 7.60 ± 1.91,9.23 ± 2.66 vs 13.18 ± 2.72,39.28 ± 20.35 vs 54.64 ± 23.34,all P < 0.01),while HOMA-IR reduced(4.41 ± 1.58 vs 3.52 ± 1.44,P <0.05).But in IGla group only HOMAIR was reduced (4.92 ± 1.84 vs 4.57 ± 1.80,P <0.05).The index of △I30/△G30,AUCCP180 and Matsuda index in LIRA group are higher than those of indexes in IGla group(respectively,7.60 ± 1.91 vs 4.18 ± 1.00,13.18 ± 2.72 vs 10.53 ± 2.68,54.64 ± 23.34 vs 41.65 ± 17.84,all P < 0.05),while HOMA-IR is lower (3.52 ± 1.44 vs 4.57 ± 1.80,P < 0.05).(4) The rate of HbA1 c ≤ 6.5 % and the dosages of oral anti-diabetic drugs in LIRA group were significantly better than that in IGla group.(5) No significant differences were observed in hypoglycemic episodes and adverse events between two groups.Conclusion It seems that liraglutide is superior to insulin glargine in newly diagnosed T2DM patients with HbA1c > 9% in improving beta-cell function,insulin sensitivity and glucose homeostasis.

The ratio of insulin positive area in pancreas tissue was obviously rised, while the ratio of glucagon positive area in pancreas tissue was clearly descended in T2DM+LIRA+UC-MSCs group. And the same difference in valuating islet cells apoptosis by TUNEL could be observed ( P<0. 05). The expression of NF-κB and TLR4 protein in pancreas tissue of T2DM+LIRA+UC-MSCs group were the least amongthefourgroups[(0.75±0.10)vs(0.60±0.08),(0.47±0.08),(0.31±0.04),P<0.05]and[(1.24± 0. 12) vs (0. 93 ± 0. 10), (0. 95 ± 0. 09), (0. 74 ± 0. 07), P<0. 05 ]. Conclusion The combined treatment of liraglutide with umbilical cord mesenchymal stem cells is superior over a single treatment of liraglutide or umbilical cord mesenchymal stem cells in improving the islet function in type 2 diabetes mellitus models, which may be related to the down modulating the TLR4/NF-κB inflammatory signaling pathway.
10.Effect of liraglutide combined with bone marrow mesenchymal stem cells on glucose metabolism in experimental type 1 diabetic rats by regulating gastrointestinal hormones

Objective To investigate the effect of liraglutide combined with bone marrow mesenchymal stem cells (BM-MSCs) on glucose metabolism in experimental type 1 diabetic (T1DM) rats.Methods T1 DM rats were established by injecting 60 mg/kg streptozotocin.According to random number table,they were divided into T1DM group (n =10),BM-MSCs group (n =10),LIRA group (n =10),and LIRA+BM-MSCs group (n =10),and were treated for 8 weeks respectively.Random blood glucose,24 h urine volume and protein excretion were tested.Serum concentrations of insulin,C-peptide,glucagon,gastrin,cholecystokinin,and glucagon-like peptid 1 (GLP-1) were assayed by ELISA.Expressions of insulin and glucagon in pancreas were measured by immunohistochemistry.Results After 8 weeks,the levels of random blood glucose,HbA1C,24 h urine volume and 24 h urinary protein excretion in group 4 were significantly decreased compared to the other three groups (P<0.05).Compared to T1DM group and BM-MSCs group,the levels of insulin,C-peptide,gastrin,cholecystokinin and GLP-1 in LIRA+BM-MSCs group were significantly increased,while glucagon was decreased (P<0.05).Compared to group LIRA,the levels of insulin,C-peptide,gastrin,and cholecystokinin in LIRA + BM-MSCs group were increased (P < 0.05),there was no significantly difference in glucagon or GLP-1 (P>0.05).The analysis revealed that the level of insulin was positively correlated with gastrin (r =0.544,P<0.01),cholecystokinin (r =0.710,P<0.01) and GLP-1 (r =0.669,P< 0.01),but was negatively correlated with glucagon (r =-0.506,P<0.01);the level of glucagon was negatively correlated with gastrin (r =-0.364,P<0.05),cholecystokinin (r =-0.433,P<0.01) and GLP-1 (r =-0.591,P< 0.01).Compared to the other three groups respectively,immunohistochemistry displayed that the positive area of insulin in pancreas was significantly increased in LIRA + BM-MSCs group,while that of glucagon was decreased (P< 0.05).Conclusions By means of regulating gastrointestinal hormones efficiently,combination of liraglutide withBM-MSCs may improve glucose metabolism more efficaciously than treatment with a single agent in T1DM rats.
