2.Distribution characteristics of overweight and obesity in 0-5 years old children born small for gestational age in Shanghai city

Objective To understand the distribution of overweight and obesity in 0-5 years old children who were born small for gestational age ( SGA) in Shanghai through a cross-sectional investigation. Methods All resident children aged 0-5 years were included, covering all 18 districts in Shanghai. Health-check cards were prepared for SGA children. The check-up included weight,length/height and head circumference. Body mass index ( BMI) was used to evaluate the overweight and obesity according to the standard of World Health Organization ( WHO) . Results This study investigated 728 602 children aged 0-5 years in Shanghai,and ultimately 23 871 of them were defined as SGA,a-mong whom 9 805(41. 4%) were boys and 14 066(58. 9%) were girls. The BMI of SGA children were higher than that of appropriate for gestational age( AGA) from 4 to 18 months,while for the rest of the time,they were basically the same. There was no difference in changing tendency of BMI between SGA children and those AGA children. The distri-bution of overweight and obesity according to the standard of WHO in SGA boys among different ages was 7. 7% to 20. 7%, and 15. 7% in average;the distribution of SGA girls among different ages was 5. 9% to 18. 3%,and 12. 9% in average. The proportion of overweight at the age of 4-18 months was significantly higher than that of other ages. There was no correlation between overweight at 0-2 years old and overweight at the age of 5(P>0. 05). Conclusions Com-pared with SGA girls,overweight and obesity in SGA boys were more serious. The age of 4-18 months were the period of high incidence of overweight. There was no correlation between overweight at 0-2 years old and overweight at the age of 5.
3.Comparison in physical and neurological development between two types of children born small for gesta-tional age

Almost 3% of newborns are diagnosed as small for gestational age(SGA)worldwide.Born SGA is one of the important causes of perinatal morbidity and mortality,and is also associated with metabolic diseases in adulthood.Low birth weight by itself is insufficient to characterize growth restriction,as it does not include information about the neonate’s body proportionality.Depending on the origin,timing and severity of in-sult,small for gestational age infants are classified into two types:proportionate or symmetric growth restriction (SGR)and disproportionate or asymmetric growth restriction(AGR).There may be differences in physical and neurological development of these two types.This study compares three classification indexes,and to find differ-ences in postnatal growth of these two types.
4.Curative effects observatin of Edaravone on the patients with acute massive cerebral infarction

Objective To observe the curative effects of treatment with Edaravone for acute massive cerebral infarction.Methods 48 patients with acute massive cerebral infarction were randomized into the treatment with Edaravone group(Edaravone group,24 patients) and the conventional treatment control group(control group,24 patients).Two groups patients were admitted conventional treatment for cerebral infarction.Edaravone group patients were admitted with Edaravone 30 mg into 100 ml saline infusion introvenously,twice a day,linked 20 ~ 25 d.Respectively before and after the treatment,neurologic function dificit score(NDS),plasma fibrinogen(Fib) content,coagulation blood function,activity of superoxide dismutase(SOD) were examined.The clinical efficacy was compared between the two groups.Results NDS of tow groups after treatment were significantly lower than those of before treatment,activity of SOD were significantly increased than those before treatment(all P0.05).Significant efficiency ratio of the Edaravone group(87.5%) was significantly higher than that of the control group(45.8%)(P
5.Effects of Danshen Dripping Pill on glycolipid metabolism in insulin resistant rats

AIM:We investigated the effects of Danshen Dripping Pill(DSP) on the glucose metabolism in rats during the insulin resistant(IR) statue. METHODS:HOMA-IR was used to show the degree of insulin resistant and Sprague-Dawley rats were randomly divided into 5 groups,normal control,IR control,Pioglitazone(PIO),Danse Injection(DSZ) and DSP groups,and the model rats were given dexamethasone to induce the insulin resistant statue.After being treated for 8 weeks,the serum adiponectin concentration and the relative expression amount of Glut-4 mRNA in skeletal muscles were detected by ELISA and real time PCR,respectively. RESULTS:The insulin resistant model rats induced by dexamethasone had a series of characters with the high level of HOMA-IR,and the serum adiponectin concentration was decreased,the Glut-4 mRNA relative expression amount in skeletal muscle cell and its translocation on the plasma membrane were inhibited.The effects of dexamethasone were significantly reversed by PIO,DSZ and DSP,respectively(P

Objective To investigate the effects and the mechanism of Danshen dripping pill(DSP,复方丹参滴丸) on the lipid metabolism in rats under insulin resistant(IR) status.Methods Twenty-four Sprague-Dawley(SD) rats were randomly divided into normal control(n=8),IR control(n=8) and DSP groups(n=8).The rats were given dexamethasone(1 mg/kg) intramuscular injection once every other day to induce the IR status in the latter two groups,and the rats in DSP group were administered intra-gastrically with DSP 0.5 g/kg dissolved in normal saline once a day in the morning at 9 o′clock,and in the mean time,the rats in the normal and IR control groups were given intra-gastrically with equal amount of 0.9% NaCl. After 8 weeks of treatment,fasting blood glucose(FBG),fasting insulin(FINS),total cholesterol(TC),triglyceride(TG),nitric oxide synthase(NOS),nitrogen monoxidum(NO),smooth muscle depth/wall thickness of thoracic aorta were detected,and serum adiponectin concentration was measured by enzyme linked immunosorbent assay(ELISA),respectively.Results After IR model was successfully established,the insulin resistance index(HOMA-IR),TC and TG were increased,the serum NOS production and NO were decreased, smooth muscle depth/wall thickness of thoracic aorta was increased significantly,and the secretion of adiponectin concentration was decreased(P
7.Effect role of initial planes on the examination of fetal heart by four-dimensional echocardiography with spatiotemporal image correlation

Objective To investigate whether different initial planes have an effect on examination of the fetal heart by four-dimensional ultrasound with spatiotemporal image correlation (STIC).Methods The standard examination planes of fetal heart were rendered using the heart volume data which were acquired in different initial planes with STIC,then these rendered planes were compared and scored.Results① At the four-chamber initial planes,the reconstruction rate of longitudinal planes of the aortic arch (46.27%) and the pulmonary artery (49.25%) were poor,in addition,the reconstruction showed low quality,and the average scores were 1.366 and 1.403,respectively.② At the left outflow-tract initial planes,reconstruction rates of all planes were above 75%,and there was no statistical difference(P=0.169).Each reconstruction plane had good score,even if the scores of longitudinal planes of the aortic arch/pulmonary artery were low,whose scores reached 1.936 and 2.051,respectively.③ At the initial longitudinal planes of the aortic arch,the reconstruction rate (96.67%) and average reconstruction score (2.617) of longitudinal planes of the pulmonary artery were obviously higher than those of other planes.④ Compared with different initial planes,volume data acquired from left outflow-tract initial planes could lead to high-quality reconstructed planes of all standard examination planes.While four-chamber initial planes acquired high-quality standard examination planes except longitudinal planes of the aortic arch and pulmonary artery.Volume data from initial longitudinal plane of the aortic arch acquired low-quality standard examination planes except longitudinal planes of the aortic arch and longitudinal plane of the pulmonary artery.Conclusions With STIC,sonographers can not only save the volume data for off-line analysis and remote consultation but also choose the better initial planes for themselves during the examination of fetal hearts.
8.The efficacy and safety of human glucagon-like peptide-1 analogue liraglutide in newly diagnosed type 2 diabetes with glycosylated hemoglobin A1c > 9%

Objective To evaluate the efficacy and safety of human glucagon-like peptide-1 analogue liraglutide in newly diagnosed type 2 diabetes mellitus (T2DM) with glycosylated hemoglobin A1c (HbA1c) > 9%.Methods This was an open-labelled,randomized,parallel-group,treat-to-target trial.Newly diagnosed T2DM patients with HbA1c > 9% were enrolled.These patients were treated with metformin with repaglinide and randomized to receive once-daily liraglutide (LIRA,n =25) or the insulin glargine (IGla,n =24) at bedtime.Efficacy and safety were assessed and compared after 18-month treatment.Results (1) Compared with the baseline,patients with LIRA had significantly reduced mean body weight,BMI and waist circumference (P < 0.01),whereas,the above indexes were increased (P < 0.01) in patients treated with IGla.(2) After 18 months of treatment,fasting plasma glucose (FPG),2-hour plasma glucose after a 75g oral glucose load(2hPG) and HbA1c were significantly improved in all patients (P < 0.01),with 2hPG,mean blood glucose (MBG),the largest amplitude of glycemic excursions (LAGE),mean amplitude of glycemic excursions (MAGE) were significantly lower in LIRA group than in IGla group (all P < 0.05).(3) HOMA-IR decreased in both groups (P < 0.05).However,△I30/△G30,AUCCP180 and Matsuda index were only significantly increased in patients treated with LIRA (respectively,4.88 ± 1.55 vs 7.60 ± 1.91,9.23 ± 2.66 vs 13.18 ± 2.72,39.28 ± 20.35 vs 54.64 ± 23.34,all P < 0.01),while HOMA-IR reduced(4.41 ± 1.58 vs 3.52 ± 1.44,P <0.05).But in IGla group only HOMAIR was reduced (4.92 ± 1.84 vs 4.57 ± 1.80,P <0.05).The index of △I30/△G30,AUCCP180 and Matsuda index in LIRA group are higher than those of indexes in IGla group(respectively,7.60 ± 1.91 vs 4.18 ± 1.00,13.18 ± 2.72 vs 10.53 ± 2.68,54.64 ± 23.34 vs 41.65 ± 17.84,all P < 0.05),while HOMA-IR is lower (3.52 ± 1.44 vs 4.57 ± 1.80,P < 0.05).(4) The rate of HbA1 c ≤ 6.5 % and the dosages of oral anti-diabetic drugs in LIRA group were significantly better than that in IGla group.(5) No significant differences were observed in hypoglycemic episodes and adverse events between two groups.Conclusion It seems that liraglutide is superior to insulin glargine in newly diagnosed T2DM patients with HbA1c > 9% in improving beta-cell function,insulin sensitivity and glucose homeostasis.
9.Effects of pioglitazone on the expressions of HIF-1α and VEGF in renal tissues of diabetic rats

The effects of pioglitazone on the expressions of hypoxia-inducible factor-1 α (HIF-1 α) and vascular endothelial growth factor (VEGF) in renal tissues of diabetic rats were observed. Diabetic rat model was established by feeding high-carbonhydrate-fat diet and injecting streptozotocin. After the treatment with pioglitazone, the kidney index, 24 h urinary albumin, blood urea nitrogen, serum creatinine, fasting blood glucose, fasting insulin, homeostasis model assessment for insulin resistance (HOMA-IR), total cholesterol,triglycerides, and low-density lipoprotein-cholesterol of diabetic rats were significantly lower than those of untreated ones, high-density lipoprotein-cholesterol was increased, the expressions of HIF-1α and VEGF in renal tissue were decreased ( all P<0. 01 ). It suggested that pioglitazone may improve renal function and the balance of glucose-lipid metabolism in diabetic rats via down-regulating HIF-1/VEGF pathway.
10.Influence of convergence angle on the retention of cast metal post and core restorations

BACKGROUND: The residual root of anterior teeth are usually repaired by means of post and cores plus porcelain fused metal crown in clinic, and the retention force is mainly provided by the post because of the deletion of crown, so the retention force provided by the post is directly correlated with the long-term effect of repair.OBJECTIVE: To analyze the changes of retention of cast metal post and core restorations with different convergence angles, and investigate the influence of convergence angles on retention.DESIGN: A repeated observational measurement.SETTING: Department of Prosthodontics, West China School of Stomatology, Sichuan University.MATERIALS: The experiments were carried out in the building of high polymer material of Sichuan University in May 2005. Fifty-four simulated tooth roots were manufactured by the numerical control machine tools.They were divided into six groups according to the convergence angles of post being 0°, 3.93°, 5.71°, 7.48°, 11.31° and 14.71°, with nine specimens in each group.METHODS: ① Manufacture of the standard specimens: The tooth roots with the convergence angles of 0°,3.93°, 5.71°, 7.48°, 11.31° and 14.71° were prepared with the digital control precision machine. ② Manufacture of the cast post and cores: The post and core wax patterns of the specimens were made in the condition similar to the clinical practice. The post and cores were cast and tried in, necessary grinding was made to ensure the adoption of the posts and the specimens. Then the posts were luted to the specimens with ZPC after the posts and the simulated root canals having een cleaned by alcohol. The posts and the specimens were pressed until the ZPC solidified. ③ Retention Force test: After having been dipped in physiological saline for 24 hours, the bonded castings and specimens were subjected to a tensile testing at the drawing speed of 10 mm per minute.The retention force of post was the load recorded when the post was pulled out from the specimen. ④ Planar bonding strength test: We cut a PMMA stick into the same little cylinders and made angular milling on the flank of the cylinders. The angle between the milled bevel and the central axis of the cylinder was controlled to 0°, 1.9°, 3°, 6°, 9°, 12° and 15°. Afterwards we cut the brass into pieces. Eight wafers were cast with the same diameter of 8 mm and thickness of 1 mm. With the same procedure of bonding the posts to specimens, the brass pieces were bonded to the cylinder bevels and the wafers were adhered to the upside of some unmachinized cylinders.Immerged in physiological saline for 24 hours, their bonding force were tested at the speed of 10 mm per minute, and the load was recorded when the pieces and wafers were separated from the cylinders. ⑤ Calculation of the frictional and interlocking forces: As the post can be regarded as a frustum, the bonding surface area can be calculated with the formula calculating the flank area of frustum, and the area of the occlusal bonding surface is the sum of the annulus part of the root surface and the circular part of the post tip. Some regressions were made to the retention force, frictional strength, bonding strength per unit area, retention force per unit area and frictional force per unit area to convergence angles.MAIN OUTCOME MEASURES: ① Bonding strength of different convergence angles; ② Influence of different convergence angles on the area and bonding strength of the post; ③ Retention force and frictional force of posts with different convergence angles; ④ The results of the one-way analysis of variance (ANOVA) of bonding strength, retention force and frictional force per unit area.RESULTS: ① The bonding strength of different convergence angles: It was 0.309 0 N/mm2 for the occlusal plane, 0.128 3 N/mm2 for the axial plane with a convergence angle of 0°, 0.108 7 N/mm2 for the 1.9°axial plane, 0.107 2 N/mm2 for the 3°axial plane, 0.084 9 N/mm2 for the 6°axial plane, 0.056 7 N/mm2 for the 9°axial plane, 0.046 3 N/mm2 for the 12°axial plane, 0.027 4 N/mm2 for the 15°axial plane. ② Bonding strength and bonding area of posts with different convergence angles: The post with a 0°convergence angle had a total post area of 108.047 mm2 and bonding strength of 19.041 N; The most tapered post with 14.71°had a total post area of 90.245 mm2 and bonding strength of 5.131 N. ③ The retention force of cast post and the bonding strength of ZPC declined with the increasing of convergence angles of the post. For the parallel post (convergence angle=0°), the retention force was 321.60 N, the frictional force was 302.559 N, retention force per unit area was 2.976 N/mm2,frictional strength was 3.885 N/mm2; for the most tapered post (convergence angle=14.71°), the retention force was 9.93 N, frictional force was 54.799 N, retention force per unit area was 0.664 N/mm2, frictional force was 0.681 N/mm2. ④ A one-way ANOVA showed that there were significant differences in the bonding strength, retention force and frictional force per unit area among the groups (P < 0.05).CONCLUSION: The retention force, frictional force and the bonding strength of ZPC decrease with the increasing of convergence angles of posts, so the convergence angles of posts should be reduced as much as possible to obtain greater retention.
