1.Estimating the burden of dengue in the Philippines using a dynamic transmission model.

OBJECTIVE: This study aimed to describe dengue burden in the Philippines. Specifically, health and economic costs of the disease were estimated.
METHODS: A published serotype-specific and age-stratified dengue dynamic transmission model was populated with Philippine-specific dengue epidemiology and cost data. Data were gathered from literature and record reviews. Dengue experts were consulted to validate the model parameters. Sensitivity analyses were performed to test the uncertainty of input parameters on model outcomes.
RESULTS: By 2016 to 2020, it is estimated that annually, average hospitalized cases will amount to 401,191 and ambulatory cases will amount to 239,497; resulting to USD 139 million (PhP 5.9 billion) and USD 19 million (PhP 827 million) worth of aggregate costs shouldered by the public payer for hospitalized and ambulatory cases, respectively. Average annual productivity losses may amount to USD 19 million (PhP 821 million) and DALY lost is expected to be 50,622.
CONCLUSION: The cost of dengue is high especially since the Philippines is an endemic country. Thus, there is a need to optimize government interventions such as vector control and vaccination that aim to prevent dengue infections.

2.The creation and maintenance of a hospital-based cancer registry system.

Introduction: Cancer registries contain information essential to any rational program of evidence-based cancer control, including cancer epidemiology and outcomes, and can be site-specific, hospital-based, or population-based. The creation of a national population-based cancer registry and hospital-based cancer registries was mandated in the National Cancer Control Act of 2019. This paper reports on the creation and maintenance of the Cancer CARE Registry and Research Philippines (CARE PH) app, the country's first hospital-based cancer registry system, and its future directions in registry and research.
Methods: A cancer registry in the form of a web-based application was developed through the collaboration between a clinician and a health information technology specialist. This registry was designed to follow the cancer patient's journey from diagnosis to staging to treatment and cure, relapse, or progression into death. Patient information is collected in a structured and secure process from designated catchment areas in each hospital by trained tumor registrars, with the main catchment area being the hospital's Surgical Pathology department. The CARE PH application is given to member hospitals for free through the support of grants given to the CARE PH Foundation, Inc.
Results and discussion: Today, 31 member hospitals in the CARE PH system have recorded a collective total of 9,880 new cancer patients for the year 2020. The most common cancer types recorded in CARE PH for 2020 include breast, colorectal, cervical, and head and neck cancers. In addition, the registry captures a myriad of information that can potentially answer questions relevant to the individual cancer patient and clinicians, and hospital administrators.
Conclusion: HBCRs are an indispensable part of effective cancer control programs as they facilitate making evidence-based decisions that would result in better healthcare for Filipino cancer patients.

3.A systematic review of epidemiologic studies on scabies in the Philippines

Background and Objective:
Scabies is the second most common cause of disability due to skin disease in the
Philippines. However, there were no cited studies in Global Burden of Disease 2019 and the disability-adjusted life years (DALY) computations were most likely based on statistical modelling. The Philippine Department of Health has embarked on a program to estimate the disease burden of priority diseases in the country, which include scabies. The last nationwide prevalence survey was 23 years ago. This systematic review aimed to estimate the prevalence or incidence of scabies in the Philippines.
Methods:
We searched PubMed, Scopus, Herdin, and Philippine Health Research Registry (search date October
2022) for studies on prevalence/incidence (including systematic reviews, cross-sectional studies, cohort studies,
case series, registry or census studies) of patients diagnosed with scabies in the Philippines. We excluded narrative reviews, commentaries, and conference proceedings or abstracts. Two reviewers independently screened titles and abstracts, assessed full text reports for eligibility, appraised the quality of included studies, and collected data using a pretested data extraction form. We did not pool studies due to clinical heterogeneity but plotted the individual studies in a forest plot with prevalence estimates and confidence intervals. We reported the median and interquartile range for entire group or relevant subgroups (age, setting) of studies. We used the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach to assess the certainty of evidence.
Results:
We included nine studies (N=79,065). Most were clinic-based prevalence studies, retrospective chart
reviews, conducted in dermatology outpatient clinics, Metro Manila area, and on pediatric populations. Prevalence of scabies was moderate (i.e., between 2 and 10%), ranging from 2.75% (national prevalence survey) to 6.8% (communitybased review), to high (> 10%) among pediatric patients in clinic-based retrospective chart reviews (29 to 36%) and institution-based surveys (39.0% to 45%), and a tertiary government university training hospital dermatology clinic (22.9%). The most affected age group was from 0 to 14 y/o, while males tended to have a higher prevalence than females. The cooler month of January had higher prevalence than the hotter month of June in one study.
Conclusion
Scabies is common in the Philippines, especially among children and elderly in institutional settings,
and during the month of January. There is a need to do a national prevalence survey to identify high-risk areas
and to monitor the prevalence of scabies, especially in crowded settings and vulnerable populations. This
information can be used for estimating the burden of disease for scabies and guide appropriate health resource allocation.

4.Scabies in the Philippines: A secondary analysis of local patient registries

Background:
Scabies is the second most common cause of disability among skin diseases in the Philippines as of 2019. There is no large nationwide study describing the epidemiologic profile of scabies in the country.
Objective. This study aimed to describe the demographic, seasonal, and geographic profile of scabies in the Philippines.
Methods:
We compared secondary data of two local patient registries (Philippine Dermatological Society, PDS, 2010 to 2021; and Philippine Pediatric Society, PPS, 2009 to 2021) for reported cases of scabies in the Philippines. We reported the frequency and percentage distribution according to age, sex, month, year, and type of diagnosis, and region.
Results:
The median annual frequency of scabies cases (mostly outpatient) for PDS (from year 2010) was 4087 (range ([QR], 342-6422 [3271.5]), while it was 183 (range [IQR], 64-234 [96.5]) (all inpatient) for PPS (from year 2009). There was a reduction to one-third (PDS) and one-fourth (PPS) of pre-pandemic numbers during the pandemic years (2020- 2021). The peak months for scabies cases were the cooler months: January (median, 12.1% of annual cases; range [IQR], 2.6%-31.4% [3.6%]) to February (median, 10.0% of annual cases; range [IQR], 1.5%-27.8% [2.5%]) based on PDS data, and November (median, 10.0% of annual cases; range [IQR], 0.0%-24.3% [7.0%]) to January (median, 9.0% of annual cases; range [IQR], 0.0%-24.3% [6.6%]) for PPS data. Overall, for PDS, age 1-4 years is the most affected age group (median, PDS, 17.5% of annual cases; range [IQR], 11.9%-25.4% [8.1%]), while it was the less than 1-yearolds (median annual cases, 48.9%; range [IQR], 29.1%-67.3% [13.20%]) among PPS pediatric population aged 0 to 18 years. Males (median, 53.9% of annual cases; range [IQR], 45.0%-67.2% [8.8%]) were more affected than females in PPS. While for PDS during earlier years (prior to 2015), males (median, 51.6% of annual cases from 2010 to 2014; range [IQR], 47.4%-52.9% [0.2%]) were more affected than females. However, males became less affected than females with median, 44.7% of annual cases from 2015 onwards (range [IQR], 43.4%-46.5% [1.2%]). NCR was the region with the highest frequency of cases in PPS (median, 52.6% of annual cases; range [IQR], 22.7%-75.0% [20.4%]). The 2nd most affected regions were Central/Eastern Visayas (34.2%, 2009-2013; range [IQR], 17.9%-54.1% [5.3%]), Bicol region (12%; 2014 to 2018; range [IQR], 17.9%-54.1% [7.4%]), Central Luzon (18%; 2019), Central/Eastern Visayas (29%, 2020), and Northern/Central Mindanao (17%, 2021).
Conclusion
Scabies was commonly seen in the younger age group, slightly more in females in the PDS, while slightly more among males in the PPS, in the cooler months of the year, and in the urbanized NCR.

5.Impact on transmissibility and case fatality rate of COVID-19 of the mandatory face shield use in addition to mask during the pandemic: The Philippine experience

Background:
While wearing face shields and other gears to protect the facial mucous membranes not covered by face masks are proven to decrease the odds of infection in the hospital setting, there is no concrete evidence of its efficacy in the general public.
Objective:
To determine the effectiveness of face shield use in the general public in the local setting.
Methods:
This study utilized an ecological study design, with the weeks when the policy was implemented serving as the exposure variable while the weeks when the policy was not in effect, whether prior to or after, serving as control. Primary outcomes were mean incidence of COVID-19 and case fatality rate (CFR) per week.
Results:
When the mandatory face shield use was implemented, the mean incidence of COVID-19 per week was higher compared to weeks when it was not implemented [93 cases per 1000 population per week (ptpw) vs 65 cases, relative risk:1.43, z=-3.79, p=0.0001]. Moreover, during weeks when only less than 50% of the population was vaccinated with first dose (93 cases ptpw vs 52 cases, RR: 1.79, z=-4.3, p<0.0001) and complete doses (93 cases ptpw vs 66 cases ptpw, RR:1.41, z=-3.69, p=0.0002), the mean incidence of COVID-19 per week were statistically higher in weeks when face shield use was in effect. Controlling the status of vaccination and the predominant strain, face shield use increased the incidence of COVID-19 cases ptpw by 38 (F=13, R2=39%, p=0.026) to 50 (F=3.06, R2=12.2%, p=0.032) compared to no face shield use. No difference in CFR between weeks with face shield use and no face shield use was seen (29 deaths ptpw vs 32 deaths per ptpw, p=1.0). Nevertheless, when the weeks with no vaccination (27 deaths ptpw vs 48 deaths ptpw, RR=0.56, p=0.0018), less than 50% of the population were vaccinated with f irst dose (30 deaths ptpw vs 50 deaths ptpw, RR:0.6, p=0.0005), and complete doses (30 deaths vs 47 deaths ptpw, RR:0.64, p=0.0042) were only considered, face shield use significantly decreased the mean CFR per week. Controlling the incidence rate of COVID-19, vaccination status, and prevalent strain, face shield use decreases the number of deaths by 26 per 1000 COVID-19 diagnosed cases (F=7.4, R2=28.3, p=0.010).
Conclusions
In general, although face shield use increased susceptibility to COVID-19, it decreased case fatality rate in the Philippines. However, a more robust and controlled study in the future may be needed to truly justify its recommendation for the public.

OBJECTIVE
This study compared the demographics, clinical profile, treatment, and outcomes of retinoblastoma patients seen at medical institutions in the Philippines between two time periods: 2010 to 2015 and 2016 to 2020.
METHODSThis was a multicenter, analytical, cohort study using review of medical charts and databases of retinoblastoma patients seen in 11 medical institutions from 2010 to 2020.
RESULTSThere were 636 patients (821 eyes) included in this study: 330 patients were seen in 2010 to 2015 while 306 in 2016 to 2020. More cases per annum were seen in the latter timeline. The number of patients with unilateral disease was not significantly different between the two time periods (p=0.51). Age at onset of symptom, age at initial consultation, and delay in consult were also not significantly different between the two time periods (p > 0.05). Patients had significantly different distributions of intraocular grades (p < 0.0001) and systemic staging (p < 0.0001) between the two time periods. Enucleation was the most common surgical treatment performed in both timelines. There was significant difference in the status of patients based on the need for systemic chemotherapy (p < 0.01). There was significant difference in outcome between the two time periods, including the proportions of living and deceased patients.
CONCLUSIONThis study compared the most comprehensive data on retinoblastoma patients in the country. There was no improvement in early health seeking behavior based on similar age at initial consult and delay in consult. Enucleation remained the most common treatment mode as opposed to chemotherapy due to similar percentage of patients with unilateral disease, an indication for enucleation rather than chemotherapy.

7.Clinical profile of cerebrovascular disease population in Sorsogon: A hospital-based study

Objectives:
In the Philippines, an estimated half million are affected annually by stroke. It is the third most common cause of mortality among Filipinos. Locally, there are limited data on the epidemiology of stroke in the country. This study aimed to study cerebrovascular disease in the rural setting in the country, primarily exploring the demographic characteristics, risk factors, clinical profile, and outcomes of patients assessed with cerebrovascular disease in the province of Sorsogon.
Methods:
This was a retrospective study of all adult patients admitted to two tertiary hospitals in Sorsogon between February 1, 2020, and January 31, 2021, with a stroke diagnosis (International Classification of Diseases, Revision 10). A manual review of the charts and demographics, risk factors, clinical presentation, neuroimaging findings, and outcome were recorded.
Results:
A total of 721 cases with a mean age of 63.06 ± 13.96 years were involved in the analysis. Of all the
stroke cases, 64.7% were ischemic, and 29.7% were hemorrhagic strokes. The most common risk factors for stroke occurrence were hypertension (65%), history of stroke (16.2%), and diabetes (11.4%). Most sought consultation was due to one-sided weakness (41.3%) and slurring speech (14.2%).
Conclusion
In a third-class province in the Philippines, the most common type of stroke was an ischemic stroke.
Analysis showed that diabetes was more associated with ischemia while hypertension was significantly associated with hemorrhagic stroke. A mortality rate of 26.8% was seen in this cerebrovascular disease population.

8.Factors associated with adverse outcomes among SARS-CoV-2 positive children in a Tertiary Government COVID-19 Referral Hospital in the Philippines

Background and Objective:
Pediatric COVID-19 epidemiology and factors associated with adverse outcomes-mortality, need for invasive mechanical ventilation, and ICU admission, are largely unstudied. We described the clinicodemographic characteristics of Filipino pediatric COVID-19 patients and determined the factors associated with adverse outcomes.
Methods:
This is a retrospective cohort study of 180 hospitalized SARS-CoV-2-confirmed cases 0-18 years old
from April 2020 to August 2021 in a tertiary COVID-19 referral hospital in Manila, National Capital Region. Crude associations were determined using chi-squared or Fisher’s exact tests; and medians were compared using the Mann-Whitney test. Factors predictive of mortality were determined using Cox proportional hazards regression analysis. The survivor functions were depicted in graphs.
Results:
About 41.67% had mild disease, 58.33% were males, 39.4% aged 0-4 years, and 69.44% had at least one comorbidity. About 9.44% died (adjusted 9.2 persons per 1000 patient-days, 95% CI 5.5%-15.2%), 17.78% needed invasive mechanical ventilation, and 20% needed ICU admission. Independently, severe-critical COVID-19 (HRc 11.51, 95% CI 3.23, 41.06), retractions (HRc 10.30, 95% CI 3.27, 32.47), alar flaring (HRc 4.39, 95% CI 1.53, 12.58), cyanosis (HRc 4.39, 95% CI 1.72, 14.11), difficulty of breathing (HRc 7.99, 95% CI 2.25, 28.71), poor suck/appetite (HRc 4.46, 95% CI 1.59, 12.40), ferritin (HRc 1.01, 95% CI 1.00, 1.01), IL-6 (HRc 1.01, 95% CI 1.00, 1.01), aPTT (HRc 1.05, 95% CI 1.01, 1.10), IVIg (HRc 4.00, 95% CI 1.07, 14.92) and corticosteroid (HRc 6.01, 95% CI 2.04, 17.67) were significant hazards for mortality. In adjusted Cox analysis, only retractions (HRa 34.96, 95% CI 3.36, 363.79), seizure (HRa 9.98, 95% CI 1.76, 56.55), and corticosteroids (HRa 8.21, 95% CI 1.12, 60.38) were significantly associated with mortality while alar flaring appeared to be protective (HRa 0.10, 95% CI 0.01, 0.95). Several clinical characteristics were consistently associated with adverse outcomes.
Conclusions
Majority of hospitalized pediatric COVID-19 patients were very young, males, had mild
disease, and had at least one comorbidity. Mortality, invasive mechanical ventilation, and ICU admission were relatively low. Except for alar flaring which appeared to be protective, retractions, seizure, and use of
corticosteroids were associated with adverse outcomes.

10.The Vibrio Cholerae Isolated During Cholera Epidemics in Korea in 1969 and 1970.

Recently, two successive epidemics of cholera were observed in Korea. The first one started in Suhchun-Goon of Choong-Chung-Namdo in August 1969, and the 2nd in Changyoung-Goon of Kyung Sang-Namdo in August 1970. With stool specimens collected from patients in Suhchun, Ko-Chang, Seoul, Inchun, Youngkwang, Chang-hang and Wooljin were epidemic areas in l969, and from patients in Chang-Young, Pusan, Taegu, and Seoul which were epidemic areas in l970, studies were carried out in 1) the isolation and identification of cholera vibrio strains 2) the differentiation of E1 Tor vibrio from classic cholera strains 3) the liberation test of Kappa-type phages and 4) El Tor phage typing. Five strains, which were isolated in the epidemic area of the Philippines in l969 were included for a comparative study. The results are summarized as follows. 1) The epidemic strains of 1969 were identified as Vibrio cholerae, Celebes type El Tor and those of 1970 epidemic as Vibrio cholerae, biotype El Tor, El Tor phage type IV. 2) Korean strains and Philippine strains of 1969 epidemic appeared to be identical in biochemical and serological tests and phage susceptibility tests.
