2.Facts about Stock Control of Medical Supplies and Inventory Management System: Results of a Survery

How should the “management of things” be? We carried out a questionnaire survey to find out the facts about the stock control of medical supplies and inventory management system in hospitals affiliated with the National Federation of Agricultural Cooperatives for Health and Welfare. Questionnaires were sent out to 98 hospitals with not less than 100 beds. Fifty-one hospitals responded. The number of effective replies was 49. We made analyses of the data as best we could, especially the data on the operation of the inventory management system and stock control.(1) After the adoption of an inventory management system, 41% of the hospitals took more than half a year before the system was put on the right track. (2) The system has turned out to be conductive to the streamlining of such work as placing orders, delivery, storing and stocktaking. (3) Concerning the management of the master file in the system, many respondents gave the accuracy of data, prompt processing and working efficiency as the problems yet to be addressed. (4) In the hospitals using an inventory management system, the in-hospital type SPD took 17.7 days in terms of the holding period in the case where users are supposed to control the consumption of medical materials as against 8.3 days in the case where clerks in charge of warehousing deliver medical materials and take on consumption control. (5) About 80% of the hospitals have introduced their own inventory management system in an effort to promote the standardization of materials, purchase materials in proper quantities, carry out inventory and consumption control, and create an efficient supply system.

Phenylpropanolamine (PPA) is the major ingredient of over-the-counter drugs, including diet pills and nasal decon-gestants. We present a case of ICH induced by PPA in diet pills in a 36-year-old woman. She didn't have any medical concerns when she began taking diet pills, containing PPA, since 10 days before admission. A brain CT showed large ICHs on the right basal ganglia and lateral ventricle. Physicians should be alerted to the potential fatal side effects of PPA, in commonly used OTC drugs. 4.A Clinical Effect of Ornade Spansule in Stress Incontinence.

There were 18 woman with mild to moderate stress incontinence treated with the Sympathomimetic agents, with were combined with phenylpropanolamine hydrochloride, chlorpheniramine maleate, and isopropamide. The majority of these patients achieved significant improvement for 2 months to 2 and half years after beginning therapy. These patients were performed physical examination, voiding cystourethrogram, stress test lateral-chain cystourethrogram, cystoscopy in order to confirm stress incontinence. We thinked that these conservative therapy were advisable for stress incontinence especially when symptoms were mild.

5.Cost-Benefit Analysis Methods For Assessing Air Pollution Control Programs in Urban Environments --- A Review

The most common method of evaluating beneficial impacts of environmental policies is cost-benefit analysis (CBA). In the present review, CBA methods for air pollution impacts are reviewed. Three types of air pollution effects are identified, including health, productivity, and amenity. Market valuation, stated preference methods, and revealed preference methods are identified for valuing benefits. Three types of costs are described, including private sector costs, societal costs, and governmental regulatory costs. A benefits valuation approach based on Freeman’s principals is described. A costs valuation approach based on U.S. Environmental Protection Agency and Dixon et al. principals is described. Limitations associated with estimates of benefits and costs are summarized. Input assumptions and results are compared for several existing air pollution control analyses. The importance of CBA in environmental policy studies is discussed. Our conceptual approaches should be useful in analyses of urban air pollution impacts and air pollution prevention policies.

6.Global standardisation of HbA1c.

HbA1c is used for assessing glycaemic control in patients with diabetes. It is also used for treatment goals and as a target for therapeutic intervention. The Direct Control and Complications Trial in the USA showed that HbA1c can be used to predict the risk of complications. Hence, it is important for HbA1c assays to be standardised. The National Glycohemoglobin Standardization Program (NGSP) in the USA was formed in 1996 so that HbA1c results from different laboratories would be comparable to those reported in the DCCT study. There were also HbA1c standardisation programmes in Sweden and Japan. These three standardisation programmes are, in fact, direct comparison methods (DCMs), and yield different HbA1c results. In 1994, the International Federation of Clinical Chemistry and Laboratory Medicine (IFCC) established a Working Group on Standardisation of HbA1c. This working group has developed a global HbA1c reference system with very much improved intra-assay and inter-assay coefficients of variation. Recommendations have been made to report HbA1c results as IFCC-HbA1c values in SI units (mmol HbA1c/mol Hb) and NGSP-HbA1c (%) as well as estimated average glucose (eAG), once a tight relationship has been shown to exist between eAG and HbA1c.

7.The Clinical Effects of Amitriptyline on the Nocturia of Patients with Benign Prostatic Hyperplasia.

PURPOSE: We evaluated the effects of amitiptyline, as one of the first-line therapies, on the nocturia of patients with benign prostatic hyperplasia (BPH). MATERIALS AND METHODS: Between June 2005 and December 2006, 50 patients completed this study(Group I=20, Group II=14, Group III=16). Group I was treated with doxazocin 4mg, group II was treated with doxazocin 4mg and tolterodine 4mg and the third group was treated with doxazocin 4mg and amitriptyline 10mg. We measured the treatment efficacy, the clinical parameters and we examined three days of the voiding diaries at baseline and after 4 weeks of treatment, respectively. RESULTS: After 4 weeks of treatment, all the patients had significant improvement for the International Prostate Symptom Score(IPSS) and the quality of life(QoL) score among the clinical parameters and they also showed improvement of their frequency of micturition per 24 hours, per night(nocturnal frequency) among the voiding diary parameters(p<0.05). For the post-treatment comparison of the nocturnal frequency, there was a significant difference between group I and group II as well as between group I and group III(p<0.05), and there was no difference between group II and group III(p>0.05). Although there was 1 case of mild dry-mouth in group II and 1 case of mild dry-mouth and drowsiness in group III, none of the patient dropped out due to side effects. CONCLUSIONS: We found significant improvement in the IPSS, the QoL score and the nocturnal frequency after treatment with amitriptyline 10mg. Therefore, amitriptyline 10mg would be helpful as a first-line therapy for BPH patients with nocturia.

Phenylpropanolamine is a sympathomimetic amine used widely as a decongestant or appetite suppressant. Reports of the myocardial injury from the use of phenylpropanolamine are rare and the mechanism of the myocardial injury is not known clearly. We experienced a case of myocardial injury after ingestion of phenyl-propanolamine. A 46-year-old woman was admitted because of chest pain and dyspnea after ingestion of 5 tablets of anorectic pill containing phenylpropanolamine 75 mg per tablet. The serum creatine kinase MB isoenzyme levels were elevated and electrocardiographic abnormalities suggesting myocardial infarction were seen in the precordial lead. In echocardiograpy, left ventricular anteroseptal wall motion was nearly akinetic but coronary angiography showed normal coronary arteries except sluggish blood flow in left anterior descending artery.

9.Dissimilar Effects of Tolterodine on Detrusor Overactivity in Awake Rats with Chemical Cystitis and Partial Bladder Outlet Obstruction.

PURPOSE: We investigated bladder function, with a special focus on nonvoiding contractions (NVCs), in awake rats with chronic chemical cystitis and bladder outlet obstruction (BOO) by use of simultaneous registrations of intravesical and intraabdominal pressures. In addition, we tested the effects of tolterodine on the NVCs in these models. METHODS: A total of 20 female Sprague-Dawley rats were used in this study. In eight rats, chemical cystitis was induced by intravesical instillation of HCl. Twelve rats were subjected to sham instillations or partial BOO. Four weeks after intravesical instillation or 2 weeks after partial BOO, cystometrograms were obtained by use of simultaneous recording of intravesical and intraabdominal pressure in all unanesthetized, unrestrained rats in metabolic cages. RESULTS: A total of 17 rats survived. In the rats with acute injury by HCl, 50% showed detrusor overactivity (DO), which was not seen in the sham group. The cystitis group had lower DO pressure without a difference in DO frequency compared with the BOO group. After the administration of tolterodine, the cystitis group showed no difference in DO frequency or pressure, whereas the BOO group showed decreased values for both parameters. CONCLUSIONS: Our study showed that toleterodine produced no effect on DO during the filling phase in rats with chronic chemical cystitisbut decreased the frequency and pressure of DO in rats with BOO. Clinically, studies are needed to improve the treatment effect of anticholinergic drugs ininterstitial cystitis patients with overactive bladder.

10.Stent Position Is More Important than alpha-Blockers or Anticholinergics for Stent-Related Lower Urinary Tract Symptoms after Ureteroscopic Ureterolithotomy: A Prospective Randomized Study.

PURPOSE: To evaluate the clinical factors that impact ureteral stent-related lower urinary tract symptoms (LUTS) after ureteroscopic ureterolithotomy, including the stent position and medication. MATERIALS AND METHODS: Fifty-three patients who underwent ureteroscopic ureterolithotomy with indwelling a stent were distributed into three groups. On demand analgesics were given to the group 1 (n=18). Daily tamsulosin 0.2 mg was added for group 2 (n=15) and daily tamsulosin 0.2 mg and tolterodine 4 mg was added for group 3 (n=20). The patients were also subclassified into appropriate or inappropriate group according to stent position. All the patients completed a visual analogue scale (VAS) and International Prostate Symptom Score (IPSS) on the 1st and 7th postoperative days. The VAS and IPSS were analyzed according to the medication groups and the stent position. RESULTS: In the appropriate stent potion group, only the storage symptom scores of groups 2 and 3 on the 1st postoperative day were significantly lower than those of the group 1 (p=0.001). This medication effect on LUTS was not observed in the inappropriate stent position group. In this group, total IPSS (p=0.015) and storage symptom scores (p=0.002) were higher than in the appropriate stent position group on the 7th postoperative day. CONCLUSIONS: Correct placement of the stent was more important than medication for lessening stent-related storage symptoms.
