1.Laparoscopic appendectomy in children at Hue central hospital

101 children from 4 to 16 years of age were performed laparoscopic appendectomy at Hue Central hospital from January 2001 to June 2006. The result: the complication rate accounted for 1%, the rate of cases had to open surgically operate was 1%, postoperative infection 0%. The surgeons had to use 3 trocars; abdominal pressures changed from 8 to 10mmgHg, abdominal cavity volume was 1.21(0.6-2.41l). Laparoscopic surgery in children had the same effect to adult with many advantages as reduce of incision, hospitalization time, pain after surgery. There were no significant differences about laparoscopic appendectomy indications and techniques between children and adults

2.Endoscopic Sphincterotomy for bile duct stones technique aplications and results

From March 1998 to December 2004, there were 92 patients (mean age: 46.9) treated bile duct stone by endoscopic retrograde cholangio-pancreatography and Oddi sphincter at Hue Central hospital had. Indication: the patients with bile duct stone were diagnosed by clinical and echographic examination; the patients who had a high risk with open surgery, or had recurrent biliary stone. The complete remove rate of bile duct stones was achieved by 81.5% of cases. This rate related to patient’s condition, the size, the location and the number of biliary stone. 3.Laparoscopic cholecystectomy: experiences of 436 cases

From January 1999 to September 2004, 436 patients were performed laparoscopic cholecystectomie at Hue Central hospital. Mean age of the patients was 47 years; ratio of men/women was 1/2. The cases of laparoscopic cholecystectomie included normal cholecystitis, chronic cholecystitis, fibro-atrophy cholecystitis, and 5 cases with necrotic cholecystitis. The conventional open cholecystectomy was performed on the patients with necrotic, fibro-atrophy cholecystitis and patients without indication of laparoscopy (26.10%). The abnormality of gall-bladder arteries acquired 20% of cases, consisted of: more than 2 arteries, originated from gastroduadenal artery or common hepatic artery. The common complications of laparoscopic cholecystectomie were perforated gall-bladder, bleeding, and main bile duct injury which led to death

4.Laparoscopic surgery performed at Hue Central Hospital: evaluation on 1161 cases

Study on 1161 cases of laparoscopic surgeries at Hue Central Hospital from 1999 to September 2004. Results: laparoscopic surgery was applied, developed and expanded effectively, safely, and economically, etc in many kinds of diseases, with 23 kinds of surgeries were applied for 1139 patients in 5 years. The rate of complications was low (1.75%), with the most severe complication was gallbladder lesion (0.34%), bleeding (0.43%). The rate of laparotomy conversion was 2.15%, mainly due to technical conversions rather than complications. Death rate was 0.09% in cholecystectomy. Laparoscopic surgery is performed in nephrectomy, colectomy, or in the treating of rectal prolapse or genital prolapse, etc, and contributed to break through the new fields of modern surgery.

5.Outcomes of laparoscopic appendectomy at Hue Central Hospital

Study on 523 patients with acute appendicitis (269 females, 254 males) underwent laparoscopic appendectomy at Hue Central Hospital between January 1999 and September 2004. Results: 3.25% patients had history of laparotomy, 74% of appendices was on normal locations. The rate of laparotomy conversion was 0.19%. The time of operation was relative long (mean time: 43 minutes), especially in cases of appendices was on difficult locations. The advantages of laparoscopic appendectomy are less post-operative pain; patients can eat and move early after operation, duration of hospital stay is short, no post-operative infection, etc.

6.Treatment of splenic trauma in children: the experiences of Hue Central Hospital

Background: Pediatric closed abdominal trauma is a common disease in surgical practice, in which spleen is the most vulnerable organ. As adults, diagnosis of pediatric splenic trauma is not difficult but treatment attitude of splenic trauma in children having more important differences compared with in adults, tending to conservative treatment without surgery. Objectives: To assess the results of pediatric splenic trauma treatment in Hue Central Hospital from 2001 to 2005. Subjects and method: To study on 43 pediatric patients (23 male, 20 female), mean age 11.35\xb13.51 years, were diagnosed with splenic trauma due to closed abdominal trauma, treated in Hue Central Hospital from September, 2001 to September, 2006. Results: 72.1% hospitalized patients with maximum blood pressure in normal limitation. There was not a statistic significant association between hypotension and indicated surgery. The rate of patients with splenic trauma due to traffic accidents, living accidents and sport activities were 62.8%, 32.6% and 4.7%, respective. 2 patients with combined traumatic brain injury without surgery (4.65%), 3 patients with combined hand fracture (6.97%). 3 patients in conservative treatment group changed to surgery (8.33%). A total of 76.7% patients treated with successful conservation. Conclusion: More than 60% patients with splenic trauma caused by traffic accidents and having stable hemodynamic index during hospitalization. The difference between position of splenic trauma and rupture degree of spleen both in surgical treatment group and successful conservative treatment group is statistically significant.
