Esophageal retrograde peristalsis is known to be absent normally. This rare finding, if it happens, usually suggests a possibility that proximal ends of the manometric catheter are connected to the pressure transducers in a reverse order. We report a case showing repeated retrograde peristalsis during an esophageal manometric examination due to an inadventently hooked catheter in the esophagus during insertion. We suggest that if a retrograde peristalsis is repeatedly observed during a manometric session, an erroneous recording due to a hooked catheter in the esophageal lumen should be considered as one possibility.

Myxoglobulosis of the appendix is a rare and peculiar form of appendiceal mucocele. It is characterized by pearl or fish egg-like mucin globules in the lumen. We report a case of myxoglobulosis of the appendix in a 31-year-old man. Myxoglobules are composed of eosinophilic necrotic cell debris and mucin in the central nidus and lamellar structures alternating with necrotic cell debris and mucin in the peripheral zone. A stirring effect produced by vigorous appendiceal peristalsis may contribute to its lamellar growth.

Foreign bodies in the upper esophagus should be removed as soon as possible to avoid serious complications. These foreign bodies can penetrate the bowel wall and cause severe complications. The peristalsis of the esophagus is not strong enough to prevent it from retaining swallowed objects. Hence, perforation from a foreign body is more likely to occur in the esophagus than in the rest of the gastrointestinal tract. A razor blade is a rare foreign body of the esophagus. Its sharpness and large size make it difficult to remove. A razor blade was very firmly impacted in the esophageal wall in our case, and the razor blade had not moved from the upper esophagus. A standard overtube has limitations to remove a razor blade inside the overtube's lumen. We report here on a case of using a wedge resected overtube made it possible to successfully extract a razor blade and no serious complications occurred after extraction of the razor blade.

The study was designed to evaluate the radiological and pathological changes in the periureterally extravasated left ureter of rabbit following the irrigation with normal saline without dilatation and the effects of steroid in 30 rabbits. The rabbits were divided into two groups :the periureterally extravasated group without steroid administration( irrigation group) and the steroid administered group before and after ureteral irrigation( irrigation + steroid group). The results were as follows: 1. The frequency of ureteral peristalsis was decreased in both irrigation group and irrigation steroid group compared to the that of pre-irrigated one but with no significant difference between two groups. 2. The ureteric wall was easily liable to hydrostatic inflammatory reactions even with only periureteral extravasation in both irrigation group and irrigation +steroid group but with no significant difference between two groups. 3. The mechanism for the ureteral and periureteral fibrosis at an area where ureteroscope hasn't contacted is thought to be related to the microscopic or visible injury of transitional epithelium of ureter followed by extensive mucosal or periureteral extravasation of irrigation fluid resulted from high pressure of the irrigant within the ureteral lumen.

The massively dilated ureter is one of major therapeutic challenges that face the urologists. The goals of reconstructive procedures are the elimination of residual urine, effective ureteral peristalsis, efficient and/or urgent drainage. 6 cases of primary obstructive megaureter were presented with review of literatures. The results obtained were as follows ; 1. 6 cases were divided into 2 groups according to etiology with 4 adynamic segments and 2 intrinsic stenosis. 2. The underlying histopathologic features included 1 submucosal inflammation,2 submucosal fibrosis with inflammation and 2 submucosal fibrosis. 3. 5 cases were treated with Hendren's technique and 1 case treated by ureteral meatotomy. 4. All cases showed obvious improvement of upper tract without complication during follow-up period from 5 to 26 months.

The continent ileal neobladder is an ileal pouch that is anastomosed to the urethral stump for urinary diversion after radical cystectomy. We report our results in 4 patients who underwent this type of diversion. The 1st case is Camey procedure. In another 3 cases, to achieve a low pressure system, disruption of directional bowel peristalsis with a longitudinal incision at tire antimesenteric border of a 40cm to 60cm ileal segment is performed. A spherical pouch, the neobladder, is fashioned and anastomosed to the urethra. The ureters are implanted according to the method of Le Duc and Camey (Case 2). In case 3 and 4, the ureters are implanted by an antireflux nipple ureteroileostomy. The use of this ileal neobladder in male patient undergoing radical cystectomy offers an alternative free of a stoma to urinary diversion.

Between august 1986 and march 1988, 75 transurethral ureteroscopic lithotripsies were performed in 65 patients with 69 ureters. Analyzed sequentially, removal was successful in 40.6 per cent among the initial 30 patients(32 ureters) and in 81.1 percent among the subsequent 35 patients(37 ureters). The overall success rate was 62.3 percent. The common causes of failure were; Upward migration of stone, difficult ureteroscopic maneuvers to retch calculi, failure of ureteroscopic insertion in order. Of complications, 3 cases of ureteral perforation and 2 cases of ureteral avulsion were most severe. In one case who has circumferential denudation of ureteral mucosa with no urine flow in ureter, the ureteral orifice was noted with marded adhesion and total obstruction. To prevent this kind of complication, ureteral stenting is thought to be indicated. Even in the case of successful transurethral ureteroscopic lithotripsy without any immediated complications, the long-term follow-up will be necessary to detect late complications such as disturbance of peristalsis or VUR due to the fibrotic changes of ureter resulted from the insertion of ureteroscope.
