1.The effect of newly designed toothbrush on plaque control, treatment of gingivitis and periodontitis.

The present study investigate the effect of newly designed toothbrush(Chess tip : experimental group , conventional toothbrush(control group) on plaque control, gingivitis, and peridontitis. The results of 4weeks post-research by clinical comparison between the two groups are as follows. 1. In analysis of plaque index between groups there was significant difference in the experimental group at 2, 4 week. 2. In analysis of gingival index between groups there was significant difference in the experimental group at 1, 2, 4 week. 3. Both experimental group and control group showed decrease in periodontal pocket depth after 4week, but there was significant difference in the experimental group at 4 week. 4. Both experimental group and control group showed decrease in bleeding on probing after 4week, but there was significant difference in the experimental group at 4 week. These results indicate that newly designed toothbrush has beneficial effects as additional aid of mechanical treatment at the point of periodontal pocket, plaque control, and bleeding on probing.

2.The study of the peri-inplant soft tissue around osseointegrated implants in partial edentulous patients.

The keratinized mucosa around the implant is an important key in health of soft tissue and hard tissue. The purpose of this study is showed that the keratinized mucosa is associated with the keratinized mucosa index, plaque index, gingival index, probing depth. which is investigated to observing the peri-implant mucosa of mandibular partial edentulous patuent using periodontal parameter by previously published paper. It was estimated 6 site with regard to 80 fixture for 28 person, and the average age is 46.8. Each estimation is the order of less trauma, that is, plaque index, keratinized mucosa index, gingival index and probing depth. In this study, statstically analyzed treatment is used for Spss V 7.0 for Windows(Spss Inc, USA). The Kruskal Walis Test is used to compare the amount of the keratinized mucosa is into the 0~3 index, with plaque index, gingival index and probing depth. Mann-whitney Test is used to interpreate the relation of plaque index and probing depth, which is showed significant difference. The Result are as follows 1. The kertinized mucosa index 3 amounts to 47.7%, which is much higher than the other indices and the index order is followed 3, 1, 2 and 0. 2. The plaque index 1 amounts to 61.7%, which is much higher than the other indices and the index order is followed 1, 2, 3 and 0. The plaque index 0 is significant to each of index(P<0.05). The plaque index is decrease as the keratinized mucosa index is increased. 3. The probing depth for 2mm, 1mm, 3mm is 48.9%, 23.5%, 16.8% respectively, which is most occupied. The probing depth 2mm and 3mm for the keratinized mucosa index is significant(P<0.05). The probing index is decreased as the keratinized mucosa index is increased. 4. The gingival index 0 amounts to 58.0%, which is much higher than the other indices and the index order is followed 0, 1, 2 and 3.

3.Effect of Mouthrinses prepared by Sterilized Water-Generating Device on the Control of Periodontal Disease.

The aim of this study was to evaluate the clinical efficacy of mouthrinses prepared by sterilized water-generating device(Purister(R)) on the control of gingivitis and incipient periodontitis when it was used as a adjunctives to the mechanical plaque control. 40 healthy patients with gingivitis or incipient periodontitis were divided into two groups. Patients in the experimental group use mouthrinses prepared by sterilized water-generating device(Purister(R)) after tooth brushing while patients in the control group do only tooth brushing for plaque control. All patients received scaling and tooth brushing instruction. 1 week after scaling was set as baseline. Probing pocket depth, clinical attachment level, and bleeding on probing were scored at baseline, 4 weeks. Gingival index and plaque index were scored at baseline, 2 weeks, and 4 weeks. The results were as follows: 1. In the experimental group, gingival index, plaque index, probing pocket depth, and clinical attachment level showed statistically significant decrease, but in the control group, significant increase(p<0.05). 2. There was no significant difference between the experimental and the control group in bleeding on probing, but significant decrease in the experimental group and significant increase in the control group in a time-dependent manner(p<0.05). From these results, it can be concluded that regular use of mouthrinses prepared by sterilized water-generating device(Purister(R)) as adjunctives of mechanical plaque control, may be effective to prevent and treat gingivitis and incipient periodontitis.

4.An analytical on the periodontal conditions and treatment in leprous patients.

The purpose of this study was to investigate the oral health periodontal conditions (CPITN(0), CPITN(1) CPITN(2) CPITN(3)) and periodontal treatment (non-surgical periodontal treatment and surgical periodontal treatment) of patients who have leprous disease. The objects for this study were 754patients (male: 453, female: 301, 60-69ages: 340, 70-79ages: 414) treated in the oral health center among Leprous disease patients with gingivitis and periodontal disease, residents in the Sirocco from 2003, May 1st to 2005, April 30th. The subdivided result carrying out periodontal conditions and treatment were as follows: 1. 5.A Clinical Study On The Occurrence Of Food Impaction.

The purpose of this study was to investigate the causes of food impaction and to explore solutions as well. For this study, 39 patients with food impaction wee selected. 77 contact areas in these patients were investigated mobility, tightness of contact area, gingival index, plaque index, attachment loss, alveolar bone loss, proximal caries, marginal ridge distance and occlusal relationships. The results were as follows ; 1. Teeth without distal support were found to be the most frequent site of food impaction (41.6%). Food impaction was found to be more frequent in the upper teeth (66.2%) than the lower teeth (33.8%). 2. Food impaction was found in tight contact cases (71.4%). Alveolar bone loss was not found in the early stage of food impaction (83.1%) 3. The distance between the marginal ridges of food impaction sites (mean=0.48mm) was shorter than that of the control group. (mean=0.77mm) (p<0.001) 4. In 18.2% of t he cases, proximal carries were found at the food impaction site. 5. Food impaction affected patient's occlusion with the following frequencies ; cusp to marginal ridge relationship (72.7%), cusp to fossa relationship (3.9%) and stepped relationship (23.4%).

6.Clinical study on the abutment periodontal condition with extracoronal attachment denture.

OBJECTIVETo evaluate the abutment periodontium condition after restored with the distal-extension extracoronal attachment dentures.
METHODS20 mandibular distal-extension cases were followed up, to measure the gingival index (GI) and plaque index (PLI) of the distant-abutments before restored with attachment dentures and after restored for one month, six months and one year; to compare the length of distant alveolar bone before restored with that after restored one year by taking digital stomatology tomogram photos.
RESULTSThere were not significantly different in the PLI of distant-abutment between before repaired and after repaired one month, six months, one year (P > 0.05). There were not significantly different in the GI of distant-abutment between before repaired and after repaired one month, six months (P > 0.05), but the GI of distant-abutment after repaired one year was significantly higher than that before repaired (P < 0.05). There were no significant differences on the length of distant alveolar bone in distant-abutment before repaired and after repaired one year(P > 0.05).
CONCLUSIONIt is important to protect the periodontal health of the abutment when restored with distal-extension extracoronal attachment dentures. It is necessary to examine periodically after restored in order to keep the periodontal health of the abutment.

7.Comparison of periodontal indices and Porphyromonas gingivalis between conventional and self-ligating brackets.

OBJECTIVETo compare the periodontal indices and Porphyromonas gingivalis (P. gingivalis) between the use of self-ligating brackets and conventional brackets.
METHODSThirty patients were divided into 2 groups(n=15). Self-ligating brackets were used in the experimental group. Conventional brackets were used in the control group. Clinical periodontal indices, including plaque index (PLI), gingival index (GI) and probing depth (PD) of observed teeth were examined at three different time points: Before orthodontic treatment, the first month after treatment and the third month after treatment. Subgingival plaques were collected simultaneously at each time point. The number of total bacteria and P. gingivalis in each sample were detectd and quantitated by real-time quantitative polymerase chain reaction, the percentage of P. gingivalis in total bacteria was obtained.
RESULTSBefore treatment, the periodontal indices and the percentage of P. gingivalis in total bacteria had no difference between the two groups (P>0.05). After 1 and 3 months respectively, the periodontal indices and the percentage of P. gingivalis in total bacteria increased with time (P<0.05) and were obviously lower than those of the control group (P<0.05).
CONCLUSIONCompared with conventional brackets, the self-ligating brackets are better for periodontal health. But it is adverse effect on oral health.

8.Accuracy of partial-mouth examination protocols in periodontal epidemiological surveys: a systemic review.

OBJECTIVETo evaluate the accuracy of partial- mouth periodontal examination (PMPE) protocols frequently used in epidemiological periodontal surveys.
METHODSArticles in English or Chinese published before Jan 31st 2014 were searched, which compared the results of PMPE protocols with those of gold-standard protocol, i.e.full-mouthmesialbuccal-midbuccal-distobuccal-mesiolingual-midlingual-distolingual (MB-B-DB-ML-L-DL) protocol.
RESULTSTwelve articles were included and nine that frequently used PMPE protocols were evaluated. All these protocols underestimated the prevalence scores. For prevalence of probing depth (PD) ≥ 4 mm, 6 mm and attachment loss (AL) ≥ 4 mm, 6 mm, smaller amount of underestimation was observed in community periodontal index of treatment needs (CPITN) teeth (-12.6%--3.5%), full-mouth MB-B-DB (-16.1%--3.5%), full-mouth MB-B-DL (-10.8%--6.1%) and half-mouth MB-B-DB-ML-L-DL (-23.6%--2.0%) protocols. For severity and extent estimates, half-mouth MB-B-DB-ML- L-DL provided lowest biased results (relative bias: -1.0%- 1.1% for severity and -6.7%-0.1% for extent). Full-and half-mouth MB-B-DB also performed well, with relative bias within ± 5.0% in most cases. CPITN overestimated the severity and extent of periodontal disease, the relative bias of which amounted to 42.3% and 38.1%, respectively.
CONCLUSIONSHalf-mouth MB-B-DB-ML-L-DL and full-mouth MB-B-DB protocols caused lower biased results in prevalence, severity and extent estimates of PD and AL.

9.The clinical study on the use of diode laser irradiation in the treatment of periodontal-endodontic combined lesions.

OBJECTIVETo evaluate the adjunctive effect of diode laser irradiation for the treatment of periodontal-endodontic combined lesions.
METHODS30 cases with periodontal-endodontic combined lesions were selected. All cases were randomly divided into experimental group and control group (n=15). In the control group, patients were treated with root canal therapy and conservative periodontal treatment (scaling and root planning), and patients in the experimental group were treated with root canal therapy and conservative periodontal treatment followed by diode laser irradiation. Clinical measurements including pocket probing depth (PPD), clinical attachment level (CAL), modified bleeding index (mBI), periapical index (PAI) were taken before treatment (baseline). Three and six months after the treatment, the adjunctive effects of diode laser in the treatment of periodontal-endodontic combined lesions were evaluated.
RESULTSThree months after the treatment, the average PPD, CAL and mBI levels were reduced significantly in both groups. But the average PPD reduction in the experimental group was significantly greater (P<0.05) than in control group. However the average CAL and mBI reduction were not significantly different between two groups. Six months after the treatment, the average PPD, CAL and mBI levels were further reduced in the experiment group, but not in the control group. The average PAI levels were reduced significantly in both groups, but the PAI reduction was not significantly different between two groups.
CONCLUSIONThe use of diode laser as an adjunct procedure in the treatment of periodontal-endodontic combined lesions can aid in achieving success.

10.The role of serum lipoxin A4 levels in the association between periodontal disease and metabolic syndrome

PURPOSE: An unresolved inflammatory state contributes to the pathogenesis of periodontal disease and metabolic syndrome (MetS). Therefore, the purpose of this study was to evaluate the role of lipoxin A4 (LXA4), a proresolving lipid mediator, in the association between periodontal disease and MetS. METHODS: Sixty-seven patients with MetS and 65 patients without MetS were included in the study. Sociodemographic information was obtained via a questionnaire, and detailed medical diagnoses were made. Periodontal parameters (plaque index [PI], gingival index [GI], probing pocket depth [PD], and clinical attachment level [CAL]) and metabolic parameters were measured, and serum LXA4 levels were determined. The associations among MetS, periodontal parameters, and serum LX levels were evaluated by adjusted multivariate linear regression analyses. RESULTS: Patients with MetS were older and had a higher body mass index than patients without MetS. Periodontal parameters (PI, GI, PD, and CAL) were higher in patients with MetS than in those without MetS. Serum LXA4 levels were higher in patients without MetS. Multivariate linear regression analysis indicated a positive association between MetS and periodontal parameters (PD and CAL). Negative associations were established between MetS and LXA4 levels, and between LXA4 and periodontal parameters (PI, PD, and CAL). CONCLUSIONS: The presence of higher values of periodontal parameters in patients with MetS and the negative relationship of LXA4 with MetS and periodontal disease may support the protective role of proresolving lipid mediators in the association between periodontal disease and MetS.
