6.Management of the Infected Penile Prostheses.

Infection of the prosthesis remains a significant complication that almost always results in loss of the device. From 1983 through 1993, 290 penile prostheses have been placed at our hospital. Among them 13(4.5% ) required removal of the prosthesis secondary to gross infection and 8 underwent reimplantation. The infection rates were higher in diabetogenic impotence( 7.9%) than in nondiabetogenic ( 4.0% ) and higher in the simultaneous implantation of artificial sphincter( 40.0 %). The primary isolated organism was Staphylococcus epidermidis(53.8 %). The removed penile prostheses were malleable(AMS 600) in 8 patients and inflatable( Hydroflex 1, AMS 700 2, AMS 700CX 1, AMS Ultrex 1) in 5. Among the 8 cases who underwent reimplantation 2 were inserted at the time of removal( immediate), 2 one week later(early) and 4 six months later (late). The two cases of immediate reimplantation were those with 3-piece inflatable prostheses of which pump or tube was infected. The infected inflatable prostheses were removed totally and replaced with self-contained prostheses. The reimplanted prostheses were removed again due to reinfection in two cases of the early reimplantation and in one case of the late reimplantation. In conclusion, a delay of at least 6 months after removal of the infected prosthesis is thought to be the safest for reimplantation. And in case of 3-piece inflatable prosthesis of which pump and/or tube is infected, total removal of the prosthesis and immediate replacement with noninflatable or self-contained prosthesis would be a good method.

7.Penile Prosthetic Surgery Under Local Anesthesia.

Penile prosthetic surgeries were performed under local anesthesia in 36 cases of erectile dysfunction. Pudendal block and intracorporeal and local infiltration was effective for implantation of the malleable and hydroflex penile prosthesis.

9.Assessment of Erectile and Ejaculatory Function after Penile Prosthesis Implantation.

PURPOSE: We investigated erectile and ejaculatory function after penile prosthesis implantation. MATERIALS AND METHODS: A total of 121 patients were enrolled in the surgery group (SG) and 120 patients in the nonsurgery group (NSG). All subjects were evaluated by use of the following questionnaires: the erection function and intercourse satisfaction domains of the International Index of Erectile Function (IIEF) and the ejaculation domain of the Male Sexual Health Questionnaire (MSHQ-EjD). Comparisons were made between the SG and the NSG, by prosthesis types, and of postoperative periods and complication rates for each prosthesis type. RESULTS: Differences in the erection function and intercourse satisfaction domains of the International Index of Erectile Function (IIEF-EF and IIEF-IS) between before and after treatment were significantly higher in the SG group than in the NSG group (p=0.02, 0.03, respectively). When comparing prosthesis types, differences in the erection confidence and intercourse satisfaction items between before and after surgery were significantly higher in the SG group (p=0.03, 0.04, respectively). In the comparison of each prosthesis type by postoperative period, differences in the IIEF-EF and IIEF-IS between before and after surgery were not statistically significant but the MSHQ-EjD domain after surgery was significantly lower in cases of >5 years (p=0.02, 0.03, respectively). CONCLUSIONS: Subjective symptoms such as erectile confidence and erectile function were improved more in the SG group than in the NSG group, especially in the inflatable group. It appeared that there was no significant difference in improvement in ejaculatory function depending on the treatment method, but that ejaculatory function decreased as time passed.

10.Biomedical engineering and erectile restoration: design considerations for urologic prosthetics.

For patients with moderate-to-severe erectile dysfunction, implantable penile prostheses continue to be a viable treatment. Medical device developers apply design controls during the development cycle to ensure that a product performs as intended in the final use environment. This process relies heavily on the principles of systems engineering and documents every facet of performance, unmet need, and risk. To better understand design philosophy, it is important to frame benchmarked performance outcomes in the context of the ideal state. Careful consideration of erectile anatomy and physiology, including flaccid state, transitional phases, and full tumescence, informs penile prosthesis design philosophy and provides the foundation for product advancement.
