1.The expression and clinical significance of interleukin -6 in non -Hodgkin's lymphoma

Objective To investigate the expression and clinical significance of interleukin -6 (IL -6)in aggressive and indolent lymphoma patients.Methods Serum specimens obtained from 1 07 non -Hodgkin's lympho-ma (NHL)patients and 50 healthy controls were collected.Chemiluminescence immunoassay was used to assess the expression of IL -6 in the serum.Results The level of serum IL -6(1 8.54 ±4.53)ng/L in aggressive lymphoma group was significantly higher than (6.90 ±1 .78)ng/L in the inert lymphoma group,and the serum IL -6 levels of two groups were significantly higher than the healthy control group (3.87 ±0.76)ng/L.The lymphoma patients,with B symptoms,elevated CRP or bone marrow involvement presented higher IL -6 level than the other patients (P <0.05).IL -6 level of the IPI high -risk group was higher than the intermediate -risk group and low -risk group, with significant difference (P <0.05 ).The correlation study showed that IL -6 and CRP,IPI /FLIPI,Ann Arbor stage,B symptoms,bone marrow involvement had significantly positive correlation (P <0.05);Through analysis of the effect of the patients with aggressive lymphoma,the serum level of IL -6 in patients with complete remission was sig-nificantly higher than the non complete remission group.By Logistic regression analysis,IL -6 was one of the inde-pendent risk factors affecting the treatment effect of NHL,and the difference was statistically significant (P <0.05). Conclusion The serum IL -6 was significantly elevated in NHL patients,in association with tumor invasion,progno-sis and therapeutic effect.IL -6 may help the diagnosis and prognosis of NHL.

Objective To investigate the incidence of middle ear barotrauma due to hybaric oxygen therapy by using Health Education Atlas.Methods 100 patients were divided into two groups by random number table. The research group(49 patients)was educated by Health Education Atlas.The control group(51 patients)was educated by traditional education approach.During the first three days,we observed and recorded the eardrum injury and asked patients ear discomfort everyday.Results The incidence rate of middle ear barotrauma of the research group was 6.1%,which of the control group was 19.6%.The eardrum injury of the research group was milder than the control group(χ2 =4.02,P <0.05).Conclusion Education using Health Education Atlascan reduce the incidence of middle ear barotrauma due to hybaric oxygen therapy.
3.Effect of ulinastatin on expression of interleukin 15, connective tissue growth factor and malondialdehyde in rat peritoneal mesothelial cells induced by high glucose

Objective To observe the effect of ulinastatin on the expression of interleukin 15 (IL-15), connective tissue growth factor (CTGF) and malondialdehyde (MDA) in rat peritoneal mesothelial cells (RPMCs) induced by high glucose. Methods RPMCs were isolated, cultured and passaged by trypsin, then identified. The third generation of cultured RPMCs were used in the experiment. RPMCs were divided into normal control group, high glucose (1.5%, 2.5%, 4.25%) for 6 hours and 12 hours, high glucose (2.5%) for 3, 6, 12, 24 hours or ulinastatin (160, 320, 640U/ml) for 12 hours. IL-15 mRNA was detected by real-time PCR. IL-15 and CTGF protein in supernatants was detected by ELISA. MDA protein was detected by TBAS. Results Compared with the control group, the expression of IL-15, CTGF and MDA was significantly increased in the groups stimulated by high glucose (P<0.05) in dose- and time-dependent manner. Ulinastatin could significantly decrease the expression of IL-15, CTGF and MDA induced by high glucose in dosedependent manner both in protein and gene levels (P<0.05). Conclusions High glucose can up-regulate the expression of IL-15, CTGF and MDA in RPMCs. Ulinastatin can reverse these changes.
4.The present situation and countermeasures of the discipline management of the standardized trainees in the department of orthopedics

In order to improve the quality of standardized training in department of orthopedics, it is necessary to ensure and strengthen the discipline management of standardized training. According to management experience of the standardized trainees in the department of orthopedics in the past years, this paper analyzes present situation of standardized training in orthopedics department, including the composi-tion and characteristics of the students and the structure of existing management staff and discusses the management measures, such as the establishment of electronic supervision system and the establishment of a new management structure. By doing so, we hope to achieve the institutionalization and normalization of the discipline management of standardized training and provide valuable experience in discipline manage-ment for other bases.
5. Prognostic significance of NCCN-International Prognostic Index (NCCN-IPI) for patients with peripheral T-cell lymphoma treated with CHOP-based chemotherapy

Objective:
To validate the prognostic value of NCCN-International Prognostic Index (NCCN-IPI) for patients with peripheral T-cell lymphoma (PTCL) treated with CHOP-based chemotherapy.
Methods:
A retrospective analysis in 162 PTCL patients who were initially diagnosed and treated in Rui Jin Hospital from January 2003 to May 2013 was conducted. Baseline characteristics were collected, and survival analysis was performed according to the IPI and NCCN-IPI model.
Results:

6.Study on the Relationship between Five-state Personality and Cattell's 16 Personality Factors Based on Structural Equation Model

ObjectiveTo explore the relationship between five-state personality and Cattell's 16 personality factors. MethodsA total of 913 students recruited from Beijing University of Chinese Medicine from November 2020 to January 2021 were assessed by using the Five-state Personality Test Form and the 16 Personality Factors Questionnaire (16PF). The five-state personality scores were compared with national norms and gender differences were examined. Additionally, a structural equation model was established and the model fit was assessed using comparative fit index (CFI), Tucker-Lewis index (TLI), and root mean square error of approximation (RMSEA). The five-state personality (Taiyang, Shaoyang, Yin-yang balance, Shaoyin and Taiyin) was used as individual outcome variables while the 16PF factors was employed as predictor variables. Based on standardized parameter estimation results, we explored the relationships between the five-state personality and 16PF. ResultsA total of 913 students were recruited, and 756 valid samples were obtained after excluding unqualified questionnaires. Among these, the scores of Taiyang, Shaoyang and Taiyin in the 756 school students were significantly lower than the national norms, while the scores of Yin-yang balance was significantly higher (P<0.05 or P<0.01). The scores of Taiyang, Shaoyang, Yin-yang balance and Shaoyin were significantly higher than in males compared to females (P<0.05 or P<0.01). Regarding the relationship between the dimensions of five-state personality and 16PF, the model demonstrated a good fit with CFI and TLI values of 0.993 and 0.980, respectively, and an RMSEA value of 0.027. The 16PF factors, including dominance, social boldness, vigilance, rule-consciousness, tension, and emotional stability, positively predicted the Taiyang personality traits, while privateness and apprehension negatively predicted the Taiyang personality traits (P<0.05 or P<0.01). For Shaoyang personality traits, factors such as liveliness, social boldness, dominance, and tension positively predicted them, while rule-consciousness, apprehension, and privateness negatively predicted Shaoyang perso-nality traits (P<0.05 or P<0.01). In the case of Yin-yang balance personality traits, perfectionism, rule-consciousness, emotional stability, and self-reliance positively predicted them, while apprehension negatively predicted Yin-yang balance personality traits (P<0.05 or P<0.01). Regarding Shaoyin personality traits, perfectionism, emotional stability, self-reliance, rule-consciousness, and vigilance positively predicted them, while dominance, social boldness, and liveliness negatively predicted Shaoyin personality traits (P<0.01). Apprehension, tension, vigilance, and self-reliance positively predicted Taiyin personality traits, while social boldness, rule-consciousness, and intelligence negatively predicted Taiyin personality traits (P<0.05 or P<0.01). ConclusionThere is a significant correlation between five-state personality test and 16PF with 16PF factors capable of predicting five-state personality factors. The five-state personality demonstrates scientific validity and effectiveness.
7.Intestinal innate immune barrier and genetic regulatory mechanisms employed by enteropathogenic bacteria to evade innate immune system in intestines

Intestinal pathogenic bacteria utilize their own virulence factors to interfere with the nor-mal function of intestinal epithelial cells and evade clearance by the host immune system through precise gene regulation and expression strategies,thereby causing a variety of diseases.It has been found that the mechanisms affecting the gene expression strategies of pathogens include the toxin-antitoxin system,secretory system,and quorum sensing system.Novel antimicrobial strategies are being developed to interfere with the inherent gene regulatory system of bacteria and inhibit the expression of virulence genes,thus enabling a modest approach to the treatment of intestinal disea-ses.This paper initially discusses the components of innate immunity in the gut,followed by a dis-cussion of the common gene regulatory system of intestinal pathogenic bacteria.This article aims to clarify the mechanisms by which intestinal pathogenic bacteria evade clearance by the innate im-mune system through gene regulation and expression strategies,and to provide new ideas for the treatment of intestinal bacterial diseases.
8.Effects of electroacupuncture on activation of microglia cells in spinal cord in rats with neuropathic pain.

OBJECTIVETo observe the effects of electroacupuncture (EA) on the activation of microglia cells in the Lto Lspinal cord in rats with neuropathic pain, so as to investigate whether EA could inhibit the activation of spinal microglial cells and regulate the expression of brain-derived neurotrophic factor (BDNF) to achieve the analgesic effect.
METHODSForty male Sprague Dawley rats were randomly divided into a normal group, a sham-model group, a model group and an EA group, 10 rats in each one. The rats in the normal group received no treatment; the rats in sham-model group were treated with operation to exposure sciatic nerve for 2 to 3 min (no knot); the rats in the remaining groups were treated with model establishment of chronic constrictive injury (CCI). 7 days after model establishment, the rats in the EA group were treated with EA at "Zusanli" (ST 36) and "Yanglingquan" (GB 34), 30 min per time, once a day for consecutive 7 days. Only immobilization was used in the remaining groups the mechanical withdrawal threshold (MWT) and thermal withdrawal latency (TWL) of affected side feet were respectively measured before model establishment and 3 days, 5 days, 7 days, 10 days, 12 days and 14 days after model establishment; 14 days after model establishment, rats were sacrificed; the immunohistochemical method was used to measure the expression of Iba1 and BDNF in the sample of Lto Lspinal cord; real-time fluorescent quantitative PCR was used to measure the expression BDNF mRNA.
RESULTSCompared with the sham-model group, the pain threshold was decreased significantly in the model group (<0.05), leading to hyperpathia. After EA treatment, compared with the model group, the pain threshold was increased significantly in the EA group (<0.05). 14 days after operation, the microglia cells in the Lto Lspinal cord, expression of BDNF and level of mRNA in the model group were significantly higher than those in the normal group and sham-model group (all<0.01); those in the EA group were significantly lower than those in the model group (all<0.01).
CONCLUSIONSThe analgesic effect on neuropathic pain is likely to be achieved by EA through inhibiting the activation of spinal microglia cells and down-regulating the expression of BDNF.
9.Moderate and severe persistent allergic rhinitis treated with acupoint application therapy of the different intensity: a randomized controlled trial.

OBJECTIVETo evaluate the clinical therapeutic effects and safety on moderate and severe persistent allergic rhinitis treated with acupoint application therapy of the different intensity during the dog days.
METHODSOne hundred and sixty patients of moderate and severe persistent allergic rhinitis were randomized into a No.1 treatment group, a No.2 treatment group, a No.3 treatment group and a placebo group, 40 cases in each one. The same acupoints were used in the four groups, named Dazhui (GV 14), Dingchuan (EX-B1), Feishu (BL 13), Pishu (BL 20), Mingmen (GV 4), Gaohuang (BL 43), Shenshu (BL 23) and Qihai (CV 6). In the three treatment groups, the fine powder of the ingredients (,,,) of compoundformula was used. In the No.1 treatment group, the herbal paste (ginger-prepared paste) was prepared with ginger juice and the above herbal powder. In the No.2 and No.3 treatment groups, the herbal paste (honey-prepared paste) was prepared with honey with the above herbal powder. In the placebo group, the pseudo-herbal paste of the same appearance was prepared with millet powder and distilled water. The acupoint application was given for 2 h in the No.1 and No.2 groups and was for 6 h in the No.3 treatment group and the placebo group. The acupoint application therapy was given once every week during the dogdays, continuously for 5 weeks. The total nasal symptom score (TNSS), the score of the rhinoconjunctivitis quality of life questionnaire (RQLQ) and the count of blood eosinophils (EOS) were observed in the patients of the 4 groups before and after treatment. The clinical therapeutic effects were compared among the 4 groups. The incidences of the skin adverse reactions were observed in each treatment group.
RESULTSAfter treatment, the scores of TNSS and RQLQ were all reduced as compared with those before treatment in the three treatment groups (<0.05,<0.01), in which, the improvements in the No.3 treatment group were better than those in the No.1 treatment group and the No.2 treatment group (both<0.05). After treatment, the count of EOS was all reduced as compared with that before treatment in the three treatment groups (all<0.05). The differences were not significant statistically among the three treatment groups (all>0.05). The total effective rate was 85.0% (34/40) in the No.3 treatment group, better than 76.3% (29/38) in the No.1 treatment group, 71.8% (28/39) in the No.2 treatment group and 5.0% (2/40) in the placebo group (<0.05,<0.01). The incidences of the skin adverse reaction in the No.3 treatment group and the No.2 treatment group were lower than those in the No.1 treatment group (both<0.01).
CONCLUSIONThe acupoint application of the different intensity relieves the symptoms and improves the living quality in the patients of moderate and severe persistent allergic rhinitis. The stimulation of the ginger-prepared herbal paste is strong and induces skin blisters after 2 h herbal application. The stimulation of the honey-prepared herbal paste is moderate and does not induce blisters. The 6 h stimulation of the honey-prepared herbal paste is mild and the therapeutic effect is optimal.

Objective:To investigate the bacterial composition and antimicrobial resistance profile of clinical isolates from bloodstream infections in China.Methods:The clinical bacterial strains isolated from blood culture were collected during January 2020 to December 2020 in member hospitals of Blood Bacterial Resistant Investigation Collaborative System (BRICS). Antibiotic susceptibility tests were conducted by agar dilution or broth dilution methods recommended by Clinical Laboratory Standards Institute(CLSI, USA). WHONET 5.6 was used to analyze data.Results:During the study period, 10 043 bacterial strains were collected from 54 hospitals, of which 2 664 (26.5%) were Gram-positive bacteria and 7 379 (73.5%) were Gram-negative bacteria. The top 10 bacterial species were Escherichia coli (38.6%), Klebsiella pneumoniae (18.4%), Staphylococcus aureus (9.9%), coagulase-negative Staphylococci (7.5%), Pseudomonas aeruginosa (3.9%), Enterococcus faecium (3.3%), Enterobacter cloacae (2.8%), Enterococcus faecalis (2.6%), Acinetobacter baumannii (2.4%) and Klebsiella spp (1.8%). The prevalence of methicillin-resistant Staphylococcus aureus (MRSA) and methicillin-resistant coagulase-negative Staphylococcus aureus were 27.6% and 74.4%, respectively. No glycopeptide- and daptomycin-resistant Staphylococci were detected. More than 95% of Staphylococcus aureus were sensitive to rifampicin and SMZco. No vancomycin-resistant Enterococci strains were detected. Extended spectrum β-lactamase (ESBL) producing Escherichia coli, Klebsiella pneumoniae and Proteus mirabilis were 48.4%, 23.6% and 36.1%, respectively. The prevalence rates of carbapenem-resistance in Escherichia coli and Klebsiella pneumoniae were 2.3% and 16.1%, respectively; 9.6% of carbapenem-resistant Klebsiella pneumoniae strains were resistant to ceftazidime/avibactam combination. The prevalence rate of carbapenem-resistance in Acinetobacter baumannii was 60.0%, while polymyxin and tigecycline showed good activity against Acinetobacter baumannii. The prevalence rate of carbapenem-resistance of Pseudomonas aeruginosa was 23.2%. Conclusions:The surveillance results in 2020 showed that the main pathogens of bloodstream infection in China were gram-negative bacteria, while Escherichia coli was the most common pathogen, and ESBL-producing strains declined while carbapenem-resistant Klebsiella pneumoniae kept on high level. The proportion and the prevalence of carbapenem-resistant Pseudomonas aeruginosa were on the rise slowly. On the other side, the MRSA incidence got lower in China, while the overall prevalence of vancomycin-resistant Enterococci was low.
