Objective To study the effect of modified Shengmal decotion treating viral myocarditis, and on the basis of syndrome differentiation, to discuss the type and period for treating VM.Methods The PubMed, Embase, the Cochrane Library, the Chinese biomedical literature database(SinoMed), China hownet(CNKI), and VIP database(VIP) were retrieved for the articles on VM treated by modified Shengmal decoction with RCT method. RevMan 5.0 software was adopted for Meta-analysis.ResultsA total of 7 articles and 495 patients were involved. The results showed the modified Shengmal decoction treatment group had statistical difference to the control group in treating VM[OR(95%CI) was 3.61 (2.11,6.16),Z=4.70,P<0.01]. Among them, the decoctions contalning milkvetch root or not had statistical difference compared to the control group[contalning milkvetch root[OR (95% CI) was 2.67 (1.29, 5.52),Z=2.64,P=0.008; not contalning milkvetch rootOR (95%CI) was 5.11 (2.30,11.38),Z=3.99,P<0.01].Conclusion Modified Shengmal decoction has certaln effects in treating VM of Qi and yin deficiency type or VM in the advanced stage showing Qi and yin deficiency. The effectiveness of adding milkvetch root in Shengmal decoction to treat VM should be further discussed, the use of milkvetch root must be based on the premise of careful syndrome differentiation.

Objective To study the changes of inferior adjacent segmental after different segmental fixation ,and to analyze the accelerating mechanism of the adjacent segment degeneration .Methods Sixty sheep lumbar spine specimens (Waist 1 vertebral body-S5) with pedicle screws were collected ,according to the different fixed section ,they were divided into complete control group(not fixed) ,L6-L7 single segment fixed group ,L5-L7 short segmental fixed group ,L3-L7 long segmental fixed group .The biomechan-ical experiment was measured in the 6 N · m torque loading ,L7-S1 sacral vertebra clearance were measured in different fixed sec-tion .Results Applied in fixed ,beneath the adjacent segment in the stretch direction of angular displacement ,intervertebral disc pressure was increased significantly ,amplitude in creased with the increase of fixed range .The difference between groups was sta-tistically significant(P< 0 .05) .Conclusion After fixing spine in pedicle screws ,the longer the segmentation is fixed ,the more effect it has on subjacent segment degeneration .

Objective A distintegrin and metalloproteinase 33 (ADAM33) is one of the asthma susceptibility gene, which is closely related to the pathogenesis of asthma and airway hyperreactivity. The aim of this study was to evaluate the expression of serum ADAM33 in patients with different severity of asthma and the relation to airway chronic inflammation through clinical trials.MethodsPatients diagnosed as mild-to-moderate asthma (n=54), severe asthma (n=35) and healthy controls (n=30) were recruited from May 2015 to May 2016.The serum IgE, ADAM33, IL-4 and IL-13 1evels and Eosinophil (EOS) in peripheral blood were detected, and the correlation analysis was performed Results The serum levels of ADAM33 in mild-to-moderate asthma group, severe asthma group and healthy control group were 23.8±6.21pg/mL,64.8±12.8pg/mL and 18.3±4.49pg/mL, respectively. The ADAM33 levels in mild-to-moderate asthma group and severe asthma group were higher than those in control group (P<0.05).The ADAM33 levels in severe asthma group were higher than those in mild to moderate asthma group.(P<0.05).The correlation analysis showed that ADAM33 had positive correlation with IL-4 and IL-13(r=0.79 and r=0.81), but no correlation with EOS and IgE(r=0.54 and r=0.46).Conclusion The expression of serum ADAM33 was up-regulated in asthmatic patients along with the severity of asthma.ADAM33 was positively correlated with serum IL-4 and IL-13, implying that the expression of ADAM33 may be regulated by Th2 type cytokines.

STOMATOLOGY 2023;43(2):125-129
Objective:
To evaluate the clinical effect of clear aligners in the anterior region in non-extraction cases by establishing a three-dimensional model of crown-root fusion to guide clinical application.
Methods:
Eleven patients visiting the orthodontic department of Xuzhou Stomatological Hospital from December 2020 to December 2021 were collected, and the orthodontic plan was designed using Maestro 3D Dental Studio scheduling software to obtain the expected three-dimensional model of the patient's orthodontic treatment result. CBCT, intraoral scan, and 3D reconstruction software were used to create a postoperative model of the patient. The crown and root data were aligned in Geomagic Studio 2014, and differences in torque and axial inclination between the actual model after treatment and the predicted model of the anterior teeth before treatment were compared in 3-matic.
Results:
The actual torque angles of the anterior teeth were all smaller than the predicted angles before treatment, with the highest realization rate of 77.55% for lateral incisors and the lowest of 60.70% for central incisors; the actual axial inclination angles of the anterior teeth were also smaller than the predicted angles before treatment, with the highest realization rate of 81.49% for central incisors and the lowest of 74.95% for cuspids.
Conclusion
A digital model of crown-root integration based on a combination of 3D reconstruction and intraoral scanning techniques is advantageous in assessing the efficacy of clear aligners. In non-extraction cases with clear aligner, the efficiency of movement is higher for small areas of the anterior region.

Clinical Medicine of China 2023;39(4):276-280
Objective:To understand the occurrence of major adverse cardiac events (MACE) during the perioperative period of cervical spine surgery and analyze its risk factors. Methods:A retrospective analysis of 426 cervical spine surgery patients admitted to the from March 2017 to March 2021. The basic information of the patients, including gender, age, body mass index (BMI), underlying diseases, and the Charlson comorbidity index (CCI) was calculated based on the underlying diseases. The preoperative serum cardiac troponin I(cTnI) level and the operation-related indicators were collected, including the type of cervical spine surgery, surgical procedure, approach, duration of surgery, duration of anesthesia, intraoperative bleeding, postoperative pain visual analogue scale (VAS), and electrolyte disturbances. Count data were expressed as cases (%), and comparisons between groups were made using the χ 2 test; logistic regression models were applied to perform a multifactorial analysis of the factors influencing the perioperative occurrence of MACE in patients undergoing cervical spine surgery. Results:Among the 426 patients, 54 (12.68%) experienced MACE during the perioperative period, including 4 cases of unstable angina (7.41%), 4 cases of acute myocardial infarction (7.41%), 33 cases of severe arrhythmia (61.11%), and 13 cases of acute heart failure (24.07%). The results of multivariate logistic regression analysis showed that the older the age group (50-59 years old: odds ratio=1.34, 95% confidence interval: 1.14-1.78; 60-69 years old: odds ratio=1.48, 95% confidence interval: 1.20-1.86; 70-79 years old: odds ratio=1.71, 95% confidence interval: 1.34-2.57; 80-89 years old: odds ratio=1.95, 95% confidence interval: 1.46-2.85), as well as females, CCI scores>3, and preoperative cTnI>0.04 μg/L, postoperative VAS score>5, and electrolyte disorders are all influencing factors for the occurrence of MACE in cervical spine surgery patients during the perioperative period (odds ratios of 1.84, 2.12, 2.34, 2.57, 2.20, 95% confidence intervals of 1.34-2.68, 1.50-3.41, 1.63-3.72, 1.53-4.01, 1.43-3.69, all P<0.05). Conclusions:The incidence of MACE in the perioperative period of cervical spine surgery is relatively high. Elderly age, female, high CCI score, high preoperative serum cTnI, postoperative pain and electrolyte disturbance are independent risk factors for the perioperative period of cervical spine surgery.
