1.Random-controlled Trial of Kechuanning Capsules on Chronic Obstructive Pulmonary Disease Patients

Objective To observe the clinical efficacy of Kechuanning capsules for the treatment of chronic obstructive pulmonary disease (COPD) in stable phase.Methods Totally 110 patients with stable COPD were randomly divided into the therapy group and the control group, 55 cases in each group. Patients in both groups were given ipratropium bromide solution for inhalation. On the basis of western medicine treatment, the therapy group took Kechuanning capsules orally at the same time, 4 softgels for each time, 3 times a day;on the basis of western medicine treatment, the control group took the same amount of placebo capsules. The treatment lasted three months. Meanwhile, the clinical efficacy, BODE index, CAT score and ADRs of patients in the two groups were observed.Results The clinical efficacy in the therapy group was 96.00% (48/50), and 88.24% (45/51) in the control group, with statistical significance (P<0.05). BODE index and CAT score in the two groups decreased and the therapy group was lower than the control group (P<0.05). Liver and kidney function had no obvious abnormalities in the treatment group before and after treatment.Conclusion Kechuanning capsules can effectively reduce the BODE index and CAT score in patients with stable COPD, improve patients' life quality and clinical outcomes, and no obvious side effect has been found.

Based on the theory of the rise and fall of qi-blood in meridians, Professor GAO Yu-chun harmonizes the qi-blood and yin-yang for diagnosis and treatment of hypertersion from liver, spleen and kidney. With reference of the academic idea of“Preventive Acupuncture”, he investigated the qi and blood, treated diseases in the upper part by managing the lower part, pay attention to the syndrome differentiation of excess and deficiency, the different of acupoint property, and the treatment of acupuncture or moxibustion. Thus, through balancing yin and yang, the qi and blood are regulated to reach the therapeutic aim of“yin and yang in equilibrium”.
3.Efficacy observation on Governor Vessel-unblocking and mind-calming acupuncture for insomnia

Objective:To observe the effect of Governor Vessel-unblocking and mind-calming acupuncture on insomnia and further develop the academic idea on Gao’s lineage acupuncture. Methods:A total of 68 insomnia cases were randomly allocated into an acupuncture group (n=34) and a medication group (n=34) according to their sequences of consultation. Patients in the acupuncture group received acupuncture treatment once a day, 5 d a week, for 4 weeks. Patients in the medication group took 1 mg of Estazolam before sleep for each dose, one dose a day, for 4 weeks. Before and after treatment, the sleep quality was evaluated using the Pittsburgh sleep quality index (PSQI), and the efficacy was assessed using the PSQI reductive rate. Results:After treatment, the global PSQI score and scores of each component in the acupuncture group were significantly decreased (allP<0.01). The global PSQI score in the medication group was significantly decreased (P<0.01). Except for sleep disturbance, scores of the rest of components were significantly decreased (P<0.01 orP<0.05). The between-group difference was statistically significant in global PSQI score (P<0.01). Except for sleep latency and duration, scores of the sleep quality, efficiency, disturbance and daytime dysfunction in the acupuncture group were significantly lower than those in the medication group (allP<0.05). In addition, the recovery and marked effect rate in the acupuncture group was significantly higher than that in the medication group (P<0.05). Conclusion:The Governor Vessel-unblocking and mind-calming acupuncture is safe, reliable for insomnia and has better efficacy than Estazolam.
4.Efficacy observation on Governor Vessel-regulating and brain-unblocking acupuncture for post-stroke depression

Objective:To observe the clinical effect of Governor Vessel-regulating and brain-unblocking acupuncture on post-stroke depression.
Methods:A total of 70 eligible cases were randomly allocated into an observation group and a control group according to the random number table. Patients in the observation group were treated with Governor Vessel-regulating and brain-unblocking acupuncture, whereas patients in the control group were treated with oral Escitalopram Oxalate tablet. The treatment lasted for 8 weeks. The Hamilton depression scale (HAMD), Montgomery Asberg depression rating scale (MADRS), and Barthel Index (BI) were used to evaluate the patients’ condition before, 4 weeks and 8 weeks after treatment respectively. In addition, adverse reactions were also recorded.
Results:After 4 weeks of treatment, the HAMD and MADRS scores were significantly decreased in both groups (P<0.05); and the BI scores were significantly increased in both groups (P<0.05). There was no between-group statistical difference in HAMD score (P>0.05); the MADRS score in the observation group was significantly lower than that in the control group (P<0.05) and the BI score in the observation group was significantly higher than that in the control group (P<0.05). After 8 weeks of treatment, there were no between-group statistical differences in all aspects (P>0.05); however, the adverse reactions in the observation group were significantly less than those in the control group.
Conclusion:The Governor Vessel-regulating and brain-unblocking acupuncture is safe and effective for post-stroke depression and can obtain similar efficacy as Escitalopram Oxalate tablet. In addition, it can fast alleviate core symptoms of depression and improve the activities of daily living (ADL) and has fewer adverse reactions.
5.Effects of Kechuanning Capsules on Airway Inflammatory Cells and Infammatory Mediators in Patients with Stable Chronic Obstructive Pulmonary Disease

Objective To observe the effects of Kechuanning capsules on airway inflammation cells and inflammatory mediators in patients with COPD.Methods Totally 110 patients with stable COPD were randomly divided into therapy group and control group, 55 cases in each group. Both of the groups were given compound ipratropium bromide solution for inhalation, at the same time, patients in the therapy group took Kechuanning capsules orally, 4 softgels each time, 3 times a day, and patients in the control group took the same amount of placebo capsules. The treatment lasted three months. Differential counting of inflammatory cells and levels of inflammatory factors in serum and sputum supernatant were observed.Results In terms of differential counting of inflammatory cells, the sputum neutrophils of the therapy group decreased significantly (P<0.05) and macrophages increased (P<0.05) after treatment, with statistical significance compared with the control group (P<0.05). In terms of inflammatory mediators, the levels of IL-6, IL-8, LTB4 in serum did not change significantly (P>0.05) in two groups before and after treatment;IL-6, IL-8, LTB4 levels in the sputum supernatant of the therapy group were lower than those before treatment (P<0.05), with statistical significance compared with the control group (P<0.05). Conclusion Kechuanning capsules have an obvious inhibitory effect on airway inflammatory cells in patients with stable COPD as well as the release of inflammatory mediators.
6.Reliability and validity of Chinese version of the Osteoporosis Knowledge Assessment Tool in the elderly

Objective To test the reliability and validity of the Chinese version of Osteoporosis Knowledge Assessment Tool (OKAT).Methods The English version of OKAT was introduced and the Chinese version was confirmed by translation,revision,retroversion and expert discussion.A sample of 400 community-living aged people were recruited to complete the OKAT (Chinese version).Internal consistency test,split-half reliability,correlation analysis,items analysis,content validity,exploratory factor analysis,confirmatory factor analysis and ROC curve were used for examination.Results The Cronbach's α of the total scale was 0.821.The split half reliability coefficient was 0.934,test-retest reliability was 0.866,inter rater reliability coefficient was 0.786,the correlations between items and total scale ranged from 0.163 to 0.542.A two-factor model containing 20 items was got by exploratory factor analysis,the cumulative contribution rate of the items was 75.528 percent.The confirmatory factor analysis revealed a good model fit.Conclusions The Chinese version of the OKAT in the elderly is proved to be reliable and valid.It can be used to assess osteoporosis for the aged people in China.
7.Immunotherapy of mite allergic rhinitis in children

Objective:To study the effect of sublingual immunotherapy on children with mite allergic rhinitis.Methods:Four hundred and ninety patients with mite allergic rhinitis have been recruited by Beijing Children′s Hospital from March 2014 to September 2019 and divided into 4 groups of young children group, different treatment duration group, individualized dose adjustment group and multiple allergy evaluation group, the clinical scores of total nasal symptoms score (TNSS), visual analogue scale scores (VAS) and total medication score were recorded at the first visit, 12 months, 24 months and 36 months after treatment, and the combined symptom and medication score(CSMS) score was calculated.Results:A total of 374 patients (76.32%) completed this study.Among them, the CSMS(2.20±1.61, 2.50±1.78), TNSS(2.80±2.32, 3.60±2.71) and VAS(3.50±1.16, 3.90±1.43) in ≤3-year-old group and children over 3-year-old group of young children set after use of 12 months were significantly lower than the score at the first time of diagnosis (respectively CSMS: 4.50±1.44, 5.30±1.32; TNSS: 6.20±1.89, 7.50±2.19; VAS: 5.40±2.33, 5.90±1.61). In addition, in the duration and efficacy set, the patients who completed the immunotherapy for 36 months can only be observed in the 3-year group, the scores were TNSS(0.90±0.97), VAS (1.30±1.19), CSMS (1.70±1.28); the scores of patients who completed the immunotherapy for 24 months in 2-year group and 3-year group were TNSS (2.10±0.95, 2.00±0.97), VAS (3.00±1.56, 3.10±1.68) and CSMS (3.10±1.15, 2.90±1.19) and the patients who completed 12-month immunotherapy were scored in 1-year group, 2-year group and 3-year group with TNSS(3.20±1.27, 3.10±1.41, 3.20±1.41), VAS(4.50±2.11, 4.70± 2.19, 4.50±2.17) and CSMS(4.20±1.39, 3.70±1.32, 4.10±1.39) respectively; patients with poor efficacy in sublingual immunotherapy achieved a score similar to the control group after 6 months of dose adjustment (equals to 12 months after treatment), that were CSMS(2.90±1.56, 2.90±1.88, 2.40±1.69), TNSS(4.70±2.98, 3.90±2.77, 3.80±2.45) and VAS(4.20±1.29, 4.50±1.65, 4.20±1.14) of 4 drops group, 5 drops group and control group; sublingual immunotherapy for patients with multiple allergens for 2 years finally achieved similar efficacy to patients with single allergen, with CSMS (2.30±0.50, 2.10±1.01, 1.90±1.01), TNSS (3.50±2.62, 3.70±2.62, 3.20±2.82) and VAS (4.50±1.00, 4.10±1.57, 3.80±1.54) in single allergen group, combined with 1-2 allergens group and combined with 3+ allergens group.Conclusions:Sublingual immunotherapy plays a corresponding role in the treatment of low-age children, multiple allergy children, and some children get better after dose adjustment.
8.Application of enhanced recovery after surgery-based multidisciplinary cooperation model in reducing preoperative fasting time of reception surgery

Objective:To explore the effect of enhanced recovery after surgery(ERAS)-based multidisciplinary collaboration model on shortening the time of forbidden eating before receiving surgery, provide the basis for the selection of the preoperative diet prohibition scheme.Methods:From January 2017 to February 2019, a total of 384 patients who received the operation in Qingdao Municipal Hospital were analyzed retrospectively. The patients who under the traditional preoperative diet prohibition scheme were taken as the control group(156 cases) while those who under the multidisciplinary cooperation mode nursing under the concept of eras were taken as the experimental group(228 cases). The experimental group formulated the perioperative diet prohibition process according to the guidelines of eras, and the experimental group carried out the perioperative diet management for the patients according to the procedure. The difference between the two groups in the time of fasting, hunger, thirst incidence, insulin resistance, temporary stop will be observed and compared.Results:The time of fasting was (4.01±1.55) h in the experimental group and (10.12±1.57) h in the control group,there was significant difference between the two groups( t value was -1.65, P < 0.01). The incidences of thirst,hunger were 13.2%(30/228), 11.8%(27/228) in the experimental group and 89.7%(140/156), 87.2%(136/156) in the control group, there were significant differences between the two groups(χ 2 values were 220.20, 215.20, P < 0.01). The levels of insulin resistance on the first and third day after operation were 1.85 ± 0.43,1.52±0.61 in the experimental group and 1.99±0.51, 1.67±0.49 in the control group, the differences were statistically significant ( t values were -2.90, -2.56, P < 0.05).The temporary stop rate was 1.75%(4/228) in the experimental group and 7.69%(12/156) in the control group, the difference was statistically significant( χ2 value was 8.19, P<0.01). Conclusions:The ERAS-based multidisciplinary collaboration model can effectively shorten the preoperative fasting time, reduce the level of insulin resistance, reduce the incidence of hunger and thirst, and improve the rate of temporary stop and adjustment.
9.Isolation and identification of lung cancer stem-like cells based on a 3D cell culture system

Objective:To highlight the developmental process of 3D cell culture technology system, which is more suitable for isolating and identifying lung cancer stem-like cells than 2D cell culture technology system, and to explore the application of 3D cell cultures in the evaluation of proliferation, apoptosis, invasion, and drug resistance of lung cancer. Methods:Cells (104/well) from the human lung adenocarcinoma cell lines A549 and RPMI 1640 were cultured in complete medium containing 10%fetal bovine serum. Cell suspension was cultured in a BME basal medium. A growth curve was drawn after 7 d of culture. The stem-like cell was identified through a mammosphere culture, drug resistance and invasion assay, and flow cytometry. Data of A549 cells cultured in 3D and 2D tra-ditional cell culture technologies were compared. Results: Cells from the 3D cell culture had higher tumor formation rates [(20.75 ± 0.85) d vs. (60.25 ± 1.49) d, P<0.01)] and tumor sphere formation (28.50%± 1.17%vs. 8.67%± 0.80%, P<0.01) than those from the 2D cell culture. Moreover, cells from 3D cell culture were more invasive and resistant to therapy (58.17%± 2.19%vs. 41.70%±5.81%in 48 h, P<0.01;33.27%±5.76%vs. 27.30%±4.25%in 72 h, P<0.01). Phenotype experimental results demonstrated that the CD44 and CD326 cells were double-positive, whereas the CD24 cell was negative. Conclusion:The proportion of stem-like cells in A549 cell line after 3D cell culture significantly increased compared with 2D cell culture. The 3D cell culture can promote the proliferation of lung cancer stem cells.
10.Risk factors for yersiniosis: a case-control study

Objective:
To investigate the risk factors for yersiniosis, so as to provide insights into prevention of yersiniosis.
Methods:
The patients with yersiniosis admitted to the clinics in the surveillance site of Chengbei Township of Jin'an District and Chengnan Township of Yu'an District in Lu'an City from 2013 to 2021 were included as the case group, and the healthy family members matched to cases were selected as the family control group, while normal residents with a 1︰2 match in the same village, gender, and age difference within 5 years were included in the community control group. Participants' demographics, hand-washing and eating habits, living environment hygiene, poultry and livestock feeding were collected using questionnaire surveys, and factors affecting yersiniosis were identified using a multivariable conditional logistic regression model.
Results:
There were 43 cases in the case group, with a median (interquartile range) age of 45 (34) years, 91 cases in the family control group, with a median (interquartile range) age of 36 (36) years and 86 cases in the community control group, with a median (interquartile range) age of 46 (34) years. Multivariable conditional logistic regression analysis showed that compared with the family control group, the habit of drinking unboiled water (OR=6.721, 95%CI: 1.765-25.588), and direct consumption of food stored in the refrigerator (OR=7.089, 95%CI: 1.873-26.829) were risk factors for yersiniosis in the case group; and compared with the community control group, not washing hands after contacting with poultry and livestock (OR=50.592, 95%CI: 2.758-927.997), habit of eating raw vegetables and fruits (OR=5.340, 95%CI: 1.022-27.887), direct consumption of food stored in the refrigerator (OR=19.973, 95%CI: 2.118-188.336), and unclean refrigerator (OR=12.692, 95%CI: 1.992-80.869) were risk factors for yersiniosis in the case group. Compared with the family and community control groups, not washing hands after contacting with poultry and livestock (OR=4.075, 95%CI: 1.427-11.637), habit of drinking unboiled water (OR=4.153, 95%CI: 1.331-12.957), habit of eating raw vegetables and fruits (OR=4.744, 95%CI: 1.609-13.993), and direct consumption of food stored in the refrigerator (OR=5.051, 95%CI: 1.773-14.395) were risk factors for yersiniosis in the control group.
Conclusion
Unhealthy habits such as eating raw vegetables and fruits, drinking unboiled water, direct consumption of food stored in the refrigerator, unclean refrigerator, and not washing hands after contacting poultry and livestock may increase the risk of yersiniosis.
