1.Prognostic correrelation factors and analysis of molecular subtypes on 108 cases young breast cancer patients

Objective To analysis the correlation factors which influence the prognosis and relationship of molecular subtypes on young patients.Methods Clinical data of 108 cases young breast cancer patients (≤ 35 years old) who were treat in the Third Affiliated Hospital of Zhengzhou University from January 2007 to October 2014 were retrospective analyzed.According to immunohistochemistry such as estrogen receptor (ER),progesterone receptor(PR),human epidermal growth factor receptor-2 (Her-2/neu),Ki-67 index,patients were divided into molecular subtypes and follow up.Results LuminalA subtype,LuminalB subtype,human epidermal growth factor receptor-2 overexpression subtype and basal-like accounted for 22% (24/108),33% (36/108),20% (21/108) and 25% (27/108) respectively,and there's 5-year disease-free survival rate were74.1%,62.8%,.56.8%,58.5% respectively.Kaplan-Meier method was used to draw the disease-free survival curves through.There was no significant difference in the 5-year disease-free survival rate of each subtype (x2 =0.318,0.802,0.876,0.277,0.239,0.074,P>0.05).Univariate analysis of prognostic factors of Log-rank showed that there were significant differences between recurrence with tumor size,age,lymph node metastasis and surgical methods in young patients with breast cancer(x2 =9.612,12.099,51.345,5.928,P>0.05),while no significant difference with adjuvant chemotherapy,radiotherapy,endocrine therapy and molecular typing(x2 =2.381,1.958,0.027,2.612,P>0.05).Multivariate Cox proportional hazards regression model analysis showed that tumor size (RR =5.487,95% CI 1.519-19.822,P =0.009) and lymph node metastasis (RR =5.655,95% CI 2.939 -10.884,P<0.001) were independent risk factors for disease recurrence.Conclusion Tumor size and lymph node metastasis are important factors of prognosis on young patients.Young patients need the screening and therapy in the early time.
2. Problems for the conversion therapy in advanced gastric cancer

Patients with advanced gastric cancer have a poor prognosis, which remains the clinical concerned hot topic. The main previous treatments for advanced gastric cancer were adjuvant chemotherapy and palliative surgery, however, the application of conversion therapy has improved the survival in recent years. There are still many problems and challenges for conversion therapy because of its initial stage, such as the definition of advanced gastric cancer and conversion therapy, the selection of suitable population for conversion therapy, and the role of surgery in conversion therapy. Precision medicine will be applied to conversion therapy for advanced gastric cancer in the future, which would benefit more patients.
3. Optimization of TNM staging and control of pathological quality in gastric cancer

Comprehensive treatment of gastric cancer is mainly based on the pathological staging. The T stage mainly depends on the accurate determination of the depth of the tumor invasion. The accurate T stage should be standardized pathological examination and continuous sectioning. N stage may be influenced by the number of lymph node examined. Insufficient lymph node examined may lead to stage migration. Therefore, standardizing lymph node dissection and lymph node harvest after surgery is important. M stage is mainly to improve the detection rate of peritoneal lavage cytology (CY), identify high risk factors for peritoneal metastasis, and optimize the prediction of peritoneal metastasis molecular markers, as a complementary methods of clinical examination. Currently, the quality of standardized pathological diagnosis of gastric cancer in China still needs to be improved. This article mainly elucidates the related studies and clinical experience of our center on how to do better in the optimization of gastric cancer TNM staging and pathological quality control.
4.Recent advance in anti-fingolimod glioblastomas

Fingolimod (FTY720) as a new immunosuppressive agent,is the prodrug of an sphingosine-1-phosphate receptor agonist.It is a potent immunosuppressant which has been approved by the US Food and Drug Administration to treat relapsing-remitting multiple sclerosis,and shows its unique and novel mechanism of action.Unlike the traditional immunosuppressive agents,fingolimod exerts immunosuppressive and immunoregulatory functions mainly through interaction with shhingosine-l-phosphate receptors on the cell surface without affecting activation and proliferation of lymphocytes.In addition,FTY720 has been shown to inhibit a variety of cancer related signal transduction pathways.It also presented significant anticancer effects in the in vivo and in vitro experiments.And in the treatment of glioblastoma multiforme (GBM) experiment,FTY720 displayed excellent activity as well had a synergistic effect in addition to temozolomid,the current standard chemotherapeutic agent to treat malignant gliomas.This article reviewed the advances in study on the anti-GBM effect and mechanism of FTY720.
5.Identification of a de novo MAP2K1 gene variant in an affected patient with Cardio-facio-cutaneous syndrome.

OBJECTIVE:
To explore the genotype-phenotype correlation of Cardio-facio-cutaneous syndrome (CFCS) caused by MAP2K1 gene variants.
METHODS:
Genomic DNA was extracted from peripheral blood sample from a child patient and his parents. Whole exome sequencing (WES) was carried out for the patient. Suspected variant was verified by Sanger sequencing.
RESULTS:
The patient was a 1-year-8-month old Chinese male who manifested short stature, psychomotor retardation, relative macrocephaly, distinctive facial features, and congenital heart disease. WES test revealed a heterozygous missense c.389A>G (p.Tyr130Cys) variant in the MAP2K1 gene. Sanger sequencing has confirmed the variant as de novo. According to ACMG/AMP guidelines, the variant was classified as pathogenic.
CONCLUSION
Compared with previously reported CFCS cases due to MAP2K1 variants. The patient showed obvious behavioral problems, good appetite and tricuspid regurgitation, which may to be novel features for CFCS.

6.Discussion and treatment of bullous pemphigoid from traditional Chinese medicine point of view "lung governing skin and hair"

Bullous pemphigoid is a chronic obstinate dermatological disease. Hormone is the main therapeutic method, but the disease course is rather long, relapse is frequently seen and difficult to be radically cured; many complications may occur such as pulmonary infection, etc. From traditional Chinese medicine (TCM) basic principal points of view "lung governing skin and hair" and "strengthen the body resistance to eliminate pathogenic factors", the author explored the TCM therapy of bullous pemphigoid. By using clearing away lung heat and invigorating lung qi as the main principles supplemented by invigorating spleen and kidney, eliminating phlegm and blood stasis for treatment of such disease, relatively satisfactory therapeutic results were obtained.
7. The importance of occupational history in clinical thinking from the diagnosis and treatment of a case of Tsutsugamushi disease

A farm worker in Baodi District of Tianjin was diagnosed with Tsutsugamushi disease due to fever and intermittent cough for more than 3 months.The patient's diagnosis and treatment process was complicated, and the diagnosis was delayed in the early stage of the disease because the clinician neglected the occupational history and lacked knowledge about the prevention and control of tsutsugamushi disease. As an important part of the epidemiological history, occupational history is crucial for the diagnosis of occupational related diseases. Clinical thinking runs through the whole process of clinical diagnosis and disease treatment, and correct clinical thinking can effectively reduce the occurrence of misdiagnosis.Clinicians should ask and record career history in detail to improve the quality of health care.
