1.Clinical effect of atorvastatin combined with lopidogrel in patients with transient ischemic attack

Objective To investigate the clinical effect of atorvastatin combined with lopidogrel in patients with transient ischemic attack.Methods Totally 92 cases of patients with transient ischemic attack in Xingyuan Hospital of Yulin City from January 2013 to August 2015 were divided into observation group and control group,46 cases in each group.Patients in observation group were treated with atorvastatin combined with lopidogrel,and patients in control group were treated with lopidogrel.The differences in short term effect,long-term effect,and adverse reaction between two groups were compared.Results The total effective rate of observation group was significantly higher than that of control group (P < 0.05);After treatment,the levels of hs-CPR,Lp-PLA2TC,TG,and LDL,the recurrence rate of transient ischemic attack and the incidence of cerebral infarction of the observation group were significantly lower than that of the control group and before treatment (P < 0.05),but the differences in PLT,PT,and APTT between two groups before and after treatment were not significant.Conclusion The clinical effect of atorvastatin combined with lopidogrel in patients with transient ischemic attack were remarkable,and the incidence of adverse events do not increase.
2.The value of MRI in differentiating hepatic epithelioid angiomyolipoma from hepatocellular carcinoma

Objective To compare the value of dynamic contrast-enhanced MRI(DCE-MRI) and diffusion weight-imaging(DWI) in differentiating hepatic epithelioid angiomyolipoma (HEAML) from hepatocellular carcinoma(HCC).Methods The MR data of 15 HEAML and 50 HCC proved surgically and pathologically were analyzed retrospectively.The DCE-MRI and DWI were performed preoperatively.The size, location, margin, T1-weighted images(T1WI),T2-weighted images(T2WI) and DWI signal intensity(SI),enhancement pattern, central vessel, early draining vein, pseudocapsule were recorded.The apparent diffusion coefficient (ADC) value was also calculated.The differences of these MR features between two groups were compared statistically.Results There was one lesion in each patient.For all 15 HEAML lesions, early draining vein, central vessel and pseudocapsule were displayed in 8,11 and 6 lesions, respectively.The wash in and slow out enhancement pattern was shown in 10 lesions and wash in and wash out pattern shown in others (5/15).The mean ADC value was (1.15±0.31)×10-3 mm2/s.For all 50 HCC lesions, early draining vein, central vessel and pseudocapsule were displayed in 6,5 and 45 lesions, respectively.29 lesions were shown as wash in and wash out enhancement pattern and the others (21/50) as wash in and slow out pattern.The mean ADC value was (1.23±0.29)×10-3 mm2/s.There were significant differences in these MR features including draining vein, central vessel and pseudocapsule (P<0.05), but no differences in size, location, margin, SI(T1WI, T2WI and DWI), dynamic enhancement pattern, ADC value(P>0.05) between HEAML and HCC.Conclusion DCE-MRI is more helpful in differentiating HEAML from HCC than DWI.
3.Remifentanil for deliberate hypotension in intracranial aneurysm clamp operation

Objective To discuss the effects of remifentanfl for delthemte hypotension in intracranial aneurysm clamp operation,evaluating the feasibility and safety.Methods Twenty intracranial aneurysm patients undergoing occlusion surgery were prospectively randomized in to two groups:Group Ⅰ and Group Ⅱ.Group Ⅰ adopted remifentanil for deliberate hypotension and Group Ⅱ adopted sodium nitroprusside.The data were analyzed with SPSS 11.5 for Windows.Results Deliberate hypotension was achieved at the target mean arterial pressure(MAP)for remifentanil and nitroprusside respectively.During deliberate hypotension,HR in Group Ⅰ were lower than that in Group Ⅱ(P<0.01).RPP in Group Ⅰwere lower than that in Group Ⅱ(P<0.01).Blood loss in Group Ⅰ were lower than that in Group Ⅱ(P<0.05).Conclusion For general anesthesia in intracranial aneurysm clamp operation,using remifentanil for deliberate hypotemion is a good choice.
4.Analysis of Antibodig Spectrum of Anti- membranous Lahyrinth Proteins in Experimental Animals with Autoimmune Inner- ear Disease

ObjectiveTo define the components of membranous labyrinth proteins which take part in the development of autoimmunity in experimental animals with autoimmune inner- ear disease(AIED). MethodsAn animal modee of AIED was es tablished by using homologous crude inner ear antigen to immune the guinea pig. The autoantibodies against inner ear antigenin the sera of animals were examined by immunoblotting assay. ResultsThe components of membranous labyrinth proteins were very complicated, which had five main bands in electrophoresis; most of immunized animals had only developed two sorts of autoantibod ies which were anti-68 kD and 136 kD membranous labyrinth proteins. There was no positive reaction in sera of control animals. Conclusion This results showed that the immune reactions against the 68 kD and 136 kD proteins of membranous labyrinth may be autoimmune bases for AIED in animals.
5.Relationship between the major histocompatibility complex class I chain-related gene A expression and clinicopathologic features in laryngeal squamous carcinoma.

OBJECTIVE:
To investigate the expression of major histocompatibility complex class I chain-related gene A (MICA) in laryngeal squamous cell carcinoma(LSCC), and its clinical significance.
METHOD:
Immunohistochemistry and RT-PCR were used to detect the expression of the MICA in LSCC and normal tissue samples. The relationship between the expression of MICA and the clinicopathologic features features were was analyzed.
RESULT:
Compared to the expression of MICA in normal tissues samples, the expression of MICA in LSCC tissue was significantly increased (P < 0.01). MICA expression level in carcinoma tissue was closely related to the tumor-differentiation degree and TNM staging.
CONCLUSION
Our study suggests that MICA may play an important role in the invasion and metastasis of LSCC, and could be a potential tumor maker for LSCC.

6.Curative Effects of Tympanoplasty with Two Different Ossicles Protheses

Objective To investigate the therapeutic effects of titanium artificial auditory ossicles in closed type mastoid radical tympanoplasty ,in comparison with ceramic artificial auditory ossicles .Methods A retrospec‐tive analysis was done on of 150 patients (155 ears) with the closed type of mastoid radical tympanoplasty using arti‐ficial auditory ossicles for hearing reconstruction (from January 2008 to December 2012) ,cund were ,and followed up for 12 months .Based on the two kinds of artificial auditory ossicle materials ,the group was divided into two :122 cases (127 ears) for the titanium auditory ossicle group and 28 cases (28 ears) for the ceramic auditory ossicle group .We compared the average air conduction hearing threshold and air bone gaps (ABG) at 0 .5 ,1 .0 ,2 .0 ,and 4 .0 kHz ,at preoperative 6 months and postoperative 6 ,12 months .Results For the Titanium artificial auditory os‐sicle group:117 ears (92 .13% ) had hearing threshold improvement >15 dB HL ,106 ears of the ABG change ≤20 dB ,which were statistically significant .The total success rate of hearing reconstruction was 83 .46% (106/127) . For the ceramic auditory ossicle group:5 ears (17 .86% ) had hearing threshold improvement >15 dB HL ,3 ears (10 .71% ) of the ABG change ≤20 dB ,which was no statistical significance between the two groups .Conclusion The hearing reconstruction effects of titanium artificial auditory ossicle in closed type mastoid radical tympanoplasty is excellent .It is suitable for application in closed-mastoidectomy tympanoplasty ideal artificial auditory ossicle .
7.Analysis of etiology and prognosis of neonatal gastric perforation

Objective To investigate the etiology,clinical features and prognosis of neonatal gastric perforation.Methods The medical records of 18 patients with neonatal gastric perforation with respect to sex,age,birth-weight,course of disease,clinical presentations,and prognosis were retrospectively analyzed.Results There were 13 boys and 5 girls,8 of whom were full term infants and 10 preterm infants.The most common initial manifestations were poor activity,abdominal distension,and respiratory distress.All neonates were treated by surgical operation,the overall survival rate was 11/18.Prematurity,birth-weight,course of disease,were the statistically significant risk factors (P < 0.05).Conclusions Neonatal gastric perfora tion is mainly caused by congenital gastric wall defect,and associated with high mortality,particularly in premature infants.It is necessary to early diagnosis,surgical operation in time,nurse intensively,and observe the condition closely.
8.Investigation to Theoretical Origin of the Prevailing "Five Orbiculi" Theory

The article made a study on the earliest literature and the initial time of the "Five Orbiculi" theory in TCM. By analyzing the masons for different locations of "Five Orbiculi" recorded in ancient TCM books, the author argued against that the term of "Five Orbiculi" originated from ancient India and the "Five Orbiculi" theory was a product with the combination of traditioinal Chinese and Indian cultures. The author further put forward that the "Five Orbiculi" theory most probably was a Chinese traditional medical innovation under the influence of Internal Classic of Medicine, a great development to the theory recorded in Miraculous Pivot On Serious Confusion.
9.A novel reporter system monitoring sortase A catalyzed protein ligation efficiency.

Efforts on directed evolution of sortase A to optimize its catalytic properties have been undertaken and shown the promise. To facilitate screening of sortase A mutants with expected catalytic properties, a novel ligation efficiency monitoring system, including reporter substrates GFP-LPETG and GGGYK-Biotin, was developed. GFP-LPETG, wild type sortase A, and a recently reported high activity sortase A mutant were prepared recombinantly from Escherichia coli BL21 (DE3). Taking advantage of the newly designed reporter system, the ligation efficiency catalyzed by wild type and mutant form of sortase A could be sensitively monitored via SDS-PAGE directly. Consistent with previous report, the mutant sortase A displayed much higher catalytic activity compared to wild type enzyme, indicating the new reporter system is easily and fast handled and sensitive. The application of this reporter system into systemic screening will facilitate future directed optimization of sortase A.

10.MR Imaging of endometrial benign and malignant polypoid lesions: correlations with clinicopathological findings

Objective To investigate the diagnostic value of conventional and dynamic gadolinium-enhanced T1-weighted (T1W) MRI in benign and malignant endometrial polypoid lesions. The pathologic basis of MRI features was also evaluated. Methods The conventional and dynamic contrast enhanced MRI features in 48 cases with clinicopathologically-proved endometrial polypoid lesions were retrospectively analyzed, including 26 cases with malignant tumors (23 eases with endometrial carcinoma and 3 cases with endometrial sarcoma) and 22 cases with benign polypoid lesions (12 cases with endothehal polyps and 10 cases with submucons leiomyomas). The occurrence probability of central fibrous core, intratumoral cysts and intact junctional zone were evaluated by Fisher's exact test. Results Benign and malignant endometrial polypoid lesions had specific characteristics on MRL Endometrial carcinomas usually appeared as homogeneous intermediate or slightly high signal-intensity masses on T2-weighted images, and relatively homogeneous hypedense masses on contrast-enhanced images with myometrial invasion (P < 0.05). An irregular central fthrous core and small non-enhanced intratumoral cysts were seen more frequently in endometrial polyps (P < 0.05) than others. All 3 cases with uterine sarcomas showed obviously inhomageneous intensity on T2WI and demonstrated areas of early and persistent marked enhancement.Submucous leiomyomas demonstrated isodense or slightly low-density masses with defined outline on T2 WI and the endomerium was displaced. The diagnostic sensitivity, specificity and accuracy of malignant polypoid masses were 92.3% (24/26), 83.3% (20/24) and 95.8% (46/48) respectively. Conclusion Conventional and dynamic contrast-enhanced MRI are valuable in characterizing the benign and malignant endometrial polypoid lesions.
