1.Effect of ulinastatin on postoperative delirium in elderly patients receiving hip fracture surgery

Objective To explore the effect of ulinastatin treatment on postoperative delirium (POD) in elderly patients receiving hip fracture surgery.Methods Ninety-six elderly patients (38 males,58 females,aged 70-93 years,ASA grade Ⅱ or Ⅲ) undergoing elective hip fracture surgery were randomly divided into two groups using a random number table: ulinastatin group (group U) and control group (group C),48 cases in each group.After spinal anesthesia and fascia iliaca compartment block,ulinastatin 5 000 U/kg diluted with normal saline to a volume of 50 ml (group U) was administered intravenously over 10 min before surgical incision and the equal doses on post-operative days 1,2.The equal volume of normal saline was administered intravenously in group C at the same time.POD was assessed by using the Confusion Assessment Method (CAM) on post-operative days 1-3.Serum samples were collected to measure the levels of IL-6,IL-10 and S100β before the anesthesia (T0),at the end of surgery (T1) and three days after surgery (T2) by ELISA.Results The incidence of POD in group U was significantly lower than that in group C (4.3% vs.28.2%) (P<0.05).Compared with T0,the levels of serum IL-6 and IL-10 in group C at both T1 and T2 significantly increased (P<0.05).Compared with group C,serum IL-6 levels in group U decreased at both T1 and T2 (P<0.05).Compared with T0,the levels of serum S100β in group C at T1 significantly increased (P<0.05).Compared with group C,ulinastatin significantly inhibited the release of serum S100β at T1 (P<0.05).Conclusion Ulinastatin can significantly reduce the incidence of POD in elderly patients undergoing hip fracture surgery.The mechanism may involve inhibition of IL-6 and S100β in serum.
2.Effect of umbilical cord mesenchymal stem cell transplantation on cognitive function in endotoxemic rats

Objective To evaluate the effect of umbilical cord mesenchymal stem cell (UC-MSC) transplantation on cognitive function in endotoxemic rats.Methods Seventy-two pathogen-free male Wistar rats,aged 180-250 g,were randomly divided into 4 groups (n=18 each) using a random number table:control group (group C),endotoxemia group (group E),UC-MSC group (group M),and endotoxemia+ MSC group (group PM).Cognitive function was assessed using Morris water maze test.At 4 h before training on 2nd day of place navigation test,lipopolysaccharide 100 μg/kg was injected intraperitoneally in group E,the equal volume of normal saline was given in group C,UC-MSC suspension 300 μl (5× 105cells,the fluid was normal saline) was injected via the tail vein immediately after intraperitoneal lipopolysaccharide in group PM,and UC-MSCs were injected via the tail vein in group M.At 4 h after injection,Morris water maze test was continued.At 4,24 h and 5 days after intraperitoneal injection or injection via the tail vein (T1-3),6 rats were selected,and blood samples were collected from hearts.The rats were then sacrificed,and the hippocampus was removed.The levels of interleukin-lbeta (IL-1β),IL-6 and tumor necrosis factor-alpha (TNF-α) in serum and hippocampus were determined.Results Compared with group C,the escape latency was significantly prolonged on 2nd and 3rd days of Morris water maze test,the number of crossing the platform was significantly decreased,the percentage of swimming distance in 1 st quadrant in the total swimming distance was significantly decreased,and the levels of IL-1 β,IL-6 and TNF-α in serum and hippocampus were significantly increased at T1,2 in group E (P<0.05).Compared with group E,the escape latency was significantly shortened on 2nd and 3rd days of Morris water maze test,the number of crossing the platform was significantly increased,the percentage of swimming distance in 1st quadrant in the total swimming distance was significantly increased,and the levels of IL-1β,IL-6 and TNF-α in serum and hippocampus were significantly decreased at T1,2 in group PM (P<0.05).Conclusion UC-MSC transplantation can improve the cognitive decline in endotoxemic rats.
3.Comparing the clinical characteristics of IgG4-related sclerosing cholangitis with primary sclerosing cholangitis

Objective To analyze and compare the clinical characteristics of primary sclerosing cholangitis (PSC) with IgG4-related sclerosing cholangitis (IgG4-SC).Methods The clinical data of 32 PSC patients and 72 IgG4-SC patients who were hospitalized in Peking Union Medical College Hospital (PUMCH) from January 2004 to December 2014 were retrospectively analyzed.Results Of the 32 PSC patients,there were 16 male and 16 female.Of the 72 IgG4-SC patients,there were 61 male and 11 female,(ratio =5.5∶ 1).The average ages were 44.9 (11 ~ 77) and 59.8 (28 ~ 83) years,respectively (P <0.05).The most common symptoms of PSC and IgG4-SC were abdominal pain and jaundice,and the incidences of abdominal pain and jaundice were 50.0% and 68.1%,78.1% and 81.9%,respectively.The serum IgG4 level of the IgG4-SC patients was significantly higher than the PSC patients (P < 0.05).The total protein in serum of the IgG4-SC patients was higher than the PSC patients (P < 0.05).The rate of bile duct wall thickening as detected on endoscopic ultrasonography (EUS) was higher than by abdominal ultrasound and abdominal CT,which were 91.2%,11.5% and 33.3%,respectively (P <0.05).12 PSC patients were followed up for over 2 years,including 2 patients who underwent liver transplantation after failure of conservative treatment,5 patients who died from hepatic failure and infection,and 3 with stable condition.43 IgG4-SC patients were followed up for over 2 years,including 16 patients with relapse.The recurrence rate was 37.2% (16/43).The more the extrabiliary organs or bile duct segments were involved,the higher was the recurrence rate.Conclusions Both PSC and IgG4-SC are cholestatic diseases,and they have many similarities in clinical and imaging manifestations.However,they still have unique features.IgG4-SC is sensitive to glucocorticoids therapy and has good prognosis.Thus,it is important to differentiate PSC from IgG4-SC.
4.Prognostic index construction and analysis of immune infiltration combined with RNA binding protein and transcription factor in hepatocellular carcinoma

Objective:To comprehensively analyze the prognostic prediction value of RNA binding protein, transcription factor gene expression and immune infiltration in hepatocellular carcinoma (HCC).Methods:Common gene sets associated with RNA-binding proteins and transcription factors were screened in TCGA ( n=365) , GSE54236 ( n=78) and GSE14520 ( n=221) datasets. Univariate Cox regression was used for primary screening. The survival regression model was constructed by LASSO-Cox. And a complex index [CIRT=(score-min)/max] was calculated. According to the median of CIRT, the HCC patients were divided into CIRT high group ( n=182) and CIRT low group ( n=182). The differences of prognosis, immune infiltration between the two groups were analyzed. Results:Of 37 prognostically relevant RNA binding protein and transcription factor genes were identified. The prognosis prediction model based on seven selected genes was determined by stepwise regression. Patients in the CIRT high group exhibited a lower percentage of macrophages in M1 ( P=0.032), macrophages in M2 ( P=0.009), resting mast cell ( P<0.001), activated NK cells ( P=0.007), and resting memory CD4 + T cells ( P<0.001), while patients in the CIRT low group showed a lower level of resting dendritic cells ( P=0.048), macrophages in M0 ( P<0.001), neutrophils ( P=0.049), follicular helper T cells ( P=0.004) and regulatory T cells ( P=0.001). GSEA analysis has shown that CIRT high groups were highly enriched in cell cycle, DNA repair pathways in TCGA and GSE14520. In the TCGA cohort, the CIRT low group had better overall survival than the CIRT high group. Analysis of 5-year follow-up data in the TCGA cohort showed that CIRT had a good predictive value for long-term survival of patients with liver cancer (area under receiver operating characteristic curve was 0.71). Conclusion:A novel prognostic index and classifier based on RNA-binding protein expression, transcription factors and immune expression profiles were developed and cross-cohort validated. CIRT could be used as an independent predictor.
5.Clinical analysis of 11 cases multisystem inflammatory syndrome associated with SARS-CoV-2 Omicron variant infection in children

Objective:To explore the clinical characteristics, diagnosis, treatment, and follow-up of multisystem inflammatory syndrome in children (MIS-C) related to SARS-CoV-2 Omicron variant infection.Methods:A retrospective analysis was conducted on clinical data of 11 children with MIS-C, who were admitted to the Department of Pediatrics of Peking University First Hospital from December 2022 to January 2023. Clinical characteristics, treatment, and follow-up of MIS-C were summarized in this study.Results:The 11 cases contained 7 boys and 4 girls, with an age of 4.4 (2.0, 5.5) years on admission. All the patients had fever, with a duration of 7(5, 9) days. Other clinical manifestations included rash in 7 cases, conjunctival hyperemia in 5 cases, red lips and raspberry tongue in 3 cases, lymphadenopathy in 3 cases, and swollen fingers and toes in 2 cases. There were 8 cases of digestive symptoms, 8 cases of respiratory symptoms, and 3 cases of nervous system symptoms. Eight patients had multi-system injuries, and one of them had shock presentation. All 11 patients were infected with SARS-CoV-2 Omicron BF.7 variant. The laboratory examination results showed that all cases had elevated inflammatory indicators, abnormal coagulation function and myocardial damage. Six patients had elevated white blood cell counts, 5 cases had liver function abnormalities, 3 cases had kidney function abnormalities, and 8 cases had coronary artery involvement. All 11 patients received anti-infection treatment, of which 3 cases received only 2 g/kg intravenous immunoglobulin (IVIG), while the remaining 8 cases received a combination of IVIG and 2 mg/(kg·d) methylprednisolone. Among the 8 cases with coronary artery disease, 6 cases received low molecular weight heparin anticoagulation therapy. All patients were followed up in 2 weeks after being discharged, and their inflammatory markers had returned to normal by that time. The 8 cases with coronary artery disease and 3 cases with pneumonia showed significant improvement or back to normal at the 4-week follow-up. All patients had no new complications or comorbidities during follow-up of more than 3 months.Conclusions:MIS-C may present with Kawasaki disease-like symptoms, with or without gastrointestinal, neurological, or respiratory symptoms. Elevated inflammatory markers, abnormal coagulation function, and cardiac injury contribute to the diagnosis of MIS-C. IVIG and methylprednisolone were the primary treatments for MIS-C, and a favorable short-term prognosis was observed during a follow-up period of more than 3 months.
6.Effects of aerobic exercise on learning and memory functions, hippocampal synaptic plasticity and the adiponectin signaling pathway in diabetic rats

Objective:To explore the effects of aerobic exercise on learning and memory functions, hippocampal synaptic plasticity and the ADPN signaling pathway in diabetic rats.Methods:6-week-old male SD rats were randomly divided into a blank control group(NC group)and a high-fat diet group, and a rat model for diabetes was induced by feeding rats in the high-fat diet group with a high-fat diet combined with intraperitoneal instillation of low-dose streptozotocin(STZ)for 5 weeks.Rats in the high-fat diet group were further divided into a diabetic group(DC group)and a diabetic aerobic exercise group(DM group)after successful establishment of the model.Rats in the DM group were subjected to aerobic exercise for eight weeks and then the Morris water maze test was conducted to assess learning and memory functions, relevant serum markers were measured, Golgi staining was used to examine synaptic changes in the hippocampus, and Western blot was carried out to detect hippocampal protein expression levels of adiponectin(ADPN), AMP-activated protein kinase(AMPK), glucose transporter 4(GLUT4), synaptic plasticity-related protein synaptophysin(SYN)and postsynaptic density protein 95(PSD-95)for rats in each group.Results:Serum FBG and HBA1c in diabetic rats were markedly significantly decreased after 8 weeks of aerobic exercise( P<0.01), and serum ADPN and insulin were significantly increased after 8 weeks of aerobic exercise( P<0.05).When test results from the three groups of rats compared, the F value was 69.248 for FBG, 6.740 for INS, 7.017 for HBA1C and 14.315 for serum ADPN.The results of the water maze test and hippocampal Golgi staining showed that the escape latency of diabetic rats was highly significantly decreased after 8 weeks of aerobic exercise( P<0.01).The platform crossing times, the number of dendritic branches and the dendritic spine density in the hippocampal CA3 region of diabetic rats were significantly increased after 8 weeks of aerobic exercise( P<0.05).When results from the three groups of rats were compared, the F value was 13.934 for escape latency, 5.864 for platform crossing times, 9.307 and 6.734 for the number of dendritic branches and the density of dendritic spine in hippocampal CA3 region.Hippocampal PSD-95, SYN, ADPN, p-AMPK, and GLUT4 protein expression levels of diabetic rats were significantly increased( P<0.05)after 8 weeks of aerobic exercise.When results from the three groups of rats were compared, the F value was 15.137 for SYN, 5.415 for PSD-95, 9.687 for ADPN, 27.761 for GLUT4, and 9.298 for p-AMPK. Conclusions:Eight weeks of aerobic exercise can improve the learning and memory functions of diabetic rats, and the mechanisms may be related to exercise-induced hippocampal ADPN/AMPK/GLUT4 signaling activation in rats, leading to enhanced synaptic plasticity in the hippocampus.
7.Detection of IgG protein in human urine based on vertical flow paper microfluidic chip.

The kidney is the body's most important organ and the protein components in urine could be detected for diagnosing certain diseases. The amount of IgG protein in urine could be used to determine the degree of kidney function damage. IgG protein in human urine was detected by vertical flow paper-based microfluidic chip, double-antibody sandwich immunoreaction, and cell phone image processing. The results showed that using an IgG antibody concentration of 500 μg/mL and a gold standard antibody concentration of 100 μg/mL, the image signal showed a good linear relationship in the range of IgG concentration of 0.2-3.2 μg/mL, with R2=0.973 3 achieved. A complete set of detection devices were designed and the detection method showed good non-specificity.

8. Investigation and analysis on characteristics of a cluster of COVID-19 associated with exposure in a department store in Tianjin

Objective:
To describe the epidemiological characteristics of a cluster of COVID-19 cases reported in Baodi district of Tianjin as of 18 February, 2020, which might be associated with the exposure in a local department store, and provide suggestions for prevention and control strategy development.
Methods:
The basic characteristics, time and area distributions, clinical manifestations, epidemiological history and transmission mode of the COVID-19 cases associated with the department store exposure were analyzed.
Results:
A total of 40 COVID-19 cases were associated with the department store exposure, accounting for 75.47% of the total confirmed cases (53 cases) reported in Baodi district. The cases were mainly at the age of 60 years or older (35.00%) and farmers (40.00%). The main clinical manifestations included fever (95.00%), cough (35.00%), and diarrhea (15.00%). The proportion of confirmed severe cases was 32.50%. The incidence curve showed that the incidence peak occurred on 31 January, 2020. Among the 40 cases, 6(15.00%) were department store employees, 19(47.50%) were customers and 15(37.50%) were close contacts (secondary cases). The first case occurred on 21 January, 2020, this case was a department store employee who had a purchasing history at whole sale markets in other provinces and cities before the onset, and 3 employees were still on duty after symptom onsets. The median of the incubation period of customer cases was 6 days, and the median of the interval between onset and medical treatment of customer cases was 7 days.
Conclusion
This was a cluster epidemic of COVID-19, which might be associated with the exposure in the department store. By now, the current prevention and control measures have achieved satisfied effects.
