Chinese Journal of Rehabilitation Theory and Practice 2013;19(4):362-364
The artificial urinary sphincter (AUS) has been used for the treatment of various incontinence due to intrinsic sphincter deficiency, and is considered as the gold standard treatment for incontinence after a prostatectomy. The long-term satisfaction rates exceed 80%, but almost a third of patients with AUS require revision surgery within 5 years. This paper reviewed the common complications of AUS implantation and the treatment.

Objective To explore the technique, efficacy and complications of transurethral sphincterotomy with Holmium laser for the treatment of male patients with neurogenic bladder and examine its long-term outcome. Methods 20 male cases who received transurethral sphincterotomy with Holmium laser were included in this retrospective analysis, in which 7 cases injuried at cervical spinal cord, 6 at thoracic spinal cord, 3 at cauda equine; 1 was spinal meningocele, 1 was sacral crack, 1 was ependymoma, and 1 was myelitis sequel. The age was 20~64 years, averaged 45.1 years. They were followed up for 3~95 months (averaged 43.6 months) since the last sphincterotomy. Results The 20 patients significantly improved in symptoms, laboratory examination, imaging demonstration and urodynamic index after the operation. There were 3 patients who required repeated laser surgery during the follow- up. 17 patients were stable. Conclusion Transurethral sphincterotomy with Holmium laser is effective on neurogenic bladder with less bleeding for the appropriately selected patients.

Objective To summarize and analyze the clinical features, diagnostic methods and treatment measures of patients with advanced spontaneous esophageal rupture. Methods Retrospective analysis of clinical characteristics of 10 patients with advanced spontaneous esophageal rupture was conducted. Results The average age of the patients was 49. 3 years old. The average time of diagnosis was 82. 6 hours. The cause of onset was drastic vomiting except for one case falling down. The main clinical manifestations were chest pain, abdominal pain, and shortness of breath, fever and elevation of hemogram, pleural effusion appeared in all patients,1 case was not treated in time, and 9 cases of the first checks were delayed for diagnosis and treatment in other specialties. Conservative treatment(closed thoracic drainage, gastrointestinal decompression, enteral nutrition support and antimicrobial therapy) was given to all patients. All 10 cases were cured by conservative treatment, the average time of hospitalization was 49. 4 days, followed up for 2 years, no chronic empyema, stricture of the esophagus and reflux esophagitis were observed. Conclusion Late spontaneous rupture of the esophagus is caused by delays in the diagnosis and treatment of the esophagus, the effect of comprehensive conservative treatment is satisfactory.
