1.Study on the sustainability gap of funds for Urban Employees' Basic Medical Insurance in Beijing

Objective:To forecast Urban Employees' Basic Medical Insurance ( UEBMI) in the next 85 years based on population size, age and gender distribution, and to calculate its sustainability gap. Methods:According to the population policy of Beijing and economic development of the region;based on the data of the sixth census in Bei-jing, the estimation model was constructed, and its parameters were defined and selected. Based on the period of 2010, the future of UEBMI in the next 85 year was predicted based on the population size, age and gender distribu-tion. Results:The population of UEBMI in Beijing will reach the maximum value in 2020, and then the downward trend will increase. The aging rate of the insured population will accelerate year after year and will reach the peak in the middle of this 21st century. The sustainability gap is predicted to reach at least 9541. 35 Billion dollars, and the Beijing municipal government is expected to level an average of at least 44 . 693 billion yuan per year to set aside the financial funds in order to fill the gap, so sustainable development of the fund is ensured. Conclusion:From the point of view of the insured population, there is a huge financial sustainability risk in the future of UEBMI in Beijing.
2.Analysis of methomyl and its metabolite in rat plasma by HPLC-MS

Objective: No complete study has been reported on methomyl and its metabolite in rat plasma at home and abroad.This study aimed to detect methomyl and its metabolite in rat plasma by high performance liquid chromatography-mass spectrometry(HPLC-MS).Methods: Ten Wistar rats received intragastric administration of methomyl 50mg/kg,and then executed and kept at room temperature for 72 hours.After that,methomyl and its metabolite in the heart-blood of the rats were determined by solid phase extraction(SPE) and HPLC-MS.Results: Compared with the blank controls,methomyl and its metabolite were detected in the plasma of the experimental rats,and so were protonate ions and multistage fragment ions.Conclusion: The methomyl metabolite in the rat plasma was S-methyl-N-hydroxythioacetamidate.
3.Determination of methomyl in rat plasma by SPE-HPLC

Objective To establish a method of solid phase extraction-reversed-phase high performance liquid chromatography(SPE-HPLC)for determination of methomyl in plasma of rat.Methods The sample pretreatment method,the test conditions,the linear range,the sensitivity,the specificity,the precision, the accuracy,the stability and the recoveries for plasma were investigated by using rat plasma spiked with standard methomyl and intemal standard substance.Results The linear range was 0.1~20μg/mL ( r= 0.9993,P<0.001).The limit of detection was 0.03μg/mL(S/N ≥3).The intra and inter-day precision of assay for methomyl was less than 8.33%and 11.11%in plasma respectively.The intra and inter-day accuracy of assay for methomyl was between 90%and 120%in plasma respectively.The recoveries for methomyl were more than 88%±4.4%in plasma.Conclusion The HPLC method for quantitative and qualitative analysis of methomyl is simple,rapid and accurate,which is suitable for the identification of methomyl in the cases.
4.Relationship among CEA, CA199, clinical pathological characteristics and prognosis in patients with rectal cancer

Objective To investigate the relationship among CEA, CA199, clinical pathological characteristics and prognosis in patients with rectal cancer. Methods A total of 142 patients diagnosed rectal cancer were collected for the study between January 2006 and December 2010. The clinical data including age, sex, size of tumor, differentiation, depth of tumor (T), lymph node metastasis (N), distant metastasis (M), lymphatic invasion, venous invasion, stage, and preoperative serum levels of CEA and CA199 were obtained. Patients′ follow-up ranged from 42 months to 90 months. Results Preoperative elevated CEA levels were significantly associated with distant metastasis (P < 0.05). Median overall survival time was 150 weeks in the group with normal preoperative CEA and CA199, and 101 weeks in the group with preoperative abnormal preoperative CEA and/or CA199, and Kaplan-Meier analysis showed that the difference was significant between two groups (P = 0.031). Conclusion Elevated preoperative CEA level can predict distant metastasis. The patients with abnormal preoperative CEA and/or CA199 have shorter overall survival time.
5.Study of Hippocampal Perfusion in Temporal Lobe Epilepsy Patients with MRI Arterial Spin Labeling(ASL) Technique

Objective To perform the main application of MRI arterial spin labeling (ASL) in cerebral blood flow (CBF) on hippocampus of patients with temporal lobe epilepsy for quantitative measurement,to assess the relationship between temporal lobe epilepsy occurrence and hippocampal perfusion and to explore the critical value of hippocampal CBF predict the temporal lobe epilepsy early.Methods Forty-two subjects by video EEG (VEEG) and conventional MR sequences screend were divided into 3 groups eventually,namely group of healthy control,the group with one temporal lobe abnormal discharge,that conventional MRI in hippocampus is not abnormal and the last group with one side hippocampus sclerosis confirmed by conventional MRI in temporal lobe epilepsy patients,then for the hippocampal 3D ASL examination.We provided six same size regions of interest (ROI) in the hippocampal head,body,tail respectively with the best level display to measure and record bilateral hippocampal cerebral blood flow values (CBF values) of every subject,and to explore the relationship between the cerebral blood flow value in the hippocampus and temporal lobe epilepsy.Results Mean cerebral blood flow (CBF) value of each subject's unilateral hippocampus was calculated.The average CBF value of the hippocampus in the healthy control group was 53.82 ± 0.98ml/(100g · min).The average CBF of the hippocampus in epileptic unilateral abnormal discharge group with the ipsilateral and contralateral were 49.12 ± 5.31ml/(100g · min) and 55.99 ± 1.65ml/(100g · min).The average CBF of the hippocampus in epileptic unilateral hippocampal sclerosis group with the ipsilateral and contralateral were 39.57 ± 2.08ml/(100g · min) and 48.06 ± 1.74ml/(100g · min).Then pairwise comparison of the unilateral hippocampal average CBF value among the 3 groups of experimenters in different groups was performed.The results showed that in the eight times,the first six times had statistically significant differences.Comparing between the healthy control group and epilepsy unilateral temporal lobe abnormal discharge group,to calculates the best critical point by the ROC curve,the cut-off value was 46.76.Conclusion Magnetic resonance arterial spin labeling (ASL) technique can evaluate the cerebral blood flow in the hippocampus of patients with temporal lobe epilepsy by quantitative measurement of hippocampal CBF value.The changes of hippocampal perfusion in patients with temporal lobe epilepsy should be preceded by the changes of imaging.ASL quantitative measurement of hippocampal CBF value is helpful for early diagnosis and treatment of temporal lobe epilepsy.
6.Analysis of Infection Sources and Care seeking Behaviour among STD Clinic Clients

Objective To understand the infection sources, care seeking behaviour and STD prevention in STD clinic patients. Methods Four hundred and seventy four patients of 3 STD clinics in Shanghai were investigated with questionaires and interviewed. Results 94.1% respondents contracted STDs via sexual contact. 70.3% married females attributed their infection source to their husbands, while more than 80% males and unmarried patients were infected through casual or commercial sex partners. High risk populations were observed among salesclerks, hotel attendants, private businessmen and unemployed, who were liable to contract STD through extramarital sexual contact and had a low rate of condom use. 74.5% of patients who visited the clinics after their symptoms had appeared. 82.7%, 62.2%, 57.1% and 30.0% of patients, whose sex partners were their spouses, lovers, commercial sex workers and casual partners, respectively, promised to encourage their partners for STD check up. Although the care seeking behaviour among females was slightly more active than that among males, postponed treatment seeking behaviour, low rate of simultaneous treatment of both male and female partners and low rate of condom use were observed among total patients. Majority of patients practised unprotected sexual intercourse, 4.2% used condom in commercial sexual activities, only 9.7% used condom even after having contracted the present STDs. Conclusion It is implied that counselling and health education must be strengthened in STD clinics, aiming at improving the awareness of STD prevention, insistently and correctly using condom, carefully choosing sex partners and reducing the number of sex partners, and encouraging the patients to adopt healthier life style.
7.Isolation and identification of four isoflavones and one pterocarpan from BUYANGHUANWUTANG

Object To study the antithombotic portion and bioactive fraction of BUYANGHUANWUTANG (BHT), a decoction of astragalus in combination with peony and several other ingredients. Methods The EtOAc portion of BHT extract was repeatedly separated by chromatography and identified by means of spectral analysis. Results One pterocarpan, (6aR, 11aR) 9, 10-dimethoxypterocarpan-3-O-?-D-glucopyanoside (Ⅴ) and four isoflavones, formononetin (Ⅰ), calycosin (Ⅱ), formononetin-7-O-?-D-glucopyanoside (Ⅲ), and calycosin-7-O-?-D-glucopyranoside (Ⅳ) were identified. Conclusion All five compounds were isolated for the first time from BHT.
8.Alkaloids isolated from Buyang Huanwu Decoction and their structure identification

Objective To study the antithrombotic compounds and bioactive fraction from Buyang(Huanwu) Decoction(BHD).Methods Four alkaloids have been isolated from BHD by repeated chromatographies.Their structures were elucidated by means of spectral and single-crystal Xray diffraction analysis.Results They are N-(3′-maleimidyl)-5-hydroxymethyl-2-pyrrole formaldehyde(Ⅰ),4-carbamoyl-2-pyrrole carboxylic acid(Ⅱ),N-(1′-D-deoxyxylitolyl)-6,7-dimethyl-1,4-dihydro-2,3-quinoxalinedione(Ⅲ),and 2,3,4,9-tetrahydro-1H-pyrido indole-3-carboxylic acid(Ⅳ).Conclusion All these compounds are isolated from BHD for the first time and compounds Ⅰ-Ⅲ are new alkaloids.
9.Isomers of amygdalin in BUYANG HUANWU TANG and its production

Object To study the isomers of amygdalin in BUYANG HUANWU TANG and its production Methods Amygdalin was isolated from both peach seeds and BUYANG HUANWU TANG by using various column chromatography Their structures were identified by the various spectral data Results Amygdalin had been isolated from n BuOH fraction of aqueous extract of BUYANG HUANWU TANG and found to be a pair of D , L epimers and their ration was 1∶1 It was also found that the structures and the ration of D , L epimers of amygdalin in decoction of single peach seed were similar to that in BUYANG HUANWU TANG. The peach seed only gave D amygdalin when it was extracted in 95% EtOH at reflux temperature, and D amygdalin cannot be isomerized when it was treated in water at 100 ℃ Conclusion Isomerization of D amygdalin results from interaction between it and other compounds of peach seed in water at high temperature, and has no evident relation to other constituents in BUYANG HUANWU TANG L amygdalin is a new compound generated due to decoction of peach seed
10.Dynamic contrast-enhanced MRI in the early diagnosis of brucella spondylitis

Objective To explore the value of dynamic contrast-enhanced (DCE).MRI in the early diagnosis of brucellosis spondylitis.Methods Fifty-six patients (24 female and 32 male) with Brucellosis with average age of (49± 3)years were retrospectively analyzed.Inclusion criteria:The patients with clinically diagnosed brinell coli spondylitis,drops of serum agglutination test degree 1:100>(++),Hu>red plate agglutination test (+ +),enzyme-linked immunosorbent assay to detect specific antibody IgG/IgM (+).Exclusion criteria:Pregnant and lactating women,patients with MRI examination contraindications and spinal tuberculosis,myeloma or other spinal disease,patients with the cervical,thoracic and lumbar 5 vertebral body involvement.The patients were classified into early lesion group and lesion group.Early lesion group was defined as low back pain less than a month,enzyme-linked immunosorbent assay positive IgM and negative IgG results,and no abnormality in conventional MR imaging,while lesion group was defined as low back pain for longer than 3 months,enzyme-linked immunosorbent assay positive IgG and negative IgM and marked lesion in conventional MR imaging.All the patients conducted with conventional MRI and DCE-MRI scan.The differences of the Ktrans,Kep,Ve and Vp between the vertebral lesions,early lesions of vertebral body and normal tissues were measured and compared.Results The values of Ktrans,Kep,Ve and Vp were significantly different between the vertebral lesions [(0.856±0.539) ml/min,(1.482±0.711) ml/min,0.542±0.267,0.034±0.017] and normal tissues [(0.315±0.298) ml/min,(0.713±0.548) ml/min,0.358±0.259,0.056±0.03](all P<0.05).The values of Ktrans,Kep and Vp were significantly different between the early lesions of vertebral body and normal tissues (all P<0.05),while no difference was found for Ve between the two groups (P>0.05).Conclusion DCE-MRI quantitative analysis plays a role in the early diagnosis of the brucella spondylitis.
