1.Progress of interference screw in the reconstruction of anterior cruciate ligament
Chinese Journal of Orthopaedics 2016;36(12):810-816
The anterior cruciate ligament (ACL) injury in sports is a common knee joint injury. It is widely accepted to reconstruct ACL under arthroscopy with autologous patellar ligament or popliteal ligament fixed in tibial or femoral tunnel with interference screws.Interference screws are widely used as intra-articular fixation devices in ACL reconstruction. The ideal materials for interference screw provide rigid fixation of graft with high mechanical strength,long-lasting stiffness and the bioactivity to stimulate osteointegration and tendon-bone healing.Current interference screws are made of metals and bio-absorbable polymers.Metallic interference screws provide high mechanical strength and stiffness for tendon graft fixation, but require a second surgery for removal.Bioabsorbable interference screws made of polymers avoid second surgery for removal, but have lower mechanical strength and might cause aseprtic inflammation during its degradation as well as screw slip, tunnel enlargement and many other complications.Magnesium (Mg) has been investigated as a novel and potential biomaterials for biodegradable interference screws.Mg is essential nutrient element and has good biocompatibility. Its mechanical strength and related stiffness are much higher than that of biodegradable polymers and are close to cortical bone.Furthermore, the degradation rate of Mg in vivo could be modulated via techniques such as alloying. In previous studies, the relationship between screw design and its pull-out force has been investigated, based on which the design of interference screw is optimized. The mechanical properties (the ultimate load to failure and the related stiffness) of interference screws proved to be higher than that of bioabsorbable interference screws sharing the same design both in vitro and in vivo.Furthermore,Mg performs good biocompatibility without severe intra-articular inflammatory reactions,as well as unique bioactivities to promote osteointegration, mineral deposition and the regeneration of fibrocartilaginous entheses. The present study reviews the researches of Mg interference screws both in vitro and in vivo, gives reference for further research and has great importance for the application of Mg interference screws in ACL reconstruction.

Objective To prepare an antiserum of human putative magnesium transporter protein (HPT protein) to study its biological effects on the drug resistance of gastric cancer cell by observing its expression level. Methods The cDNA encoding HPT protein was cloned and sequenced. Prokaryotic expression vector was constructed and HPT protein fused with histidine tag was expressed. Mice were immunized with the polyacrylamide gel particles containing the fusion protein, and the antisera of them were detected for antibody activity by Western blot. Using the antiserum, HPT protein expression level was assessed in different gastric cancer cell line. Results The sequence of cloned cDNA fragment was consistent with that of HPT. After BL21/pRSETB bacterium was induced with IPTG, a new protein band with a relative molecular weight of 55kD was showed on SDS PAGE profile, and further proved to be fusion protein with histidine tag by Western blot. The antiserum of immunized mice only recognized a unique protein band with a relative molecular weight of 50kD. HPT protein expression level in SGC7901/ADR was higher than in SGC7901. Conclusion The antiserum of HPT protein was successfully prepared, and HPT was found to probably take part in the gastric cancer cell MDR

International Journal of Cerebrovascular Diseases 2016;24(3):275-278
Inflammation is involved in the pathogenesis of many vascular disorders. Hemodynamics cause the activation of inflammatory factors in the vascular wal , resulting in dysfunction of vascular endothelial cels and vascular smooth muscle cels, activating vascular wal inflammatory cascade. Inflammatory cels, especialy macrophage infiltration and secretion of enzymes gradualy weaken the vessel wal to form aneurysm, and even cause aneurysm rupture. Therefore, inflammation plays a key role in the formation and rupture of intracranial aneurysms.

Objective To study on transcatheter closure of membranous ventricular septal defect (VSD) with pseudoaneurysm by patent ductus arteriosus(PDA) occlusion devices in children and summarize the skill and clinical experience. Methods The study included 20 membranous VSD cases in children.According to the finding of the left ventricular angiography, various kinds of the PDA occlusion devices was implanted. The mean diameter of the waist of the occluder was ( 10.4 ± 2.6) mm. Examination by transthoracic echocardiography (TTE) immediately and left ventricular angiography after the occluder was implanted 15minutes later to evaluate the efficacy. Results In the 20 patients, one of the Ⅳ type VSD patient was quitted because of the significant residual shunts(≥2 mm). Slightly residual shunts ( < 2 mm) was found in one Ⅲ type VSD patient with multi-outlet. And disappeared in 1 month after the procedure, which VSD patient was confirmed by TTE. Thirteen cases were normal by EKG examination (or the same before procedure).Incomplete right bundle branch block was found in 4 cases. First degree atrioventricular block was found in 1 case and paroxysmal junctional tachycardia was found in 1 case. All of them were recovered in 1 week. Conclusions Transcatheter interventional therapy with PDA occlusion devices for membranous VSD with pseudoaneurysm is safe and effective. The key of the procedure is to select suitable occluder and suitable position to plant them according to the membranous morphologic characteristics, size and position of the pseudoaneurysm. It is a facultative method for transcatheter therapy this kind of congenital heart disease.
